1.Analyzing the occupational health literacy level and its influencing factors among workers in non-metallic mineral product industry in Yunfu City

Objective To analyze the occupational health literacy (OHL) level and its influencing factors of workers in non-metallic mineral product industry in Yunfu City. Methods A total of 947 frontline workers from 24 non-metallic mineral products enterprises in Yunfu City were selected as the research subjects using the stratified random sampling method. The OHL level of the workers were assessed using the Occupational Health Literacy Questionnaire of National Key Populations. Results The overall OHL level of the research subjects was 58.3% (552/947). The OHL levels across four dimensions, from highest to lowest, were basic knowledge of occupational health protection (94.7%), healthy work practices and behaviors (81.8%), legal knowledge of occupational health (65.5%), and basic skills of occupational health protection (25.9%). The results of binary logistic regression analysis showed that workers with 2.0-<10.0 years and ≥10 years of work experience had higher OHL levels than those with <2.0 years of work experience (all P<0.01). Workers with a high school education or above had higher OHL levels than those with a junior high school education or below (all P<0.01). Workers in large- and medium-sized enterprises had higher OHL levels than those in small and micro-sized enterprises (both P<0.01). Conclusion The OHL levels of workers in Yunfu City's non-metallic mineral products industry can be further improved, particularly the occupational health protection skills and related legal knowledge. Workers with short seniority, low educational level, and in small and micro enterprises should be the key groups for improving OHL levels.
2.Analysis of the safety, economic benefit and social psychological satisfaction of day breast conserving surgery for breast cancer

Objective To investigate the safety, economic benefits and psychological effects of day breast conserving surgery for breast cancer. Methods The demographic data and clinical data of breast cancer patients undergoing day (day surgery group) and ward (ward surgery group) breast conserving surgeries in West China Hospital of Sichuan University from March 2020 to June 2021 were retrospectively collected; the demographic data, clinical data, medical and related transportation costs, and preoperative and postoperative BREAST-Q scores of breast cancer patients undergoing day (day surgery group) and ward (ward surgery group) breast conserving surgery in West China Hospital of Sichuan University from June 2021 to June 2022 were prospectively collected. The safety, economic benefit, and psychological satisfaction of day surgery was analyzed. Results A total of 42 women with breast cancer were included in the retrospective study and 39 women with breast cancer were included in the prospective study. In both prospective and retrospective studies, the mean age of patients in both groups were <50 years. There were only statistical differences between the two groups in the aspects of hypertension (P=0.022), neoadjuvant chemotherapy (P=0.037) and postoperative pathological estrogen receptor (P=0.033) in the prospective study. In postoperative complications, there were no statistical differences in the surgical-related complications or anesthesia-related complications between the two groups in either the prospective study or the retrospective study (P>0.05). In terms of the overall cost, we found that the day surgery group was more economical than the ward surgery group in the prospective study (P=0.002). There were no statistical differences in postoperative psychosocical well-being, sexual well-being, satisfaction with breasts or chest condition between the two groups (P>0.05). Conclusion It is safe and reliable to carry out breast conserving surgery in day surgery center under strict management standards, which can save medical costs and will not cause great psychological burden to patients.

Yttrium-90 selective internal radiation therapy (90Y-SIRT) is a treatment technique that delivers radioactive microspheres precisely to the arterial vascular bed of neoplasms, utilizing beta radiation to administer a high local dose of radiation to the neoplasm tissues. This technology has demonstrated significant efficacy in patients with unresectable pirmary liver cancers and liver metastases. This article systematically reviews the development history and clinical application status of 90Y-SIRT in the treatment of liver cancer, and looks forward to future development directions.
4.Analysis of factors influencing the trough concentration of voriconazole and adverse drug reactions in renal transplant patients

