Objective To observe the clinical effect of combination of clopidogrel and simvastatin on coro-nary heart disease and transient ischemic attack(TIA). Methods 76 patients with coronary heart disease and TIA were randomly divided into test group (n=40) and control group (n=36). The control group was treated with en-teric-coated aspirin 50 mg×2 every night after supper, and the test group was treated with clopidogrel 25 mg×2 and simvastatin 10 mg×2 every night before sleep. Liver and kidney function, blood coagulation function and blood lipids were measured before treatment and after. 1 year followed-up. Results The effective rate was 95.0% (38/40) in test group and 55.5% (20/36) in control group(χ2=6.45,P<0.01). LDL-C was (3.18±1.24) mmol/L and (2.60±1.03)mmol/L(t=2.67,P<0.01),TC was(5.18±1.24) mmol/L and (4.02±2.18) mmol/L(t= 4.91, P<0.01),TG was (1.50±1.02) mmol/L and (1.30±1.03) mmol/L(t=1.02, P>0.05), respectively in test group before and after treatment. However, there was no statistical difference in LDL-C, TC and TG (t=0.17, 0.00,0.52,0.57,P>0.05 for each) in control group. The two groups showed no difference after treatment (t= 1.51,2.55,0.57, P>0.05 for each). Conclusions Glopidogrel combined with simvastatin capsules is safe in pre-vention of TIA attack.

Objective To compare the specificity and sensitivity of antibodies against cell membrane associated DNA (cmDNA)、 nucleosome (AnuA)、 double stranded DNA (dsDNA)、 deoxyribonucleoprotein (DNP) and antinuclear antibodies (ANA) in systemic lupus erythematosus (SLE). To analyze the relationship between these auto-antibodies in SLE. Methods One hundred and twenty five SLE patients and 118 other rheumatic diseases patients were studied. The latter included primary Sj?觟gren syndrome, ploymyositis, dermatomyositis, rheumatoid arthritis, osteoarthritis and systemic sclerosis. Indirect immunofluorescence was used to measure the anti-cmDNA antibodies and ANA. The enzyme-linked immunosorbent assay (ELISA) was used to measure the AnuA. Anti-DNP antibodies were detected by latex agglutination text and anti-dsDNA antibodies were detected by colloidal gold-technique. Results The seropositive rates of AnuA、 anti-cmDNA、 DNP、 dsDNA antibodies and ANA were 68%、 38.4%、 51.2%、 49.6% and 95.2% respectively in SLE patients. OBJECTIVETo observe the efficacy of Xingnaojing Injection (XI) in treatment of sepsis-associated encephalopathy (SAE).

METHODSTotally 65 SAE patients were retrospectively analyzed at EICU from September 2010 to September 2013. They were assigned to the control group (32 cases) and the treatment group (33 cases) according to whether they received XI. Patients in the control group received anti-infection and symptomatic support, while those in the treatment group were intravenously injected with XI at 20 mL per day for additional 7-10 days. The fever clearance time, Glasgow coma scale (GCS), C-reactive protein (CRP), neuron-specific enolase (NSE), and improvement of electroen-cephalogram (EEG) were observed in the two groups.

RESULTSCompared with the control group, the fever clearance time was shortened, CRP levels decreased, GCS score and efficacy of EEG was alleviated in the treatment group after treatment with statistical difference (P < 0.05). No adverse reaction occurred during medication.

CONCLUSIONX1 was safe and effective in treatment of SAE.

HPLC fingerprint of Congrong Zonggan capsule was established in order to provide basis for quality evaluation. With acteoside as the reference, HPLC was adopted for fingerprint analysis on Congrong Zonggan capsule. The chromatographic conditions wereas follows. Waters C18 column (4.6 mm x 150 mm, 5 μm) was used, with methylalcohol-0.1% formic acid as the mobile phase for gradient elution at the flow rate of 1.0 mL x min(-1). The detection wavelength was 330 nm, and the column temperature was 30 °C. This method was highly accurate and reproducible. All of the 13 components in tested samples reached the baseline resolved peak, and 15 batches of finished products showed the similarity of above 0.95. The method was accurate and feasible and could be used as a simple and effective method to evaluate the quality of the traditional Chinese medicines.

