1.Several surgical skills and the discussion to the question of primary retroperitoneal tumor

Objective To investigate the strategy and announcements of surgery in primary retroperitoneal. tumor for improving the rescetable rate and decreasing the surgical risk.Methods Clinical data of 50 cases undergoing surgery of retroperitoneal tumor in our hospital from January 2004 to December 2008 were retrospectively analyzed.Results Tumors were benign in 20 cases and malignant in 30 cages.Complete resection was achieved in 39 cases(78%).in which combined organ resection carried out in 10 cases(20%),3 cases(6%)underwent partial resection.non-resection and had biopsy only were performed in 8 cases(16%),so the rescetable rate of this clinical weTe 84%(42/50).Conclusions Abundant preoperative preparation,wide open operation field and distinct anatomical dissection were the essential condition of the resecting primary retroperitoneal tumor.The relevant surgical skill should decrease the surgical risk.The tumor should be completely reaeeted in principle,and invaded neighboring organs should be resected with the discretion at the gane time.Completely resected was the key 10 improving the surviral rate and the strategy to decreasing the reeurence rate for recurrent retroperitoneal tumor.
2.Clinical features and surgical strategy for the senile patients with cardiac cancer

ObjectiveTo investigate rational surgical therapy for patients aged at 70 and up suffering from cardiac cancer. Methods 79 cases admitted from 1991 to 2000 were treated by transabdominal total gastrectomy with a radical resection rate of 73% and palliative resection rate of 19%. Results The average age was 74 6 years with a ratio of male to female of 4 6∶1. The average course of the disease was 3 1 months, and only 1 (2%) case got early diagnosis,while the misdiagnosis rate was 54%. Postoperative complication rate was 38%, mortality rate was 3%. Conclusions With the preoperative concurrent disease properly managed and the operation appropriately performed senile patients with cardiac cancer could also be managed successfuly as young patients .
4.Effect of Baclofen on Spasticity in Spinal Cord Injury:136 Cases Report

Objective To study the antispasticity effect of Baclofen on spasticity in spinal cord injured patients.Methods 136 cases were treated with Baclofen administration for at least 2 months.They were assessed with revised Ashworth Scale.Results The spasticity was reduced 1 grade in average in 131 cases.Conclusion Baclofen shows a good antispasticity effect on spasticity in spinal cord injury with different age,sex,duration of postinjury,level or completeness of injury.No hepatic or renal side-effects have been found.
5.Retrospective Analysis of Slight and Moderate Hydronephrosis in 58 Spinal Cord Injured Patients

ObjectiveTo investigate the clinical manifestation, mechanism and optimal treatment of hydronephrosis after spinal cord injury. Methods58 slight and moderate cases were retrospectively analyzed. ResultsThere were no differences in the remitted time among the therapies of indwelling catheter, intermittent catheterization and medicine, mixed therapy. 56 (96.55%) SCI hydronephrosis remitted after treatment, 28(48.28%) recurred within 3 months. The recurrence rate of the patients with detrusor sphincter dyssynergia (57.14%) was higher than that of the detrusor hyperreflexia (28.57%) and the detrusor hyporeflexia (14.29%). The recurrence was associated with the fact that many patients couldn't keep up with the systematic therapy.ConclusionThe onset of SCI hydronephrosis is indistinct and recurrent easily. Keeping up with intermittent catheterization and taking medicine can prevent SCI hydronephrosis recurrence effectively.
6.Retrospective analysis of effect of treatment on hydronephrosis in 46 spinal cord injured patients

ObjectiveTo search for the optimal treatment of hydronephrosis after spinal cord injury .Methods46 cases were treated by indwelling catheter, intermittent catheterization and drug therapy, and comprehensive therapy respectively. The course of treatment and the recurrence rate were observed after hydronephrosis remitted. Results There were no differences in the course of treatment among the three different kinds of therapy. The recurrence rate of the detrusor sphincter dyssynergia(85.7%) was higher than that of the detrusor hyperreflexia (35%) and the detrusor hyporeflexia(25%).ConclusionsThe optimal therapy on slight and moderate hydronephrosis should be intermittent catheterization and drug therapy to avoid the complications of long term indwelling catheter. SCI hydronephrosis is indistinct and recurrent easily. B ultrasound for kidneys is helpful in discovering SCI hydronephrosis on early stage. The earlier the treatment, the better the prognosis. Urodynamics plays an important role in the management of SCI hydronephrosis.

Functional electrical stimulation(FES) offers a vast potential for partial restoration of parlayzed movements. This three part article will review the basic concepts,system design and applications of FES. In part I,concepts such as activation threshold, recruitment order are discussed. Part Ⅱ introduces stimulation waveforme,safe parameters and tissue damage,as well as the design principle of stimulators for use with percutaneous electrodes. Part Ⅲ outlines clinical applications of FES,in particular,for restoration of hand grasp function for C5/C6 patients.
8.Defective expression of TGFBR3 gene and its molecular mechanisms in non-small cell lung cancer cell lines.

