1.Preparation and Pharmacokinetics of Nevirapine Nanosuspensions in Rats

Objective:To prepare nevirapine nanosuspensions ( Nev-NS) and study the pharmacokinetics in rats. Methods:Nev-NS was prepared by a high pressure homogenization technology. The particle size, PDI and Zeta potential of the nanosuspensions were used as the indices to determine the influencing factors in the preparation process. Nevirapine plasma concentration was detected by HPLC and the pharmacokinetic parameters were calculated by 3P97 software. Results: The particle size, PDI and Zeta potential of Nev-NS was (456. 1 ± 72. 1) nm, 0. 441 ± 0. 072 and ( -24. 4 ± 4. 7) mV, respectively. AUC0-12 of Nev and Nev-NS was (7. 57 ± 0.52) and (11.72 ±1.83) mg·h·L-1, t1/2 was (2.45 ±0.31) and (3.16 ±0.39) h, Tmax was (1.43 ±0.38) and (1.61 ± 0. 32) h and Cmax was (1. 62 ± 0. 42) and (3. 15 ± 0. 52) mg·L-1 , respectively. Conclusion:Nev-NS can improve the pharmacoki-netic behavior of Nev in rats significantly, and obviously enhance the bioavailability when compared with nevirapine suspensions.
2.Research progress and application of the aortic taper angle

Endovascular repair has gained worldwide popularity for the treatment of aortic dissection and aortic aneurysm.The non-tapered stents are selected usually.The aortic diameters vary in different level.High incidence of distal stent-induced complications is closely related to the aortic taper angle.According to several published studies,the aortic taper angle could be evaluated by establishing geometric model.Applications of tapered stents can significantly reducing the incidence of distal stent-induced comnplications.This review summaries the latest researches in the studies of the progress and the application of the aortic taper angle and the tapered stents,providing a reference for endovascular stent-graft selecting in the treatment of aortic dissection and aortic aneurysm.
3.Management of hypogastric artery in endovascular repair of abdominal aortic aneurysm

Objective To summarize our experiences of management for hypogastric artery in endovascular repair(EVR) of infrarenal abdominal aortic aneurysm(IAAA).Methods From July,1997 to March,2007,62 cases of IAAAs required special management of the internal iliac artery(IIA) during the EVR.Among them,57 cases were type C AAA who included 35 cases of unilateral common iliac artery(CIA) bifurcations and 22 cases of bilateral iliac bifurcations.The remaining 5 cases involved superior iliac bifurcations.Various techniques including simple coverage by stent-graft,embolization of the IIA trunk combined with stent-graft covering,reconstruction of the IIA and combination of the above techniques were applied according to the different conditions of the IIA involved.Results None of the 62 patients required conversion to surgical repair.Immediate post operational angiography demonstrated type Ⅰ endoleak in 6 cases(9.7%).Claudication due to gluteus ischemia occurred in 5 cases(8.1%),leg numbness in 1 case(1.6%),and severe constipation in 1 case(1.6%),but no buttock and colorectal necrosis was recorded.Conclusion It is important to preserve one of the hypogastric arteries and avoid occlusion of both hypogastric arteries in EVR of IAAA.
4.Feasibility study about modified ligation of the intersphincteric fistula tract in the treatment of low anal fistula

Objective To assess the efficacy and safety of modified ligation of the intersphincteric fistula tract (LIFT) for low anal fistula.Methods We follow-up visited 20 patients with low anal fistula underwent modified LIFT procedures (Since the outer edge shape of anal fistula incision fistula and the branch into the intersphincteric groove,proximal lateral internal sphincterotomy ligation.The wound was closed to the whole layer of closed suture after removal of the pipe wall).Median follow-up duration was 10(range 3-15) months.To compile statistics on the wound healing time,the clinical healing rate,the recurrence rate and the fecal incontinence score (Wexner score) of those patients.Results The wound healing time was (15.3 ± 4.8) d.The prime success rate of fistula healing was 75 % (15/20) after the modified LIFT procedure.2 cases of low complex anal fistula presented with wound infection,and 3 cases (including 1 cases of simple low anal fistula,2 cases of low complex anal fistula)had fistula with infection.They were cured after appropriate treatment.During follow-up of 3 to 15 (median 10)months,1 cases of simple low anal fistula recurred in third months after surgery.To the end of the follow-up,the total clinical healing rate was 95% (19/20),of which 12 cases of low complex anal fistula total cure rate was 100% (12/12).Preoperative fecal incontinence scores (Wexner score)were all 0 points.At the final follow-up,18 (90%)cases of fecal incontinence score were 0 points,1 (5 %) cases were 2 points and 1 (5 %) cases were 1 points.Conclusion Modified LIFT is better balance the relationship between cure rate and anal function.The sphincter preservation is complete.The fistula is thoroughly cleared.The cure rate is high.The recurrence rate is low.Does not cause anal defects and deformities.It has little influence on the function of anal control,the operation is simple,and it is suitable to be popularized.
5.Allogeneic bone for repairing bone defects after resection of benign bone tumor and tumor-like lesions