Objective To compare the expression level of interferon regulator factor 5 (IRF5) of the health controls and systemic lupus erythematosus (SLE) patients and analyze the relationship between IRF5 and SLE disease activity. Methods Peripheral blood monoeytes (PBMCs) from SLE patients and healthy donors were separated with Ficoll density gradient eentrifugation and total cellular RNA was isolated with Trizol from the PBMCs, the mRNA was reverse transcripted to cDNA. Real-time PCR was applied to determine the expression level of IRFS. The expression level of IRF5 between the two groups were compared. The correlations of expression level of IRF5 with SLE disease activity and other laboratory or clinical parameters of SLE patients were analyzed. Results The level of IRF5 was (2.1±2.2) in SLE patients and (1.5±1.2) in healthy controls, the difference was not statistically significant (P=0.161). And the levels of IRF5 in SLE patients were significantly correlated with their SLEDAI (r=0.616,P<0.01), but not correlated with other parameters such as bemoglobulin complements, immunoglobulin etc. Anti-dsDNA antibody positive patients had significantly higher expression of IRF5 compared to the anti-dsDNA-antibedy-negative patients. The IRF5 mRNA levels of SLE patients with fever or neuropsyehiatric symptoms were significantly higher than those of patients free of neuropsychiatrie involvement. Conclusion High expression level of 1RF5 may contribute to the pathogenesis of SLE in disease activity and antibody production.

Between January 2011 and December 2011,86 patients undergoing operation for anorectal disease were randomized in a double-blind manner to receive a mixed injection of methylene blue,ropivacaine and sodium chloride (observation group,n =43) or traditional analgesic methods (control group,n =43).Patients in observation group had less visual analog scale (VAS) scores of pain at 6-48 h postoperation than control group (P ＜0.05).The scores had no significant difference at 72 h postoperation between two groups (P ＞ 0.05).The total postoperative complications (8,19％) and hospitalization duration [(5.3 ± 2.2) days] in observation group were significantly less than those [15,35 ％ and (6.4 ±1.7) days] in control group (P ＜ 0.05).A mixed injection of methylene blue and ropivacaine intraoperatively can relieve the postoperative pain of patients with anorectal disease and reduce the incidence of postoperative complications and the duration of hospitalization.

Objective To study the effects of postoperative immune-enhancing enteral nutrition (IEEN)on the liver function and postoperative infectious complications after hepatectomy in patients with hepatocellular carcinoma.Methods Between January 2010 and December 2011,96 patients undergoing hepatectomy for hepatocellular carcinoma were randomized in a double-blind manner to receive postoperative enteral nutrition with IEEN (IEEN group,n =48) or standard enteral nutrition (control group,n =48) for 5 days,respectively.Calorie and nitrogen of the same quantity were provided for both groups from the first day after surgery.The liver function tests and routine blood tests were performed.The postoperative hospital stay and postoperative infectious complications were also compared between these two groups.Results The serum albumin,prealbumin,alanine aminotransferase,and total bilirubin levels had no significant differences between the two groups (all P ＞ 0.05).On the 6th day after operation,the level of total lymphocyte count was significantly higher in the IEEN group than that in the control group [(2106 ± 1057) vs.(1648 ± 1079) / μL,P =0.038).The postoperative hospital stay was (10.3 ±5.6) days in the IEEN group and (13.4 ±6.7) days in the control group (P =0.016).Postoperative infectious complications were observed in 8 (16.7％) patients in the IEEN group and 17 (35.4％) patients in the control group (P =0.036).Conclusion Early postoperative IEEN can improve the clinical outcomes of liver cancer patients after hepatectomy.

Objective To assess the effect of neutrophil?to?lymphocyte ratio ( NLR) on the recurrence rate of patients with alpha?fetoprotein (AFP) ?negative hepatocellular carcinoma (HCC) and the value of NLR in predicting prognosis. Methods The clinical data of seventy?seven patients diagnosed with AFP?negative HCC and treated with hepatocellular carcinoma surgery in the First Hospital in Weinan from June 2015 to March 2017 were analyzed. According to the recurrence at the end of the follow?up, the patients were divided into the recurrence group and the non?recurrence group. Cox single factor analysis was used to analyze the relationship between the clinicopathological features and postoperative recurrence, and the clinical risk factors with statistically significance in the univariate analysis were placed in the Cox multivariate regression analysis to determine whether it is independent risk factor. Results The differences between the two groups in the number of tumors (31/15,28/3),tumor size (>5 cm) (5. 53±1. 83,4. 65±1. 73),portal vein tumor thrombus (18/28,2/29),microvascular invasion (14/32,2/29) were all statistically significant (P<0. 05). AlM: To explore the clinical significances and changes of related parameters of corneal endothelial cell in keratouveitis patients before and after treatment.
METHODS:Fifty-two keratouveitis patients ( 52 eyes ) diagnosed and treated in our hospital from October 2012 to December 2013 were collected. Before and after treatment, the related parameters of corneal endothelial cell in all patients were measured with non - contact corneal specular microscope and these parameters were statistically analyzed.
RESULTS: Compared with the normal group, obvious endothelial cell edema and great variation was observed in diseased group. The shorter duration of treatment, the better the recovery of endothelial cells and the fewer cells lose. Conversely, the longer the healing of normal hexagonal cell ratio was smaller, the larger the coefficient of variation. Before and after treatment, the differences of each parameter were statistical significance (P<0. 05).
CONCLUSlON: The functional recovery of corneal endothelial cell has directly relation with disease times.
