1.Meta-analysis of Qingkailing Injection in treatment of viral hepatitis

AIM The proofs of evidence-based medicine of Qingkailing Injection's effect on viral hepatitis are lack in spite of the wide use of Qingkailing Injection in medical practice.This study aims to use Meta-analysis to evaluate its clinical effect and safety.METHODS Such databases as CNKI,VIP and Wanfang were searched for randomized controlled trials related to Qingkailing Injection in treatment of viral hepatitis,which were published officially between 1996--2016.Revman 5.3 software was used to conduct Meta-analysis for the effective rate,rate of adverse reaction,the decrease rate of ALT,AST and TBIL of Qingkailing Injection.Meanwhile,subgroup analysis was conducted to the clinical effect of doses [< 0.5 mL/(kg · d),0.5-0.67 mL/(kg · d),>0.67 mL/(kg · d)],treatment courses (≤ 14 d,> 14 d),manufacturers (Beijing University of Chinese Medicine Pharmaceutical Factory,Shenwei Pharmaceutical Company,Shanxi Taihang Pharmaceutical Company),year (1996 to 2002,2003 to 2009,2010 to 2016).RESULTS Fourteen studies were included according to the criterion.The results of Meta-analysis showed that the total effective rate,the decrease rate of ALT,AST and TBIL in Qingkailing Injection group were significantly higher than those in the control group (P < 0.05).Whereas,no significiant differences were found between Qingkailing Injection group and the control group of its rate of adverse reaction (P > 0.05).Subgroup-analysis showed that the effective rate of Qingkailing in doses subgroup,treatment courses subgroup,manufacturers subgroup and year subgroup were significantly higher than those of the control group.CONCLUSION Qingkailing Injection has obvious benefit in the treatment of viral hepatitis,which can reduce the activity of ALT and AST,also can reduce the content of TBIL.But we should pay attention to the prevention of allergic reaction in the clinical use.
2.Clinical significance of histological examination and noninvasive measurements in renal osteodystrophy

Objective To analyze the histological changes of bone diseases and to investigate the noninvasive measurements for diagnosing renal osteodystrophy (ROD) in maintenance dialysis patients . Methods Ninety-one patients were selected to receive bone biopsy . The bone samples were stained with HE, toluidine blue and Masson, and were examined with light microscopy . The levels of immunoreactive parathyroid hormone (iPTH), osteoprotegerin (OPG),sRANKL and osteocalcin (OCN) were determined in the patients enrolled from 2004 to 2006 . The level of iPTH was measured by radioimmunoassay . OPG and sRANKL were measured by ELISA,and OCN was measured by chemiluminescence . Results The incidence of ROD in the maintenance patients was 100% . According to the histological appearance, 50 cases (54 .9%) were high turnover bone disease (secondary hyperparathyroid bone disease), 9 cases (9 .9%) were low turnover bone diseases(osteomalacia and adynamic bone disease), and 32 cases(35 .2% ) were mixed bone disease . The level of iPTH in patients with ROD was significantly increased compared with healthy controls . It was the lowest in low turnover bone diseases . There was no difference among three types of ROD . OPG level was significantly increased compared with healthy controls [(2176 .58±1576 .08) pmol/L vs (1310 .46±1254 .00) pmol/L, P<0 .05] . The level in high turnover bone diseases was higher than that of the healthy controls [(2261 .85±1712 .22) pmol/L vs (1310 .46±1254 .00) pmol/L, P<0 .05] . There was no difference among three types of ROD .sRANKL level in high turnover bone disease was significantly increased compared with healthy controls [(0 .328±0 .524)pmol/L vs (0 .084±0 .190) pmol/L, P<0 .05] . OCN level was also higher than that of the healthy controls (P<0 .05), and the OCN level in low turnover ROD was the lowest among three types of ROD . OCN level in mixed ROD was dramatically increased as compared to low turnover ROD [(226 .633±66 .455) pmol/L vs (193 .03±104 .269) pmol/L, P <0 .05] .Conclusions The histological changes of bone disease can be indicated by iPTH level, but the types of ROD can not be distinguished according to iPTH level neither be differentiated by the levels of OPG, sRANKL and OCN . Bone histomorphometry is still the golden standard for diagnosing renal osteodystrophy .
3.Pharmacodynamics and toxicoligy of Longkai Granules against prostatic hyperplasia

