1.Early Intervention on Psychosocial Development of Children in Single Child Family

Objective: To study the effect of early intervention on psychosocial development of children in single child family. Method:30 babies were enrolled as intervention group from they were fetus. Another 37 were enrolled as control group. The former received intervention under the Guide of Different Stages from 0 to 6 years old. Achenbach Children Behavior Checklist (CBCL) and Binet Intelligent Test were used to assess the results.Results:The rate of behavior group was 6.7%, significantly lower than that of the control group (13.5%) and Chinese norm (14.2%). The average IQ of the intervention group was significantly higher than control. But there was no significant difference in Social Adaptability between the two groups. Conclusion:Early education and intervention can improve psychosocial development children in single child family.
2.Whole-genome expression analysis of synovial fibroblasts in rheumatoid arthritis

Objective To identify the crucial gene implicated in rheumatoid arthritis (RA) pathogenesis by comparing microarray-based gene expression profiles of synovial fibroblast in arthritis patients and that in control.Methods The public datasets were obtained from NCBI GEO and EBI ArrayExpress.The qualified microarray-based gene expression profiles were integrated and normalized using the method implemented in GeneSpring software.Furthermore,the differentially expressed genes (DEGs) were identified using significance analysis of microarrays (SAM) method.The online tool DAVID and STRING were applied to conduct the enrichment analysis and gene product interaction analysis respectively.Results There were two datasets that were qualified and analyzed in the present study.A total of 336 significant DEGs were identified by comparing the whole-genome gene expression profiles from synovial fibroblast of RA patients and control group.Among these DEGs,261 were significantly downregulated and 75 upregulated.About 13.6% of the downregulated genes were associated with extracellular matrix degradation.The COL9A3 and COL4A5,indispensable component of hyaline cartilage and basement membrane respectively,were significantly downregulated,as well as genes in WNT family,including WNT2,WNT11,and WNT16.In contrast,matrix metalloproteinase 13 (MMP13) was found to be significantly upregulated in RA patients.MMP13 is a matrix metallopeptidase that degrade extracellular matrix and hyaline cartilage,and it could possibly interact with other proteins to regulate morphogenesis.Conclusion Molecular mechanisms underlying RA pathogenesis were investigated by analyzing the public datasets.A few genes that associated with extracellular matrix degradation,construction and regulation,including MMP13,WNT2,WNT11,WNT16,COL9A3 and COL4A5,could be regarded as therapeutic targets in RA treatment.
3.An analysis on knowledge, attitude and practice regarding antibiotics use among community residents in Hangzhou

Objective To assess the current status of knowledge, attitude and practice (KAP) towards antibiotic use among community residents in Hangzhou, and to explore the correlations among them. Methods A total of 449 permanent residents in Hangzhou were randomly selected using a multistage stratified random sampling method. Self-reported data on basic demographic factors, and relevant KAP information were collected by the questionnaire survey. Differences in KAP scores according to each demographic factor were assessed by the t test or ANOVA test, and AMOS 21.0 was used for the path analysis. Results Scores for knowledge, attitude and practice regarding antibiotic use were (6.17±2.45), (6.45±0.99) and (6.21±1.02) respectively. Results of the path analysis showed that education level and age had effects on the knowledge (coefficients: 0.57 and -0.38 respectively) . Age, gender and knowledge had effects on the attitude (coefficients: -0.27, 0.12 和 0.02 respectively), and attitudes, gender, monthly income and the level of education had effects on the practice (coefficients: 0.48、 0.37、 0.06 and 0.02 respectively) . Conclusion Community residents in Hangzhou lack relevant knowledge, and there are some irrational attitudes and practices regarding antibiotic use. There is a correlation between knowledge and attitudes, as well as between attitudes and practices, but the knowledge and practices are not correlated.
4.Study on treatment of tibial avulsion fractures of posterior cruciateligament combined with anterior cruciate ligament

Objective To explore the clinical efficacy of one-stage cannulated screw fixation in treatment of tibial avulsion fracture at the insertion of posterior cruciate ligament(PCL) under posterior direction version and in treatment of tibial avulsion fracture at the anterior cruciate ligament (ACL)under arthroscopy.Methods From January 2010 to June 2015,25 patients with tibial avulsion fractures of the posterior cruciate ligament combined with the anterior cruciate ligament in our hospital were retrospectively analyzed.Firstly,arthroscopic exploration was conducted in all patients in their horizontal position,aiming to observe whether combined injures existed or not and to confirm fracture fragments and the degree of tibial avulsion fractures of the cruciate ligaments.All patients recieved arthroscopic fragment fixation by using cannulated screws,then,were treated with a minimally posteromedial incision in the prone position by cannulated screw fixation.All knee joints were protected by adjustable orthopedic brace for 4-6 weeks.The clinical knee joint function of patients were measured by using International Knee Documentation Committee (IKDC) scoring scale and Lysholm scoring scale.The X-ray or CT image were used to evaluate healing process of the fractures.Results All patients were followed up for 6-24 months,the median follow-up time was 15 months.All fractures healed in one stage,time of fracture healing ranged from 9 to 24 weeks,and the median time was 12 weeks.The average IKDC scores at the finial follow-up were as follows:claudication (4.88±0.29)points,support (4.65±0.36)points,pain (4.78±0.49)points,swelling (4.88±0.52)points,squat (4.85±0.35)points,all were higher than those before operation,there were statistically significant differences when compared with those assessed before operation (P<0.05).The average Lysholm score at the finial follow-up was (93.4±2.8)points,the rate of excellent and fine was 96.0%,no statistically significant difference was found compared with that at the healthy side (P>0.05).Conclusion The one-stage cannulated screw fixation in treatment of tibial avulsion fracture at the insertion of PCL under posterior direction version and in treatment of tibial avulsion fractures at the ACL under arthroscopy is easy and convenient to operate,which has the advantages of reliable fixation and satisfactory recent curative efficacy.
6.Investigation on mechanism of ovarian cancer scarcely developing brain metastasis