OBJECTIVE To analyze the influencing factors of voriconazole trough concentration and adverse drug reactions (ADR) in renal transplant recipients. METHODS Data from inpatients who received voriconazole and therapeutic drug monitoring in our hospital between January 2022 and August 2023 were retrospectively analyzed. Patients were divided into renal transplant group and non-renal transplant group based on transplantation status. A 1∶1 propensity score matching (PSM) method was used to balance differences in baselines between the two groups. Voriconazole trough concentrations, target attainment rate, clinical efficacy, and ADR were compared between the two groups. Multiple linear regression (backward) was used to analyze the factors influencing voriconazole trough concentrations in the renal transplant group. Univariate analysis and binary Logistic regression were used to identify independent risk factors for ADR in the renal transplant group. RESULTS After PSM, 48 patients were included in each group. There were no statistically significant differences in the mean voriconazole trough concentration, target attainment rate or efficacy rate between the two groups (P>0.05). The total incidence of ADR was significantly higher in the renal transplant group than in the non-renal transplant group (P<0.05). Multiple linear regression analysis showed that age, average daily dose, pulmonary infection, total bilirubin during medication, day-1 loading dose, use of the original drug, concomitant immunosuppressant use, and the occurrence of ADR were factors influencing voriconazole trough concentration in renal transplant patients (P<0.05). Binary Logistic regression analysis showed that abnormal direct bilirubin during medication [OR=7.747, 95%CI (1.334, 45.005), P=0.023] was an independent risk factor for ADR in renal transplant patients receiving voriconazole. CONCLUSIONS Age, average daily dose, pulmonary infection, use of the original drug, day-1 loading dose, total bilirubin during medication, concomitant immunosuppressant use, and the occurrence of ADR are the factors influencing voriconazole trough concentration in renal transplant patients. Furthermore, patients with abnormal direct bilirubin during medication are more susceptible to ADR.
5.Analyzing the influencing factors of occupational health literacy among noise-exposed workers in the automotive manufacturing industry

Objective To analyze the current status and influencing factors of occupational health literacy (OHL) among noise-exposed workers in the automotive manufacturing industry. Methods A total of 1 188 front-line workers in noise-exposed positions in the automotive manufacturing industry were selected by judgment sampling method. The OHL status of the workers was investigated using the Occupational Health Literacy Questionnaire of National Key Population, and the influencing factors of OHL were analyzed. Results The overall OHL level was 30.13% (358/1 188). The result of binary logistic regression analysis showed that marital status, educational level, personal monthly income, household registration, enterprise scale and weekly working hours were influencing factors of OHL level (all P<0.05). Specifically, being married, having a high school education or higher, monthly personal income >5 000 yuan, being from a non-agricultural household, working in a medium-sized enterprise, and working more than >40-48 hours per week were protective factors for high OHL levels (all P<0.05). Conclusion There is considerable room for improvement in the OHL of noise-exposed workers in the automotive manufacturing industry. Focus should be placed on workers who are unmarried, widowed, or divorced, have educational levels below high school, earn less than 5 000 yuan per month, have agricultural household registrations, work in small or micro-sized enterprises, or work ≤40 hours per week, with targeted strategies to improve their overall OHL level.
6.Application of excellence-performance management model in macro-quality management for occupational medical examination institutions

As the management system of Occupational Medical Examination (OME) institutions is moving to a record-filing model, the traditional management approach might not fulfill the current policy requirements. There is a pressing need for development of high-quality occupational health management system. In this context, the Guangdong Provincial Occupational Health Examination Quality Control Center (hereinafter referred to as the "Provincial Quality Control Center") pioneered the integration of the Performance Excellence Model (PEM) framework into the macro-quality management of OME institutions in Guangdong Province. In light of the current research gaps in the field of macro-quality management for OME institutions, the Provincial Quality Control Center, based on the core principles and standards of PEM, has closely aligned with the professional characteristics of occupational health examinations. The established macro-quality management innovation in OME institutions in Guangdong Province consists of seven dimensions: strategic direction, leadership development, identification of key management subjects, data analysis and management, human resources allocation, process optimization, and the application of quality control outcomes. This system has provided a robust impetus for the ongoing improvement of quality control efforts. The initial implementation of PEM demonstrated notable improvements in macro-quality management for OME institutions. Moving forward, the Provincial Quality Control Center will continue to strengthen the development of key technical personnel, innovate training models, harness the advantages of big data technology, restate and upgrade the system continuously, and refine the macro-quality management mechanisms of OME institutions to ensure that the health rights and interests of workers and employers are effectively safeguarded and enhanced.
7.Preparation and content determination of extended-release tablets of altrenogest