AIM:To demonstrate the inhibitoary effects of Longkai Granules(LKG) against experimental prostatic hyperplasia and evaluate its toxicity on animals taking the granules orally. METHODS: The prostate exponent,DNA content in prostate tissue、the activity of acid phosphatase in serum or the wet weight of spermatophores and testicles in normal immature mice and in the hyperplasia model mice induced by subcutaneous injecting testooslerone spropionate or by implanting of the urogenital sinus were determined after administrating of LKG intragastrically to the mice.The single maximum dosage of LKG in mice and its long-term(13 weeks) toxicity in Wistar rats and Beagle dogs in orally was evaluated. RESULTS: LKG could decrease the weights of prostates and DNA content in the tissue in the normal immature mice in the amount of 20 and 40 g/kg once a day.LKG,in the amount of both 10,20 and 40 g/kg for 10 days and 20 and 40 g/kg for 30 days,could inhibit the hyperplasia of ventral prostates in the model mice induced respectively by the injection of testooslerone spropionate and by implanting urogenital sinus.LKG,in the(amount) of 100 g/kg for 13 weeks to Wistar rats,would lead to prostatic atraphy in alight degree,and its epithelial cells change in shape from column to flat and prostatic cavity being small,which did not recover in 4 weeks after stopping administration of tested drug to the animals.The single maximum dosage by ig in mice was 200 g/kg.There was no significant toxicity reaction in rats in the amount of 10,40 and 100 g/kg for 13 weeks or in Beagle dogs in the amount of 12 and 60 g/kg for 13 weeks. CONCLUSION: LKG can inhibit the prostatic hyperplasia and shows no visible toxic reaction in animals orally.
4.Design and implementation of an improved invasive antenna for microwave hyperthermia.

A new 2450MHz invasive antenna for microwave hyperthermia is designed in this paper. The finite element method is employed to simulate the choke antenna and improved choke antenna, i.e., choke-umbrella antenna. The specific absorption rates (SAR) in tumors are calculated with the two kinds of antennas. S11 of choke-umbrella antenna is also compared with that of choke antenna. Multi-layered block of tissue-equivalent phantom is heated with the fabricated invasive antenna. The distribution of the temperature field in the invasive layer as well as the three-dimensional distribution of the temperature field in phantom is obtained with an infrared thermo graphic technique. Research results show that the improved choke-umbrella antenna can generate more uniform temperature distribution and meets the requirements of the actual clinical microwave hyperthermia better.

5.Transitional care interventions in reducing readmission for patients with heart failure: a literature review

It focuses on eight transitional care models which can be applied to decrease the readmission for heart failure patients, and puts forward current challenges, so as to provide references for applying transitional care to reduce heart failure readmission in China.
6.Investigation on knowledge, attitude and practice of nutrition for stroke patients among community nurses and hospitals

Objective? To understand the status quo of knowledge, attitude and practice (KAP) of nurses in hospital and community about nutrition for stroke patients, compare their differences, and analyze the common related factors, so as to provide basis for carrying out nutritional nursing education for stroke patients in hospital-community interaction. Methods? A total of 160 neurology nurses from 7 Class Ⅲ Grade A hospitals and 161 nurses from 10 community hospitals in Shanghai were selected from August to October 2017 by convenience sampling method. The Chinese version of the Staff Attitudes to Nutritional Nursing Care Geriatric scale were applied in the investigation. A total of 340 questionnaires were distributed and 321 valid questionnaires were recovered, with a recovery rate of 94.4%. SPSS 20.0 software was used for statistical analysis. Results? The scores of nutrition knowledge, nutrition attitude and nutritional practice of the 160 hospital nurses were (58.44±12.67), (72.72±10.73) and (85.50±15.49) respectively, while those of the 161 community nurses were (53.98±13.15), (75.87±8.24) and (69.81±16.75) respectively. The differences in scores between the two groups were statistically significant (P< 0.05). Multiple linear stepwise regression analysis showed that the factors related to nutritional knowledge were academic qualifications and further study at work. Academic qualifications, duration of continuous nursing for stroke patients and nutritional course in school were related to nutrition attitude. Age, academic qualifications and clinical nutrition nursing training were the main factors influencing nutrition practice scores. Conclusions? There are differences in scores of nutrition KAP between hospital and community nurses. Effective nutrition nursing training can improve nurses' KAP of nutrition for stroke patients, so as to ensure the effective development of nutrition nursing services for patients.
7.Development and effect assessment of transitional care on stroke in communities of Shanghai

