1.Study on Antagonism of GSH and Taurine to Acute Oxidative Damage Induced by Mercury

Objective To study the antagonism of glutathione(GSH) and taurine on acute oxidative damage caused by mercury(Hg). Methods 32 Wistar rats were randomly divided into four groups. The rats of control group were given 0.9% saline by subcutaneous injections. The rats in mercuric chloride (HgCl2) group were subcutaneously injected with 2.5 mg/kg HgCl2. Rats of the other two groups were pretreated with 3 mmol/kg GSH and 4 mmol/kg taurine, respectively and two hours later injected subcutaneously with 2.5 mg/kg HgCl2. Urine creatine and mercury contents were determined; mercury level, contents of GSH, MDA and GSH-Px in liver and kidney were evaluated. Results Compared with the control, urine mercury level, level of mercury in liver and in the renal cortex, contents of GSH, MDA and GSH-Px in kidney in the group treated with 2.5 mg/kg HgCl2 were significantly increased. In the rats pretreated with GSH and taurine, contents of MDA in kidney were significantly decreased compared with those treated with mercury only. The levels of GSH-Px in kidney in the group pretreated with GSH and taurine were significantly higher than that not only in the mercury group but also in the control. Compared with the mercury group, levels of mercury in urine and liver in GSH pretreated group were distinctly reduced. Conclusion Pretreated with GSH and taurine have certain protection for the acute oxidative damage caused by mercury.
2.Development of a New Multi-Purpose Face Mask for Oxygen Provision in First Aid

Objective To develop a new multi-purpose face mask for oxygen provision in first aid,which can be used in both war time and peace time.Methods The structure,operating principle and application of a multi-purpose face mask,whose transparent silicone mask body has even the entrance of nested endoscopy and oxygen concentration regulating twist,are introduced in this paper.Results This mask can be used in painless endoscopy and emergent oxygen supply in tracheal intubatton.Conclusion The mask can greatly enhance the safety of painless endoscopy and it can also be used as mask for the use of general anesthesia.
3.Screening of nutritional risk in 112 patients with inflammatory bowel disease

Objective To assess the nutritional risk of patients with inflammatory bowel disease (IBD).Methods The nutritional status of 112 IBD patients from PUMC Hospital were evaluated by Nutritional Risk Screening 2002 (NRS 2002).Using Chinese standard,a body mass index (BMI) that was lowered than 18.5 kg/m~2 according to clinical material was regarded as malnutrition,and the score was recorded as 3.The se-verity of ulcerative colitis (UC) was evaluated using True-Love criteria as mild,moderate,and severe.Crohn's disease (CD) was evaluated using Harvey-Bradshaw Index as in remission stage (≤4),moderately active stage (4-8),and severely actively stage (≥9).Results All these 112 patients,including 70 UC cases and 42 CD cases,were evaluated by NRS2002.Forty-five patients (40.2%) were judged as at the risk of malnutrition.The incidence of malnutrition was significantly higher in CD patients than in UC patients (52.4% vs.32.9%,P<0.05).It was also significantly correlated with the severity of disease.In patients with severe IBD,the rate of mal-nutrition evaluated by BMI according to clinical material was significantly lower than the rate of nutritional risk.Con-clusions The risk of malnutrition is high in IBD patients.NRS 2002 can be used for nutritional risk screening.
4.Clinical study on the role of immunosuppressant agents in prevention of postoperative recurrence of Crohn's disease

Objective To investigate the effects of different therapeutic strategies on recurrence of postoperative Crohn's disease (CD) patients.Methods From September 2009 to September 2014,85 CD patients with intestinal resection were enrolled.The clinical features and maintenance therapeutic medication were retrospectively analyzed.The patients were divided into non-treatment group (induding continuously or cumulatively taking medicine less than three months),5-aminosalicylic acid (5-ASA) group and immunosuppressant agents group (including azathioprine,methotrexate and thalidomide).Kaplan-Meier method was performed to compare the recurrence rate in postoperative CD with different therapeutic medication and the risk factors of postoperative recurrence were also analyzed.Results Among 85 CD patients,there were 32,21 and 32 patients in non-treatment group,5-ASA group and immunosuppressive agents group,respectively.After surgery,the one year accumulated clinical recurrence rate of immunosuppressant agents group was 12.5% (4/32),which was significantly lower than that of non-treatment group (56.3%,18/32) and 5-ASA group (38.1%,8/21),and the differences were statistically significant (x2 =12.250,P<0.01;x2 =4.102,P =0.043).After surgery,the two years accumulated clinical recurrence rate of immunosuppressant agents group was 12.9 % (4/31),which was significantly lower than that of non-treatment group (75.9%,22/29) and 5-ASA group (47.6%,10/21),and the differences were statisitcally significant (x2 =17.840,P<0.01;x2 =6.597,P=0.010).After operation,the one year accumulated endoscopic recurrences rates of non-treatment group,5-ASA group and immunosuppressant agents group were 39.1% (9/23),5/16 and 34.6% (9/26),respectively;while the two year accumulated endoscopic recurrence rates were 59.1% (13/22),6/16 and 44.0% (11/25),respectively.However,there was no statistically significant difference among the groups (all P>0.05).Penetrating lesion was an risk factor of postoperative clinical recurrence in CD patients (x2 =4.963,P=0.026,oddsratio (OR) =2.221,95 % confidence interval (CI) 1.121 to 5.775).Conclusions Immunosuppressive agents rather than 5-ASA have remarkable effects in preventing postoperative clinical recurrence in CD patients.Postoperative clinical recurrence is more likely to happen in patients with penetrating lesions.
5.Low-dose celiac CT angiography with fixed current-time product

