1.A comparison between external fixation with external fixation apparatus in entire period with external fixation apparatus and plaster in different period for treatment of open fractures of tibia and fibula

Objective To study the results of treatment of open fracture of tibia and fibula by external fixation with unilateral multifunction external fixation apparatus(UMEFA) and plaster in different period. Methods 89 cases with open fracture of tibia and fibula had got reduction and external fixation with UMEFA and then divided into two groups, group A of 53 cases followed by external fixation with UMEFA, 8 cases of them were forced to remove UMEFA because of complications, the other 45 cases followed by external fixation with UMEFA till bone union, group B of 36 cases were removed UMEFA and changed with plaster when the skin was healed, the sweal was eliminated and the bone was steady. Compared and analyzed the results by complication, healing time and quality.Results In group B, there was less complications(P
2.Effects of ginkgo biloba tablets plus combined with routine therapy on HPA axis in patients with steroid-dependent asthma

Objective:To investigate the effects of ginkgo biloba tablets plus relevant mechanism on HPA axis in patients with steroid-dependent asthma.Methods:80 patients with steroid-dependent asthma were divided into control group and experimental group at random,40 cases in every group.The control group were treated with pulmicort respules(1 mg/d) ,patients in the experiment group were given ginkgo biloba tablets in addition to routine treatment(120 mg/d).The therapeutic courses were both 12 weeks.The effective rate of reducing hormone,lung function,the plasma concentrations of ACTH and COR and the levels of PBMC GCR were deter mined before and after treatment.Results:The effective rate of reducing hormone was 89.47%in experimental group,which was 66.67%in control group.There were significant difference at the tows groups(P<0.01).There were no significant difference at lung function,the plasma concentrations of ACTH and COR and the levels of PBMC GCR at pre-treatment( P>0.05).After treatment,the lung function, the levels of plasma concentrations of COR and the levels of PBMC GCR were increased significantly than control group( P<0.01,P<0.05),the levels of plasma concentrations of ACTH was decreased significantly than control group(P<0.01).There was a significant positive correlation between the value of FEV1/FVC and the plasma concentrations of COR,the levels of PBMC GCR( r=0.724,0.632, P<0.01).There was a significant negaitive correlation between the value of FEV1/FVC and the plasma concentrations of ACTH( r=-0.668,P<0.01).Conclusion:Ginkgo biloba tablets combined with routine treatment therapy can increase the effective rate of reducing hormone in patients with steroid-dependent asthma,which can improve the lung function maybe by regulate the HPA axis and upgrade the levels of GCR.
3.Ten-year case review of solid-pseudopapillary neoplasm of pancreas

Objective To summarize the methods of diagnosing and treating solid-pseudopapillary neoplasm of pancreas SPN so as to provide reference for its early clinical diagnosis.Methods We collected the clinical data of 62 patients with pathologically confirmed diagnosis of SPN treated in the First Affiliated Hospital of Xi'an Jiaotong University between Feb.2004 and Sep.2014.Then we retroactively analyzed and summarized the clinical characteristics,diagnosis and treatment of the disease.Results Among the 62 patients,55 were female and 7 were male;the mean age was (31.58±12.67) years old.The clinical features showed no specificity and the tumor was mostly detected upon physical check-up.The tumor was seen to be located mostly in the body and tail of the pancreas,and the average maximum diameter was (7.81±3.54)cm.We did not find obvious abnormality in routine pre-operative blood test results or liver and kidney functions.The imageological examination indicated tumor occupation in the pancreas.All the patients underwent surgical resection with no complications or death and had a good recovery after operation.Pathological diagnosis after operation was SPN without metastasis in lymph modes.Until the last time we followed up all the patients,we found relapse in two patients (3.2%).Conclusion SPN is a tumor that tends to affect young and middle-aged females.The malignancy grade of SPN is low,and patients with this disease show no specificity in clinical manifestation.The imageological examination is of vital importance in diagnosing SPN and surgical resection is an effective way to treat SPN.
4.Clinical observation of postprandial blood pressure changes in the very elderly with nasal feeding