This study aims to prepare altrenogest extended-release tablets,evaluate their quality and establish a content determination method.The hydrophilic gel skeleton type,dosage and core thick-ness of altrenogest extended-release tablets were used as the investigating factors,and the release degree of the tablets was used as the investigating index,the prescription process of altrenogest ex-tended-release tablets was optimized by one-factor screening and central combinatorial design re-sponse surface method,and quality evaluation was carried out,the in vitro release model was es-tablished,and a high-performance liquid chromatography(HPLC)assay method was set up for the determination of altrenogest extended-release tablets.The results showed that the optimal pre-scription of altrenogest extended-release tablets was 2%as the main drug,70%as the solubilizer,0.5%as the lubricant,19.1%as the filler,8.4%as the hydrophilic gel skeleton material,and the thickness of the tablets was 3.8 mm.The in vitro drug release conformed to the Higuchi model,and the altrenogest showed a good linear relationship with the R2=0.999 98 in the range of 10-80 mg/L.The optimized process for the extended-release tablets was stable and had a good quality.The extended-release tablets were stable and had significant slow-release effect.The HPLC method is accurate and reliable and can be used for the determination of altrenogest in extended-release tablets.
8.Formulation screening and content determination of compound albendazole sulfox-ide pouring agent

Albendazole sulfoxide and ivermectin compound pouring agent were prepared with dime-thyl sulfoxide and 1,2-propanediol as solvents.The central composite design response surface method was used to optimize the formula of pouring agent.Franz diffusion cell method was used to investigate the transdermal performance of pouring agent in vitro.The permeation amounts of the two drugs were determined by HPLC.The best formula of pouring agent was ivermectin 0.5%,al-bendazole sulfoxide 5%,dimethyl sulfoxide 52%,propylene glycol 39%,and the rest was 100%anhydrous ethanol.The cumulative permeation amounts of ivermectin and albendazole sulfoxide were up to 20.78 μg/cm2 and 249.02 μg/cm2,respectively.The in vitro release model of the two drugs accords with the first-order kinetic equation.There is a good linear relationship between al-bendazole sulfoxide and ivermectin in the range of 1-100 mg/L and the peak area.The precision and stability RSD of the two methods are less than 2%.The preparation process of albendazole sul-foxide compound pouring agent is simple,stable and easy to pour.The established HPLC method is simple and accurate,and can be used for the determination of albendazole sulfoxide and ivermectin in pouring agent.
9.Differences in the Elderly Care Service Demand,Preference,and Tendency Between Urban and Rural Areas in the Pearl River Delta

Objective To understand the differences in the demand,preference,and tendency for elderly care services between urban and rural areas in the Pearl River Delta(PRD),and to provide reference for the planning and balanced allocation of elderly care resources in urban and rural areas.Methods Using the multi-stage stratified random sampling method,we selected 7 community health service centers in 2 prefecture-level cities in the PRD and conducted a questionnaire survey on the elderly care service demand,preference,and tendency among 1919 regular residents aged 60 years and above who attended the centers.Results A total of 641 urban elderly residents(33.4%)and 1278 rural elderly residents(66.6%)were surveyed in the PRD.The urban and rural elderly residents showed differences in the child number(χ2 =43.379,P<0.001),willingness to purchase socialized elderly care services(χ2 =104.141,P<0.001),and attitudes to the concept of raising child to avoid elderly hardship(χ2 =65.632,P<0.001).The proportion(71.8%)of rural elderly residents who prefer family-based elderly care was higher than that(57.1%)of urban elderly residents(χ2 =41.373,P<0.001).The proportion(62.2%)of urban elderly residents clearly expressing their willingness to choose institutions for elderly care was higher than that(44.0%)of rural elderly residents(χ2 =57.007,P<0.001).Compared with family-based elderly care,the willingness to choose institutional or community-based in-house elderly care was low among the urban elderly residents with surplus monthly household income or balanced income and expenditure;urban males,those with college education background or above,and those who purchased so-cialized elderly care services tended to prefer community-based in-house elderly care.In rural areas,the elderly residents who had local household registry were prone to choose institutional or community-based in-house elderly care,while those who had more than one child and those who were satisfied with the current living conditions were less willing to choose community-based in-house elderly care.Conclusions It is suggested that the urban-rural differences in the elderly care service demand,preference and tendency should be fully considered in the planning and allocation of urban and rural elderly care resources.Efforts remain to be made to develop diversified social elderly care services tailored to the characteristics of urban and rural areas.
10.HuiNet report of 2023: The distribution and antimicrobial resistance profile of clinical bacterial isolates in Anhui

