1.Analysis of the phenotypes and the function of CD56+T cells during primary HIV-1 infection

Objective To investigate the changes of phenotypes and function of CD56+T cells during primary HIV-1 infection and their relationship with disease progression.Methods Peripheral blood mononuclear cells (PBMCs) were collected from 53 subjects with primary HIV-1 infection and 31 HIV-1-negative healthy subjects.The percentages of CD56+T cells and the expression of several phenotypic markers on CD56+T cells including CD16, CD161, NKB1, NKG2A, NKp46, NKG2D, NKG2C and CD158a were analyzed by flow cytometry.IFN-γand TNF-αreleased by CD56+T cells with and without K562 stimulation and the levels of cytotoxic molecular CD107a were measured.Results The percentages of CD56+T cells in patients with primary HIV-1 infection were significantly lower than those of healthy subjects (P=0.025). The levels of CD56+T cells were negatively related to the viral loads in plasma samples ( P=0.021, r=-0.316).Compared with healthy subjects, the expression of CD16 (P=0.003), CD161 (P=0.023), NKB1 (P=0.023) and NKp46 (P=0.021) on CD56+T cells were decreased in patients with primary HIV-1 infection.The levels of NKB1 were positively related to the CD4+T cell counts ( P=0.007, r=0.364), but were negatively related to the viral loads in plasma samples (P=0.030, r=-0.299).Sponta-neous secretion of IFN-γand TNF-αby CD56+T cells and the expression of CD107a were dramatically in-hibited in patients with primary HIV-1 infection as compared with healthy subjects ( all P<0.001 ) . Moreover, the killing ability of CD56+T cells against K562 target cells was weakened in patients with prima-ry HIV-1 infection as the levels of IFN-γ-, TNF-α-and CD107a-producting CD56+T cells were significantly decreased (P<0.001 for IFN-γand TNF-α, P=0.016 for CD107a).Conclusion Inhibited expression and altered phenotypes of CD56+T cells were identified during primary HIV-1 infection.Lower levels of cy-tokines and cytotoxic molecular were also detected, indicating the dysfunction of CD56+T cells appeared dur-ing early stage of HIV-1 infection and was associated with disease progression.
2.Study on the Effect of Various Polarity Fractions Extracted from Sancaofang and Its Combination on Human Lung Adencarcinoma SPC-A-1 Cells

OBJECTIVE: To study the effect of various polarity fractions extracted from Sancaofang (SCF) and its combination on proliferation of human lung adenocarciaoma SPC-A-1 cells for bolting the most effective position of anti-tumor and suitable compatibility regimes.METHODS: Ethanol extraction and water extraction were adopted to prepare 95%,60% and 30% ethanol extract portion,water extract portion of SCF and compound decoction.The MTT assay was used to determine the effect of various polarity fractions of SCF,decoction and its combination on SPC-A-1 cells proliferation.RESULTS: IC50 of 60% ethanol extract was the smallest for SPC-A-1 cells.60% ethanol extract combined with 95% ethanol extract acts as a stimulus to anti-tumor activity significantly.CONCLUSION: The best suitable compatibility regimes were 95% ethanol extract combined with 60% ethanol extract.The liposolubility extract of SCF can be applied for anti-tumor.
3. Effects of hydroxycamptothecin on hepatic expression of Bax and Bcl-2 genes and α-SMA protein and hepatic fibrosis in rats

