1.Infection situations of fungi and trichomonas and changes of cell counts in patients with non-bacterial vaginosis

Objective To explore the infection situation of fungi and trichomonas and the changes of pus cell and epithelial cell counts in patients with non-bacterial vaginosis in Lanzhou .Methods The pus and epithelial cell counts of vaginal secretion samples from patients with vaginitis were detected .Fungi and trichomoniasis were checked under high power microscope ,and the samples were defined positive if fungal spores or pseudohyphae were found .Results In the 4 404 cases of patients with vaginitis ,the total rate of fungi and trichomoniasis infection was 28 .63% (1 261/4 404) .The infection rate for fungi was 25 .39% ,and for trichomonad was 3 .25% ,respectively .The dual infection rate of fungi and trichomonad was 0 .54% .Compared with ≤20 age group ,the counts of pus cells and epithelial cells in other age groups were significantly different(P<0 .05) .Conclusion Fungi were the primary in-fection etiology of non-bacterial vaginosis in Lanzhou .
2.Influence of glutamine on barrier function of heat-stressed intestinal epithelial Caco-2 cells

Objective To investigate the effect of Glutamine (Gln) on heat stress-induced dysfunction of intestinal epithelial barrier. Methods Human intestinal epithelial Caco-2 cells were pre-incubated with Gln for 24h and then exposed to heat 43℃ for 1h. Cell counting kit-8 (CCK-8) was used to detect the cellular proliferation with various concentrations of Gln and choose an optimum concentration for subsequent experiments. The barrier integrity was measured by transepithelial electrical resistance (TEER) and horseradish peroxidase (HRP) permeability. Levels of tight junction protein occludin and ZO-1 were analyzed by Western blotting. Cytoskeleton using Coomassie blue staining was observed by microscopy. Results At 0.7mmol/L concentration, Gln showed the most effective cell proliferation compared with other concentration groups (P<0.05). Therefore, 0.7mmol/L Gln was used as effective concentration in following experiments. Gln attenuated the TEER decrease and impairment of intestinal permeability induced by heat exposure compared with 43℃ group (P<0.01). The expressions of occludin and ZO-1 were significantly elevated by pretreatment with Gln. The distortion of cytoskeleton was also effectively prevented. Conclusion 0.7mmol/L Gln is potentially beneficial for protecting against heat stress-induced permeability dysfunction and epithelial barrier damage.
3.Diagnostic value of SUVmax/ADCmin obtained by 11 C-choline PET/MR in patients with prostate cancer

Objective To evaluate the value of SUVmax/ADCmin obtained by 11 C?choline PET/MR imaging in the differential diagnosis of benign and malignant prostate disease. Methods Suspicious prostate cancer patients who underwent PET/MR before transperineal prostate biopsy in People′s Liberation Army General Hospital from April 2014 to January 2015 were observed. Prostate gland was divided into five dis?tricts. A ROI with the diameter of 1 cm was drawn at the highest radioactivity concentration and/or lowest ADC signal area in each district, and SUVmax , ADCmin and SUVmax/ADCmin were then calculated. Taking the pathological results as the gold standard, malignant and benign prostate disease districts were separated. Pearson correlation analysis and two?sample t test were used for statistical analysis. Results Biopsy pathol?ogy showed 9 cases were malignant ( 25 prostate cancer districts, and 20 benign prostate disease districts) , and 6 cases were benign (30 benign prostate disease districts). The SUVmax had a negative correlation with ADCmin( r=-0.521, P<0?05) . The SUVmax/ADCmin of benign and malignant districts was significantly dif?ferent(5.67±3.03 and 13?76±7.13; t=6.9, P<0.01). The ROC AUC for ADCmin, SUVmax and SUVmax/ADCmin were 0?880, 0?809 and 0.898. Using SUVmax/ADCmin of 7.78 as a cutoff, the sensitivity, specificity and accuracy of 11 C?choline PET/MR imaging for differentiating the benign and malignant prostate lesions were 84.0%(21/25), 86.0%(43/50) and 85.3%(64/75), respectively. Conclusions Simultaneous 11C?choline PET/MR could be used for detecting prostate cancer. SUVmax/ADCmin may serve as an effective diag?nostic indicator for differentiating benign and malignant prostate lesions.
4.Diagnosis of brain tumor with multi-modality imaging of 11C-MET PET combined with functional MRI