Objective To observe the changes of postprandial blood pressure and heart rate with two types of nasal feeding in the very elderly and explore the method for decreasing the postprandial hypotension (PPH).Methods Totally 49 patients (aged>80 years) were randomly divided into two groups of continuous versus intermittent nasal feeding.The clinical symptoms and changes in blood pressure and heart rate before and after meals were observed.Results The incidence of PPH was 100.0%(26 cases)in the group of intermittent nasal feeding,and 47.8% (11 cases) in the group of continuous nasal feeding (P<0.01).Postprandial blood pressure in the group of intermittent nasal feeding was decreased significantly as compared with the group of continuous nasal feeding (P <0.05); and the heart rate was accelerated in the group of intermittent nasal feeding compared with continuous nasal feeding (P<0.05).The incidence of serious cardio-cerebral ischemia was 7.7% (2cases)in the group of intermittent nasal feeding.The patients had no clinical symptoms in the group of continuous nasal feeding.Conclusions Continuous nasal feeding for the very elderly with gastrointestinal nutrition can reduce the extent of the falling blood pressure and the incidence of PPH,and avoide cardiocerebrovascular events.
5.Application of needle-knife precut papillotomy in difficult common bile duct cannulation of endoscopic retrograde pancreatic duct radiography

Objective To investigate the effects and safety of needle?knife precut papillotomy in diffi?cult common bile duct cannulation of endoscopic retrograde pancreatic duct radiography ( ERCP ) . Methods The data of 104 cases treated with needle?knife precut papillotomy were retrospectively analyzed. The effect of pre incision and short?term complications were observed,and compared with 1326 cases( conventional group) of con?ventional selective intubation in the same period of ERCP. Results Successful cannulation of the bile duct was achieved in 97 of 104 patients who underwent needle?knife precut papillotomy, the total success rate was a?chieved in 93. 3%. The complications of needle?knife precut papillotomy occurred in 8 patients(7. 7%),inclu?ding mild bleeding in 4 cases,mild pancreatitis in 2 cases and billiary infection in 2 cases. There were 1326 ca?ses conventional selective intubation in the same period of ERCP,and 98 cases(7. 4%) with complication. There was no significant difference in the incidence of complications between the needle knife cut group and the con?ventional selective intubation group(χ2=0. 055,P>0. 05 ) . Conclusion In case of difficult common bile duct cannulation of ERCP,needle?knife precut sphincterotomy surgery can be safely and effectively catch the high ?ERCP the success of treatment,is worthy of wider application.

Harmful substances are produced during the metabolism of alcohol in the human body, which causes liver disease.The mechanism of oxidative stress is the main pathogenesis of alcoholic liver disease, which plays a major role in the occurrence and development of alcoholic liver disease.Alcohol-induced oxidative stress mediated by reactive oxygen species(ROS)/reactive nitrogen species(RNS) brings about liver disease via apoptotic signaling pathway and MAPK signaling pathway, and alleviates liver disease through Nrf2 signaling pathway.In this paper, the studies on the mechanism of oxidative stress in alcoholic liver disease are reviewed.
7.Screening of LRRK2 interactants by yeast 2-hybrid analysis

Objective To isolate and identify the potential binding partners of LRRK2,a gene linked to both dominant familial form and sporadic form of Parkinson's disease,thus to further our knowledge of its function.Methods We used a sequence containing full-length of COR domain and part of ROC and MAPKKK domain as bait.The bait amplified by polymerase chain reaction(PCR) was then cloned into a yeast expression plasmid pGBKT7.After being sequenced and analyzed,pGBKT7-bait was transformed into the yeast strain AH109.Western blot was performed to confirm the expression of pGBKT7-bait in AH109 yeast strain.Then human fetal brain cDNA library was trarnsformed into that yeast strain.which could express pGBKT7-bait fusion protein.The yeast strain which contained pGBKT7-bait and human fetal brain cDNA library was plated on quadruple dropout medium (SD/-Trp/-Leu/-His/-Ade)containing X-a-gal.We retested these positive colonies using 2 independent yeast strains AH109 contained pGBKT7-bait or pGBKT7,respectively.At last,these plasmids isolated from these true positive colonies were analyzed by bioinformatics.Results We obtained 9 true positive colonies,these colonies were sequenced, and we performed sequence Blast in GenBank.Three colonies of the 9 positive colonies were not in open reading-frames.Among other 6 colonies,there were known proteins including spermatid perinuclear RNA-binding protein(STRBP)and BCL2-associated athanogene 5 isoform b(BAG5),as well as unknown proteins including tyrosine phosphatase non-receptor type(PTPN23),1(3)mbt-like 3 isoform b(L3 MBTL3),RALY RNA binding protein-like isoform 1(RALYL),and Homo sapiens mRNA for KIAA1783 protein,partial cds(KIAA 1783).Conclusion True positive colonies of LRRK2 are successfully obtained by the yeast 2-hybrid.Our screened proteins may provide a new research clue for revealing biological functions of LRRK2,pathogenesis of Parkinson's disease,and other neurodegerations.