Objective To observe the effects of hydroxycamptothecin (HCPT) on hepatic expression of Bax, Bcl-2 genes, and crsmooth muscle actin (α-SMA) and hepatic fibrosis in rats. Methods Sixty-four SDrats were randomly divided into 5 groups: normal control group, model group, low-dose HCPT group, intermediate-dose HCPT group and high-dose HCPT group. Hepatic fibrosiswas induced in rats by abdominal injection of CC14- The normal control group was injected with normal saline only; the low-dose, intermediate-dose, and high-dose groups were injected with HCPT (3 times a week for 8 weeks) at 0. 25, 0. 5 and 1. 0 mg/kg, respectively. At the end of the 8th week, liver tissues were obtained from each group for H-E staining and Masson staining to observe the degree of hepatic fibrosis. The expression of Bax, Bel-2 mRNA and the Bax/ Bel-2 mRNA ratio in liver tissues were examined by RT-PCR. Immunohistochemical staining was used to observe crSMA protein expression and TUNEL staining wasused to observe cell apoptosis. Results Notable hepatic fibrosis was found in model group (HI stage in2rats, II stage in8 rats). Compared with the model group, each HCPT group had significantly improved fibrosis (P<0. 05; low-doseHCPT group: E stage in 1, H stage in 8, II stage in 1; intermediate-doseHCPT group: E stage in 7, H stage in 3; and high-dose HCPT group: E stage in 1, H stage in 7, II stage in 2). RT-PCR results showed that the expression of Bax and Bel-2 mRNA in the model group was significantly higher than that in the normal control group (P< 0. 05), and the expression in the three HCPT groups were significantly lower than that in the model group (P<0. 05), with the ratio of Bax/Bcl-2 mRNA in the model group being significantly lower than those in the three HCPT groups (all P<0. 05). The immunohistochemistry result showed that the hepatic α-SMA level in the model group was significantly higher than those in the intermediate- and high-dose HCPT groups (P<0. 05). The TUNELstaining showedno significantly positive staining in the normal control group ormodel group, and positive staining in all the three HCPT groups. Conclusion HCPT has protective effect on CC14-induced hepatic fibrosis in rats; inhibiting proliferation of hepatic stellate cells, up-regulating Bax/Bcl-2 mRNA ratio might be part of the related mechanism.

On the basis of the component structure theory, and according to the material basis of S. miltiorrhiz injection, we discussed the multi-dimensional structure and process dynamic quality control technology system of the preparation, in order to achieve the quality control over the material basis with safety and effectiveness of S. miltiorrhiz injection, and provide new ideas and methods for production quality standardization of S. miltiorrhis injection.

6.AO elastic intramedullary nailing for the treatment of clavicle fractures.

OBJECTIVETo evaluate the outcome of Titanium elastic intramedullary nailing (TEN) for the treatment of displaced midclavicular fractures.
METHODSBetween February 2010 and February 2013, 62 patients with displaced midclavicular fractures were treated by TEN, including 27 males and 35 females with an average age of 37.6 years old ranging from 15 to 67 years. The course of disease was from 1 to 9 days (means 2.7 days). Thirty-nine cases were treated by closed reduction and 23 cases by assistant small incision. Based on OTA (the Orthopaedic Trauma Association classification) classification, 31 cases of simple fractures involved 5 cases of 06-A1,15 cases of 06-A2,11 cases of 06-A3; 31 cases of wedge fractures involved 4 cases of 06-B1,12 cases of 06-B2,15 cases of 06-B3. Postoperative pain relief were evaluated by VAS score,and operation time,fracture healing time were recorded. After 6 weeks and after removing internal fixation shoulder joint function was evaluated by Constant score, and shoulder joint function were assessed by Herscovici score after 6 weeks.
RESULTSExcept 2 cases were lost to follow-up, 2 cases did not remove internal fixation, 3 cases' fractures were nonunion, the remaining 55 patients received follow-up for an average time of 11.4 months. The average preoperative VAS score was 5.20±0.71, and it turned to be 1.550.59 at 3 days after operation. The average operative duration was 40 min (15 to 65 min). The average bone healing time was (2.71±0.54) months (2 to 5 months). No difference about bone healing time was found between simple fractures and wedge fractures,and between the patients treated by closed reduction and assistant small incision. According to Herscovici standard, the shoulder function was excellent in 49 cases, good in 4, fair in 1, and poor in 1. Simple fractures achieved better Constant score than wedge fractures at 6 weeks postoperatively, whereas no difference was found at 4 weeks after fixation removal.
CONCLUSIONTitanium elastic nails (TEN) for treatment of displaced midclavicular fractures has advantages of good clinical effect,rapid pain relief,fast recovery of shoulder joint function. For OTA classification type B patients with comminuted fracture can replace steel to achieve very good effect, but to grasp the appropriate.

7.Surgical treatment for posterior Pilon fracture through posterolateral approach.