Objective To characterize the feasibility of low-dose CT angingraphy on 16-slice multislice computed tomography (16-MSCT), and its relationship to the noise in the pre-contrast image and enhancement value.Methods Forty-three consecutive patients (male 21, female 22, mean age 59 years, median age 56 years) underwent abdominal 16-MSCT (Toshiba Aquilion 16) with constant scanning parameters including 120 kVp, a 0.5-second gantry rotation time, a pitch of 0.938: 1, and 16×1-mm detector collimation.The mA was set at 200 in the pre-contrast scan and 160 in the contrast-enhanced scan.The arterial phase images were retrospectively reconstructed with 1-mm slice thickness, 0.8 mm interval.The pre-contrast noise was defined as the standard deviation (SD) of the aorta at the level of right posterior crura of diaphragm.The enhancement of aorta was also measured at level of celiac artery.The volume rendering of CT angiography was made and classified into three grades (excellent, good, bad).Receiver operating characteristic curve (ROC) was used to evaluate the relationship between the image quality of CT angiography and noise in the pre-contrast image and enhancement value.Results Twenty-five cases had the aorta enhancement between 300.0--400.0 HU.The sensitivity and specificity of excellent CTA image was 75% and 62%, respectively when the SD was 12.00.Eighteen cases had the aorta enhancement more than 400.0 HU.The sensitivity and specificity of excellent CTA image was all 100% when the SD was 12.25, and 100% and 75%, respectively when the SD was 13.35.The area under curve of CTA image quality and enhancement in receiver-operated characteristic analysis was O.907.The enhancement was more than 356.7 HU when the sensitivity and specificity of excellent CTA image was 100% and 60%, respectively, and 389.8 HU when 78% and 80%.When the enhancement was more than 442.4 HU, the specificity of excellent CTA image was 100%.Conclusions MSCT angiography with low radiation dose is feasible.Good CT angiography demands great enhancement, and then, the needed mAs is lower when the SD is smaller.
6.Expression and location of hypoxia inducible factor-1α and -2α in the remnant kidney of 5/6 nephrectomy rats

Objective To investigate the location and expression of hypoxia inducible factor (HIF) subunits in the remnant kidney of 5/6 nephrectomy rats. Methods Remnant kidneys were produced in adult male SD rats by 5/6 nephrectomy. The renal function and histopathological changes were evaluated at week 1, 2, 4, 6, 8 and 12 after operation. Tissues of remnant kidneys were collected to detect the location and expression of HIF-1α and HIF-2α by immunohistochemistry staining and Western blotting. The mRNA levels of HIF targeted genes vascular endothelial growth factor (VEGF) and heme oxygenase-1 (HO-1) were determined by RTPCR. Results (1) 5/6 nephrectomy rats underwent one week of acute renal failure at first[Scr (122.8±22.1) μmol/L] and then developed compensative chronic renal failure [(66.0±3.7)-(66.4±8.4) μmol/L], but the level of Scr increased quickly after week 6 [(66.4±8.4)-(127.8±22.7) μmol/L],concomitantly with progressive tubulointerstitial fibrosis in remnant kidney cortex. (2) In cortex, HIF-1α was expressed only in the atrophic and dilated tubular cells while HIF-2α was located in endothelial, interstitial fibroblasts, and vascular smooth muscle cells. The semiquantitative results of imunohistochemistry and Western blotting revealed that HIF-1α and HIF-2α were both gradually up-regulated during the early stage of remnant kidney, peaked at week 4 and 6, and then gradually down-regulated. (3) The mRNA levels of HIF targeted genes VEGF and HO-1 transiently peeked at week 4 and 6, and then decreased gradually. Conclusions The increased stabilization of HIF-αprotein and transcription of HIF targeted genes at the early stage of this model is a compensation reaction towards hypoxia. The mechanism of decreased expression of HIF-α at the end stage of chronic kidney disease deserves further investigation.