Objective? To explore the development and effects assessment of transitional care on stroke in communities of Shanghai so as to provide a reference for improving the development and assessment of transitional care on stroke. Methods? From 30 to 31 October 2018, we selected nurses in 55 communities of Shanghai as subjects by convenience sampling. The self-designed questionnaire was used to investigate the general condition, development program and effects assessment of transitional care on stroke in communities. SPSS 20.0 was used to statistical data analysis. Results? Among the 55 communities, 44 (80.0%) communities carried out transitional care on stroke. A total of 29 communities were with the general level of development in transitional care on stroke, and 26 communities were the general level of effects assessment. A total of 41 communities built the linkage model, and only 14 of them had effects assessment indexes. Besides, 93.2% of communities developed two-way referral and telephone follow-up. Conclusions? Transitional care on stroke is developed in most communities of Shanghai with limitations in developing program, regional cooperation and effects assessment. The communities need further expand the development program of transitional care on stroke, improve the regional cooperation between hospitals and communities, build effects assessment indexes and pay attention to transitional model and outcome assessment of caregivers.
8.Potential Antitumor Activity of SIM-89 in Non-Small Cell Lung Cancer Cells.

PURPOSE: c-Met and its ligand, hepatocyte growth factor (HGF), play a critical role in oncogenesis and metastatic progression. The aim of this study was to identify inhibited enzymogram and to test the antitumor activity of SIM-89 (a c-Met receptor tyrosine kinase inhibitor) in non-small cell lung cancer. MATERIALS AND METHODS: Z′-LYTE kinase assay was employed to screen the kinase enzymogram, and mechanism of action (MOA) analysis was used to identify the inhibited kinases. Cell proliferation was then analyzed by CCK8 assay, and cell migration was determined by transwell assay. The gene expression and the phosphorylation of c-Met were examined by realtime-PCR and western blotting, respectively. Finally, the secretion of HGF was detected by ELISA assay. RESULTS: c-Met, activated protein kinase (AMPK), and tyrosine kinase A (TRKA) were inhibited by SIM-89 with the IC₅₀ values of 297 nmol/L, 1.31 µmol/L, and 150.2 nmol/L, respectively. SIM-89 exerted adenosine triphosphate (ATP) competitive inhibition on c-Met. Moreover, the expressions of STAT1, JAK1, and c-Met in H460 cells were decreased by SIM-89 treatment, and c-Met phosphorylation was suppressed in A549, H441, H1299, and B16F10 cells by the treatment. In addition, SIM-89 treatment significantly decreased the level of HGF, which accounted for the activation of c-Met receptor tyrosine kinase. Finally, we showed cell proliferation inhibition and cell migration suppression in H460 and H1299 cells after SIM-89 treatment. CONCLUSION: In conclusion, SIM-89 inhibits tumor cell proliferation, migration and HGF autocrine, suggesting it's potential antitumor activity.

9.Efficacy comparison of interlocking intramedullary nail and locking plate in the treatment of varus proximal humeral fractures in the elderly

