1.Observation on levels of trace elements and cytokines in patients with leukoderma

Objective To observe and analyze the change situation of trace elements and cytokines in the patients with leukoderma.Methods Sixty-two patients with leukoderma in our hospital from June 2013 to January 2016 were selected as the observation group and contemporaneous 62 individuals undergoing physical examination were selected as the control group.Then the serum trace elements and cytokines levels were compared between the two groups and the detection results of above serum indexes in the observation group were compared among different grades and stages too.Results The serum trace elements levels of the observation group were all lower than those of the control group,the serum interleukin 10(IL-10)level was lower than that of the control group,while other serum cytokines levels were all higher than those of the control group,moreover the detecfion results of above serum indexes in the observation group had statistical differences among different grades and stages(P<0.05).Conclusion Leukoderma has obvious effect on trace elements and cytokines and the influence of differente grades and stages of leukoderma on their expressions is inconsistent.
2.Histological changes of tooth root and periodontal tissue during mini-implant anchorage for incisor intrusion in dogs

BACKGROUND: Tooth intrusion easily leads to root resorption. Previous studies regarding orthodontic treatment-caused tooth root resorption or retrospective clinical studies based on X-ray films have great errors in outcome evaluation because of intrusion force which cannot be precisely controlled. OBJECTIVE: This study established dog models of mini-implant anchorage for incisor intrusion to observe the histological changes of tooth root and periodontal tissue and to evaluate the feasibility and safety of mini-implant anchorage for incisor intrusion. METHODS: Nine dogs were assigned to one control group (n = 1) and four experimental groups per time to sacrifice (1, 2, 4 and 12 weeks; n = 2 dogs for each experimental group). No force was added to the control group. In the experimental groups, mini-implant as an anchorage was placed in the buccal alveoli between maxillary second and third incisors on each side. A traction force of 100 g was imposed to each side to intrude the maxillary first and second incisors on each side. At 1, 2, 3, and 4 weeks (traction force was imposed for 4 weeks and after withdrawal of extraction force, mini-implant was retained in place for 8 weeks), dogs were sacrificed. The first and second incisors together with gingival and alveolar bone were completely resected to prepare histological specimens. Following hematoxylin-eosin staining, histological changes of tooth root and periodontal tissue were observed. RESULTS AND CONCLUSION: Compared with the control group, in the 1-week group, histological changes were primarily at the root tip and alveolar ridge crest, alveolar bone and cementum were absorbed and peridental membrane presented glassy degeneration in local region; in the 2-week group, bone resorption degree and range were obviously enlarged, and bone resorption developed from root tip, root middle part to cervical part; in the 4-week group, bone resorption was still active and the glassy degeneration of peridental membrane disappeared; in the 12-week group, significant improvement in alveolar bone and cemental surface was observed, bone lacuna had deposition of newly formed bone, and peridental membrane was orderly arranged. These findings reveal that in the mini-implant anchorage for dog incisor intrusion, early histological changes primarily appear in the root tip and alveolar ridge crest, presenting as alveolar bone and cemental resorption and the glassy degeneration of the peridental membrane. Bone resorption extent and range expand with the persistence of traction force. After withdrawal of traction force, tooth root and periodontal tissue were gradually repaired
3.Effect of an Internet support program on uncertainty in illness in breast cancer survivors

Objective To evaluate the effect of an Internet support program on uncertainty in illness in breast cancer patients after operation. Method One hundred and nineteen breast cancer patients after surgery who were discharged from Shanghai Cancer Hospital were assigned into the control group (63 cases) and the intervention group (56 cases). A 12-week Internet support program was offered to the patients in the intervention group, while the patients in the control group received the routine follow-up. Measurement of uncertainty in illness was taken at the sixth week and the twelfth week after the baseline survey. Results The total score of the uncertainty in illness as well as the factor scores of ambiguity and lack of information showed downward trend in both the intervention group and the control group, and the three indexes in the intervention group declined more rapidly than those in the control group. The scores of the three indexes in the intervention group were significantly lower than those in the control group at the twelfth week. Conclusion The Internet support program which offers abundant information, multi-disciplinary cooperation and real-time interaction intervention is an effective approach in reducing uncertainty in illness for breast cancer survivors.
4.Immediate breast reconstruction by implanting silicone prosthesis wrapped by ectopectoralis after nipplesparing mastectomy

