1.Management of multiple segment lumbar pedicle and vertebral body fractures combined with spondylolisthesis

Objective To evaluate neurofunctional and radiographic results of transpedicular screw fixation reduction and anterior column fixation with use of screw-red system in treatment of multiple segment lumbar pedicle and vertebral body fractures combined with spondylolisthesis. Methods A consecutive series of 12 patients with unstable multiple segment lumbar pedicle fracture, vertebral body fracture, spondylolisthesis and neurologic deficit were managed with posterior transpedicular screw fixation including fractured pedicle and anterior screw-rod fixation instrumentation from January 2002 to December 2007. Results Patients were followed up for 24-30 months (mean 26 months). All the patients with incomplete neurologic deficits got improvement by at least one Frankel grade. Transpedicular screw brought satisfactory reduction. At the time of the latest follow-up, no screw breakage occurred. Con-clusions Excellent reduction of unstable multiple segment lumbar pedicle fractures combined with spon-dylolisthesis can attain better reduction and maintenance by means of selective pedicle screw fixation via fractured pedicle and anterior screw-rod instrument.

Objective To evaluate X-ray,CT and MRI findings of soft tissue infections in AIDS. Methods Three cases of soft tissue infections with AIDS were retrospectively analyzed by comparing the imaging findings with pathological results.All patients were performed MRI,X-ray was in 1 case,CT was in 1 case.Results Cellulitis was in 1 case:MRI showed extended thickening of subcutaneous tissues, ill-defined hypointense areas on T_1WI and hyperintensity on T_2WI,and reticular pattern on GRE. Necrotizing fasciitis was in 1 case:MRI showed obvious thickening of subcutaneous tissues and deep fasciae, abnormally increased signal intensity on T_1 and T_2WI.Fluid collections were within muscles and muscles interval on fat-suppressed T2 WI.Tuberculosis was in 1 case:CT demonstrated multiple low density areas in the subcutaneous tissues and clear peripheral rim enhancement.MRI appeared hypointense on T_1WI and hyperintensity on T_2WI,and peripheral rim enhancement following gadolinium injection.Conclusion Infections of soft tissue are common complication in patients with AIDS,radiology is important in early diagnosis and treatment planning in this population.

OBJECTIVEThe exercise rehabilitation in patient with chronic heart failure (CHF) is standard clinical practice, but it is rare using CardioPulmonary Exercise Testing (CPET) guide to prescribe exercise rehabilitation in China.
METHODSWe performed symptom limited maximal CPET in 10 patients with CHF, randomly divided into two groups: 5 patients as control without exercise and 5 exercise patients used Δ50%W intensity to exercise 30 min/d, 5 d/w, x12 w. Before and after 12 w rehabilitation, we evaluated functions.
RESULTSThere were no significant difference between two groups patients (P > 0.05). The exercise duration was increased from 8 min to 23 min after rehabilitation (P < 0.001); distance 6 minutes walking was increased from 394 m to 470 m (P < 0.05); score of Minnesota quality of life was decreased from 25 to 3 in exercise group (P < 0.01). However, there were nosignificant changes in control group (P>0.05) and their changes were smaller than those in exercise group (P < 0.01).
CONCLUSIONThe CPET guiding exercise rehabilitation is safe and effective for patients with CHF.

5.A descriptive study on the smoking pattern among general population in Xuzhou,2008

Objective To understand the prevalence of smoking in different sub-populations in Xuzhou area so as to develop effective tobacco control policies. Methods Through multi-stage randomized cluster sampling,a face-to-face study with standard questionnaire was carried out among residents aged above 15 years,from June to December 2008,with descriptive nature. Results Totally,44 686 people,with 21 524 males and 23 162 females at age 15 and over were investigated,including 34 391 of them from rural areas and 10 295 from the urban population. Data regarding rates on ever smoked and current smoking,regular smoking,heavy smoking,and average numbers of cigarettes smoked per day,types of tobacco products smoked,rates on quitting smoking successfully,relapse,passive smoke exposure etc.,were 22.45%,21.40%,15.49%,9.49%,15.09/d,4.68%,5.91% and 14.12% respectively. There were no significant differences in the rates of overall smoking,current smoking and regular smoking among urban and rural residents (P>0.05). However,rates of successfully quitting smoking and passive smoke exposure in the urban areas were higher than those in the rural areas (P<0.05). Rates regarding relapse,heavy smoking and average numbers of cigarettes smoked per day in the rural areas were higher than those in the urban areas(P<0.05). Rates on smoking,current smoking,regular smoking,heavy smoking,relapse and average numbers of cigarettes smoked per day were higher in males than those in females (P<0.05). Rates of quitting smoking successfully and passive smoke exposure were higher in females than those in males (P< 0.05). The major types of tobacco products smoked by people aged 15 years old and above were cigarettes (85.17%),and Chinese pipes (3.24%). Conclusion Smoking was quite common in people from Xuzhou. Our data suggested that health education should be strengthened and sustainable intervention measures be developed and implemented to control the tobacco use in the area.
6.Therapeutic effect of diet guidance on dysphagia after stroke