Objective:To compare the effect of interlocking intramedullary nail and locking plate in the treatment of varus proximal humeral fractures in the elderly.Method:A retrospective case-control study was conducted to analyze the clinical data of 46 elderly patients with varus proximal humeral fractures treated in Central Theater General Hospital of PLA from June 2016 to January 2019, including 27 males and 19 females, at age of 60-84 years[(71.9±5.7)years]. All fractures were fresh. Overall 25 patients were treated with interlocking intramedullary nail(intramedullary nail group), and 21 patients were treated with locking plate(bone plate group). The incision length, operation time, intraoperative bleeding and fracture healing time were compared between the two groups. Visual analogue scale(VAS)was used to evaluate the degree of pain relief at 1 week and 1 month after operation, and Constant-Murley score was used to evaluate the recovery of shoulder function at 1 month, 3 months and 1 year after operation. The cervical trunk angle was recorded at 2 days and 1 year after operation to judge whether there was a loss of cervical trunk angle. Postoperative complications were observed.Results:All patients were followed up for 12-32 months[(19.7±6.6)months]. The incision length[(7.1±0.6)cm], operation time[(60.8±5.2)minutes], intraoperative bleeding[(64.4±8.4)ml]and fracture healing time[(10.0±1.0)weeks]in intramedullary nail group were significantly less than those in bone plate group[(13.6±0.9)cm,(80.2±8.1)minutes,(151.0±15.2)ml,(11.0±1.5)weeks]( P<0.05). In both groups, the VAS decreased significantly over time, and markedly increased Constant-Murley score was detected as well( P<0.05). The VAS in intramedullary nail group[(2.8±0.2)points,(1.1±0.2)points]was significantly lower than that in bone plate group[(4.0±0.2)points,(1.5±0.1)points]at 1 week and 1 month after operation( P<0.05). The Constant-Murley score in intramedullary nail group[(59.9±6.9)points,(79.1±6.8)points]was higher than that in bone plate group[(50.1±8.5)points,(73.6±8.4)points]at 1 month and 3 months after operation( P<0.05), but the score showed no significant difference between intramedullary nail group[(89.1±5.3)points]and bone plate group[(86.4±6.4)points]at 1 year after operation( P>0.05). According to Constant-Murley score, 10 patients were evaluated as excellent and 15 patients as good in intramedullary nail group at 1 year after operation, with the excellent and good rate of 100%, while 8 patients were evaluated as excellent, 11 patients as good and 2 patients as fair in bone plate group at 1 year after operation, with the excellent and good rate of 91%( P>0.05). The cervical trunk angle in intramedullary nail group[(140.2±2.9)°,(139.6±2.3)°]had significant difference from that in bone plate group[(139.6±3.2)°,(138.8±3.3)°]at 2 days and 1 year after operation( P<0.05). In both groups, the cervical trunk angle had slight lost at 1 year after operation compared with that at 2 days after operation, but the difference was not statistically significant( P>0.05). In intramedullary nail group, 1 patient had subacromial impact. In bone plate group, 1 patient had screw cutting, 2 patients subacromial impact, and 2 patients delayed fracture healing. The incidence of complications in intramedullary nail group was 4%(1/25), lower than 24%(5/21)in bone plate group( P<0.05). Conclusion:Compared with locking nail plate, interlocking intramedullary nail in the treatment of senile varus proximal humeral fracture has the advantages of small trauma, early fracture healing, less pain, early function recovery and less complications.
10.Clinical effect of peroneal fracture line in the treatment of posterior Pilon fracture

Objective:To investigate the clinical effect of peroneal fracture line in the treatment of posterior Pilon fracture.Methods:The data of 26 patients treated with fibular fracture line from January 2017 to July 2019 were analyzed retrospectively, including 11 males and 15 females; the age ranged from 28 to 69 years, with an average of 42.2 years. There were 10 cases of falling injury, 9 cases of falling injury and 7 cases of traffic injury; all of them were fresh closed fibular fractures;According to Yu Guangrong's classification, there were 11 cases of type I, 8 cases of type II and 7 cases of type III; AGH was divided into 10 cases of type I, 5 cases of type IIa, 2 cases of type IIb, 5 cases of type IIIa and 4 cases of type IIIb. All cases were treated by opening the fibular fracture line through the posterolateral approach, the quality of fracture reduction was evaluated by Burwell Charnley radiological evaluation standard after operation; At the last follow-up, ankle function was evaluated by American Association of Foot and Ankle surgery (AOFAS) ankle and hindfoot scores.Results:All 26 patients were followed up for 12-23 months, with an average of 14.9 months; Bone healing was achieved in all fractures. The healing time was 3-6 months, with an average of 4.0 months. The quality of fracture reduction was evaluated according to the Burwell Charnley radiology evaluation standard after operation, including anatomical reduction in 23 cases and acceptable reduction in 3 cases. The anatomical reduction rate was 88% (23/26). At the last follow-up, AOFAS ankle and hindfoot scores ranged from 80 to 100, with an average of 89.9 points, of which 17 cases were excellent and 9 cases were good, and the excellent and good rate was 100%. At the last follow-up, no patient had complications such as reduction loss, skin necrosis, infection, internal fixation loosening or ankle stiffness.Conclusion:After the treatment of Pilon fractures via peroneal fracture line, the distal tibial articular surface and posterior ankle fracture gap can be fully exposed, which can be repositioned and fixed under direct vision, with high anatomical repositioning rate and good and safe clinical results.
