1.Correlation factor analysis in older type 2 diabetes patients with osteoporosis

ObjectiveTo investigate the correlation factors in older type 2 diabetes patients with osteoporosis.MethodsThe related risk factors including their age,body mass index(BMI),history of disease and bone mineral density(BMD) were analyzed in 106 older type 2 diabetes patients with osteoporosis,who were hospitalized in the first affiliated hospital of Guangzhou Medical College during January 2000 to December 2010.Results BMD of lumbar[ (0.756 ± 0.099) g/cm2 ],neck of femur [ (0.658 ± 0.111 ) g/cm2 ] and wards triangle [ (0.525 ± 0.064) g/cm2 ] in the eighty group were distinctly reduced than the sixty group[ (0.883 ±0.112)g/cm2,(0.781 ±0.130) g/cm2,(0.627 ±0.118) g/cm2] ( P <0.05).BMD of lumbar[ (0.738 ± 0.114) g/cm2 ) ] and neck of femur [ ( 0.664 ± 0.112 ) g/cm2 ) ] in low weight patients ( BMI≤20 kg/m2 ) were distinctly reduced than normal BMI group [ ( 0.816 ± 0.138 ) g/cm2,( 0.727 ±0.134)g/cm2] ( P <0.05).BMD of lumbar[ (0.743 ±0.122) g/cm2] and neck of femur[ (0.719 ±0.147)g/cm2] in patients with course of diabetes more than 10 years were distinctly reduced than the patients with less course[ (0.886 ± 0.132 ) g/cm2,( 0.792 ± 0.122 ) g/cm2 ] ( P < 0.05 ).BMD was negatively correlated with age and history of diabetes ( P < 0.05 ).BMD was positively correlated with BMI ( P < 0.05).ConclusionsOlder,low BMI and long history of diabetes are the risk factors of older type 2 diabetes patients with osteoporosis.
2.Pemetrexed with geiftinib or pemetrexed alone in advanced non-small cell lung cancer with acquired resistance to EGFR tyrosine kinase inhibitors

Background and purpose:New treatment methods should be explored for non-small cell lung cancer (NSCLC) patients with acquired resistance to epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI). This study compared the curative effect of pemetrexed with geiftinib or pemetrexed alone in advanced NSCLC with acquired resistance to EGFR-TKI.Methods:This study included 62 NSCLC patients with advanced EGFRgene mutation and acquired resistance to EGFR-TKI. Among those, 32 patients were treated with pemetrexed and geiftinib, and 30 patients treated with geiftinib alone. The differences in outcomes between the two strategies were assessed.Results:Objective response rate (ORR) was 46.9% for those treated with pemetrexed and geiftinib and 20%for those treated with pemetrexed alone(χ2=4.933,P<0.05). There was no signiifcant differences between the two groups on disease control rate (DCR) (P>0.05). The median progression-free survival (PFS) was 8.0 months on pemetrexed and gefitinib group and 6.3 months on pemetrexed alone (χ2=8.063,P<0.05). There was no significant differences between the two groups on overall survival (OS) (P>0.05). Higher occurrence of leukocytopenia and rash was observed in the pemetrexed and geiftinib group than in the pemetrexed group (P<0.05). There was no signiifcant differences be-tween the two groups on grade 3-4 toxicities (P>0.05).Conclusion:This study was to demonstrate that continuation of EGFR-TKI with pemetrexed in patients with acquired resistance improves outcomes compared with pemetrexed alone. An improved response rate and PFS were observed in this study. A larger prospective clinical trial is needed to further evaluate this promising strategy.
3.Influence of holistic nursing intervention on psychological state and gastrointestinal function of patients after abdominal operation