It is very important to establish and execute the all -sided experimental animal quality monitoring in order to guarantee human health , animal health and welfare as well as the authenticity , validity, and repeatability of the experimental research results.Setting corresponding sentinel animals in the experiment can effectively monitor the quality of experimental animals.This article gives a general review of the selection of sentinel rats in the microbiological quality monitoring of the experiment animals , contact form and time between the sentinel rats and the rats being monitored , the placement of sentinel rats, and the number of rat cages being monitored by each cage of sentinel rats , as well as the test quantity, test frequency and the project.
8.Descriptive epidemiological study of hyperkalemia among patients in the emergency department

Objective:To describe the incidence, diagnosis rate, treatment rate and treatment pattern of hyperkalemia, and serum potassium retesting rate among hyperkalemia patients in the emergency department.Methods:Data were derived from Military Data Center for Rational Use of Drugs. Patients who accessed emergency medical services (≥18 years old) with record(s) of serum potassium between 2015 and 2017 were included. The data of laboratory test, diagnosis, and treatment were analyzed. The main outcomes included the incidence of hyperkalemia, the diagnosis rate, the treatment rate, treatment pattern and the 7-day retesting rate.Results:A total of 1 039 245 patients who met the above criteria were included, of whom, 36 615 (3.52%) had at least one hyperkalemia event. Among the emergency patients with chronic kidney disease, heart failure, diabetes mellitus and hypertension, the proportions of patients who experienced hyperkalemia were 47.69%, 29.13%, 21.69% and 10.16%, respectively. The diagnosis rate of emergency hyperkalemia patients was 9.23%. The overall hyperkalemia treatment rate was 42.1%. Insulin + glucose injection was the most commonly used therapy for emergency hyperkalemia patients. The overall serum potassium retesting rate within 7 days was 28.8%.Conclusions:Hyperkaliemia is more common and more severe in patients with chronic kidney disease, heart failure, diabetes and hypertension. The diagnosis rate and retesting rate of hyperkalemia are low, suggesting that the identification and management of hyperkaliemia in emergency patients should be strengthened.

Objective To evaluate the impact of nutrition support on the clinical outcomes of gastrointestinal disease patients at nutritional risk and explore the the cost-effectiveness of various nutrition support options.Methods A prospective cohort study was designed.The patients who met the predetermined inclusion criteria were followed up during the hospital stay.Nutritional risk was determined using the Nutrition Risk Screening 2002 on admission.The information with respect to nutritional support,occurrence and treatment of complications,length of hospital stay,and discharge destination was monitored and recorded.The direct costs of nutritional support and the costs of diagnosing and treating complications were calculated after discharge.Infectious complication-free patient was used as the effectiveness indicator in the cost-effectiveness analysis.Results Patients who had received nutrition support had significantly lower infectious complications incidence (6.8% vs.19.6%,x2 =9.0,P=0.003) and significantly higher total hospitalization costs (P =0.0001).The adjusted (by general linear model) cost of parenteral nutrition (PN) cohort,enteral nutrition (EN) cohort,PN combined EN cohort,and the cohort without nutritional support were 5635,1212,5220,and 1339 China Yuan,respectively.The incremental cost effectiveness ratios were 36 101,-794,and 33 748 China Yuan for PN,EN,and PN-EN combination groups,respectively.Conclusions For the patients at nutritional risk,nutritional support can remarkably reduce the incidence of infectious complications.The preliminary resuits of cost-effectiveness:due to lack of enough data required by health economic professional,it can not be cited directly.
10.A Progress on ADME and the Toxicity of Fructus Psoralea

Fructus psoralea is a tonic traditional Chinese herb commonly used in clinic.The chemical constituents of Fructus psoralea are complicated,mainly containing coumarins,flavonoids and monoterpene phenols with various pharmacological effects.Since the increasing number of reports on the toxicity of Fructus psoralea in clinic,the side effects including toxicity on the liver and kidney,as well as skin allergies have gradually attracted attention.The toxicity of Fructus psoralea is produced from ADME (absorption,distribution,metabolism and excretion) in vivo.In this paper,we collected and clarified the studies of ADME of Fructus psoralea in vivo,and summarized recent adverse clinical events and research over its toxicity.We propose to make a thorough study on the toxic material basis of Fructus psoralea and the toxicological mechanism of its extraction,fractions and compounds.The review provided a possible reference and the direction of research for the safe clinical use of Fructus psoralea.