Objective To evaluate the diagnostic efficiency of the parameters obtained from PET/MR in brain tumors.Methods In this prospective study,28 patients (21 males,7 females,age range: 6-82 years) with clinical suspicion of brain tumor from November 2012 to September 2015 underwent PET/MR multi-modality imaging.The examination of PET/MR included 11C-MET PET and multiple MR sequences.The qualities of images were estimated firstly.The ROC curve and the accuracy of SUVmax,ADCmean,rCBF and NAA/(Cho+Cre) ratio were calculated.The pathology or final clinical diagnosis was taken as the standard.The diagnostic efficiency of the multi-modality imaging was determined based on the cutoff values of the four parameters.Two-sample t test was used to analyze the differences of parameters between glioma group and inflammatory group.Results SUVmax,ADCmean,rCBF and NAA/(Cho+Cre) ratio were validated to be effective parameters in diagnosing brain tumors with the diagnostic accuracy of 89.3%(25/28),82.1%(23/28),78.6%(22/28) and 75.0%(21/28),respectively.The SUVmax exhibited the highest diagnostic accuracy,while the diagnostic efficiency of the combination of four parameters was superior to the separate parameter.The values of SUVmax,rCBF and NAA/(Cho+Cre) ratio were significantly different between glioma group(n=10) and inflammatory group(n=11;t values:-2.31,-3.11,-2.77,all P<0.05).Conclusions PET/MR can provide a one-stop examination with multi-modality imaging of brain.The obtained parameters SUVmax,ADCmean,rCBF and NAA/(Cho+Cre) ratio,especially their combination,have effective diagnostic values on brain tumor.
5.Principal Items Respond to Activities of Daily Living for Persons with Physical Disability

Objective To investigate the principal indicators of self-care ability in activities of daily living for the persons with physical disability. Methods Persons with physical disability were asked to select the top 3 items out of 27 items of activities of daily living in 3 levels. Results A total of 1960 questionnaires were send out, and 1862 were returned. For all the subjects, the items related with personal hygiene, such as toileting, self-cleaning and bathing, were selected 899 times (16.1%). The items related with personal health, as visiting community clinics and community exercising, were selected 570 times (10.2%). The items related with social interaction, as making a telephone and chatting, were selected 500 times (9.0%). For the persons with physical disability of first grade, the major items most selected were eating, entertaining, self-cleaning and transferring; and self-cleaning and housework for those of second grade; self-cleaning, community activities and housework for those of third grade; and community and interaction for those of fourth grade. Conclusion The persons with physical disability mostly focused on the activities related with personal hygiene, health and social interaction, and varied with the severity of disability, from self-cleaning to housework and social participation.
6.To compare the effects of false hernia sac exclusion,barbed suture and circular suture in transperitoneal preperitoneal laparoscopic hernia repair for direct hernia

Objective To investigate the difference of surgical effect of exclusion,simple continuous suture and circular suture suspension in TAPP for the treatment of false hernia sac in patients with direct inguinal hernia.Method From May 2020 to May 2022,120 patients diagnosed with direct inguinal hernia and treated with TAPP in our hospital were retrospectively.The false hernia sacs were divided into three groups according to different methods of treatment patients treated with false hernia sac exclusion were included in group A,those treated with simple continuous suture were included in group B,and those treated with circular suture suspension were included in group C.There were 40 patients in each group.The perioperative indicators(operation time,intraoperative blood loss,postoperative hospital stay,hospitalization cost)and postoperative effects(chronic pain,seroma,incision or mesh infection,foreign body traction feeling)were compared among the three groups.Results All 120 patients successfully completed TAPP surgery.There was no significant difference in general condition,intraoperative blood loss,postoperative hospital stay,wound or mesh infection and chronic pain among the three groups(P>0.05).The operation time of group B and C was longer than that of group A,and the incidence of seroma was significantly lower than that of group A,the difference was statistically significant(P<0.05).The incidence of foreign body traction in group A and group C was lower than that in group B,and the difference was statistically significant(P<0.05).The hospitalization cost of group B and group C was lower than that of group A,with statistically significant difference(P<0.05).Conclusion In clinical practice,direct hernia and false hernia sac often need to be treated.In direct hernia TAPP operation,simple continuous suture method and circular suture suspension method have the effect of improving the condition of the false hernia sac,but in terms of economy and postoperative effect,the circular suture suspension method can benefit patients more.
7.Evaluation of carotid artery plaque stability with ¹⁸F-FDG PET/CT and contrast-enhanced ultrasonography.

OBJECTIVETo assess the value of contrast-enhanced ultrasonography (CEUS) and fluorodeoxyglucose positron emission tomography-computed tomography (¹⁸F-FDG PET/CT) in evaluating the stability of carotid atherosclerosis.
METHODSSeventeen patients with 21 carotid artery plaques received examinations with CEUS. According to the nature of the plaques, the patients were divided into soft and mixed plaque group and hard and calcified plaque group. The maximal enhancement intensity of the plaques (I(MAX)) and maximal plaque density (D(MAX)) were measured to quantify the neovasculature. The patients also underwent concurrent ¹⁸F-FDG PET/CT, and ¹⁸F-FDG uptake was quantified by the mean standard uptake values (SUV(mean)), an index reflecting the inflammatory activity in the plaque. The findings in CEUS and PET/CT were comparatively analyzed for these cases.
RESULTSThe D(MAX) of the plaque in soft and mixed plaque group was significantly greater than that in hard and calcified plaque group (4.26±3.65 vs 1.41±1.47, P<0.05); the I(MAX) was also greater in the former group, but this difference was not statistically significant (26.83±19.61 vs 24.73±29.85, P=0.869). The soft and mixed plaques tended to have higher SUVmean than the hard and calcified plaques (1.70±0.45 vs 1.47±0.12, P=0.099). The values of I(MAX) and D(MAX) were not found to correlate to SUV(mean) in these patients.
CONCLUSIONCEUS can sensitively show the status of neovascularization within the carotid atherosclerosis plaques, and PET/CT reflects the inflammatory activity in the plaques. The combination of these two imaging modalities allows the evaluation of plaque stability in terms of neoangiogenesis and inflammatory activity.

