1.Evaluation the global and regional myocardial function of patients with hypertrophic cardiomyopathy by two-dimensional speckle tracking imaging

Two-dimensional speckle tracking imaging(2D-STI)can accurately evaluate myocardial strain with non-angle dependent and good reproducibility. In this study, 16 patients with hypertrophic cardiomyopathy (HCM), 18 athletes and 20 normal subjects underwent 2D-STI examination. The global longitudinal strain (GLS) and regional peak systolic strain (PSS) were assessed by 2D-STI in the apical four-chamber-view. Compared to athletes and normal subjects, GLS in patients with HCM was significantly reduced [GLS: -(8. 0±3.6)% , P < 0. 01]. There was no significant difference in GLS between athletes and the controls, but GLS in basal or middle segments of interventricular septum and middle segments of lateral wall; PSS in basal, middle or apical segments of interventricular septum and basal or middle segments of lateral wall of the normal controls were significantly higher than those of the athletes. A cut-off value of less than -10% in GLS for diagnosis of pathologic hypertrophy resulted in a sensitivity of 84. 0% and a specificity of 94. 0%. The results suggest that 2D-STI can quantify global and regional myocardial function and may be used for the differentiation between physiological and pathologic LVH.
2.Relationship between torsion and remodeling or function of left ventricle in patients with dilated cardiomyopathy

Objective To evaluate the effect of torsion on remodeling and function of left ventricle by two-dimensional speckle tracking imaging(2D-STI) in patients with dilated cardiomyopathy(DCM).MethodsThirty-nine patients with DCM and thirty-five controls accepted conventional echocardiography,pulsed-wave Doppler tissue imaging examination.Basal and apical short-axis view of left ventricle were accorded for Q-lab analysis.Indices included:left ventricular diameter in end diastole (LVEDd),left ventricular ejection fraction (LVEF),left ventricular sphericity index (LVSI),left ventricular mass index (LVMI),e-wave of mitral valvular flow (E),velocity of mitral annulus in systole (S),velocity of mitral annulus in early distole ( E'), E/ E',rotation of left ventricle at basal and apical level(Rbasal,Rapical),twist (T),torsion (T').Results Compared with controls,LVEDd,LVMI of patients with DCM increased significantly,LVEF and LVSI decreased significantly,S and E' decreased significantly,E/ E' increased significantly.Patients with DCM were divided into two groups:group DCM-1 (clockwise rotation at apical level) and group DCM-2 (countclockwise rotation at apical level).Compared with controls,Rasal,Rapical,T,T' of two DCM groups decreased significantly.Compared with group DCM-1,Rbasal,Rapical,T,T' of group DCM-2 decreased significantly,LVEDd,LVMI increased significantly,LVEF and LVSI decreased significantly,E' decreased significantly,E/ E' increased significantly.Conclusions The torsion of left ventricle decrease in patients with DCM,DCM patients with decreased or reversed torsion of left ventricular apex have more severe LV remodeling or damage of LV function.

4.Ultrasound evaluation of left atrial function in patients with old myocardial infarction by strain rate imaging

Objective To evaluate the changes of the left atrial function in patients with old myocardial infarction(OMI) by strain rate imaging(SRI). Methods The velocity of the left atrial lateral wall and atrial septum was measured by SRI in 30 patients with OMI(OMI group) and 25 normal subjects (control group). Left atrium passive eject volume index(LAPEVl) and left atium active eject volume index (LAAEVI) were measured using Simpson method. Results ①Compared with the control group, the velocity of the left atrium was reduced obviously in ventricular systole and early ventricular diastole in OMI group(P<0.05) ,but was increased in late ventricular diastole(P<0.05). ②Compared with the controls, LAAEVI was increased significantly in OMI group, but LAPEVI was decreased signicantly(P <0.001). Conclusions SRI could be used to evaluate the left atrial function accurately in patiens with OMI.

Lumbar dysfunction closely relates to the weakness or deficiency of lumbar proprioception, and accurate and comprehensive lumbar proprioception test is the important basis of training program formulation, efficacy assessment and prognostic evaluation. Comprehensive lumbar proprioception test includes position sense test, kinesthesia test and vibration sense test. It has been widely used in clinic at abroad, and is rich in test equipments and methods. Domestic research is still at the preliminary stage in this field, and is lack of accurate, objective and unified test methods. According to an overview of relevant literature, this article discussed the lumbar proprioception test methods, the influential factors and reliability of the test, finally, put forward the prospects about the research direction in this field.

