1.AN EVALUATION OF SURGICAL TREATMENT OF EARLY GASTRIC CANCER (EGO

This is a retrospective study of 103 cases of early gastric cancer undergoing surgery during the years of 1974-1988 with a special discussion on surgical treatment.The lesions were localized to the mu-cosal layer in 54.3%,to submucosal layer in 45.7%,In 10% of patients there was lymphnode metastasis,all of them were in the first station.Operation consisted of radical subtotal gastrectomy in 94.2%.and total gastrectomy in 5.8%.The extent of lymphatic excision was:Ro in 12.6%,R1 in 61.2% and R2 in 26.2% Postoperative chemotherapy was given in 61.2%.However no statistical difference of 5 years survival rate was found in respect to the extent of lymphatic excision as well as postoperative chemotherapy.Since 60.2% of EGC lesions were of minute,multiple and plane type,preoperattve en-doscopy and intraoperative biopsy of gastric mucosa,if necessary,should be carefully done to ascertain that no lesion was overlooked in the remnant of the stomach.Follow-up rate was 96%,and the survival rates of 3 and 5 years were 97% and 93.7%.This makes the authors believe that a radical operation of R1 is justified and routine postoperative chemotherapy is unnecessary.
2.Imaging with PET and fMRI on brain function in acupuncturing the ST36 (Zusanli)

ObjectiveTo discuss the experimental method and the mechanisms on treating diseases by acupuncturing the ST36(Zusanli).MethodsUsing Positron Emission Tomography(PET) and functional Magnetic Resonance Imaging(fMRI) to obtain the experimental data about glycometabolism and cerebral blood stream,using SPM and ROI image-analytical method to obtain the visual experimental evidence when acupuncturing the ST36. ResultsThere are certain increases of glycometabolism and cerebral blood stream in ipsilateral hypothalamus and bilateral temporal lobe, when acupuncturing the ST36. Conclusions Acupuncturing the ST36 can lead to the functional changes in vegetative nerve center and temporal lobe, which is close correlated with the therapeutical effects of ST36.
3.Initial experience on protocol optimization for integrated PET/MR

Objective To investigate the optimal workflow and protocol for integrated PET/MR by comparison with PET/CT.Methods A total of 228 patients were enrolled in this study for PET/CT and PET/MR evaluation on the same day.Six PET/MR protocols with different MR sequences but the same PET acquisition protocol were investigated and the optimal protocol was identified based on image quality,acquisition time and diagnostic performance.Results PET/MR workflow was similar to PET/CT,however,some special issues needed to be considered for PET/MR.Among the 6 protocols,protocol No.6 outperformed others for body and head regions.Types of artifacts were found more often in PET/MR than in PET/CT.Conclusions By optimizing the protocol,PET/MR could achieve almost the same diagnostic performance as PET/CT.However,the issues of long acquisition time and artifacts on PET/MR need to be further improved.
4.Regional glucose metabolic increases in left auditory cortex in tinnitus patients: a preliminary study with positron emission tomography

Objective To investigate the relationship between tinnitus and glucose metabolism in auditory cortex and whether positron emission tomography (PET) can be an objective tool in measuring tinnitus. Methods Eleven right-handed patients with severe tinnitus and ten right-handed control subjects participated in the 18 F-FDG/PET study. Analysis with regions of interests was used to calculate asymmetry indices according to the formula: [(L-R)×100/[(L+R)÷2]]. Results Glucose metabolism in the auditory cortex of tinnitus patients was asymmetric between the left and right auditory cortices, with that of the left being much higher than that of the right. The asymmetry indices of tinnitus patients was significantly higher than that of the control group (unpaired t test, P<0.001). This revealed that the increased metabolic activity was present in the predominant left hemisphere with a significant focus on the superior and transverse temporal gyri (Brodmann areas 41 and 42, respectively corresponding to primary and secondary auditory cortex), and the results were independent of the subjective localization of the tinnitus sensation. Conclusion It is suggested that the increased metabolism in the left auditory cortex is related to the tinnitus sensation. PET is capable of providing objective evidence for tinnitus and may be used as a potential tool in measuring tinnitus.
5.Multivariate predictors of intracranial aneurysm rupture and hemorrhage by regression analysis with emphasis on aneurysm angle

Objective To evaluate the influence of aneurismal inclination angle in the intracranial aneurysm rupture and hemorrhage. Methods A total of 2064 patients with intracranial aneurysms (1086 without rupture and hemorrhage, and 978 with rupture and hemorrhage), admitted to our hospital from January 2010 to April 2018, were chosen in our study. DSA examination and three-dimensional reconstruction were used for morphological evaluation of aneurysms, including aneurysm size, location, morphous feature, aneurismal inclination angle, vessel angle, aspect ratio (AR) and size ratio (SR). Risk factors of intracranial aneurysm rupture were analyzed by univariate and multivariate regression models; two-piecewise linear regression model and smooth curve fitting were used to analyze the saturation threshold effect of aneurismal inclination angle on aneurysm rupture and hemorrhage to evaluate the correlation between aneurismal inclination angle and aneurysm rupture and hemorrhage. Stratified analysis was conducted to clarify the influence of aneurismal inclination angle changes in risk of aneurysm rupture and hemorrhage in presence of various stratified factors (cerebral microbleeds [CMBs], diabetes, and hyperlipidemia). Results Aneurysm inclination angle was an independent risk factor for aneurysm rupture and hemorrhage (HR=1.02, 95%CI: 1.01-1.03, P=0.000). Risk of aneurysm rupture and hemorrhage increased when aneurismal inclination angle was more than 106.3 degrees for each additional degree; each degree of increase in aneurysm inclination increased the risk of aneurysm rupture and hemorrhage by 3%. As compared with patients without CMBs, risk of aneurysm rupture and hemorrhage increased extra 7%in patients with CMBs for each additional degree of aneurismal inclination angle;as compared with patients without diabetes mellitus MD, for each additional degree of aneurismal inclination angle, risk of aneurysm rupture and hemorrhage increased extra 2%in diabetes patients with fasting blood-glucose (FBG)≤ 6.0 mmol/L, and increased extra 4%in diabetes patients with FBS>6.0 mmol/L, respectively; as compared with patients without hyperlipemia, risk of aneurysm rupture and hemorrhage increased extra 3%in patients with hyperlipemia for each additional degree of aneurismal inclination angle. Conclusion The aneurismal inclination angle is closely related to intracranial aneurysm rupture and hemorrhage, and accurate measurement of aneurismal inclination angle has important clinical significance in assessing the risk of aneurysm rupture and hemorrhage.
6.PET imaging of brain function while puncturing the acupoint ST36.

OBJECTIVETo explore the experimental method of obtaining position emission tonogiaphy (PET) imaging evidence of changes in cerebral function by puncturing the Stomach 36 (ST36, Zusanli) acupoint.
METHODSData on changes of cerebral glycometabolism were obtained from six healthy male volunteers with positron emission tomography. Visual experimental evidence, as well as statistical parametric mapping (SPM), was gathered while puncturing the ST36 (Zusanli, right leg) acupoint.
RESULTSThere was increased glycometabolism in the hypothalamus, head of the caudate nucleus, temporal lobe, the sinistral cerebellum, postcentral gyrus, and brain stem while the acupoint ST36 was being punctured.
CONCLUSIONSAcupuncture on ST36 can lead to increase in glycometabolism in the vegetative nerve centers, which is correlated with gastric function. Visual experimental evidence of ST36 acupuncturing on functional gastrointestinal disorder was obtained in our study.