Objective To observe the intracranial tumorigenicity situation of lung cancer cell strain and ovarian cell strain by injecting them into the female nude mice with different approaches and to analyze the possible mechanism for scarce brain metastasis of ovarian cancer to provide a certain thinking for the treatment of brain metastasis of lung cancer.Methods Ovarian cancer cell strain Skov3 and lung cancer cell strain A549 were injected into female nude mice respectively through tail veins,abdominal cavity,common carotid artery and brain tissue.There were 16 nude mice in each approach.All the nude mice were killed 4-6 weeks later.Then the brain,lungs,kidneys,spleen,liver,fallopian tube,ovary and abdominal tumor mass were taken by HE staining for observing the cerebral tumorigenicity situation.Results By the caudal vein injection,there was no brain metastasis in the Skov3 strain group,while in the A549 strain group,2 cases of brain metastasis occurred.By the aabdominal cavity injection,there was no brain metastasis in the Skov3 strain group,but 2 cases of brain metastasis occurred in the A549 strain group.By the common ca rotid artery injection,there was no brain metastasis in the Skov3 strain group,but 8 cases of brain metastases occurred in the A549 group.The intracranial tumorigenicity of Skov3 strain and A549 strain had statistical difference among these 3 kinds of extracranial injection approaches(P<0.01).By intracranial direct injection approach,14 cases of brain tumorigenicity occurred in the Skov3 strain group,while 10 cases of brain tumorigenicity occurred in the A549 strain group,the difference had no statistical difference.Conclusion Blood brain barrier maybe plays an important role in preventing ovarian cancer cells from metastasizing into brain.
7.Effect of aspheric multifocal toric IOL implantation on visual acuity and contrast sensitivity in patients with cataract

AIM: To observe the effect of intraocular lens ( IOL) implantation on visual acuity and contrast sensitivity in patients with cataract.
METHODS: Fifty - eight cases ( 72 eyes ) cataract patients with regular cornel astigmatism, in our hospital from May 2014 to May 2015 were randomly divided into two groups to undergo phacoemulsification and IOL implantation: the observation group: 29 cases (36 eyes) received multifocal toric IOL implantation; the control group: 29 cases (36 eyes) received monofocal toric IOL implantation. Uncorrected distance visual acuity ( UCDVA), uncorrected near visual acuity ( UCNVA), best corrected distance visual acuity (BCDVA), the best corrected near visual acuity ( BCNVA ), total eye astigmatism, and the dark contrast sensitivity were observed for these patients at 1 and 6mo after cataract surgery.
RESULTS: There were no statistical significant difference between the two groups at postoperative 1, 6mo on UCDVA, BCNVA, BCDVA and total eye astigmatism(P>0. 05). UCNVA of observation group at 1 and 6mo were better than those of control group ( P <0. 05); there were statistically significant difference in high frequency comparison at the sixth postoperative months (P<0. 05).
CONCLUSION: Both monofocal toric IOL implantation, and aspheric multifocal toric IOL implantation for cataract with regular corneal astigmatism are effective to improve visual acuity. But the latter treatment would contribute to the improvement of uncorrected near visual acuity and the dark contrast sensitivity.
8.Effect of mirror therapy on complex regional pain syndrome (type II) after high-level spinal cord injury:a self cross-controlled study

BACKGROUND:Mirror therapy was initial y used in the treatment of affected limb pain after amputation, and has been applied as a valid way for relieving complex regional pain syndrome due to other injuries, but the efficacy for patients with upper limb complex regional pain syndrome (type II) more than 3 years after high-level spinal cord injury has not been reported in any way.
OBJECTIVE:To investigate the availability of mirror therapy for reducing pain in a patient with upper limb complex regional pain syndrome (type II) who suffered high-level spinal cord injury.
METHODS:An experimental design of self cross-control was applied for al eviating the patient’s pain with the intervention of transcutaneous electrical nerve stimulation and mirror box therapy, which were conducted 2 or 3 weeks respectively. And the elution period between two kinds of treatments was 1 week, the fol ow-up time was 1 month.
RESULTS AND CONCLUSION:The pain of the patient was not relieved, but even aggravated after 2 weeks of transcutaneous electrical nerve stimulation, while 3-week mirror box therapy significantly reduced the pain after elution period (the third week). The Hamilton anxiety scale, Hamilton depression scale, and WHO quality of life scale scores were significantly improved after treatment compared with before treatment. Experimental findings indicate that, mirror therapy may be an effective intervention for mitigating complex regional pain syndrome (type II) in addition to the comprehensive treatments, but further studies are warranted to confirm its effect.
9.Effect and safety of testosterone undecanoate in the treatment of late-onset hypogonadism: a meta-analysis.

