3.Observation of the effect of pretreatment with ibutilide and direct current on cardioversion of patients with persistent atrial fibrillation and the peri-treatment care
Chinese Journal of Practical Nursing 2012;28(8):19-21
ObjectiveTo investigate the success rate and energy requirements of ibutilide and direct current(DC)cardioversion, summarize the nursing experience.MethodsForty-six patients with persistent atrial fibrillation were enrolled and underwent DC cardioversion were selelct from May 2009 to January 2011.23 patients received pretreatment with ibutilide before DC (the ibutilide group),23 patients received non drugs before DC (the control group). The cardioversion energy started at 100 J and increased if patients could not acquire sinus rhythm. All the patients were given pertinent care before, during and posttreatment. The success rate and energy requirements of the two groups were compared.Results95.7% of the patients were successfully cardioverted in the ibutilide group, which was significantly higher than 74.0% of the control group. The energy required for cardioversion was ( 139.1±45.1 ) J in the ibutilide group, significantly lower than (197.8±53.3) J in the control group.Non patients terminated treatment or appeared complication because of improper nursing.ConclusionsPretreatment with ibutilide enhanced the efficacy of DC cardioversion for the patients with persistent AF at lower energy requirements.Proper nursing care can ensure the smooth process of treatment and decrease the complications.

AIM: To investigate the immunoloregulation effect of Angelica polysaccharide extracted from Angelica Sinensis (Oliv.) Diels. METHODS: Index of immune organs were weighed and calculated. Phagocytosis of mononuclear macrophage (M) were determined with carbon particle clearance test. Spectrophotography was used to estimate levels of serum hemolysis IgG, IgM. The cell proliferation was measured by MTT assay. RESULTS: In certain doses (10-100 mg/kg), Angelica polysaccharide significantly increased phagocytic function and spleen index in immunosuppression mice caused by hydrocortisone, while showed no obviouse influnence on thymus index. It also remarkble decreased levels of serum hemolysis IgG, IgM. In vitro experiments, Angelica polysaccharide could increase the proliferation of mouse spleen cell and macrophage. CONCLUSION: These results suggest that Angelica polysaccharide has potent enhancement on non specific immunity, however, it could suppress humoral immunity.

Aim To establish a mouse model of AIDS and observe the effects of Combivir,an anti-AIDS drug. Methods The model was set up by infecting infant mice with L6565 murine leukemia virus.Quantitative RT-PCR method was used to measure viral load in plasma. The percentage of CD4~+ and CD8~+ lymphacytes was estimated by flowcytometric method. The number of WBC,RBC and marrow karyote was counted.indexes of immune organs were weighed and calculated. Results Compared with control, the viral load in plasma in the model was at high level. The percentage of CD4~+ lymphocyte, the ratio of CD4~+/CD8~+,the number of RBC and marrow karyote, and thymus index were significantly decreased in the model, while the number of WBC and spleen index was remarkably increased. Conclusion AIDS patient symptom is shown in mouse infected with L6565 murine leukemia virus. This suggests that this model is likely to be used as a mice model to evaluate the effect of anti-AIDS drugs.

AIM: To develop an HPLC method for determintion of alliin concentration in rat plasma and to study its pharmacokinetics in rat. METHODS : The plasma samples were extracted with methanol. The analysis involved a ODS-1 column as stationary phase and distilled water as mobile phase. The flow rate was 0.5mL?min -1 ,UV detection wavelength was at 220nm. 5-fluorouracil was used as the internal standard. RESULTS : The calibration curve was linear over the range from 3?g?mL -1 to 75?g?mL -1 with a correlation coefficient of 0.9989 . The mean recovery was 95%. The RSD of within-day and between-day were all less than 5%. The HPLC method of determination of alliin in the plasma was established. After single dose of 300mg?mL -1 in 6 rat,the main pharmacokinetic parameters were estimated to be as follows: CL( 0.048 )mg?min -1 ?kg -1 ,K_ 12 ( 0.0071 )min -1 ,K_ 21 ( 0.0093 )min -1 ,K_a(0.1915)min -1 ,t_ 1/2? ( 26.85 )min -1 ,t_ 1/2? ( 131.15 )min -1 , AUC( 6228.48 )?g?min -1 ?mL -1 . CONCLUSION : This method is quick, precise and reliable. It is shown that alliin is absorbed quickly in rat.

AIM To investigate the immunolore gulation effect of Angelica polysaccharide purified from Angelica sinensis(Oliv.)Diels. METHODS Index of immune organs was weighed and caculated. Phagocytosis of mononuclear macrophage (M?) was determined with carbon particle clearance test. Spectrophotography was used to estimate levels of serum hemolysis IgG, IgM. Mixed lymphocyte reaction was measured by MTT assay. RESULTS In certain concentrations (10~100 mg?kg -1 ), Angelica polysaccharide significantly in creased phagocytic function and spleen index inboth normal and immunosuppression mice, caused by sc cyclophosphamide, but showed no obviouse influnence on thymus index. It also remarkbly decreased levels of serum hemolysis IgG, IgM. CONCLUSION These results suggest that Angelica polysaccharide has potent enhancement on non specific immunity. However, it could suppress humoral immunity.

Object To investigate the effects of Angelica polysaccharide (AP 0) and its sulfates (APS) on blood coagulation and platelet aggregation. Methods Infrared turbidimetric method was used to estimate platelet aggregation, prothrombin time (PT) and thrombin time (TT), actiated partial thromboplastin time (APTT); bleeding time (BT) was measured by cutting the mouse's tail and coagulate time (CT) was measured by glass method sheet; hemorheology was measured with heparin as anticoagulant. Results The AP 0 and APS prominently enhanced the platelet aggregation at 5 min (PAG ), while showed less effects on the maximum platelet aggregation (PAG ); they, especially APS 1, also markedly prolonged CT but shortened BT, both AP 0 and APS 1 significantly prolonged TT and APTT in all experimental dosage, while only APS 1 in high dosage (8.46 mg/kg) significantly prolonged PT; AP 0 increased the aggregation index (AI) of RBC and the whole blood viscosity (? b) at shear rate of 5 s -1 at a dose of 2.00 mg/kg, however, unlike AP 0, APS 1 decreased AI and ? b at shear rate of 5 s -1 and 230 s -1 . Conclusion These results suggest that AP 0 and APSs have potent anticoagulant and haemostasis effects. The haemostasis effect is related to promoted platelet aggregation. Ameliorative effect of Angelica polysaccharide sulfate on hemorheology is better than that of Angelica polysaccharide itself.
