1.Construction of pcDNA3-Endo eukaryon expression plasmid and angiogenesis inhibition in vitro

BACKGROUND:The eradication therapy of glioma is the major problem, and anti-angiogenesis therapy is a potential treatment of glioma.
OBJECTIVE:To confirm the inhibiting effect of endostatin on angiogenesis in vitro, and to lay the foundation in inhibiting the growth of tumor by endostatin in the future.
METHODS:Endostatin mRNA was extracted from the liver of Wistar rats by Trizol and endostatin cDNA was synthesized by RT-PCR. Endostatin cDNA and pcDNA3 were connected and pcDNA3-Endo recombined plasmid was constructed successful y. The recombinant pcDNA3-Endo was transfected into bone marrow mesenchymal stem cells by Lipofectamine. The expression of endostatin was identified by RT-PCR and western blot analysis. Endostatin proteinum activity was detected by ECV-304 cellproliferation inhibition experiment using MTT assay. The in vitro experiments were divided into four groups:recombinant plasmid group, vector plasmid group, liposome control group and blank control group.
RESULTS AND CONCLUSION:PcDNA3-Endo eukaryon expression plasmid was constructed successful y. Endostatin gene can be transcribed and expressed effectively in vitro by pcDNA3-Endo plasmid. The growth of ECV-304 cellwas inhibited obviously by pcDNA3-Endo. The growth of vascular endothelial cells can be inhibited obviously by endostatin gene.
2.Clinical efficacies of the conservative and surgical treatment of grade Ⅲ and Ⅳ traumatic hepatic rapture

Objective To investigate the clinical efficacies of the conservative and surgical treatment of grade m and Ⅳ traumatic hepatic rapture.Methods The clinical data of 77 patients with traumatic hepatic rapture were analyzed.All the patients were admitted to the Fuzhou General Hospital of Nanjing Military Command from January 2004 to June 2014.According to the Organ Injury Scale grading system of the American Association for the Surgery of Trauma (AAST-OIS) and the Becker classification,there were 44 patients and 33 patients had grade Ⅲ and grade Ⅳ traumatic hepatic rapture,respectively.The vital signs,condition of the wound and the resuhs of imaging examination were comprehensively analyzed,and the conservative and surgical treatment methods were selected accordingly.Patients were followed up via outpatient examination or telephone interview till September 2014.Results There were 19 patients received conservative treatment and 25 received surgical treatment among the 44 patients with grade m traumatic hepatic rapture,and 5 received conservative treatment and 28 received surgical treatment among the 33 patients with type Ⅳ traumatic hepatic rapture.Thirty-one patients received suture of the liver,18 received partial hepatectomy and 4 received damage control surgery.All the 77 patients were cured without perioperative death.For patients with grade m traumatic hepatic rapture,the duration of hospital stay were (12 ± 9)days and (20 ± 15)days for patients received conservative and surgical treatment,respectively; and for patients with grade Ⅳ traumatic hepatic rapture,the duration of hospital stay were (17 ± 3) days and (34 ± 25) days for patients received conservative and surgical treatment,respectively.Nineteen patients had complications,including 10 with hepatic abscess,5 with bile leakage (cured by puncture and drainage),2 with pseudoaneurysm (cured by interventional therapy with digital subtraction angiography),1 with adhesive intestinal obstruction and delayed hemorrhage (cured by operation for 2 times).Seventy-one patients were followed up,with the rate of 92.2% (71/77).The median time of follow-up was 7 months (range,3-15 months).Only 1 patient was complicated with hepatic abscess at postoperative month 13,and was cured by puncture and drainage,and the other patients were live and well.Conclusions The success rate of operation for patients with grade m traumatic hepatic rapture is relatively high with short hospital stay and quick recovery of patients.The indication of conservative treatment for patients with grade Ⅳ traumatic hepatic rapture should be strictly controlled,and the surgical treatment is the main means of therapy.Hepatic abscess,bile leakage and pseudoaneurysm are the common complications for patients with traumatic hepatic rapture.
3.Application of intracavity double pipe suction in the repair of duodenal rupture