Aim To establish the rat model of Spleen-Qi deficiency, analyse the metabolic pathways and investigate the connection between the changed urinary metabolites and Spleen-Qi deficiency, in order to explore the potential mechanisms of Spleen-Qi deficiency.Methods With the binding methods of diarrhea induced by bitter and cold, abnormal of starvation and excessive tiredness, the rat Spleen-Qi deficiency model was established.Then the activity of creatine phosphokinase(CPK) was detected.The endogenous metabolites in the urine were detected by NMR, and the data were analyzed with multivariate and statistical methods.Then the metabolites were selected that could be clearly distinct in the two groups with the fold change value(>1.2) and the P<0.05 of Student′s t-test.Both the pathway analysis and enrichment analysis were performed with Metabo Analyst 3.0.Results Compared with the normal rats, the activity of CPK decreased significantly in model rats(P<0.05).A significant separation appeared in the principal components analysis(PCA) score plot when the control group and the model group were compared, indicating that the Spleen-Qi deficiency model was successfully duplicated.The 33 differential metabolites, which mainly involved in the metabolic pathways, were distinguished from the comparision of Spleen-Qi deficiency model group and control group.The metabolic pathways was related to energy metabolism, amino acid metabolism, nucleotide metabolism and disturbance of gut microbes.Conclusions The main energy metabolic pathways (tricarboxylic acid cycle, glycolysis and liquid oxidation) may be disturbed in Spleen-Qi deficiency rats.The energy supply function is suppressed, which leads to the fatigue and weight loss in rats.
9.Effect of Yijing Fang on expression of junctional adhesion molecules and zonula occludens in testicular sertoli cells of adenine-induced infertile rats

Objective To observe the effect of Yijing Fang,a TCM compound,on the expression of junctional adhesion molecules (JSM)and zonula occludens (ZO)in testicular sertoli cells of infertile rats with kidney-yang deficiency pattern induced by adenine.Methods Altogether 48 healthy male Wistar rats of SPF grade,two-month-old,were divided according to random number table,into blank group, model group,Fufang Xuanju group and Yijing Fang group(n =12 each).After modeling,the rats were intragastrically administered by corresponding medicines for consecutive 20 days.Then the rats were sac-rificed and their testis and epididymis were resected.The sperm density and sperm activity were calcula-ted using Computer Assisted Sperm Analysis System,and the expression of JAMand ZO in testicular ser-toli cells were measured using immunohistochemical method.Results Compared with the normal group, the sperm density,sperm activity rate and expression of JAMand ZO declined significantly(P <0.01). Compared with the model group,the sperm density and the sperm activity rate increased in Fufang Xuan-Ju group and Yijing Fang group remarkably (P <0.05),while expression of JAM and ZO increased in Yijing Fang group((P <0.05 ).Conclusion Yijing Fang can repair the damage of testicular sertoli cells of kidney-yang deficiency pattern rats induce by adenine,via regulating the expression of JAM and ZO.
10.Correlation between acute stress hyperglycemia and hemoglobin A1c in patients with cerebral infarction

Objective:To investigate whether the basal blood glucose level before cerebral infarction has an impact on acute stress hyperglycemia.Methods:A total of 366 patients with cerebral infarction admitted into the neurology department of our hospital from February 2008 to May 2012 were enrolled in this study. Acute stress hyperglycemia was reflected by stress-induced glucose up-regulation ratio (SIGUR), and the basal glucose before cerebral infarction was reflected by glycosylated hemoglobin A1c (HbA1c). The correlation between SIGUR and HbA1c was analyzed in the different populations. The difference in SIGUR was compared among the non-diabetic group, diabetes with poor blood glucose control group (HbA1≥6.5%) and diabetes with well blood glucose control group (HbA1c<6.5%). The relationship between high level of SIGUR (Q4) and HbA1c was performed through logistic regression analysis.Results:SIGUR was correlated with HbA1c, no matter in the non-diabetic, diabetic or total populations ( r=-0.200, 0.195 and 0.324, P=0.010, 0.011 and 0.000). The level of SIGUR was higher in diabetes with poor blood glucose control group than in the non-diabetes and diabetes with well blood glucose control group ( F=25.842, P=0.000), but there was no significant difference between the non-diabetic group and diabetes with well blood glucose control group ( P>0.05). Logistic regression analysis showed that the high level of SIGUR was correlated to HbA1c in the total populations ( OR=1.460, P=0.000). In the diabetic group, the probability of higher SIGUR level was increased along with the increased HbA1c level ( OR=1.237, P=0.021), while the probability of higher SIGUR level was decreased along with the increased HbA1c level in the non-diabetic group ( OR=0.233, P=0.010). Conclusions:Acute stress hyperglycemia is correlated to the basal blood glucose before cerebral infarction, and blood glucose increases more prominently in those patients with high basal blood glucose level, especially in the diabetic patients.