OBJECTIVETo explore clinical results of open reduction and internal fixation (ORIF) for posterior Pilon fracture through posterolateral approach.
METHODSSeventeen patients with posterior Pilon fracture were treated through posterolateral approach from February 2010 to April 2013. Among them,including 11 males and 6 females aged from 29 to 59 with an average of 43.4 years old. All fractures were associated with more than 20% of articular surface of distal tibial. The causes of injury included falling down (11 cases), traffic accident (4 cases) and sports injury(6 cases). Fracture classification was based on posterior pilon fracture by YU Guang-rong, including type I (6 cases), type II (2 cases) and type III (6 cases). Fracture healing time, fracture reduction and postoperative complications were observed, AOFAS score were applied to evaluate clinical efficacy.
RESULTSAll patients were followed up from 13 to 24 months with an average of 20.5 months. All incisions were healed at stage I, and fractures obtained healing,the time of fracture healing ranged from 12 to 21 weeks with an average of 15.2 weeks. No incision infection, neurovascular injury, bone ununion and fracture deformity were found after operation. Postoperative AOFAS score was 92.0 ± 10.2, and 14 cases got excellent results, 2 good and 1 moderate.
CONCLUSIONPosterior pilon fracture through posterolateral approach could obtain effective reduction, stable fixation. It is a safe, simple and effective operation for treating posterior Pilon fracture, and it is worth popularizing.

8.The pathogeny and electromyography of myokymia

Objective To research the pathogeny and the electromyographic characteristics of myokymia.Methods The clinical features and electromyography of 42 elderly patients with myokymia were examined. Results Symptomatic myokymia (SM) in 27 cases was caused by low salt syndrome, thyrotoxemia, urinaemia, chronic wasting diseases (such as carcinoma of the stomach or liver), brachial plexus neuropathy, lead poisoning, chronic inflammatory demyelinating polyradiculoneuritis, succinylcholine narcosis, restless leg syndrome or Isaac's syndrome.Symptomatic facial myokymia was caused by neoplasm of the brain stem or in posterior cranial fossa, multiple sclerosis or other causes. Primary myokymia ( PM ) in 15 cases involved idiopathic generalized and benign myokymia.Compared with PM, SM was more constant and powerful. Myokymia potential appeared in the electromyograms of 42 of the patients. The majority of patients with SM had accompanying myotonic discharge. Conclusion The pathogeny and electromyographic characteristics of symptomatic myokymia are different from the primary stage.
9.Evaluation on fi ngerprint characteristic of Prunella Species with cluster analysis method

Objective:To establish the HPLC fingerprint of Prunella Species from different places and evaluate it by cluster analysis.Methods:The analysis method of HPLC ngerprint was established.Alltima C18(4.6mm?250mm,5?m) was utilized for separation.The mobile phase consisted of methanol(A) and 0.5% glacial acetic acid(B) with a ow-rate of 1.0ml/min.The detection wavelength was 280nm and the injection volume was 20?l.The column temperature was 30℃.The HPLC ngerprints of Prunella Species from three di erent places were determined and analyzed by SPSS.Results:There is a big di erence among ngerprints of Prunella from di erent places.Prunella from Hubei,Anhui,Jiangxi,Guizhou,Guangxi,Sichuan were divided into a class;Henan,Jiangsu,Zhejiang were divided into another class.Conclusion:the eatsblished ngerprint showed characteristics of Prunella Species from di erent places obviously,and the attribute of Prunella Species can be distinguished e ectively by cluster analysis method.
10.Port catheter system placement in pulmonary artery via percutaneous subclavicle vein treatment for refractory multiple metastatic tumors in the two lungs:efficacy evaluation and technique discussion

Objective To evaluate the efficacy of port catheter system(PCS)placement in pulmonary artery via percutaneous subclavicle vein treatment for multiple metastatic tumor in the two lungs and discuss the PCS technique.Methods Fifteen multiple metastatic tumor patients(13 hepatocellular carcinomas,one mandible grand adenocarcinoma,one oral bottom squamous carcinoma)were carried out with pulmonary artery PCS placement by way of percutaneous subclavicle vein.FPA/FPM/GP chemotherapy scheme were introduced every 4~6 weeks.Results The success rate of PCS placement technique was 93.3%(14/15).One case failed.Percutaneous subclavicle veins were performed 14 cases in right side and 1 in left one.Following up 2~43 months,2~7 chemotherapy cycles(mean 5 cycles)were accomplished,and the clinical CR and PR were achieved in 1 and 3 cases respectively with clinical efficacy rate 28.6%(4/14).Major side reaction was late wound healing in 1 case.Conclusion PCS placement in pulmonary artery treatment for multiple metastatic tumor in the two lungs is effective,and mastering operation technique is the key for increasing operation suc- cess rate.