Objective To investigate the effects of intracavity double pipe suction in the duodenal rupture repair.Methods The clinical data of 56 patients with duodenal rupture repair who were admitted to Fuzhou General Hospital from January 2003 to January 2014 were retrospectively analyzed.Thirty-one patients and 25 patients received the simple drainage (simple drainage group) and intracavity double pipe suction (intracavity double pipe suction group) for duodenal rupture repair.Quantitative data were presented by $ ± s,repeated measures analysis of variance and the t-test were used to evaluate quantitative data,respectively.Count data were analyzed using Chi-square test or Fisher's exact test.Results Volume of drainage of patients in the intracavity double pipe suction group at day 1,2,3,4,5,6,7 were (220 ± 54) mL,(284 ± 65) mL,(368 ± 35) mL,(413 ± 41) mL,(454 ± 62) mL,(714 ± 96) mL and (852 ± 121) mL,compared with (102 ± 30) mL,(124 ± 29)mL,(186 ±26)mL,(110 ±21)mL,(167 ±31)mL,(193 ±35)mL and (182 ±44)mL in the simple drainage group,with a significant difference between the 2 groups (F =65.214,P < 0.05).The volumes of drainage of the 2 patterns were compared at postoperative day 1 to 7,with a significant difference (t =9.532,11.624,13.421,15.257,14.147,18.311,20.135,P <0.05).The incidence of duodenal fistula,intraperitoneal infection and wound infection in the simple drainage group were 29.0% (9/31),41.9% (13/31) and 51.6% (16/31),compared with 4.0% (1/25),12.0% (3/25) and 16.0% (4/25) in the intracavity double pipe suction group,showing a significant difference between the 2 groups (x2 =4.460,6.077,7.645,P < 0.05).The incidence of pancreatic fistula and pulmonary infection in the sample drainage group and intracavity double pipe suction group were 16.1% (5/31) and 29.0% (9/31),8.0% (2/25) and 12.0% (3/25),with no significant difference between the 2 groups (x2 =0.836,2.385,P > 0.05).The duration of hospital stay and hospital expenses in the simple drainage group and the intracavity double pipe suction group were (30 ± 14) days and(12 ± 6) x 104 yuan,(21 ± 7) days and (7 ± 5) x 104 yuan,respectively,with significant difference between the 2 groups (t =3.161,2.913,P < 0.05).Conclusion The intracavity double pipe suction for duodenal rupture repair is simple and effective for significantly improving the prognosis of patients and reducing the duration of hospital stay and hospital expenses.
4.Factors affecting the risk of non-proliferative diabetic retinopathy among patients with type 2 diabetic mellitus

Objective:
To investigate the factors affecting the development of non-proliferative diabetic retinopathy (NPDR) among patients with type 2 diabetic mellitus (T2DM), so as to provide insights into manangement of NPDR.
Methods:
T2DM patients without obvious eye discomfort at ages of 18 years and older admitted to Department of Endocrinology, Jining No. 1 People's Hospital during the period from December 2019 to May 2021 were enrolled. Participants' demographics, smoking, alcohol consumption, medical history of diabetes and use of medicines were collected, and the height, weight and blood pressure were measured. The levels of glycosylated hemoglobin (HbA1c), fasting blood glucose, blood C-peptide, lipid and creatinine were tested, and retinopathy was examined with a non-mydriatic fundus camera. The factors affecting the development of NPDR were identified among T2DM patients using a multivariable logistic regression model.
Results:
A total of 486 T2DM patients were enrolled, including 354 men (72.84%), with a median age of 48.00 (15.25) years, and median diabetes duration of 35.00 (104.25) months. The prevalence of NPDR was 19.34% among the participants. multivariable logistic regression analysis identified an educational level of senior high school and above (OR=0.546, 95%CI: 0.325-0.918), duration of diabetes (OR=1.008, 95%CI: 1.005-1.012), HbA1c (OR=1.183, 95%CI: 1.034-1.354) and use of non-sulfonylurea insulin secretagogues (OR=1.859, 95%CI: 1.082-3.196) as factors affecting the risk of NPDR among T2DM patients.
Conclusion
A high risk of NPDR is found among T2DM patients with a low educational level, long duration of diabetes, poor HbA1c control and use of non-sulfonylurea insulin secretagogues.
5.Application of PDCA Cycle in Management of Teaching Quality of Rehabilitation Nurse

