1.Determination of Hesperidin in Xiangsha Yangwei Pill by HPLC

Objective To establish a method for the determi nation of hesperidin in Xiangsha Yangwei Pill(XYP).Methods Reversed -phrase HPLC was adopted.T he sample was injected into Sep -Pak C 18 cartridge for sample purifica-tion.The chromatographic conditio ns were:Prodigy ODS(150mm?4.6mm,5?m)as analytic column,methanol -wa-ter -acetic acid(35:61:4)as mobile phase,detect wavelength a t 283nmand the flowrate being 1.0mL /min.Results The linear range of hesperidin was from0.148~1.481?g,r =0.9998.The mean recovery was 100.3%,RSD =1.5%(n=6).Conclusion This method was simple,practical,accurate and suitable for the qualit y control of XYP.
2.Contribution of history-taking, physical examination and laboratory investigation to diagnoses for medical outpatients

Objective To quantitatively evaluate relative contribution of medical history,physical examination and laboratory investigation to diagnoses for medical outpatients.Methods In total,145 medical visitors to the outpatient department of Peking Union Medical College Hospital (PUMCH) during October 10 to 16,2008 were recruited and followed-up for 12 months.Results Nineteen of 145 visitors (13.1%) were lost during the period of follow-up and diagnoses were established for 86 of them (68.3%)finally with medical history and for 20 (15.9%) with physical examination or laboratory investigation,respectively.Confidence index of internists in their correct diagnosis increased to 7.3 with medical history and to 7.9 and 8.7 with physical examination and laboratory investigation in average,respectively.Conclusions Most visitors to internal medicine department could be diagnosed correctly with medical history only.On the basis of physical diagnosis,selection of adequate laboratory investigation for them is critical to improvement of clinical diagnosis.
3.Determination of Tetracyclic Triterpene Saponins in Sanqi Jiegu Pills and Stability of Saponins in Granula-tion

Objective:To establish an HPLC method for the determination gensenoside Rg1 , Rb1 and notogensenoside R1 in Sanqi Jiegu pills and to investigate the stability of saponins in the granulation process. Methods:The separation was performes on a COSMO-SIL 5 C18 MS-II(250 mm × 4. 6 mm,5μm)column with gradient elution using water and acetonitrile as the mobile phase, the flow rate was 1. 0 ml·min-1 , the detection wavelength was 203nm, the column temperature was 30℃, and the injection volume was 10 μl. The content change of triterpene saponins in Sanqi Jiegu pills prepared by the granulation process at different drying temperature was al-so investigated. Results:The linearity of notogenoside R1 , gensenoside Rg1 and gensenoside Rb1 was good within the range of 0. 211 8-2.648 0 μg·ml-1(r=0.999 7),0.566 1-7.076 0 μg·ml-1(r=0.999 7) and 0.317 2-3.964 5μg·ml-1(r=0.999 7) with the average recovery of 98. 2%(RSD=0. 82%,n=6), 98. 1%(RSD=0. 72%,n=6) and 98. 1%(RSD=0. 59%,n=6),respectively. The content of saponins in Saqi Jiegu pills was decreased obviously with the increase of the drying temperature. Conclusion:The meth-od is simple, quick and reliable, and can be applied in the quality control of Sanqi Jiegu pills. It is recommended that the drying tem-perature in the granulation process should be controlled below 60℃.
4.Clinical value of physical diagnosis for patients with functional illness at an ambulatory setting

Objective To evaluate diagnostic accuracy based on patient history and physical examinations in medical outpatients.Methods Totally, 145 consecutive patients visiting general internal medicine clinic at a university-affiliated teaching hospital during October 10 to 17, 2008 were recruited into the study and followed-up for 12 months.Results Eighteen of 145 patients ( 12.4% ) were lost to followup.Diagnosis was confirmed by follow-up for 45 ( 35.4% ) of those with medically unexplained symptoms (MUS).Sensitivity of physical diagnosis for those with MUS was 82.2 percent, with specificity of 95.1 percent, likelihood ratios of positive and negative results of 16.9 percent and 0.19 percent, its positive and negative prediction values of 90.2 percent and 90.7 percent, and overall accuracy of 90.6 percent,respectively.Conclusions MUS was common in medical clinical practice.Preliminary diagnosis for MUS based on patient history and physical examinations has been proved remarkably reliable.Carefully selected auxiliary laboratory evaluation combined with physical diagnosis is important for management of MUS.
5.Early diagnostic value of combining non-specific indicators in neonatal septicemia