BACKGROUND: Bone defects forming after resection of bone tumor and tumor-like lesion are often packed with autogeneic bone. But sample amount cannot completely meet the clinical demand and different degrees of complications are often left in the bone graft donor region. All these problems remarkably limit the application of autogeneic bone. Allogenic bone is increasingly widely used in the clinical practice due to its structure and biological characteristics similar to autogeneic bone, rich sources, long-term preservation, easy to use and other advantages. OBJECTIVE: To investigate the biocompatibility and clinical application effects of allogeneic bone in filling and repairing benign bone tumor and tumor-like lesion after resection and curettage.DESIGN: A retrospective analysis. SETTING: Department of Pelvis Surgery, Second Affiliated Hospital of Inner Mongolia Medical College. PARTICIPANTS: Totally 230 patients (156 males and 74 females, with age of 5-56 years) were admitted to Department of Bone Tumor, Second Affiliated Hospital of Inner Mongolia Medical College from December 1999 to December 2005 to undergo curettage and hyperthermia inactivation due to benign bone tumor and tumor-like lesion and to receive the treatment by filling and repairing bone defects with lyophilized small-segment allogeneic bone, and were recruited for this study. Written informed consents of treatment were obtained from all the patients. The protocol was approved by the Hospital's Ethics Committee. METHODS: Allogeneic bone grafts (Shanxi Aorui Biomedical Co.,Ltd /Shanxi Provincial Medical Tissue Banking) were used to fill and repair bone defects. Patients who had benign bone tumor, bone cyst or osteofibrous dysplasia underwent cyst curretage. Allogeneic bones were used to pack empty cavity. Therapeutic effects were assessed according to the scoring criteria of allogeneic bone transplantation from Mankin et al, consisting of satisfactory and unsatisfactory two levels. X-ray plain films of surgery sites were taken at postoperative 3, 6 and 12 months. The patients were followed up for 38 months on average in order to observe the therapeutic effects. MAIN OUTCOME MEASURES: Histocompatibility of allogeneic bone in filling and repairing bone tumor defects. RESULTS: All 230 patients participated in the final analysis. ① Biocompatibility of allogeneic bone: Postoperatively, minority of patients had mild immunological rejections. Such allogeneic bone grafts had a good biocompatibility. They could directly fuse with bone tissue in the implantation location of patients, but not inhibit the normal activity of osteocytes on the allogeneic bone grafts or interfere the natural substitution of autogeneic osteocytes, i.e. there were no or less immunological rejections. Bone union was obtained in all the patients at postoperative 6-18 months (6.5 months on average). Thirty-four patients presented exudation of light yellow liquid from incision. Incisions healed in 30 (14.8%) patients at postoperative 2 weeks and in 4 (1.7%) in later time. Altogether 196 (85.2%) patients obtained satisfactory therapeutic effects, but 34 (14.8%) obtained unsatisfactory therapeutic effects. CONCLUSION: Small-segment allogeneic bones have good histocompatibility and osteogenesis, and they are good bone grafts in the bone transplantation.
6.The effect of rosiglitazone combined with insulin on plasma aldosterone in diabetes patients

Objective To investigate if edema caused by rosiglitazone is associated with the change of aldosterone level. Methods Plasma aldosterone levels in different periods from patients treated with rosiglitazone combined with insulin were measured.Type 2 diabetes patients who have been treated with insulin alone were divided into two groups: patients treated with rosiglitazone(4mg/d) combined with insulin(RSG+insulin group), or treated with insulin alone (insulin group)(n=10). Aldosterone levels, incidence of edema and weight gain were monitored every two months during six months. Results The incidence of edema and weight gain were higher in RSG+insulin group than in insulin group. After two months, aldosterone levels were higher in RSG+insulin group than in insulin group[(77.8±25.9)vs(60.2±27.6)pg/ml,P>0.05],but it had no statistical difference; after four months, aldosterone levels were higher in RSG+insulin group than in insulin group[(87.2±27.1)vs (61.5±25.6) pg/ml,P<0.05]. After six months, aldosterone level recovered gradually,it had no statistical difference compared with that of control[(77.0±21.0)vs(69.6±21.7) pg/ml,P>0.05]. Conclusions The cause of early edema in type 2 diabetic patients treated with rosiglitazone combined with insulin was probably assosiated with elevated aldosterone level.
7.Polymorphisms in The 5' flanking Region of IGF-Ⅰ Gene are Associated With Cashmere Fibre Traits in Liaoning Cashmere Goats

