1.Construction and identification of a mifepristone inducible eukaryotic expression vector

Objective:To construct a mifepristone(an oral nontoxic chemical)-indueible eukaryotic expression vector and to evaluate its regulatory effect in vitro using luciferase reporter gene.Methods:Vector pDC-RULUC,which contains firefly lucif- erase reporter gene,promoter and mifepristone-inducible system,was constructed by molecular biological methods.A 1.2 kb in sulator was inserted to reduce the interference between two transcription units.The vector was verified by PCR,restriction en- zyme digestion,and sequencing,pDC-RULUC was used to transfect SW620 cells using Lipofectamine2000.Cells transfected with pGL3-Control and pGL3-Basic were used as positive and negative controls,respectively.Cotransfectant with pRL-TK renilla lu- ciferase reporter vector was used as internal control.Cells of experimental group were incubated for 48 h in presence of different concentrations of mifepristone after transfection and were harvested for luciferase assay by using the Dual-Luciferase Reporter Assay System.Half of the wells were replaced with fresh medium and were measured after another 48 h.Results:The recom- bined plasmid vector was identified by digestion with different enzyme restrictions,PCR and sequencing analysis.The relative activity increased with the increase of mifepristone concentration.When the concentration of mifepristone reached 1?10~(-6)mol/ L,the relative activity increased to approximately 50 folds of the original.No significant luciferase activity was detected when the mifepristone was removed.Conclusion:We have successfully established mifepristone-regulated eukaryotic expression vector, which can be used for controllable gene expression in vitro,providing a way for gene regulation and gene therapy.
2.An adaptation to changes in knee muscle strength is a basic principle for rehabilitation after anterior cruciate ligament reconstruction

BACKGROUND:How to improve reduced muscle strength and the electromechanical delay of the knee flexor at early stage after anterior cruciate ligament reconstruction is worth studying.
OBJECTIVE:To review the research development of muscle strength change and rehabilitation after anterior cruciate ligament reconstruction.
METHODA computer-based search of PubMed and Wanfang databases was performed for articles related to muscle strength change after anterior cruciate ligament reconstruction, elevation methods, muscle function rehabilitation published from January 2000 to January 2014. The keywords were“anterior cruciate ligament;hamstring;muscle strength;electromechanical delay”in English and Chinese. Final y 34 articles were included in result analysis.
RESULTS AND CONCLUSION:Removal of the hamstring tendon can decrease muscle strength of the involved knee flexor and tibial internal rotation strength, then affect the involved knee stability and functional ability. Based on the clinical signs and isokinetic testing results, we can assess the clinical effects after anterior cruciate ligament reconstruction. The change of H/Q ratio can be used to guide the rehabilitation after anterior cruciate ligament injury. Graft remodeling process after anterior cruciate ligament reconstruction has a longer period than other animal experimental studies, and tendon relaxation degree is an important factor influencing electromechanical delay. Based on the basic rehabilitation principles, the rehabilitation training is adjusted timely according to the individual functional level to ful y embody the individualized training. Both domestic and foreign researches indicate that the rehabilitation process begins before anterior cruciate ligament reconstruction, in order to start training of muscle contraction as early as possible to maximize prevent muscle atrophy. The problems are mainly concentrated in the more refined and scientific rehabilitation methods and the degree of early rehabilitation training.
3.Accurate pathological diagnosis of thyroid cancer in the era of precision medicine

Thyroid cancer is the malignant tumor with the most rapidly increasing incidence in recent years, the overwhelming majori-ty of which are derived from follicular cells. The most common tumor entity is differentiated thyroid cancer, which could be divided in-to papillary thyroid cancer and follicular thyroid cancer. The accurate pathological diagnosis of thyroid cancer is the cornerstone in the process of precision medicine which includes prevention, diagnosis and treatment. This review discusses the significance of accurate pathological diagnosis in the precision medicine of thyroid cancer based on the following four aspects:1) the risk evaluation of thyroid cancer, 2) preoperative accurate diagnosis of thyroid nodules, 3) the risk stratification of prognosis and recurrence, and 4) the guid-ance of molecular targeted therapy in advanced patients.

5.Expression and significance of bcl-2 and vascular endothelial growth factor in nephroblastoma

Objective:To study the expression of vascular endothelial growth factor(VEGF)and B-cell lymphoma/leuke mia-2(bcl-2) in nephroblastoma and to evaluate the relationship between VEGF and bcl-2.Methods:With mouse anti-human VEGF and bcl-2 monoclonal antibody,immunohistochemistry was used to examine 6)the expression of VEGF protein and bcl-2 protein in 17 cases of nephroblastoma and 7 cases of normal tissue as contrast.VEGF mRNA and bcl-2 mRNA expression was examined by in situ hybridization.Results:The positive expressions of bcl-2 protein in nephroblastoma(82.4%)were significantly higher than those in normal tissues(28.5%).The positive expressions of VEGF protein in nephroblastoma(58.8%)were significantly higher than those in normal tissues(14.3%).The rate of bcl-2,VEGF mRNA in nephroblastoma was 88.2%,82.4%,respectively.The expression of bcl-2,VEGF mRNA was increased significantly in nephroblastoma compare with control.VEGF expression and bcl-2 in nephroblastoma were positively related(rs=0.633,P

8.Distribution of Klebsiella pneumoniae infection and antimicobial resistance change in elderly patients

Objective To investigate distribution of Klebsiella pneumoniae infection and its drug resistance change in elderly pa‐tients during 2011 and 2013 .Methods API system was used to identify bacteria ,collected from clinical samples of elderly patients during 2011 and 2013 .Drug sensitivity was tested by using disk diffusion method .Results The isolation rate of Klebsiella pneu‐moniae from 2011 to 2013 was 9 .93% ,11 .56% and 13 .37% .During these three years ,the isolation rate of strains was the highest in Intensive Care Unit ,and the specimens mainly distributed in sputum .There was an increasing tendency of the resistance rates of Klebsiella pneumoniae to commonly used antibiotics ,though the resistant rate to piperacillin/tazobactam ,ceftazidime ,meropenem and amikacin was below 30% .Conclusion Observation on changes of drug resistance of Klebsiella pneumoniae and reduction of risk factors for bacteria infection ,might be important for prohibiting the increasing of drug resistance of Klebsiella pneumoniae .
9.Sterilization and Isolation Administration in Operating Room: Risks Factors and Countermeasures

OBJECTIVE To control the infection in operating room through effective administration. METHODS The risks factors and countermeasures of sterilization and isolation administration in operating room, were discussed to strengthen the consciousness of safety, and set up perfect rules and regulations in operating room. RESULTS The countermeasures were carried out,and infection rate was reduced.The infection rate of type Ⅰ incision was reduced obviously in recent 5 years. CONCLUSIONS The way of infection in operating room is controlled through perspective effectual administration and the risk of nosocomial infection is reduced.