Objective:To analyse the distribution and antimicrobial resistance profile of clinical bacterial isolates in Anhui province.Methods:Surveillance data was collected from 83 members of the Anhui Antimicrobial Resistance Surveillance Network(HuiNet)during October 2022 to September 2023,to analyze the resistance of major bacteria to commonly used antibiotics and the detection of clinically common drug-resistant bacteria. The data was analyzed using WHONET 5.6 and SPSS 25.0 software.Results:A total of 201 647 clinical bacteria isolates were collected,with Gram-negative bacteria accounting for 74.8%(150 847/201 647). The most prevalent Gram-positive bacterial strains were Staphylococus aureus(32.8%,16 648/50 800),followed by Staphylococcus epidermidis(14.0%,7 098/50 800), Enterococcus faecalis(10.7%,5 458/50 800), Enterococcus faecium(9.1%,4 613/50 800)and Staphylococcus hominis(7.4%,3 778/50 800);the most prevalent Gram-negative bacterial strains were Escherichia coli(28.9%,43 577/150 847),followed by Klebsiella pneumoniae(22.5%,34 006/150 847), Pseudomonas aeruginosa(14.7%,22 171/150 847), Acinetobacter baumannii complex(9.4%,14 194/150 847)and Enterobacter cloacae(3.5%,5 235/150 847). The prevalence of methicillin-resistant Staphylococcus aureus(MRSA)and methicillin-resistant coagulase-negative Staphylococcus aureus(MRCNS)were 39.5%(6 442/16 325)and 75.7%(12 343/16 312),respectively. No vancomycin- and teicolanin-resistant Staphylococcus were detected. The prevalence of vancomycin-resistant Enterococcus faecium and Enterococcus faecalis were 0.5% and 0.6%,respectively. The prevalence of carbapenem-resistant Escherichia coli and Klebsiella pneumoniae(CR-KPN)were 1.9%(805/42 956)and 11.7%(3 950/33 761),respectively. The resistance rate of CR-KPN to tigecycline was 3.9%. The prevalence of carbapenem-resistant Pseudomonas aeruginosa(CR-PAE)and Acinetobacter baumannii(CR-ABA)complex were 18.4%(3 936/21 447)and 62.9%(8 649/13 744),respectively,with low resistance rate to polycolistin B(6.9% and 1.7%,respectively). The detection rates of MRSA,CR-ABA complex,third-generation cephalosporin-resistant Escherichia coli(CTX/CRO-R-ECO)and quinolone-resistant Escherichia coli(QNR-ECO)in northern Anhui were the highest(50.3%,72.9%,59.2% and 55.6%,respectively),which were higher than those in central and southern Anhui( χ2=112.734 and 575.069,132.747 and 233.885,93.986 and 471.209,60.062 and 230.669,all P<0.001),and the detection rate in central Anhui was higher than that in southern Anhui( χ2=278.671,29.219,207.395 and 80.267,all P<0.001). The detection rates of CR-KPN and thirdgeneration cephalosporinresistant Klebsiella pneumoniae(CTX/CRO-R-KPN)in central Anhui were the highest(15.5% and 33.3%,respectively),which were higher than those in northern and southern Anhui( χ2=156.237 and 325.533,76.928 and 180.686,all P<0.001),and the detection rate in northern Anhui was higher than that in southern Anhui( χ2=32.202 and 25.539, P<0.001). The detection rates of CTX/CRO-R-ECO and QNR-ECO were the highest in the elderly(55.2% and 55.8%,respectively),which were higher than those in children,and young and middle aged adults( χ2=23.906 and 120.575,376.404 and 196.612, P<0.001). The detection rate of CTX/CRO-R-KPN in neonates was the highest(57.1%),which was significantly higher than that in children,adults and the elderly( χ2=46.141,38.843 and 32.093, P<0.001),and the detection rate in the elderly was higher than that in children and adults( χ2=13.604 and 13.471, P<0.001). The detection rates of MRSA and MRCNS were the highest in children(42.8% and 77.8%,respectively),which were higher than those in adults( χ2=21.766 and 10.704, P<0.001). Except MRSA and vancomycin-resistant Enterococcus faecium and faecalis,the detection rates of major drug-resistant bacteria in tertiary hospitals were higher than those in secondary hospitals( P<0.05 or <0.01). Conclusion:In 2023,the situation of antimicrobial resistance in Anhui province was serious,especially in northern and central Anhui,and targeted drug resistance control measures should be taken according to the monitoring results. At the same time,it is necessary to pay attention to the bacterial resistance in the elderly,newborns and children,and strengthen the rational use of antibiotics by clinicians to curb the spread of drug-resistant bacteria.