Objective To explore the effectiveness of plan-do-check-action (PDCA) cycle in the management of teaching quality of reha-bilitation nurse. Methods From March, 2014 to March, 2015, 108 nurses were recruited in three terms. PDCA cycle was applied in the man-agement. Problems existing in their nursing work and nursing continuing education were investigated and analyzed, then improved in the next term. Results The teaching satisfactions were 86.12%, 90.13%, and 93.32%, respectively. The scores of theoretical examination, opera-tion examination and Power Point report gradually increased in nurses of three terms. Conclusion The application of PDCA cycle is effec-tive in the management of teaching quality of rehabilitation nurse.
6.Analysis with capability of responding to emerging infectious diseases among medical university students in Chongqing city

Objective To take Ebola virus disease (EVD) as an example and analyze the capability of responding to emerging infectious diseases among medical university students in Chongqing city.Methods Medical university students of 2 094 in Chongqing were selected and investigated by cluster sampling.Questionnaire content includes social demographic information,knowledge,attitude,practice (KAP) and related factors about EVD.The variables of the respondents were described using mean and standard deviation for continuous variables and frequency distributions for categorical variables.Influencing factors were analyzed by using multiple-linear regression model.Results The average scores of KAP regarding EVD were 9.51 ±-3.97,5.93± 1.85,3.35 ±1.43,respectively.Multiple-linear regression analysis showed that the influencing factors of KAP regarding EVD were gender (x2 =0.773,P=0.000),residence (x2 =0.886,P=0.014) and health habits (x2 =0.316,P=0.008);gender (x2 =0.474,P=0.000),grade (x2 =0.118,P=0.024),residence (x2 =0.401,P=0.016) and health habits (x2 =0.307,P=0.000);gender (x2 =0.223,P=0.001),major (x2 =0.152,P=0.000) and health habits (x2 =0.231,P=0.000).Conclusion The capability of responding to emerging infectious diseases is not optimistic among medical university students.Medical universities should perform effective intervention according to the characters of different clusters.
7.Diagnosis and treatment of grade Ⅴ liver trauma combined with hilar bile duct transection injury

According to the Organ Injury Scale Grading System of the American Association for the Surgery of Trauma (AAST-OIS),grade Ⅴ liver trauma is always complicated with retrohepatic inferior vena cava injury and less bile duct injury,and it is extremely severe and difficult to be treated.Timely and fast judgment,emergent exploration and effective repair of the injured bile duct are the key points for the treatment of bile duct injury.One patient with grade Ⅴ liver trauma combined with hilar bile duct transection injury was admitted to the Fuzhou General Hospital of Nanjing Military Command on August 30,2013.The rupture of left and right liver junction was detected by preoperative multidisciplinary consultation and emergency open surgery at admission hour 4.There was left and right hepatic duct bifurcation rupture at the first hepatic hilum.Non-functional liver tissues were excised.Breakage left and middle hepatic vein were sutured by polymer suture line.Liver traumatic bleeding and bile duct were sutured and ligatured individually.Left and right hepatic duct laceration was sutured by 6-0 PDS suture line.A hole in the stomach wall was opened fist,and then most part of the gastric contents was removed and the gastric wall was reparied by stapler.Patient received the postoperative symptomatic treatment with gradual recovery,and was discharged from hospital at admission day 26.The patient was readmitted to the hospital at 31 days of discharge due to outflow of purulent fluid from abdominal cavity drainage tube,and was treated by ceftriaxone sodium and tazobactam sodium according to the results of drug sensitive test and continuous peritoneal lavage.The abdominal cavity drainage tube and left and right hepatic duct drainage tube were removed at postoperative day 83.The patient was discharged from hospital at readmission day 28,and was followed up till December 2014 with good recovery and without complication.
8.Transplacental transport mechanisms of drugs for transplacental treatment of fetal tachyarrhythmia of MDCKII/MDCKII-BCRP cell line.