Objective :To observe therapeutic effect of diet guidance on dysphagia after stroke .Methods :A total of 90 patients with dysphagia after stroke were randomly and equally divided into traditional rehabilitative training group (received traditional swallowing rehabilitative training ) and diet guidance group (received diet guidance based on traditional rehabilitative training group ) ,and 30d was treated as a course .Kubota drinking test was used to assess dysphagia before and after treatment ,and therapeutic effect was assessed by Fujishima Ichiro criteria of swallowing therapeutic effect evaluation .Results : Compared with before treatment , there was significant rise in swallowing function score in both groups after treatment ,and that of diet guidance group was significantly higher than that of traditionalrehabilitativetraininggroup[(8.72±1.33)scoresvs .(6.14±1.78)scores,P=0.001all].Totaleffec-tive rate of diet guidance group was significantly higher than that of traditional rehabilitative training group (88. 9%vs.68.9%, P= 0.020).Conclusion : Compared with pure swallowing rehabilitative training ,diet guidance com-bined swallowing rehabilitative training can significantly improve dysphagia in patients with stroke .
7.HPLC determination of insulin and its related substances in insulin powder for inhalation.

AIMTo determine insulin and its related substances in insulin powder for inhalation by reversed phase high performance liquid chromatography method.
METHODSThe initial mobile phase was solution A (0.1% trifluoroacetic acid-acetonitril 70:30) and changed to solution B (0.1% trifluoroacetic acid-acetonitril 60:40) in 30 minutes. The flow rate was 2.0 mL.min-1, the column temperature was 30 degrees C, the wave length was 280 nm, the injected volume was 20 microL.
RESULTSInsulin was well separated from other peaks induced in different conditions. There was good linear relationship between the amount of insulin and its peak area, the RSD was 1.1%, the insulin solution for determination was stable in 12 hours, and the quantity detected was near the added.
CONCLUSIONThe method is simple and accurate to detect insulin and its related substances in insulin and its preparations.

8.The overview of the property system and characterization techniques of Chinese medicine materials before the pharmaceutical

At present, the modernization of Chinese medicine preparations (CMPs) is still a challenging task. The 3 typical Chinese medicine materials (CMMs) used for preparing CMPs are the powders, extracts, and components of Chinese medicine and their properties of CMMs are important for designing CMPs. Basing on our long term research, we have established a property system for CMMs according to the state of CMMs under an exactly condition and according to the interaction characteristics between substances. The property system could be divided into 5 categories: material composition, spatial structure, body property, surface property, physicochemical properties, and they could also be divided into 18 subcategories. Furthermore, we also established the corresponding index and characterization system, where the 61 indexes and characterization techniques were systematically summarized. At last, we hope that the article will promote the modernization of CMPs.
9.Potential of Ozone Therapy for Post-stroke Pneumonia (review)

Pneumonia is the most common type of post-stroke infection, with an incidence of up to 57%, and is independently associated with poor outcome after stroke. Ozone is a harmless therapy working for anti-inflammatory, anti-oxidative stress and improving oxygenation through various molecular ways, which may be a potential therapy for post-stroke pneumonia.
10.Influence of donor physiological condition on pharmacokinetics of mycophenolic acid and its metabolite in the early period after renal transplantation

Objective To investigate the influence of the physiological conditions and kidney function of the donors on their recipients ’ myco-phenolic acid ( MPA) and mycophenolic acid glucuronide ( MPAG) levels in the early period after renal transplantation.Methods Forty-two pa-tients receiving mycophenolate mofetil ( MMF) and 23 patients receiving enteric-coated mycophenolate sodium ( EC-MPS) , as well as their do-nors were included in this study.For patients who recieving MMF , MPA and MPAG concentrations were detected by HPLC , then AUCMPA and AUCMPAG were calculated by the limited sampling strategy (LSS) model es-tablished by our lab previously.For the patients receiving EC -MPS, AUCMPA and AUCMPAG were calculated by linear trapezoidal rule.Results Spearman test showed significant correlation between recipients ’ AUCMPAG and the age of donors in MMF group , EC-MPS group and the total cohort (P<0.05).Moreover, AUCMPAG in the recipients of elder group (>55 years old) was significantly higher than that in the younger group (≤55 years old) in both MMF group and the total cohort (P<0.05).In addition, AUCMPAG was siginificantly associated with the urea levels of donors before renal transplanta-tion in MMF group and the total cohort ( P<0.05 ).Patients were divided into tacrolimus ( FK506 ) or cyclosporin A ( CsA) group according to their comedication.AUCMPAG was significantly higher in CsA group than that in FK 506 group ( P<0.01 ).AUCMPAG in the recipients of elder group was significantly higher than that in the younger group ( P<0.05), and AUCMPAG was siginificantly associated with the urea levels of donors in FK 506 group (P<0.05).In CsA group, significant correlation between AUC MPAG and the age of donors was showed (P<0.05).Conclusion Our data suggested that the age and the urea level of donors might play roles in the recipients ’ MPAG exposure in the early period after renal transplantation .