AIM:To study the effects and mechanisms of ethanol on chloride channels in poorly differentiated nasopharyngeal carcinoma CNE-2Z cells.METHODS:The effect of ethanol on the cell growth was analyzed by MTT as-say.The technique of whole-cell patch-clamp was used to detect the chloride current .The characteristics of the chloride current were analyzed by using the chloride channel blockers .The siRNA technique was used to analyze the molecular basis of the ethanol-sensitive chloride channels .RESULTS: Under isotonic conditions , the background current was weak and stable.Ethanol at concentrations of 0.17~170 mmol/L activated a chloride current in a concentration-dependent manner (an inverted U-shape), with a maximum effect at the concentration of 17 mmol/L.The currents showed obviously outward rectification and were susceptible to extracellular hypertonicity and the chloride channel blocker , 5-nitro-2-(3-phenylpropyl-amino) benzoic acid ( NPPB) .ClC-3 siRNA obviously decreased the currents activated by ethanol .CONCLUSION:Ex-tracellular ethanol induces chloride currents through activating the ClC-3 chloride channels .
7.Clinical comparative study of reforming endoscopic minimally invasive releasing versus open procedure for unilateral carpal tunnel syndrome

Objective To compare the efficacy of the reforming endoscopic minimally invasive releasing(REMIR) with open procedure for treatment of carpal tunnel syndrome.Methods Senventy patients with unilateral carpal tunnel syndrome were randomly divided into REMIR group and open procedure group,with 35 cases in each group.Kelly's standard,two-point discrimination,operation time and complication occurrence were compared between the two groups.Results All patients were followed-up for 12 months.There was no significant difference in the therapeutic results and in the improvement of two-point discrimination between the two groups (P > 0.05 ).The operation time of REMIR group was significantly shorter than the open procedure group ([ 10.03 ± 1.84] min vs [37.63 ±7.18]min,t =22.210,P <0.001 ).And there was no cases with scar tenderness in REMIR group while there was 7 cases in open procedure group.Conclusion Compared with the open procedure,the REMIR method has the same clinical efficacy while with the advantages of causing smaller skin scar and being less time consuming.
8.Chloride currents activated by cisplatin in poorly differentiated naso-pharyngeal carcinoma cells are not Ca2+-activated chloride currents

AIM:To investigate the type of chloride channel activated by cisplatin in poorly differentiated na -sopharyngeal carcinoma cells (CNE-2Z cells).METHODS:The technique of whole-cell patch-clamp was used to investi-gate the role of Ca 2+in the activation of cisplatin-activated chloride currents and to analyze the effect of hypertonic stress on these currents in CNE-2Z cells.RESULTS:Chloride currents were induced when the cells were exposed to the calcium -free cisplatin solution , showing the similar density to the currents induced by cisplatin with the presence of extracellular cal -cium.However , the latency and the peak time of cisplatin-activated currents in the absence of extracellular calcium were prolonged.The activation of cisplatin-activated chloride currents was insensitive to the depletion of intra-and extracellular calcium.Calcium channel antagonist nifedipine had no effect on the cisplatin -activated chloride currents , while hypertonic solution completely inhibited those currents .CONCLUSION:The cisplatin-activated chloride currents are independent on intra/extracellular calcium .The chloride channels activated by cisplatin are not calcium-activated chloride channels , but are probably volume-sensitive chloride channels .

In recent years, the researches on the hypoglycemic effect of Ruikang Hospital Affilliated to traditional Chinese medicine mainly started from the mechanism of new target protein and pathway, for example, Chinese medicine monomers or compounds including as-tragalus polysaccharide, curcumin, berberine, emodin, total saponins of momordicacharantia, total saponins of momordicacharantia, Yitangkang, Jiangtang Sanhuang tablets, Jianpixiaokefang and Tangnaikang granule can activate Adenosine monophosphate ( AMP)-activated protein kinase ( AMPK) to play hypoglycemic effect.
10.Borneol activated volume-sensitive chloride channels in poorly differentiated nasopharyngeal carcinoma cells

Aim To clarify the effect of Borneol on the chloride channels and cell volume in poorly differentia-ted nasopharyngeal carcinoma CNE-2Z cells.Methods The technique of whole-cell patch clamp was used to detect the chloride currents and analyze the character-istics of the currents in CNE-2Z cells.The volume changes caused by Borneol were measured by the meth-od of time-lapse live cell imaging.Results The chlo-ride currents were induced by extracellular application of Borneol (20 μmol·L -1 )isotonic condition.The currents showed a characteristic of outward rectification and did not show voltage-dependent or time-dependent inactivation.The reversal potential of the currents was close to the CI-equilibrium potential. The currents were inhibited by the chloride channel blocker tamox-ifen.The currents were also inhibited by 47% hyper-tonic solution.Borneol decreased the cell volume by 9.4% in 30 min.Tamoxifen completely inhibited the Borneol-induced cell volume decrease.Conclusion Borneol can activate volume-sensitive chloride channels and induce volume decrease in CNE-2Z cells.Chloride channels play a pivotal role in the process of volume decrease caused by Borneol.