BACKGROUND AND OBJECTIVEIt has been reported that defective expression of TGFBR3 was found in non-small cell lung cancer (NSCLC). However, its molecular mechanisms remain unclear. The aim of this study is to investigate expression of TGFBR3 in NSCLC cell lines and normal human bronchial epithelial cell (HBEpiC), and to explore potential molecular mechanisms underlying inactivation of TGFBR3 gene.
METHODSWestern blot was performed to determine the expression of TGFBR3 in HBEpiC and NSCLC cell lines. Automatic image analysis was carried out to estimate relative expression of TGFBR3 protein. We screened for mutation of the promoter region of TGFBR3 gene using DNA direct sequencing. Bisulfite-sodium modification sequencing was used to detect the methylation status of TGFBR3 promoter.
RESULTSTGFBR3 protein level was abnormally reduced in NSCLC cell lines as compared with HBEpiC. There was significant difference in TGFBR3 expression between the highly metastatic cell line 95D and non-metastatic cell lines, including LTEP-alpha-2, A549 and NCI-H460. No mutation and methylation was found in upstream sites -165 to -75 of the proximal promoter of TGFBR3 in HBEpiC and NSCLC cell lines. Hypermethylation was shown in upstream sites -314 to -199 of the distal promoter of TGFBR3 in HBEpiC and NSCLC cell lines.
CONCLUSIONReduced expression of TGFBR3 was observed in NSCLC cell lines, especially in 95D, suggesting that TGFBR3 might play an important role in development and progression of NSCLC and correlate with NSCLC invasion and migration. The methylation event occurring at TGFBR3 promoter is not a major cause for reduction of TGFBR3 expression.

9.Effect of screening and treatment of syphilis among pregnant women on prevention of intrauterine syphilis transmission in Nantong City, 2012-2019

Objective:To evaluate the intrauterine transmission of syphilis in Nantong City, Jiangsu Province from 2012 to 2019, after the introduction of a nationwide policy for preventing intrauterine transmission of syphilis in China in 2011.Methods:This study enrolled all live birth deliveries ( n=455 561) in Nantong from January 2012 to December 2019. The screening, infection rates, anti-syphilis treatment, intrauterine transmission of syphilis, and outcomes of infants with congenital syphilis were retrospectively analyzed using χ 2 test for trend, adjusted χ 2 test, or Fisher's exact test. Results:Except for three women, the remaining 455 558 subjects were all screened for syphilis antibody with a total screening rate of nearly 100%, among which prenatal screening accounted for 96.4% (439 125/455 561) and intrapartum screening for 3.6% (16 433/455 561). In total, 796 (0.17%) women were diagnosed with syphilis during pregnancy, and the prevalence increased from 0.13% (85/64 229) in 2012 to 0.24% (110/45 517) in 2019 (χ 2trend=48.985, P<0.001). The prevalence among women underwent intrapartum screening was significantly higher than those underwent prenatal screening [0.50% (82/16 433) vs 0.16% (714/439 125), χ 2=102.769, P<0.001]. Out of the women with syphilis, 716 (89.9%) received anti-syphilis therapy with 695 cases using penicillin, 16 cases using ceftriaxone and five using erythromycin/azithromycin, while the remaining 80 (10.1%) did not. Intrauterine transmission of syphilis occurred in 14 infants with a transmission rate of 1.8% (14/796). The reported rate of congenital syphilis in all live infants was 0.03‰ (14/460 552). The intrauterine transmission rate in women receiving treatment during pregnancy was significantly lower than that in the untreated women [0.4% (3/716) vs 13.8% (11/80), χ2=66.499, P<0.001]. For the untreated women, the intrauterine transmission rate increased with the rising titers of non-specific syphilis antibody ( χ2trend=5.338, P=0.021). Among infants with congenital syphilis, no obvious adverse outcomes occurred in three infants born to treated mothers, whereas the rates of preterm birth and neonatal death were 7/11 and 2/11 in those born to untreated mothers. Conclusions:Since the implementation of the policy against intrauterine transmission of syphilis, the reported rate of congenital syphilis is 3/100 000 live-birth in Nantong City, reaching the national target of below 15/100 000. Screening and treatment in the first trimester are critical for preventing intrauterine transmission of syphilis. Increased prenatal syphilis screening rate can help further reduction of the intrauterine transmission of syphilis.

A novel bioinspired phospholipid copolymer has been synthesized by the radical polymerization of poly2-Methacryloyloxyethylphosphorylcholine (MPC), stearyl methacrylate (SMA), hydroxypropyl methacrylate (HPMA) and trimethoxysilylpropyl methacrylate (TSMA). Contact angle results indicated that the coating surface rearranged to get a more hydrophilic surface at the polymer/water interface. The membrane mimic phosphorylcholine coating surface could resist the platelet adhesion and prolong plasma recalcification time significantly. Rapamycin was used as model drugs to prepare drug-eluting coating. The animal experiments showed that this novel drug-eluting stent could effectively prevent the phenomena of restenosis.
