Objective To explore the value of early arterial blood lactate clearance rate and central venous oxygen saturation (ScvO2 ) monitoring in patients with severe pneumonia .Methods A total of 56 patients with severe pneumonia treated during the period from January 2012 to December 2013 were reviewed retrospectively .The arterial blood lactate concentration and ScvO2 were determined immediately after admission to hospital and 6 hours after admission to calculate the 6‐hour lactate clearance rate .APACHE II score was evaluated at the same time .The patients were divided into three groups according to APACHE II score:10 to ＜20 (group A) ,20 to ＜30 (group B) ,≥30 (group C) .The three groups were compared in terms of early arterial blood lactate clearance rate and ScvO2 and estimate their association with the prognosis of patients .Results The initial arterial blood lactate concentration did not show significant difference between the three groups (P>0 .05) .The early lactate clearance rate and ScvO2 at 6 hours in group C were significantly lower than in group B .The values in group B were significantly lower than in group A (P＜0 .05) .The mortality rate in group C was significantly higher than in group B ,and the mortality rate in group B was significantly higher than in group A (P＜0 .05) .The APACHE II score of the dead patients was higher than that of the suevivors ,but early lactate clearance rate and ScvO2 level were lower than those of the survivors (P＜0 .05) .APACHE II score was negatively correlated to early lactate clearance rate (r=0 .661 ,P＜0 .01) and early ScvO2 level (r=0 .579 , P＜ 0 .01) .Conclusions Early lactate clearance rate and ScvO2 level are good indicators to reflect the severity of disease and predict the outcome in the patients with severe pneumonia .

Aiming at local field potential, the present paper introduces a method of estimating lag of neuron activities between brain areas based on windowed Harmonic wavelet transform (WHWT). Firstly, the WHWT of signals of two brain areas are calculated. Secondly, the instantaneous amplitude of the signals is calculated and finally, these amplitudes are cross-correlated and the lag at which the cross-correlation peak occurs is determined as the lag of neurons activities. Comparing with amplitude cross-correlation based on Gabor wavelet transform (GWT) or Hilbert transform (HT), this method is more precise and efficient in estimating the directionality and lag.

Objective To observe the clinical efficacy of Ningmitai capsule combined with Tolterodine in the treatment of overactiver bladder after transurethral resection of the prostate(TURP).Methods 63 TURP patients with bladder disease (OAB) in our hospital from October 2012 to October 2012 were selected and randomly divided into a treatment group (n=32) and a control group (n=31). From the date of operation, the control group was treated with tolterodine 2 mg, 2 times a day, while the treatment group was additional treated with Ningmitai capsule 4mg, 3 times a day based on the control group. All patients stopped to take medicine 5 days after catheter removal. The pain frequency and duration of bladder spasm after TURP in catheterization period and the urine volume per time, the number of urgent incontinence, and the number of urgent micturition average day in automatic micturition period were scored by OABSS.Results The number of bladder spasm in the first postoperative day(3.5 ± 0.5vs.4.4 ± 0.8,t=2.650), the second day(1.5 ± 0.9vs.1.8 ± 0.2,t=2.350) and the third day (0.4 ± 1.6vs. 1.1 ± 1.8,t=2.210) of the treatment group were all less than the control group (P>0.05). The 24 h average frequency of urination after catheter removal (6.2 ± 1.3vs. 9.4 ± 1.8,t=2.710), the average number of nocturia (1.5 ± 0.4vs. 3.9 ± 1.0,t=2.580), the average number of 24h urinary urgency (1.1 ± 0.3vs. 3.2 ± 0.8,t=2.660), the average number of incontinence in 24 h (0.5 ± 0.2vs. 2.4 ± 0.6,t=2.700) and OABSS total score  (4.6 ± 1.2vs. 6.9 ± 2.1,t=2.470) of the treatment group were all better than the control group (P<0.05). Conclusion Ningmitai capsule combined with tolterodine in the treatment of overactiver bladder after TURP has significant clinical effect, helping patients recovery and improving quality of life.

Objective To investigate the mechanism by which hydrogen(H2) helps prevent acute lung injury induced by hyperoxia (HALI) in rats.Methods Thirty male Sprague-Dawley rats were randomly divided into three groups: control group, HALI group and H2 group, with 10 rats in each group.The control group was exposed to air at atmospheric pressure.Rats in HALI and H2 groups were exposed continuously to pure oxygen (100%O2) for 60 hours and during this period, 10 ml/kg of normal saline or H2-saturated normal saline was given every 12 hours by intraperitoneal injection to the HALI and H2 groups, respectively.After treatment, the arterial partial pressure of oxygen was examined and histopathological examination was conducted in each group.Then,RT-qPCR and Western blotting were performed to measure the transcriptional level and protein expression of heme oxygenase 1 (human heme oxygenase 1, HO-1) in rat lung tissue.Results Compared with the HALI group, H2 group showed significantly decreased severity of lung injury and a marked increase in the arterial oxygen saturation.Besides, H2 treatment induced up-regulation of HO-1 mRNA and protein levels.Conclusion The findings suggest that HO-1 may play an important role in the protection against HALI by H2.

A large ischemic stroke caused by malignant middle cerebral artery infarction can lead to severe cerebral edema, even cerebral hernia. This condition deteriorates and progresses rapidly, usually leading to death. Due to limitations of conservative treatments, decompressive hemicraniectomy becomes a favored option in clinic. Recent years, a series of ex?perimental studies and clinical trials achieved reliable evidence that decompressive hemicraniectomy is an effective treat?ment for patients who suffer from malignant middle cerebral artery infarction. With the successful surgical procedure, most patients benefit from a reduced mortality and improved functional outcome without severe disability. Nevertheless, plenty of issues about decompressive hemicraniectomy still need to be solved. Here, we reviewed researches of early diagnosis of ma?lignant middle cerebral infarction and the application of decompressive hemicraniectomy as its treatment.

Objective To investigate the reproductive outcome after high intensity focused ultrasound (HIFU ) ablation for pa-tients with adenomyosis who have fertility requirements .Methods From October 2010 to June 2013 ,38 patients with adenomyosis who have fertility requirements treated by HIFU in our hospital were analyzed .Results In 38 cases ,the menorrhagia severity score ,the dysmenorrhea pain score and adenomyosis lesions after HIFU ablation were significantly different from the levels before HIFU treatment(P<0 .05) .Successful pregnancy occurred in 15 patients ,full-term birth was seen in 7 patients(three of Cesarean section ,four of vaginal delivery);the remaining patients accept routine prenatal examination .All pregnant patients showed no ab-normality .Conclusion Although large sample size study with long-term follow up is need ,based on our results ,HIFU ablation for patients with adenomyosis might improve the pregnancy rate safely .