8.A survey of human resources of the Anesthesiology in China: investigation of reform direction of human resources allocation of Chinese medical and health system based on the current status of hu-man resources of the Anesthesiology

Objective To survey the current status of human resources of the Anesthesiology in Chi-na and investigate the reform direction of human resources allocation of Chinese medical and health system. Methods Chinese Society of Anesthesiology organized the investigation from March 1, 2015 to June 30, 2015.All the hospitals in which clinical anesthesia was carried out were investigated only in mainland Chi-na.The questionnaires contained hospital general information, anesthesia-related information and surgery-re-lated information.Results A total of 16 280 questionnaires were released and 14 076 questionnaires (86.462%)were retrieved.There were 13 489 questionnaires(82.856%)identified as qualified anesthesia-related information ones and 7 026 questionnaires(43.157%)identified as qualified surgery-related informa-tion ones.Qualified anesthesia-related information questionnaires showed that there were 77 926 anesthesiolo-gists in total in China, 27 660 000 patients in whom anesthesia was completed inside operating room(OR)and 11 470 000 cases outside OR, one attending anesthesiologist completed anesthesia in 634 cases inside OR and in 263 cases outside OR on average; the number of per capita anesthesiologists was 5.7 per 100 000 pop-ulation in 2014.The number of per capita physicians per 10 000 population(r=0.735, P<0.05), the num-ber of anesthesiologists per 100 000 population(r=0.537, P<0.05)and the number of per capita patients requiring anesthesia per 100 000 population(r=0.571, P<0.05)were all positively correlated with GDP per capita in various provinces and cities nationwide and autonomous regions.The area having the highest num-ber of per capita anesthesiologists per 10 000 and per 100 000 population was Beijing, the area having the lowest number was Tibet; the area having the highest number of per capita patients requiring anesthesia per 100 000 population was Beijing, the area having the lowest number was Shanxi. The qualified surgery-related information questionnaires showed that the ratio of attending surgeons to attending anesthesiologists was 7.5:1.0 in China in 2014.Conclusion Although China has a huge number of anesthesiologists, the Anesthesiolo-gy is still shortage of anesthesiologists; the imbalanced distribution of human resources exists in different provinces and cities, in hospitals of different scales and between different medical specialties, and economics is one of the important influencing factors.
9.Research progress in late-onset group B Streptococcus infection in infants and its prevention

Group B Streptococcus(GBS)is a major member of the Gram-positive Streptococcus family,which occupies a dominant position in the infection spectrum of newborns and young infants.GBS infections occurring 0?6 days after birth are called GBS early-onset disease(GBS-EOD),and those occurring 7?89 days are called GBS late-onset disease(GBS-LOD).With the use of intrapartum antibiotic prophylaxis,the incidence of neonatal GBS-EOD has declined without a notable impact on GBS-LOD,which brings great pressure and challenge to clinical diagnosis and nursing work.However,the specific mechanisms by which GBS transmission triggers illness in newborns and young infants are not fully elucidated.Some factors,such as maternal GBS colonization,maternal characteristics(age,weight and ethnicity),preterm birth,GBS infection in multiple births,and exposure to human immunodeficiency virus,have been identified as risk factors for GBS-LOD.To prevent maternal and infant GBS infection,some measures,such as vaccination of pregnant women,gut microbiota regulation and lactoferrin supplementation,are developing.In addition,strengthening maternal health care and health education,along with some other conventional infection control strategies(including enhancing hand hygiene awareness among caregivers and making good disinfection of the environment and equipment)is also effective in the prevention of GBS-LOD.This review elaborates the prevalence,transmission,risk factors and prevention of GBS-LOD in infants,aiming to improve the understanding and clinical practice ability of the medical staff.
10.Definition and function identification of nucleus export signal of BRD7.

OBJECTIVE:
To localize and define the region of nucleus export signal (NES) on BRD7, and determine the role of this region in nucleus export of the external protein.
METHODS:
Based on an in vitro expressed model of green fluorescence protein (GFP), we performed DNA walking analysis to set BRD7 into several sections according to the structural characteristics of BRD7, investigated the effect of different sections of BRD7 on nucleus export of GFP, defined the region of nucleus export signal sequence of BRD7, and further ascertained the content of amino acids in BRD7 and potential localization of BRD7 NES by bioinformatics.
RESULTS:
B7C1 fragments ranged from aa219 to aa450 in BRD7 were found to target the external protein GFP into the cytoplasm detected by GFP direct fluorescence, which could be inhibited by NES inhibitor Leptomycin B (LMB). This region was rich in hydrophobic amino acid residues but no typical NES with characteristics of leucine-rich sequence by bioinformatics.
CONCLUSION
The region from aa219 to aa450 is primarily defined as an atypical NES in BRD7.