Insulin-like growth factorⅠ(IGF-Ⅰ) is essential for the growth and differentiation of hair follicles which is an important part of wool and cashmere.But there is no report on polymorphisms of the IGF-Ⅰ gene in cashmere goat,and also few candidate genes for cashmere production traits have been reported in cashmere goats.The objectives of this work were to detect the single nucleotide polymorphism(SNP) in the 5' flanking region of IGF-Ⅰ gene and to determine their association with fibre traits in Liaoning cashmere goats.The fibre traits data investigated in the experiment were combed cashmere weight,cashmere fibre length and cashmere fibre diameter.A few individuals of the Liaoning cashmere goats,selected according to phenotypic character,were used for SNPs detection in the 5'flanking region of IGF-Ⅰ gene,and four point mutation G→C(388 bp),A→G(668 bp),A→ C(719 bp),G→A(752 bp) were identified,which result in a CdxA bonding-site lack,and score increase of C/EBP site in the 5' flanking region compared with the wild type.Then different genotypes were detected in 520 Liaoning cashmere goats using create restriction site(CRS) and polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP),three genotypes,AA,AB and BB were observed coded for by two different alleles A and B for each SNP,and 13 diplotypes were identified in the groups of the four SNPs.The relationships between the genotypes and combed cashmere weight,cashmere fibre length and cashmere fibre diameter were analyzed.The statistical analysis showed that animals with the genotype AA of SNP2 in Liaoning cashmere goats had the thinnest cashmere fiber diameter compared with those with the AB and BB genotype(P
8.Determination of Paracetamol and Caffeine in New compound Folium Isatidis Tablets by HPLC

Objective:To determine the contents of paraceltamol and caffeine in New compouad Folium Isatidis Tablets simultaneously by HPLC. Methods:The determination was carried out with C 18 chemical bonded silica gel as a solid phase, methanol-water (25∶75) as a mobile phase and UV deterction wavelength at 215 nm. Results: The average recovenies of the added sample were 99.6%( RSD=0.67, n=5) for paracetamol and 99.3%(RSD=0.58, n=5) for caffine, There was a good linear relationship between the concentration and absorption area value in the rang of 1.6?g~6.4?g for paractamol or 0.16?g~0.64?g for caffeine.Conclusions: The method is simple, quick and accurate.
9.The cleaning of plaque biofilm on the surface of macromolecule ocular prosthesis material

BACKGROUND:The micro-ecological environment has been broken when the ocular prosthesis was inset into the conjunctival sac. The recede of self cleaning function is more conducive to the microbial growth and colonization. The cleaning of plaque biofilm on ocular prosthesis surface affects the patient's wearing comfort and quality of life. It is necessary to seek an effective cleaning method. OBJECTIVE:To compare the clearance effect of five cleaning methods on the palque biofilm on ocular prosthesis surface. METHODS: The conjunctival secretions from 84 patients who were subjected to ocular prosthesis repair were taken for bacterial culture and identification. Fifty pieces of self-curing resin and thermosetting resin artificial eyes were produced. The artificial eyes in each group were randomly divided into five groups, and were cleaned respectively with clear water, volume fraction of 75% ethanol, Boston SIMPLUS, polident and toothpaste. After the completion of the cleaning, the test piece was conducted residual biofilm culture. The clearance effects of different processing modes were evaluated using colony counting method. RESULTS AND CONCLUSION: Eighty-four specimens were submitted for inspection, of which 49 were positive. The Staphylococcusaureus separation rate was 14.29%.Staphylococcus epidermidis separation rate accounted for 13.10%. Maxwel Corynebacterium separation rate accounted for 7.14%. When water, Boston SIMPLUS and toothpaste were used for cleaning, theStaphylococcus aureus colony number in the self-curing resin group was higher than that in the thermosetting resin group (P< 0.05); when ethanol and polident were used for cleaning, there was no difference in the Staphylococcus aureus colony number between these two groups. In self-curing resin, the colony count in the clear water treatment group was higher than that in the other treatment groups (P < 0.05). The colony count in the ethanol treatment group was lower than that in the Boston SIMPLUS group (P < 0.05). There was no significant difference in the colony count between other groups. In thermosetting resin, the colony count in the clear water treatment group was higher than that in the other treatment groups (P < 0.05). There was no significant difference in the colony count between other groups. These results demonstrate that ethanol, Boston SIMPLUS, polident and toothpaste have better cleaning effects onStaphylococcus aureusbiofilms on the surface of two kinds of ocular prostheses than the clear water rinse. Overal, it is encouraged to clean the artificial eyes using polident and Boston SIMPLUS, in order to avoid the occurrence of microbial infection in the conjunctival sac after wearing ocular prosthesis.