To study the transport mechanisms of drugs for transplacental treatment of fetal tachyarrhythmia, MDCKII-BCRP and MDCKII cell models was used. MDCKII-BCRP and MDCKII cell monolayer model was used to investigate the bi-direction transport of sotalol, propranolol, propafenone, procainamide and flecainide. Drug concentrations were measured by HPLC-UV or chemiluminescence. The apparent permeability coefficient (P(app)), efflux rate (R(E)) and net efflux rate (R(net)) were calculated. Drugs with R(net) greater than 1.5 were further investigated using cellular accumulation experiments with or without a BCRP inhibitor. The R(net) of sotalol, propranolol, propafenone and procainamide were less than 1.5, while R(net) of flecainide with concentrations of 20 and 5 μmol x L(-1) were 1.6 and 1.9, respectively. The results showed that the transport of flecainide on MDCKII-BCRP cell monolayer could be mediated by BCRP; and the affinity increased when the concentration of flecainide decreased. Cellular accumulation experiments further suggested that accumulation of flecainide in MDCKII-BCRP cells was significantly lower than that in MDCKII cells in a concentration-dependent manner. BCRP inhibitor quercetin (50 μmol x L(-1)) significantly increased the accumulation of flecainide in MDCKII-BCRP cells (P < 0.05). Our preliminary data showed that flecainide but not sotalol, propranolol, propafenone or procainamide can be a substrate of BCRP. Thus the effect of flecainide may be affected by the BCRP in the maternal placental trophoblast membrane layer when treating fetal tachyarrhythmia.
9.Correlation Analysis on Nurse’ s Organizational Commitment and Their Professional Identity

Objective:To explore the correlation of nurses ’ organizational commitment and their professional identity status.Methods:Using Chinese employee organizational commitment scale and nurses professional identity evaluation scale questionnaire survey was conducted in 400 cases of clinical nurses , nurse compare different characteristics of organi-zational commitment and professional identity level , and USES the Pearson correlation analysis to investigate the correla-tion of both.ResultsNurses organizational commitment and professional identity total score , respectively (2.16-0.45) and (3.49 +0.84);Different cultural degree , job title, working years and the way of hiring nurses ’ professional identi-ty score comparison, the difference was statistically significant (P <0.05);Different ways of title, working years, and hire nurses’ organizational commitment score comparison , the difference was statistically significant (P <0.05);Pear-son correlation analysis showed that organizational commitment and the scores of each dimension career cognitive apprais -al, professional social skills , professional setback coping , social support and total scores of professional identity were sig -nificantly positive correlation (P <0.05).Conclusions:The nurse was significantly positively related to organizational commitment and their professional identity , and both have big room to improve .
10.Selection of Ginkgo biloba cell line for suspension culture and studies on stability of ginkgolide B in subculture

Object To select the cell line of Ginkgo biloba L that may produce high yield of ginkgolide B (GB) in suspension culture and to study the stability of GB in subculture Methods Calculus was induced with stem, root and leaf of selected elite species and high yield suspension cell lines chosen by hypoxia stress Results Seven suspension cell lines with improved yield of GB were obtained Among which cell line MH 3 gave a 3 71 fold increase of cellular bioproduct with an increased content up to 302 ?g/g DW after culturing for 18 d, a leading record country wide In shaking bottle culture, it showed a consistent yield of GB with a content of 291 ?g/g DW in 6 successive subcultures, coefficient of variation=0 131 Conclusion The suspension cell line selected by hypoxia stress gave a higher yield and stability in successive transfer culture