Objective To investigate the feasibility of immediate breast reconstruction by implanting silicone prosthesis wrapped by ectopectoralis after nipple-sparing mastectomy in early stage breast cancer.Methods From Jan.2006 to Nov.2009,28 breast cancer patients underwent immediate breast reconstruction by implanting silicone prosthesis wrapped by ectopectoralis after nipple-sparing mastectomy.Results Among the 28 patients,27 were satisfied with the apperance of their breast after operation.Conclusions Immediate breast reconstruction by implanting silicone prosthesis wrapped by ectopectoralis after nipple-sparing mastectomy not only ensures the curative effects but also beautifies the apperance of the breast.The operation has the advantages of simplity,less trauma and quick recovery.There is no difference compared with mastectomy in terms of complication,recurrence rate and mortality etc.
5.Effects of nurse-led case management intervention on arm function and uncertainty for women with breast cancer

Objective To evaluate the effect of a nurse-led case management intervention on arm function and uncertainty for women with breast cancer.Methods 90 women with breast cancer were divided into the intervention group and the control group according to their nurses,with 45 patients in each group.All participants were followed six months after surgery.The intervention group attended a 6-month nurse-led case management program,while the control group received the routine care and follow-up only.Questionnaires on arm function and uncertainty were administered 1 month,3 months and 6months after the surgery respectively.Score changes were compared by repeated-measure ANOVA and MANOVA.Results The arm function of the intervention group was better than the control group,except for the 10th day after the operation,there were significant differences at other three time points.The disease uncertainty level of the intervention group was better than that of the control group,except for the dimension of unpredictability,there were evident differences in other dimensions at other three time points.Conclusions The nurse-led case management intervention could improve arm function recovery and decrease the uncertainty to disease of breast cancer survivors.
6.The effectiveness of computer-based monitoring system for patients with breast cancer receiving adjuvant hormonal therapy

Objective To evaluate the effect of nurse-led follow-up on medication adherence and quality of life for breast cancer patients receiving adjuvant hormonal therapy.Methods A randomized controlled trial was conducted with 157 patients in the intervention group and 154 in the control group.A self-design web-database medication monitoring platform was designed for managing patients such as texting,reminding and mailing.Participants were randomized to follow-up care as usual(yearly outpatient clinic visits) or nurse-led telephone follow-up(monthly consultation with structured intervention).Telephone follow-up was performed by four trained breast care nurses (BCN) and consisted of a semi-structured interview including managing the side-effects of endocrine therapy,compliance with hormonal therapy and an open discussion of these issues.Patients' medication adherence and quality of life were evaluated by Morisky Medication Adherence Questionnaire(MAQ) and FACT-B at baseline and 3,6 18 and 24 months,respectively.Results The Nurse-led telephone follow-up did not significantly improve the quality of life(P>0.05).MAQ score in the intervention group was significantly greater than that in the control group(P<0.05) at 3,6,18 and 24 months.Conclusion Nurse-led follow-up using computer-based monitoring system can improve patients' medication adherence,but there is no obvious increase in quality of life.
7.Differential regulation of SREBP 1 c/cm on transcriptional activity and expression of PERK promoter

