1.A comparison of curative effect between Telbivudine and lamivudine in patients with chronic hepatitis B virus

Objective To analyze efficacy in patients with chronic hepatitis B virus by telbivudine or lamivu-dine. Methods To collect 100 cases of patients with chronic hepatitis B, according different drugs divided into: tel-bivudine group(50 cases)and 50 cases of lamivudine group. Methods The HBV-DNA baseline decline value、HBV DNA below the detection limit、treatment response、HBeAg negative rate、ALT normalization rote、virological response rates、primary treatment failure rate were observed. Results The treatment efficacy at 52 weeks in telbivudine group of HBV-DNA decline in the value of the baseline (6.21og10), HBV DNA below the detection limit (69.6%)、treat-ment response(64.2%), HBeAg negative conversion rate(30.5%)、ALT normalization rate (88.9%)、virologic re-sponse rates (30.5%), primary treatment failure rate(3.6%), higher than the larnivudine group(5.41 og10、43.4%、 86.8%、20.3%、75.1%、18.1%、16.0%) (t =3.65, t =6.55, t =3.95, t =3. 85, t =8.65,t =6.06, t =15.02,P <0.05~0.01). Conclusion The curative effect of telbivudine and lamivudine at 52 weeks achieve better efficacy; curative effect of telbivudine higher than the lamivudine.
2.Effect of Wenjingtongluo prescription combined with acupuncture and moxibustion on ESR, Fib and hemorheology in patients with cervical spondylosis

Objective To investigate effect of Wenjingtongluo prescription combined with acupuncture and moxibustion on ESR, Fib and hemorheology in patients with cervical spondylosis.Methods 110 cases of cervical spondylosis were divided into two groups, 55 cases in each group.The control group was treated with acupuncture and moxibustion.Experimental group on the basis of acupuncture treatment, were given Wenjingtongluo prescription.The PRI index, VAS score and blood rheology of the two groups were compared.Results The total effective rate of the experimental group was significantly higher than that of the control group (92.73% vs 76.36%) .There was a significant difference (χ2 =5.636, P <0.05) .After treatment, the two groups of PRI index ( emotional score, sensory score, total score ) , VAS score were significantly reduced ( P <0.05 ) .After treatment, the PRI index ( sensory score, total score) and VAS score of the experimental group were significantly lower than those of the control group after treatment.The difference was statistically significant(P<0.05).After treatment, two groups of ESR, Fib, PCV, whole blood viscosity, whole blood viscosity decreased significantly( P<0.05).The experimental group after treatment, ESR, Fib, PCV, whole blood viscosity, whole blood viscosity was significantly lower than the control group after treatment.The difference was statistically significant(P<0.05).Conclusion Wenjingtongluo prescription combined with acupuncture can significantly improve the clinical symptoms, reduce the pain of patients and improve the level of blood rheology.
3.Array comparative genomic hybridization screening in IVF significantly reduces number of embryos available for cryopreservation.

OBJECTIVE: During IVF, non-transferred embryos are usually selected for cryopreservation on the basis of morphological criteria. This investigation evaluated an application for array comparative genomic hybridization (aCGH) in assessment of surplus embryos prior to cryopreservation. METHODS: First-time IVF patients undergoing elective single embryo transfer and having at least one extra non-transferred embryo suitable for cryopreservation were offered enrollment in the study. Patients were randomized into two groups: Patients in group A (n=55) had embryos assessed first by morphology and then by aCGH, performed on cells obtained from trophectoderm biopsy on post-fertilization day 5. Only euploid embryos were designated for cryopreservation. Patients in group B (n=48) had embryos assessed by morphology alone, with only good morphology embryos considered suitable for cryopreservation. RESULTS: Among biopsied embryos in group A (n=425), euploidy was confirmed in 226 (53.1%). After fresh single embryo transfer, 64 (28.3%) surplus euploid embryos were cryopreserved for 51 patients (92.7%). In group B, 389 good morphology blastocysts were identified and a single top quality blastocyst was selected for fresh transfer. All group B patients (48/48) had at least one blastocyst remaining for cryopreservation. A total of 157 (40.4%) blastocysts were frozen in this group, a significantly larger proportion than was cryopreserved in group A (p=0.017, by chi-squared analysis). CONCLUSION: While aCGH and subsequent frozen embryo transfer are currently used to screen embryos, this is the first investigation to quantify the impact of aCGH specifically on embryo cryopreservation. Incorporation of aCGH screening significantly reduced the total number of cryopreserved blastocysts compared to when suitability for freezing was determined by morphology only. IVF patients should be counseled that the benefits of aCGH screening will likely come at the cost of sharply limiting the number of surplus embryos available for cryopreservation.

