Insomnia is a commen disease which disturbs life of modern persons terribly.As usual,insomnia means the subjective experience the short time of sleep and poor sleep quality which will influence the normal activity in the day-time.Here are the manifestations of insomnia:tired,dizziness,headache,tinnitus,palpitation,not concentration,poor efficiency etc.In this article,through the study on traditional twelve two-hour thery,the author connects the theory to the real reasons of insomnia and finds out the suitable time for patients to take the medicine to get twice the power with half the effort.
2.Clinical effect observation on nervous tinnitus treated with self-made Huoxue Tongqiao Decoction

Objective: To observe the clinical effect of Huoxue Tongqiao Decoction on nervous tinnitus. Methods: 60 Patients(90 ears) were randomly divided into treatment group(30 cases, 44 ears) and control group(30 cases, 46 ears). Patients of control group took orally sibelium and oryzanol. Based on the control group, patients of treatment group took Huoxue Tongqiao Decoction to treat tinnitus. 3.Experience of laparoscopic repair in 5 patients with vesicovaginal fistula.

OBJECTIVE:
To explore new methods for laparoscopic repair of vesicovaginal fistula.
METHODS:
Five patients with vesicovaginal fistula in Xiangya Hospital, Central South University, were reviewed retrospectively from May 2013 to July 2014. All patients underwent laparoscopic repair surgery, and the surgical methods were analyzed. The operative time, intraoperative blood loss, and hospital stay time were recorded. The duration of follow-up was from 4 to 12 months.
RESULTS:
Th e surgical procedures for all 5 patients were successful. No open surgery was required. The operative time was 70~120 (mean: 97) min, the intraoperative blood loss was 40~70 (mean: 54) mL, the hospital stay time was 4~8 (mean: 5.8) days. During the follow up of 4~12 (mean: 7.6) months, no recurrence was observed.
CONCLUSION
Laparoscopic repair of vesicovaginal fistula is a feasible and safe and effective procedure with less blood loss and shorter recovery time, which can minimize surgery damage and improve successful rate.

4.Clinical study of electroacupuncture assisted with facial expression muscle control rehabilitation training for the patients with severe idiopathic facial paralysis

Objective:To investigate the clinical effects of electroacupuncture assisted with facial expression muscle control rehabilitation training in the treatment of patients with severe idiopathic facial paralysis.Methods:A total of 130 patients with severe idiopathic facial paralysis who met inclusion criterion were chosen and randomly divided into 2 groups, 65 patients in each, from January 2017 to December 2019 in our hospital. The control group were treated with facial expression muscle control rehabilitation training and western medicine, and the experiment group with electroacupuncture on the basis of the control group. Both groups were treated for 8 weeks and followed up for 6 months. The facial nerve function was evaluated by H-B scale and facial nerve sunny brook scale the quality of daily life was evaluated by Facial Disability Index Physical Function (FDIP) scale and Facial Disability Index Social Function (FDIS) scale. The latency/M wave amplitude of motor evoked action on orbicularis oculi muscle and orbicularis ORIS muscle were measured by EMG evoked potential instrument. The occurrence of hemifacial spasm during follow-up was recorded. The clinical effective rates were evaluated.Results:The total effective rate of experiment group was 92.31% (60/65), which was significantly higher than that of the control group 76.9% (50/65) ( χ2=6.495, P=0.039). The H-B scale scores of experiment group after treatment were significantly less than that of the control group ( t=3.438, P<0.01). The facial nerve sunny brook scale scores of experiment group after treatment were significantly more than that of the control group ( t=2.674, P=0.032). The FDIP scores of experiment group after treatment were significantly less than that of the control group ( t=3.986, P<0.01). The FDIS scores of experiment group after treatment were significantly more than that of the control group ( t=4.621, P<0.01). The NCV latency of orbicularis oculi muscle [(2.51 ± 0.27) ms vs. (2.82 ± 0.46) ms, t=4.258] and orbicularis oris muscle [(2.97 ± 0.22) ms vs. (3.35 ± 0.40) ms, t=4.783] of observation group were significantly lower than those in the control group ( P<0.01). The M wave amplitudes of orbicularis oculi muscle [(1.83 ± 0.45) mV vs. (1.30 ± 0.39) mV, t=3.827] and orbicularis oris muscle [(2.58 ± 0.60) mV vs. (1.97 ± 0.36) mV, t=4.017] of observation group were significantly higher than those in the control group ( P<0.01). The incidence of facial spasm with follow-up of experiment group for 4.62% (3/65) was significantly lower than that of the control group for 15.38% (10/65) ( χ2=9.271, P=0.033). Conclusion:Electroacupuncture assisted with facial expression muscle control rehabilitation training in the treatment of patients with severe idiopathic facial paralysis can relieve clinical symptoms, improve facial nerve function, improve the quality of daily life and be helpful to reduce the facial spasm risk.
5.Recent advance in clinical manifestations and differential diagnoses of Kikuchi-Fujimoto disease involving nervous system

Histiocytic necrotic lymphadenitis is known as Kikuchi-Fujimoto disease (KFD).KFD is a systemic disease,which is characterized by fever,necrotizing lymphadenopathy and leukopenia.However,involvement of the central and peripheral nervous system is rare.It can present as aseptic meningitis,encephalomyelitis,peripheral neuropathy,cerebellar ataxia and tremor,which makes it difficult to be differentiated with other nervous system diseases.Our review summarizes the clinical manifestations and differential diagnoses of KFD involving nervous system in order to reduce misdiagnoses and mistreatments.
6.Application of genetic algorithm in blending technology for extractions of Cortex Fraxini.

