[Summary] Obesity is the most important risk factor of essential hypertension.Obesity-related hypertention has become a worldwide problem.Many clinical studies show that bariatric surgery has significant therapeutic effect on essential hypertension, but the specific mechanism of the condition remains to be fully understood.This paper tried to explain the mechanism of the treatment of bariatric surgery for essential hypertension from the following two aspects: the reversal of the sympathetic disorder and the decline of plasma leptin.

3.The multiple intervention study on type 2 diabetes in community comprehensive management based on clinical treatment

Objective To evaluate the necessity and practicability of optimizing the management of type 2 diabetes mellitus in community after acquiring the peer education and the music therapy to their physical and mental issues and sleep problems. Methods Totally 179 type 2 diabetes mellitus patients who were followed up in Ruijin 2nd community health service center, the random numbers table used to randomize the patients into 2 groups:control group ( 97 cases) and experiment group ( 96 cases), the conventional treatment was used in control group. Besides the conventional treatment measures, the peer support was used by patients in the experiment group. In the second step, 45 patients were met the inclusion and the exclusion criteria, the random numbers table used to randomize the patients into 2 groups:the multiple intervention group (22 cases) and the conventional treatment group (23 cases), the multiple intervention included the music therapy, the peer support and the sleep health education, the conventional treatment included the conventional treatment and the sleep health education. The t test was used to compare the patient's HbA1c and other quantitative data in two groups of patients after the intervention. Results In the first stage of research, compared with the control group patients, the patients 'HbA1c in intervention group was significantly improvement after 6 months(7.26%±1.37%vs.7.53%±1.63%,t=2.148, P<0.05),besides, the intervention group individuals achieved significant improvement in diabetic self-management behaviors and self-efficacy after 6 months, and the improvement in self-efficacy of peer support group was significant different compared with routinely educated patients(104.09±16.40 vs.110.96± 13.86,t=2.120,P<0.05), and the PHQ-9(5.95 ± 4.02 vs.2.55 ± 1.67,t=2.630,P<0.05)between the two group had significant difference, while no improvement was found in PSQI, BMI, and WHR between intervention group and control group. Conclusions Peer support could improve the blood glucose metabolism in patients with type 2 diabetes. With the effect of yoga music and physical exercises, peer support can improve the quality of sleep and decrease depression in T2DM patients, who also have sleep disorders and mild depression.
4.EFFECTS OF RECOMBINANT CILIARY NEUROTROPHIC FACTOR ON STAT3 EXPRESSION AND TYROSINE PHOSPHORYLATION IN THE INJURED NEURONS DURING PERIPHERAL NERVE REGENERATION

Objective To study effects of recombinant ciliary neurotrophic factor(CNTF) on the changes of JAK\|STAT pathway and tyrosine phosphorylation in the injured neurons. Methods Sciatic nerve of rat was resected and sutured into silicone tube with local infusion of recombinant CNTF.The distribution and quantity of STAT3 and phosphotyrosine(PTyr) immunoreactivity in the neurons of L3\|L5 spinal anteriolateral nuclei and L5 spinal ganglion were observed and measured by immunohistochemical ABC method with computer image analysis. Results There was much STAT3 immunoreactivity in the neuron located in the nucleus of spinal anteriolateral nuclei.PTyr immunoreactivity showed higher in the cell membrane of spinal anteriolateral nuclei and in the cytoplasma and neucleus of spinal ganglion in CNTF group than that in SAL group.Conclusion\ The results suggest JAK\|STAT pathway in the injured motoneurons be activated and strengthened and tyrosine phosphorylation in the injured neurons be enhanced by recombinant CNTF.\;[
5.EFFECTS OF RECOMBINANT CILIARY NEUROTROPHIC FACTOR ON GENE EXPRESSION OF SCHWANN CELLS IN THE INJURED NERVE

Objective To study effects of recombinant ciliary neurotrophic factor (CNTF) on gene expression of Schwann cells in the injured peripheral nerves. Methods Sciatic nerve of rat was resected and sutured into silicone tube with local infusion of recombinant CNTF.One or two weeks after nerve repaired,the distribution and quantity of S100 protein (S100),growth associated protein 43 (GAP-43), phosphotyrosine (PTyr) and signal transducer and activator of transcription 3(STAT3) immunoreactivity in the distal nerve of the injured sciatic nerve were observed and measured with immunohistochemical ABC method by computer image analysis. Results S100,GAP-43,PTyr and STAT3 immunoreactivity showed significantly higher in the distal nerve of the injured sciatic nerve in CNTF group than that in SAL group.Conclusion\ Recombinant CNTF could up-regulate the expressions of S100,GAP-43,PTyr and STAT3 in Schwann cells of the injured peripheral nerve.The results suggest that the JAK-STAT pathway can be strengthened,and the expressions of S100 and GAP-43 can be subsequently up-regulated by recombinant CNTF in Schwann cells.
6.Antiproliferation effects of oridonin on hepatocellular carcinoma BEL-7402 cells and its mechanism