4.Sustained efficacy of alpha-interferon therapy combined with Yixuesheng Capsule in treatment of chronic hepatitis B.

OBJECTIVETo observe the difference between the combination therapy of alpha-interferon (IFN-alpha) therapy Yixuesheng Capsule and the monotherapy of IFN-alpha in treatment of chronic hepatitis B.
METHODA total of 288 patients with HBeAg-positive chronic hepatitis B proven by liver biopsy were included in this study. During the individualized therapy, they received hypodermic injection of IFN-alpha 1b, with 5 MU x time(-1) and three times x w(-1). Of them, 125 patients received combination therapy with Yixuesheng Capsule for three months, with 1.0 g/time and three times/d; and 163 patients received only IFN-alpha 1b (the IFN-alpha monotherapy group). After the course of therapy, all patients were followed up for at least 24 months. The intention-to-treat analysis was adopted for statistic analysis.
RESULTThe two groups showed no statistical significance by gender, age, liver necroinflammation grading, liver fibrosis staging, serum ALT levels, serum HBV DNA levels and IFN-alpha therapy course. The whole course and the 24-month follow-up visit cover all of 112 patients in the combination treatment group and 141 cases in the IFN-alpha monotherapy group. The response rates of the combination treatment group and the IFN-alpha monotherapy group were 48.0% (60/125) and 35.0% (57/163) (x = 4.980, P = 0.026) at the end of treatment, respectively, 45.6% (57/125) and 33.1% (54/163) (x2 = 4.645, P =0.031) at the end of 12-month-follow-up period, respectively, and 38.4% (48/125) and 32.5% (53/163) (x2 = 1.076, P = 0.300) at the end of 24-month follow-up period, respectively.
CONCLUSIONThe combination treatment with IFN-alpha and Yixuesheng Capsule shows a slightly better sustained efficacy on HBeAg-positive chronic hepatitis B patients compared with IFN-alpha monotherapy.

5.Prediction of congenital diaphragmatic hernia with abnormal course of fetal superior mesenteric artery detected by ultrasound in first-trimester

Objective To investigate the clinical value of abnormal course of fetal superior mesenteric arteries (SMA) detected by ultrasound during the first trimester (11-13+6 weeks) in predicting congenital diaphragmatic hernia (CDH). Methods This study enrolled women who underwent fetal nuchal translucency (NT) screening during the first trimester in Central Hospital of Panyu District from March to December 2017. Low-speed high-definition flow imaging was used to observe the course of fetal SMA, and it was regarded as abnormal when the angle between SMA and abdominal aorta >90°. Once abnormal course of the SMA was suspected, the position of fetal thoracic cavity and abdominal organs would be scanned carefully. Furthermore, ultrasound examinations would be repeated at 16-18, 20-24, 28-32 and 37-40 weeks of gestation. Fetus diagnosed as CDH by ultrasound would be scheduled for MRI or autopsy to confirm the diagnosis. Pregnancy outcomes of all cases were followed up by telephone. Descriptive statistical analysis was used in this study. Results A total of 6 899 gravidas (6 964 fetuses) underwent NT scan during the first trimester were enrolled and the SMA of all fetuses were successfully displayed. Three cases with abnormal course of the SMA were identified. Two of them were diagnosed with left CDH at 17+ and 23+ weeks of gestation, which was confirmed by autopsy after termination of pregnancy, and the other one terminated pregnancy in first trimester due to a large omphalocele. Among the 6 961 fetuses with normal SMA, the pregnancy outcomes of 6 120 were successfully followed up, only one of which was found to have left CDH at 32 gestational weeks by ultrasound examination, and was later confirmed by neonatal MRI after delivery. No other fetal CDH was detected. Conclusions Abnormal course of the SMA identified in early pregnancy may be a simple and effective indicator for CDH that allow early intervention and treatment.