Objective To evaluate the significance of routine blood test and C‐reaction protein(CRP) ,procalcitonin(PCT ) in early diagnosis of neonatal septicemia .Methods Totally 95 cases of confirmed septicemia ,158 cases of the clinical diagnosis of sep‐ticemia and 160 cases of non‐septicemia who were admitted to Newborn Diagnosis and Treatment Center of Children′s Hospital of Chongqing Medical University from January 2013 to May 2014 were enrolled for this study .The routine blood test ,CRP and PCT concentration of the three groups were analyzed .Taking blood culture as the gold standard ,receiver operating characterristic (ROC) curve was drawn ,sensitivity and specificity of single or combined indicator in early diagnosis of septicemia was analysed .Results There was a significant difference between confirmed septicemia group and non‐septicemia group in PCT ,CRP ,WBC ,I/T ,PLT (P< 0 .05) ;the area under the ROC curve(AUC) was 0 .892 ,0 .847 ,0 .828 ,0 .700 ,0 .681 ,respectively ;the five indicators for con‐firmed the diagnosis positive rate of septicemia group and non‐septicemia group was statistically significant (P < 0 .05) ;the single indicator PCT had better diagnosis value ,its sensitivity and specificity was 84 .2% and 66 .9% .With the application of regression of binary Logistic the AUC of combining PCT ,CRP ,WBC was 0 .974 ,its sensitivity and specificity was 94 .7% and 87 .5% ,which was higher diagnosis value than the only one indicator alone .Conclusion The combination of PCT ,CRP and WBC have contribute to early diagnosis of neonatal septicemia .
6.Effect of ambroxol on the structure of Pseudomonas aeruginosa biofilm

Objective To observe the effect of ambroxol on the structure of Pseudomonas aeruginosa biofilm (BF) in an established biofilm model in vitro. Method Mature BF was formed after 7 days′ culture of Pseudomonas aeruginosa on plate. The BF structure was observed by scanning electron microscope (SEM). The parameters of BF structure were analyzed though pictures from confocal laser scanning microscopy (CLSM) with Image Structure Analyzer (ISA) software. Fluorescence intensity of the viable bacteria in BF treated by ambroxol alone and together with ciprofloxacin were determined with fluorescence microscopy. Results After the treatment of ambroxol, the BF was destroyed and the matrix outsides became thin and abnormal compared with the control by SEM, and small amount of bacteria dispersed. 7.Synapse function of neuron-like cells induced from mesenchymal stem cells by Salvia miltiorrhiza

Objective To explore whether the neurons induced from mesenchymal stem cells(MSCs) have synapse function or not.Methods Passage 4-5 MSCs in good shape was induced by Salvia miltiorrhiza with optimized protocol for several times,and then the observation under inverted phase contrast microscope,immunofluorocytochemistry and the measurement of neurophysiological function were carried out.Ca2+ influx and synapse function were detected with laser-scanning confocal microscopy(LSCM),taking 50 mmol/L KCl as stimuli to evoke action potential.Results The cells that had been induced for 5 h at 4th time from MSCs,looked like neurons and displayed that the processes stretched out to form complex net.Immunofluorocytochemistry presented that the rate of TUJ-1 expression was(96.7?2.8)% and that of synaptophysin was(96.2?2.1)%.When the neuron-like cells were stimulated by high concentration of KCl,intracellular Ca2+ influx enhanced quickly.When the neuron-like cells were stimulated by high KCl solution for the first time,SynaptoRed-C2 anchored onto the membrane,and after the second excitation,the fluorescence intensity decreased quickly.Conclusion The neuron-like cells derived from MSCs that are induced by salvia miltiorrhiza with optimized protocol have synapse function.
8.Comparative study on retinopathy of prematurity by color Doppler flow imaging diagnosis and clinical ophthalmology

Objective To investigate the clinical value of color Doppler flow imaging(CDFI) in the diagnosis and clinical follow-up of retinopathy of prematurity(ROP).Methods From March 2008 to February 2009,98 eyes of 49 premature infants(gestational age≤37 weeks,or birth weight≤2 000 g) underwent ROP screening by RetCam digital retinal camera and indirect ophthalmoscopy in the fourth to sixth postnatal week.At the same time,they were also examined by 8-14 MHz CDFI.Results Determined by RetCam digital retinal camera and indirect ophthalmoscopy,ROP was found in 41 eyes,including 8 eyes at stage Ⅰ,19 eyes at stage Ⅱ,6 eyes at stage Ⅲ,8 eyes at scarring stage and no eye at stage Ⅳ andⅤ.Compared with ophthalmologic examination,the specificity of CDFI for ROP diagnosis was 85.5%,and the sensitivity was 76.7%.Detected by CDFI,typical changes were not found in the infants with ROP at stage Ⅰ.In the infants with ROP at stage Ⅱ and Ⅲ,abundant blood flow signals were found in the central retinal artery(CRA).Increased amounts of the small vessels and irregular circuitousness were also observed.There was no obvious change in the systolic velocity(VS) of CRA.The diastolic velocity(Vd) was decreased and the resistance index(RI) was increased.A strong zonal echo protruded into the vitreum.Small blood flow signals which came from CRA by tracing toward their distal end were found in the same position using CDFI.Conclusions Detected by CDFI,the sonographic features of infants with ROP at stage Ⅰ are not typical,but those of infants with ROP at stage Ⅱ and Ⅲ are specific.CDFI is valuable in clinical ROP screening of high risk group.
9.Application of different cardiac assessment systems in pregnant women with congenital heart disease