Objective To investigate influence of hohstic nursing intervention on psychological state and gastrointestinal function of patients after abdominal operation. Methods 48 patients undergoing abdominal surgery with epidural anesthesia were divided into the intervention group and the control group. Holistic nursing intervention and routine nursing were given to the two groups respectively. Self-rating anxiety scale (SAS) and serf-rating depression scale (SDS) were used to evaluate the psychological state of all patients and gastrointestinal tract function were also measured. The result underwent t test. Results SAS and SDS scores were lower on the fifth day after operation and the time of borborygmus recovery, anal exsufflation and defecation was shorter in the intervention group when compared with those of the control group. Conclusions Hohstic nursing to patients after abdominal surgery can eliminate anxiety and depression and improve recovery of gastrointestinal tract function.
4.THE STUDY OF THE DIFFERENTIATION OF NSCs MODIFIED WITH ISLET-1 GENE INTO CHOLINERGIC NEURONS

Objective To explore whether Islet-1 gene of the rat could induce NSCs to differentiate into cholinergic neurons. Methods NSCs were transduced with Islet-1 recombinant retroviral expression vector.The protein expression of Islet-1 gene in NSCs was detected by immunofluorescence histochemical method.The ability of NSCs to differentiate into ChAT positive cells was observed in vivo and in vitro. Results The numbers of ChAT positive cells were significantly increased in the group of NSCs modified with Islet-1 compared with the control group in vitro.NSCs modified with Islet-1 could differentiate into ChAT positive cells when they were grafted into the corpus striatum of the adult rat brain.Conclusion Islet-1 gene could induce NSCs to differentiate into cholinergic neurons.

6.Diagnosis and treatment of obstructive jaundice following transcatheter hepatic arterial chemoembolization for hepatic cavernous hemangioma

Hepatic cavernous hemangioma is the most common benign tumor of liver.Hepatic artery embolism chemotherapy is one of the commonly used treatment methods,but more and more related complications and sequelae have been reported recently,including obstructive jaundice caused by damaged biliary tract.Because the symptoms are not typical,obstructive jaundice might be misdiagnosed as malignant biliary tumor,which brought troubles to subsequent treatment.In this article,the clinical data of 2 patients with obstructive jaundice following transcatheter hepatic arterial chemoembolization for hepatic cavernous hemangioma were retrospectively analyzed,and the experience in the diagnosis and treatment of this disease was summarized.
7.Construction of recombinant retroviral vector with rat Islet-1 gene

Objective To isolate the rat insulin gene enhancer binding protein 1(Islet-1)gene and construct plEGFP-C1-Islet-1 recombinant retroviral expression vector.Methods The cDNA encoding the rat Islet-1 gene was isolated by RT-PCR method,the cDNA was first cloned into PGET-1 TA vector to facilitate the sequence and then subcloned into the retroviral vector plEGFP-C1.plEGFP-C1-Islet-1 was transfected into PA317 packaging cells with lipofectamine 2000.Transformants were selected in medium containing G418.Results A 1 050 bp DNA fragment was obtained by RT-PCR;plEGFP-C1-Islet-1 recombinant retroviral expression vector was identified by restrictive enzymes digestion,PA317 cells transfected with recombinant vector expressed enhancer green fluorescent protein(EGFP).Conclusion The gene encoding the rat Islet-1 is obtained and plEGFP-C1-Islet-1 expression vector is constructed successfully.

Objective To explore the effects and its possible mechanism of Fluvastatin on carotid artery atherosclerotic plaque(CAAP) in elderly patients.Methods 92 patients with CAAP detected by Color Doppler Ultrasonography were divided randomly into group A(48 cases) which treated with Fluvastatin 40 mg/d and group B(44 cases) which treated with 80 mg/d respectively for 40 weeks after a washout period(Fluvastatin 40 mg/d for 8 weeks).The changes of CAAP areas,the levels of serum total cholesterol(TC),low-density lipoprotein(LDL),high sensitivity C-reactive protein(hs-CRP) were observed between the before and after treatment.Results There were 40 cases in each group completed the Fluvastatin treatment for 48 weeks.After treatment,the CAAP areas in group A and B were significantliy decreased from(0.20?0.18)cm2 and(0.20?0.19)cm2 to(0.12?0.15)cm2 and(0.11?0.12)]cm2(allP
