Objective To determine the efficacy of the double balloon catheter in ripening and dilatation of the unfavorable cervix for induction of labor .Methods A total of 332 full-term women who were underwent induction of labor with the double balloon devices were collected for retrospective analysis .Cervix was assessed before the double balloon insertion and after it was removed 12 hours later, then amniotomy alone or combined with oxytocin was performed .Cord blood gases were analyzed .Results The double balloon catheter increased Bishop score in all subgroups with a mean improvement of 2.5 ±0.7 ( P <0.05 ) .The mean insertion-deliv-ery interval was (19.07 ±4.31)h.Cesarean section was performed in 80/332 (24.1%) patients.Cord arterial pH value was 7.3 ± 0.1 .Conclusions The double balloon catheter induced significant ripening and dilatation of the unfavorable cervix .It was effective and safe.

Objective To observe the effect of reperfusion therapy on the prognosis of acute myocardial infarction (AMI) complicated by eardiogenic shock(CS)in reperfusion era. Methods 89 cases of AMI with CS were included with 57 male and 32 female.50 cases received conservative therapy and 39 cases reperfusion therapy.28 of the 39 cases had suecessflll reperfusion and 11 eases failed.18 patients had intra-aortic balloon pump (IABP) within 1 hour of CS,they constituted an early group;35 patients treated with IABP 1 hour after CS were of a late group.A group of 36 cases were not treated with IABP (no IABP group). Results The mortality of the early group with IABP Was significantly lower than that of the late and no IABP group(33.3% vs.74.2% vs.86.1%,P<0.01).The mortality of the group with sucessful reperfusion was significantly lower than that of unsuccessful reperfusion and conservative no IABP group (42.8% vs.81.8% vs.84.0%,P<0.01).logistic regression analysis showed that successful reperfusion therapy (OR 4.232,95% CI 1.407~12.730,P=0.01) and THE TIME of using IABP(OR 0.22.95% CI 0.063~0.764,P=0.017)were independent risk factors for death. Conclusion Early successful reperfusion and early institution of IABP were the most important therapeutic measures for reducing mortaliIv of AMI complicated by CS.

Objective To investigate the incidence of aspirin resistance (AR) in patients with Type Ⅱ Diabetes mellitus (DM) and the correlation between AR and advanced glycosylation end products (AGEs) as well as the homeostasis model assessment for insulin resistance index (HOMA-IR). Methods A total of 69 patients with Type Ⅱ DM and another 23 patients without DM as control group were enrolled between October 2009 and July 2010. Blood lipid, blood routine, fasting blood glucose, Glycated hemoglobin (GHb/Hb A1c) ,fasting insulin were determined at first. After aspirin treatment for at least 7 days, platelet aggregation stimulated by arachidonic acid(AA) and adenosine diphosphate (ADP) were measured. In addition, the level of serum AGEs was measured by using ELISA assay. The degree of insulin resistance was obtained by using HOMA-IR. Results The incidence of AR in patients with Type Ⅱ DM was higher than that in the controls(30.4% vs. 8.7%, P = 0.037 ); the levels of serum AGEs and HOMA-IR in patients with Type Ⅱ DM were higher than those in the controls [ (359.56 ± 120. 14) pg/mL vs. (275.45 ± 118.06)pg/mL, P=0. 004; (4.42 ±4.78) vs. ( 1.5 ±0.78), P<0.01, respectively]; platelet aggregation stimulated by AA in the diabetic group was correlated with serum AGEs and HOMA-IR( R =0.463, P <0.01; R=0.290, P =0.016, respectively); and platelet aggregation stimulated by ADP was only positively correlated with HOMA-IR(R =0.242, P = 0.045). Conclusions The incidence of AR in patients with Type Ⅱ Diabetes mellitus is higher than that in the controls, and diabetics with higher serum AGEs and HOMA-IR are more likely to develop aspirin resistant.

Objective To compare the efficacy of double balloon catheter and dinoprostone suppositories on maturing cervix and parturifacient action. Methods A total of 382 full-term nulliparous women (200) cases in double balloon group and 182 cases in dinoprostone suppositories medicine group) with labour induction indications and unfavourable cervices (Bishop score≤5scores) were collected for retrospective analysis. The promoting effects of the two methods on cervical ripening, 24-hour vaginal delivery rates and inductiondelivery intervals were compared. Results There was no difference in ceasarean delivery rates between two groups (25.5% vs 24.2%, P=0.67). Dinoprostone suppositories was more effective in promoting cervical ripening (2.68±0.87 vs 2.44±0.74, P<0.01). The induction-to-delivery interval was longer in the double balloon group (19.86±12.96)h than that in the medicine group(17.13±2.56)h (P<0.01). Conclusion Labour induction in nullipara with unfavourable cervices improve delivery rates. Dinoprostone suppositories is better than double balloon in promoting cervical ripening.

OBJECTIVETo explore the genetic cause for a child with mental retardation, developmental delay and multi-systemic developmental disorders by analyzing the copy number variations (CNVs) and correlating the genotype with the phenotype.
METHODSRoutine G-banding was performed to analyze the karyotype of the patient and her parents. In addition, single nucleotide polymorphisms array (SNP-array) was used to determine the CNVs, which was confirmed by fluorescence in situ hybridization (FISH).
RESULTSNo karyotypic abnormality was detected upon chromosome analysis. However, SNP-array has identified a de novo hemizygous deletion of 1673 kb on chromosome region 7q11.23, which has been associated with Williams-Beuren syndrome. The microdeletion was confirmed by FISH testing.
CONCLUSIONA child with Williams-Beuren syndrome has been diagnosed by SNP-array and FISH. The de novo 7q11.23 microdeletion probably underlies the clinical manifestation of the patient. Compared with routine karyotype analysis, SNP-array is more useful for diagnosing children with multiple congenital anomalies with unclear etiology.