Objective To investigate the effect of sterol regulating element binding protein (SREBP1c) and its ac-tive form (SREBP1cm) on human protein kinase R-like endoplasmic reticulum kinase (PERK).Methods Re-porter victors of PERK promoter and its truncations were constructed with pGL 3-basic and co-transfected with internal reference pRL-SV40 into cell and luciferase activity was detected .pcDNA3.1 ( -)-SREBP1c or pcDNA3.1 ( -)-SREBP1cm was co-transfected with PERK promoter transcriptional activity core regions and the detection of dual -lu-ciferase reporter gene was used to analyze the regulation of SREBP 1c/1cm on PERK promoter transcriptional activity . The expression level of PERK mRNA and protein were detected by RT-PCR and Western blot .Results PERK pro-moter and truncations were successfully constructed into pGL 3-basic, and PERK promoter core area of transcription-al activity had determined;Dual-luciferase report gene showed that SREBP 1c inhibited PERK promoter transcrip-tional activity and SREBP1cm promoted PERK promoter transcriptional activity .RT-PCR and Western blot showed that SREBP1c decreased PERK mRNA and protein expression , but SREBP1cm increased PERK mRNA and protein expression, which was consistent with the detection of dual-luciferase report gene .Conclusions SREBP1c and SREBP1cm have a opposite regulation effect on PERK promoter transcriptional activity and its expression .
8.Application of linkage teaching in pediatric nursing skills course based on integration of theory and practice

Objective:To explore the application of linkage teaching model between hospitals and colleges based on integration of theory and practice in the course of pediatric nursing skills.Methods:Two undergraduate classes of pediatric nursing in a college in 2017 were selected as the experimental group and the control group by random sampling. The experimental group adopted the combined teaching method of colleges and hospitals based on the integration of truth and practice, while the control group adopted the traditional teaching method. The results of skill operation assessment were compared between the two groups, and the results were evaluated by the self-designed teaching effect evaluation questionnaire.Results:The results of the combined nursing skills teaching in colleges and universities were higher in the experimental group (91.24±3.01) than that in the control group (87.33±2.96), the difference was statistically significant ( t value was 5.027, P<0.05). The evaluation score of teaching effect of the experimental group was 62.61 ± 2.94, which was significantly higher than that of the control group (49.67 ± 7.79) ( t value was 7.965, P<0.05). Conclusions:The combined teaching mode of colleges and hospitals based on the integration of theory and practice is beneficial to improve students′ comprehensive quality and learning effect, further shorten the gap between college theory and clinical practice, and is an effective form of pediatric nursing education.
9.Efficacy and safety of blinatumomab in the treatment of B-cell acute lymphoblastic leukemia

Blinatumomab, as a novel bispecific antibody targeting CD19 and CD3, can induce T lymphocytes to precisely target CD19 positive B lymphocytes to apoptosis. At present, it is the only bispecific antibody approved for the treatment of hematological malignancies in China. Blinatumomab is effective in the treatment of newly diagnosed, relapsed/refractory, minimal residual disease positive patients with B-cell acute lymphoblastic leukemia (B-ALL) . It can improve the survival of the patients and is well tolerated. The further study of blinatumomab can provide theoretical basis and new ideas for induction therapy, salvage therapy and subsequent hematopoietic stem cell transplantation in patients with B-ALL.
10.The systemic evaluation and clinical significance of immunological function for advanced lung cancer patients.

BACKGROUND AND OBJECTIVEThe actual evaluation of immunological function is significant for studing the tumor development and devising a treatment in time. The aim of this study is to evaluate the immunological function of advanced lung cancer patients systematically, and to discuss the clinical significance.
METHODSThe nucleated cell amounts of advanced lung cancer patients and the healthy individuals were counted. The immune cell subsets and the levels of IL-4, INF-gamma, perforin and granzyme in CD8+T cells by the flow cytometry were measured. The proliferation activity and the inhibition ratio of immune cells to several tumor cell lines were evaluated by MTT assay.
RESULTSThe absolute amounts and subsets of T, B, NK cells of advanced lung cancer patients were lower than the healthy individuals (P < 0.05); However, the proportion of regulatory T cells of advanced lung cancer patients (4.00 +/- 1.84)% was lower than the healthy individuals (1.27 +/- 0.78)% (P < 0.05). The positive rates of IFN-gamma perforin, granzyme in CD8+T cells decreased while them in IL-4 did not in the advanced lung cancer patients compared to the healthy control group (P < 0.05). The proliferation activity of immune cells, the positive rate of PPD masculine and the inhibition ratio to tumor cells in the advanced lung cancer patients was lower than the healthy subsets obviously (P < 0.05).
CONCLUSIONThere was a significant immune depression in the advanced lung cancer patients compared to the healthy individuals.