Objective To compare the application of Goldman , CARPREG and ZAHARA in predicting cardiac outcomes in pregnant women with congenital heart disease. Methods A retrospective analysis of 144 pregnant women with CHD , enrolled in The Third Affiliated Hospital of Guangzhou Medical University between Jan 1992 and June 2015 was conducted. The mortality and the incidence of cardiac events were counted by three risk estimation methods. The sensitivity and calibration of three systems were compared by using Hosmer-Lemeshow goodness of-fit analysis and Aera Under Curve (AUC). Results In terms of mortality rates, the sensitivity of CARPREG was higher than that of Goldman (AUC:0.874 and 0.889 respectively;P = 0.568 >0.05);. In terms of the incidence of cardiac events, the sensitivity of ZAHARA was higher than that of Goldman (AUC:0.912 and 0.773 respectively;P = 0.063 > 0.05); Conclusions All 3 risk estimation methods have good performance in sensitivity of predicting the incidence of cardiac events and mortality in pregnant women with CHD. The ZAHARA has a better calibration in predicting the incidence of cardiac events , and we recommend ZAHARA as the first choice of evaluation system for this kind of patients.
10.Effects of S100B on the expressions of dopamine receptors and synthesis, metabolism of neurotransmitters dopamine and 5-hydroxytryptamine

Objective To investigate the effects of S100B on the expressions of dopamine receptors and the synthesis,metabolism of neurotransmitters dopamine,5-hydroxytryptamine which are related to the abnormal motor coordination of Parkinson's disease (PD).Methods The hS100B transgenic mice were established.The mice were divided into S100B transgenic group (TG,n =14),S100B knockout group (KG,n =14) and the non-transgenic control group (CG,n =14).The motor coordination ability of mice was measured by the Rota-rod test.The expressions of dopamine D1 receptor (D1DR),dopamine D2 receptor (D2DR),tyrosine hydroxylase (TH) and phosphorylated TH at Ser19,Ser31,Ser40 in brain tissue were detected by reverse transcription polymerase chain reaction and Western blotting.The levels of Tyr,levodopa,dopamine,homovanillic acid,Trp,5-hydroxytryptamine and 5-hydroxyindoleacetic acid in mesencephalon were measured by high performance liquid chromatography with fluorescence detection.Results Compared with CG,the motor coordination ability of mice (s) showed progressive decline in TG (3 months:4.60±0.30vs4.25±0.21,q =5.194;6 months:4.52±0.31 vs4.07±0.22,q =6.139;9 months:4.43 ± 0.25 vs 3.60 ± 0.18,q =13.484;all P < 0.05),the expressions of D2DR mRNA and protein decreased (1.34 ± 0.13 vs 0.48 ± 0.07,q =21.578;1.05 ± 0.15 vs 0.69 ± 0.10,q =8.063,both P<0.05) and phosphorylated TH at Serl9 and Ser40 increased (0.95 ±0.10 vs 1.14-0.13,q =4.972;0.94 ± 0.12 vs 1.17 ± 0.14,q=5.382,both P< 0.05),the levels of levodopa,dopamine and homovanillic acid were elevated (87.04 ± 11.77 vs 115.28 ± 16.80,q =4.764;56.66 ± 9.87 vs 72.96 ± 11.02,q=3.923;26.58 ± 8.11 vs 38.65 ± 6.67,q=3.981,all P< 0.05),the leve1 of 5-hydroxytryptamine was reduced (925.50 ± 74.26 vs 637.87 ± 56.76,q =11.084,P < 0.05),the ratios of homovanillic acid/dopamine and 5-hydroxyindoleacetic acid/5-hydroxytryptamine increased (0.45 ± 0.05 vs 0.54±0.08,q =3.325;0.94±0.07 vs 1.42±0.12,q =12.367,both P<0.05) in the brain of TG at the age of 9 months old.There was no significant difference of detection indexes between KG and CG.Conclusions S100B plays an important role in the development of PD and the brain-specific S100B transgenic mice can be used to investigate the function of S100B gene on the development of PD.