OBJECTIVETo evaluate the efficacy and safety of testosterone undecanoate (TU) in the treatment of late-onset hypogonadism (LOH) by meta-analysis.
METHODSWe searched Pubmed (until April 1, 2014), Embase (until March 28, 2014), Cochrane Library (until April 17, 2014), CBM (from January 1, 2001 to February 2, 2014), CNKI (from January 1, 2001 to February 2, 2014), Wanfang Database (from January 1, 2000 to February 2, 2014), and VIP Database (from January 1, 2000 to Febru ary 2, 2014) for randomized controlled trials of TU for the treatment of LOH. We evaluated the quality of the identified literature and performed meta-analysis on the included studies using the Rveman5. 2 software.
RESULTSTotally, 14 studies were included after screening, which involved 1 686 cases. Compared with the placebo and blank control groups, TU treatment significantly increased the levels of serum total testosterone (SMD = 6.22, 95% CI 3.99 to 8.45, P < 0.05) and serum free testosterone (SMD = 4.35, 95% CI 1.86 to 6. 85, P < 0.05) but decreased the contents of luteinizing hormone (WMD = -2.23, 95% CI -4.03 to -0.42, P < 0.05), sex hormone binding globulin (WMD = 2.00, 95% CI 1.38 to 2.63, P < 0.05). TU also remarkably reduced the scores of Partial Androgen Deficiency of the Aging Males (WMD = -9.49, 95% CI -12.96 to -6.03, P < 0.05) and Aging Males Symptoms rating scale (WMD = -2.76, 95% CI -4.85 to -0.66, P <0.05) but increased the hemoglobin level (SMD = 2.35, 95% CI 0.29 to 4.41, P < 0.05) and packed-cell volume (SMD = 4.35, 95% CI 1.36 to 7.33, P < 0.05). However, no significant changes were shown in aspertate aminotransferase, alanine transaminase, prostate-specific antigen, or prostate volume after TU treatment (P > 0.05).
CONCLUSIONTU could significantly increase the serum testosterone level and improve the clinical symptoms of LOH patients without inducing serious adverse reactions. However, due to the limited number and relatively low quality of the included studies, the above conclusion could be cautiously applied to clinical practice.

10.Clinical effects of internal fixation with Herbert screws for the treatment of Pipkin femoral head fractures.

OBJECTIVETo explore the therapeutic effects of internal fixation with Herbert screws for the treatment of Pipkin type I and type II femoral head fractures.
METHODSFrom January 2008 to December 2012, 23 patients with Pipkin type I and type II femoral head fractures were treated with open reduction and internal fixation by Herbert screws through Kocher-Langenbeck approach. Twenty-three patients (aged 18 to 52 years with a mean of 35.5 years, including 18 males and 5 females patients, 8 left hips and 15 right hips) with femoral head fractures and posterior hip dislocation. The fracture was classified according to Pipkin classification based on the radiographic findings, 5 patients had type I and 18 had type II fractures. The duration time from admission to the operation ranged from 6 to 72 h (averaged 32 h). The clinical and radiographic outcomes of the patients were measured using Thompson-Epstein scoring scale. The Harris hip score(HHS) was used to evaluate and compare hip functions at the latest follow-up between affected and healthy sides.
RESULTSAll the patients were followed up, and the duration ranged from 20 to 48 months (averaged 30 months). According to Thompson-Epstein system, 12 patients got an excellent result, 6 good, 4 fair and 1 poor. The average HHS at the finial follow-up was 87.80 ± 8.46 (ranged from 66 to 95), which is similar to that in the healthy side 90.10 ± 6.35 (ranged from 72 to 98) (t = 1.044, P = 0.302). The complications such as deep infection, and deep vein thrombosis were not found. At the 3rd year during follow-up,4 patients had avascular necrosis of femoral head. At the 1st year of follow-up, 1 patient had hip pain after walking,screws loosening and shift after trauma,and serious complications of traumatic arthritis. All the 5 patients were treated with total hip arthroplasties.
CONCLUSIONThe treatment of internal fixation with Herbert screws through the Kocher-Langenbeck approach is effective for Pipkin type I and type II femoral head fracture. The method is reliable and valuable for recommendation. However, such fracture may have avascular necrosis of femoral head and complication of traumatic arthritis, which should be observed carefully in clinic with preparation of the prevention and treatment measures.