OBJECTIVETo explore the feasibility of genetic algorithm (GA) on multiple objective blending technology for extractions of Cortex Fraxini.
METHODAccording to that the optimization objective was the combination of fingerprint similarity and the root-mean-square error of multiple key constituents, a new multiple objective optimization model of 10 batches extractions of Cortex Fraxini was built. The blending coefficient was obtained by genetic algorithm. The quality of 10 batches extractions of Cortex Fraxini that after blending was evaluated with the finger print similarity and root-mean-square error as indexes.
RESULTThe quality of 10 batches extractions of Cortex Fraxini that after blending was well improved. Comparing with the fingerprint of the control sample, the similarity was up, but the degree of variation is down. The relative deviation of the key constituents was less than 10%.
CONCLUSIONIt is proved that genetic algorithm works well on multiple objective blending technology for extractions of Cortex Fraxini. This method can be a reference to control the quality of extractions of Cortex Fraxini. Genetic algorithm in blending technology for extractions of Chinese medicines is advisable.

7.Characters of urinary microbiota in male patients with non-muscle invasive bladder cancer

Objective To characterize the urinary microbial profile of male non-muscle invasive bladder cancer patients compared to healthy controls.Methods Between March,2017 and September,2017,mid-stream urine from 26 non-muscle invasive bladder cancer and 18 non-neoplastic controls were collected by the clean method,then centrifuged and processed for 16S rRNA gene sequencing.Sequencing reads were processed for evaluating alpha diversity and beta diversity using QIIME.LEfSe algorithm was performed to identify potential bacterial genera biomarker.Results The smoking cases were more in tumor group than those in control group(21 vs.7,P < 0.01).The urinary microenvironment of bladder cancer was characterized by increased bacterial richness (Observed species index,Chaol index and Ace index,125.77 ± 69.64 vs.80.38 ± 46.24;142.82 ± 76.74 vs.90.68 ± 47.62;and 147.92 ± 77.68 vs.88.19 ± 45.38,all P < 0.05) and by the enrichment of some bacterial genera (e.g.,Acinetobacter and Anaerococcus).Significant difference in β diversity was found between cancer and non-cancer group (ANOSIM,P =O.009).A clear hierarchical clustering of cancer samples was observed,suggesting a common dysbiosis associated to bladder cancer.Conclusions Patients with non-muscle invasive bladder cancer exhibit a different microbial community compared to non-neoplastic controls,suggesting a possible pathophysiological correlation between microbiome and bladder cancer.Urinary microbial community may be associated with the prognosis of NMIBC.
8.The Role of SDF-1α-CXCR4/CXCR7 in Migration of Human Periodontal Ligament Stem Cells

Background and Objectives:
Regenerative endodontic procedures (REPs) are a research hotspot in the endodontic field. One of the biggest problems of REPs is that it is difficult to realize regeneration of pulp-dentin complex and functional reconstruction. The reason is still not clear. We hypothesize that the migration may be different in different dental stem cells. Periodontal ligament stem cells (PDLSCs) may migrate faster than stem cells of apical papilla (SCAPs), differentiating into cementum-like tissue, bone-like tissue and periodontal ligament-like tissue and, finally affecting the outcomes of REPs. Hence, this study aimed to explore the mechanism that regulates the migration of PDLSCs.
Methods:
and Results: After isolating and culturing PDLSCs and SCAPs from human third molars, we compared the migration of PDLSCs and SCAPs. Then we investigated the role of SDF-1α-CXCR4/CXCR7 axis in PDLSC migration. We further investigated the impact of Porphyromonas gingivalis (P. gingivalis) lipopolysaccharide (LPS) on PDLSC migration and the potential mechanism. PDLSCs showed better migration under both noninflammatory and inflammatory conditions than SCAPs. SDF-1α can promote the migration of PDLSCs by elevating the expression of CXCR4 and CXCR7, increasing the interaction between them, promoting expression of β-arrestin1 and activating the ERK signaling pathway. P. gingivalis LPS can promote the migration of PDLSCs toward SDF-1α through increasing the expression of CXCR4 via the NF-κB signaling pathway, promoting the expression of β-arrestin1, and activating the ERK signaling pathway.
Conclusions
This study helped elucidate the potential reason for the difficulty in forming pulp-dentin complex.
9.Recent advance in diagnosis and treatment of neuro-Beh?et's disease

Beh?et's disease (BD) is a chronic, relapsed, and inflammatory vascular disease, usually with multiple systems involvement. Recurrent oral and genital ulcers, uveitis and skin lesions are common clinical manifestations. When nervous system gets involved, it is called Neuro-Beh?et's disease (NBD). NBD can lead to parenchymal damage to the brain or spinal cord, cerebral venous thrombosis and arterial vasculitis. Our review focuses on the clinical manifestations, auxiliary examination, diagnosis and treatment of NBD.
10.Recent advance in efferocytosis and stroke
