5.Clinical investigation on patient-controlled epidural analgesia with low dose ropivacaine combined with fentanyl in labor

Objective:To evaluate the analgesic effect of low dose ropivacaine combined with fentanyl in patient controlled epidural analgesia (PCEA) and its influence on labor and safety of mothers and their newborns. Methods:One hundred and forty healthy nullipara at term were randomly assigned to epidural analgesia or control group. The analgesia group received PCEA when their labor stage reached active period in the first stage of labor.The PCEA mode was background infusion 6 ml/h, bonus 2 ml,lockout time 10 min/each infusion. The VAS, modified Bromage scale(MBS),duration and mode of labor, postpartum hemorrhage,Apgar scores of newborns were recorded. Results:Fifteen minutes after application of PCEA,98.58% of patients felt no pains(VAS 0.057?0.48),only 4.8% showed a mild motor block(MAS 0.07?0.31).By the 60 min,all patients could move the lower limbs freely (MAS 0). The analgesia group had a significantly shorter duration of active period in the first stage of labor than that of the control group, but a significant longer duration of the second stage. In addition, the oxytocin usage was significantly more frequently in analgesia group than those in placebo group.There was no difference between the two groups in the duration of third stage, postpartum hemorrhage, Apgar scores of newborns in 1 min and 5 min, rate of cesarean section and instrumental delivery.Conclusion:Epidural analgesia with low dose ropvicaine combined with fentanyl is safe and effective, which has no influence on mothers and their infants. It could shorten the active period of labor, but may increase oxytocin usage. The ratio of cesarean section and instrumental delivery was similar between the two groups. This kind of labor analgesia is beneficial and do no harm to mothers as well as newborns.

7.Bone graft fusion fixation for single-segment thoracic/lumbar spinal tuberculosis:effective reconstruction of spinal stabilization and deformity correction

BACKGROUND:On the basis of thorough debridement, homochronous anterior or staging posterior fixation has been a standard scheme for spinal tuberculosis. Numerous studies confirmed that above approach has obtained good effects, but the anterior approach has some disadvantages, such as complex anatomic structure, great trauma, relatively more complications, and difficult operation and fixator implantation. OBJECTIVE:To observe spinal stabilization and deformity correction in patients with single-segment thoracic/lumbar spinal tuberculosis after posterior debridement and interbody fusion. METHODS:Clinical data of 36 patients with single-segment thoracic/lumbar spinal tuberculosis undergoing one-stage posterior debridement and interbody fusion in the Guangxi Yulin Orthopedics Hospital of Integrated Traditional Chinese and Western Medicine from January 2008 to January 2012 were retrospectively analyzed. There were 2 cases in single T11/12 segment, 4 in T12/L1 segments, 6 in L3/4 segments, 22 in L4/5segments and 2 in L5/S1 segments. Of them, 24 patients suffered from different degrees of spinal nerve injury. At 6, 12 and 24 months after surgery, al patients were folowed up. Bone graft fusion, kyphosis correction, functional recovery of the spinal cord and complications were observed. RESULTS AND CONCLUSION:Al patients were folowed up for 24-38 months. Cobb angle of kyphosis and spinal stenosis rate were significantly improved at 2 years after treatment (P < 0.05). The lumbar back pain symptoms were significantly improved in final folow-up (P < 0.05), with an intervertebral fusion rate of 100%. No lesion residue and recurrence, correction loss, fixation loosening or displacement was found. These results demonstrated that in patients with single-segment thoracic/lumbar spinal tuberculosis, posterior debridement and interbody fusion can effectively reconstruct spinal stabilization, correct deformity, and promote the functional recovery of spinal nerves.
8.Correlation of vascular endothelial growth factor and aquaporin 1 with angiogenesis in rheumatoid arthritis

ObjectiveTo study the expression of vascular endothelial growth factor (VEGF) and aquaporin 1 (AQP1) in the synovium of rheumatoid arthritis (RA) patients,and their correlation with angiogenesis.MethodsThe expression of VEGF and AQP1 in the synovial tissue was detected with immunohistoehemistry in 7 patients with RA,12 patients with osteoarthritis (OA).The expression of VEGF and AQP1 positive cells and vessels was scored,and the correlation of positive cells with vessels was analyzed.Rank-sum test and Spearman's correlation analysis were used for statistical analysis.ResultsThe expression of VEGF positive cells was significantly correlated with vessels in the synovium of RA group(rs=0.971,P<0.01 ).No significant correlation was found between VEGF positive cells and vessels in synovium of the OA group (rs=0.235,P=0.462).There was no evident difference in the expression of VEGF positive cells between RA and OA group (Z=-0.390,P=0.711 ).The expressions of AQP1 positive cells were significantly correlated with vessels in the synovium of RA group (rs=0.828,P=0.022).No significant correlation was found between AQP1 positive cells and vessels in the synovium of OA group (rs=0.396,P=0.203).There was no notable difference in the expression of AQP1 positive cells between RA and OA group (Z=-0.302,P=0.773).The expressions of VEGF positive cells were significantly correlated with AQP1 positive cells in RA group (rs=0.891,P=0.007).No significant correlation was found between VEGF positive cells and AQP1 positive cells in OA group (rs=0.338,P=0.283).ConclusionThe expressions of VEGF and AQP1 are significantly correlated with angiogenesis in the synovium of RA,and they play an important role in the pathology of RA.Moreover,there may be synergistic effect between VEGF and AQP1 in RA.

Arbuscular mycorrhizal fungi can infect the roots of Cercidiphyllaceae and form arbuscular mycorrhizae.Infection rate is from 12% to 30%,but infection strength is weak.4 species of Acaulospora and 11 species of Glomus were isolated and identified,including Acaulospora tuberculata,A.spinosa,A.foveata,A.denticulate,Glomus geosporum,G.clarum,G.constrictum,G.monosporum,G.laroideum,G.versifome,G.microcarpum,G.mosseae,G.hoi,G.halonatum and G.reticulatum.Among them,G.cercidiphyllorum is advantage species.So AMF may be a potent resource of biology which can stimulate the growth of Panax notoginseng.