AIM: To investigate the antiproliferation effects of oridonin on hepatocellular carcinoma BEL-7402 cells and its mechanisms of action. METHODS: BEL-7402 cells in culture medium were treated with different concentrations of oridonin. The inhibitory rate of the cells was measured by MTT assay. Cell apoptotic rate was detected by flow cytometry (FCM). Morphology of cell apoptosis was observed by Hoechst 33258 stain. Reverse transcriptase polymerase chain reaction (RT-PCR) and PCR-enzyme-linked immunosorbent assay (ELISA) were used to detect hTERT mRNA expression and telomerase activity before and after apoptosis. RESULTS: Oridonin could inhibit the growth of BEL-7402 cells and cause apoptosis significantly. The suppression was in both time-dependent and dose-dependent manner. Marked morphological changes in cell apoptosis including condensation of chromatin and nuclear fragmentation were observed clearly by Hoechst 33258 stain especially after the cells were treated 48-60 h by oridonin. The expression of hTERT mRNA as well as activity of telomerase decreased concurrently by treatment with oridonin in BEL-7402 cells. CONCLUSION: Oridonin has apparent antiproliferation and apoptosis-inducing effects on BEL-7402 cells in vitro, downregulation of the hTERT mRNA expression and decreasing the telomerase activity of BEL-7402 cells may be one of its important anti-hepatocellular carcinoma mechanisms.
7.Intravascular NK/T-cell lymphoma firstly manifesting as gastrointestinal tract symptom and complicated by skin lesions: a case report

A 51-year-old man presented with a 4-month history of upper abdominal distending pain and 1-month history of cutaneous nodules and plaques on the neck, trunk and bilateral thighs. The patient underwent many gastrointestinal tract examinations in several local hospitals, and symptomatic treatment did not work. The biopsy of nodules on the abdomen revealed medium- to large-sized atypical lymphoid cells within numerous small vessels in lower dermis and subcutaneous fat tissue. Additionally, the atypical cells were present exclusively within vascular lumina. Immunohistochemical labeling showed the reactivity of neoplastic cells to CD2, CD99, CD3ε, CD43, CD56, Epstein-Barr virus-encoded small nuclear RNAs (EBER), and cytotoxic proteins such as T-cell intracellular antigen-1 (TIA-1) and perforin, but not to CD4, CD8, CD20, CD79a,CD30, cytokeratin (CK), S100, or CD68. The endothelial cells lining the involved vessels exhibited the reactivity to CD31 and CD34. Based on the above findings, the patient was diagnosed with intravascular NK/T-cell lymphoma firstly manifesting as gastrointestinal tract symptom and complicated by skin lesions. Following combined chemotherapy with cyclophosphamide, daunorubicin, vincristine, prednisone and etoposide, the patient experienced a quick and satisfactory recovery and the follow-up still continued.
8.Wiltse paraspinal muscle splitting approach for treatment of L5-S1 spondylolisthesis

Forty three patients with L5-S1 spondylolisthesis undergoing surgical treatment from April 2012 to November 2014 were included for analysis,including 20 cases received transforaminal lumbar interbody fusion (TLIF group) and 23 cased received posterior lumbar interbody fusion (PLIF group).The incision length,operative time were shorter and blood loss was less in TLIF group than those in PLIF group [(9.6±0.9) vs.(16.1±1.5) cm,(125.6±13.0) vs.(156.4±11.8) minand (218.7±22.5)ml vs.(326.5 ±20.1) ml,respectively,all P =0.000].There was no statistical difference in the S1 pedicle screw (S1PS) insertion point between two approaches[(29.4 ± 1.9) vs.(28.5 ± 1.0) mm,P =0.069],but the distance from the midline to the lateral edge of the screw (12.9 ±3.6) mm,S1PS angle (23.3 ±2.1) ° and length of S1PS length with the sacral body (40.9 ± 2.6) mm in the TLIF group were better than those in PLIF group (P =0.000).Our results demonstrate that the paraspinal muscle approach for the treatment of L5-S1 spondylolisthesis may be superior with less trauma,better functional recovery and stable screw placement.
9.Clinical applications of breast imaging reporting and data system

Breast imaging reporting and data system (BI-RADS) is a standardized system for reporting breast pathology covered mammography,ultrasound and MRI.BI-RADS improves the standardization in interpretation of medical imaging and reduces the confusion of breast imaging interpretation.It is a widely accepted risk assessment and quality assurance tool in mammography,ultrasound and MRI.The development history,clinical applications,limitations of BI-RADS,as well as the clinical applications combined with other imaging techniques were reviewed in this article.
10.Influence of laparoscopic sleeve gastrectomy on nutritional status in patients with obesity

Objective To assess the influence of laparoscopic sleeve gastrectomy (LSG) on the nutritional status in patients with obesity.Methods From August 2014 to July 2015, 24 obese patients underwent LSG in Dongfang Hospital Affiliated to Tongji University.Weight and nutritional status of these patients were measured pre-operatively and 3, 6, and 12 months after surgery.Results The mean body weight of the 24 obesity patients was (81.94±21.39) kg, (78.83±24.49) kg, (62.67±10.79) kg 3, 6, and 12 months after surgery, respectively, which were significantly lower than the preoperative body weight [(99.02±23.92)kg] (P=0.004).The serum levels of albumin, hemoglobin, and trace elements (including calcium, iron, zinc, magnesium, and copper) showed no significant change after surgery (all P>0.05).Plasma vitamin D[25(OH)D] increased significantly from (13.1±4.1)μg/L preoperatively to (19.8±5.3)μg/L 12 months after surgery (P=0.031).Folic acid was (6.2±3.9)μg/L before LSG and significantly increased to (14.2±9.2) μg/L 3 months later surgery (P=0.009);the folic acid level gradually decreased 6 and 12 months after LSG but was still higher than the preoperative level.Conclusion Vitamin D deficiency is present in obese patients before LSG.After surgery, while the serum albumin and trace elements have no obvious change, the vitamin D and folic acid levels remarkably increase.
