AIM: To investigate the infection factors,diagnosis and treatment of endophthalmitis after intraocular foreign body injury.METHODS: Totally 256 cases with intraocular foreign bodies including the occurrence of endophthalmitis in 42 cases were analyzed retrospectively.RESULTS: The incidence of endophthalmitis after intraocular foreign body injury was 16.4%.Posterior segment foreign bodies and combined with traumatic cataract were the risk factors of endophthalmitis after intraocular foreign body injury (all P<0.05).Intravitreal injection of vancomycin hydrochloride and ceftazidime in the treatment of intraocular foreign body injury reduced the incidence of endophthalmitis (P<0.05).Types and sizes of intraocular foreign bodies,the time of extraction and the condition of wound had no significant effect on the incidence of endophthalmitis (all P>0.05).Vitrectomy was the main treatment of endophthalmitis.CONCLUSION: Posterior segment foreign bodies and combined with traumatic cataract were the risk factors of endophthalmitis after intraocular foreign body injury.It should be kept the integrity of the posterior capsule of lens in traumatic cataract surgery.Intravitreal injection of vancomycin and ceftazidime is recommended in emergency surgery.Vitrectomy should be performed as soon as possible when confirmed endophthalmitis.

Objective To evaluate the effects of combined induction therapy of interferon(IFN) and chemotherapy on survival of the patients with small cell lung cancer(SCLC)by meta-analysis.Methods Search all of clinical trials for addition of IFN to initial chemotherapy and single chemotherapy for induction therapy in SCLC patients in MEDLINE、EMBASE and COCHRANE Library.The references of related studies and education books of ASCO and ESMO meeting were handsearched.The quality of included trials was evaluated.Data were extracted by two reviewers independently with a designed extraction form.Revman 4.2.7 software was used for data analysis.Results The addition of IFN increased 1-year survival and response rate,but both of the results had no significant statistical difference.Conclusion IFN-chemotherapy induction treatment might not influence survival and response rate of patients with NSCLC.To resolve the problem,a series of controlled,prospective,randomized,double-blind,well-designed,multi-center trials should be performed.

Objective:To investigate the shaping and cleaning ability of Reciproc(RE),OneShape(OS)and ProTaper(PT)in oval-shaped root canal preparation.Methods:The canals of 57 distal roots of mandibular first molars were randomly divided into 3 groups and prepared by RE,OS and PT respectively.The preparation time was recorded.After instrumentation,all root canal walls were examined under SEMand evaluated by scores of debris and smear layer.The 3D data and images of the roots of pre-and post-preparation were con-structed by Micro-CT scanning.Results:Group RE and OS showed lower scores of debris and smear layer than those in group PT at the apical level(P <0.05),and less preparation time (P <0.05).Before and after preparation,the changes of root canal morphological pa-rameters (including volume,surface,area,major diameter and minor diameter)in OS group were less than those in group RE and PT(P<0.05).The changes of canal curvature in group PT was larger than that in group RE and OS (P <0.05).Conclusion:RE and OS are more effective than PT in debridement and faster in root canal preparation.RE and OS can maintain the centering of original canal better than PT in the apical part of the root canal.The shaping ability of OS is weaker than that of RE and PT.

Objective To investigate the current situation of puncture-related pain in patients with the arteriovenous fistula undergoing maintenance hemodialysis and its correlated factors. Methods A total of 180 hemodialysis patients with the arteriovenous fistula undergoing maintenance hemodialysis were investigated with demographic data questionnaire, visual analogue scale and pain self-efficacy questionnaire. Results The effective questionnaires rate was 93.89%(169/180). The median score of puncture-related pain was 5 scores, the score of pain self-efficacy was (31.42±14.59) scores;multivariate analysis demonstrated that the puncture-related pain was positively correlated with the patient gender, using time of arteriovenous fistula and pain self-efficacy ( β=0.152,-0.017,-0.409, P<0.05), OR (95%CI)were 2.069 (1.206-3.148), 0.803(0.710-0.984), 1.004(0.886-1.431). There was negative correlation between the puncture-related pain score and pain self-efficacy in these patients, r=-0.647, P<0.01. Conclusions Puncture-related pain is common sense in patients with the arteriovenous fistula undergoing maintenance hemodialysis, some factors including the patient gender, using time of arteriovenous fistula and pain self-efficacy influence this feeling.

Objective: To investigate the effect of puncture-related pain on the quality of life in patients undergoing maintenance hemodialysis through internal arteriovenous if stula. Methods: A total of 180 hemodialysis patients with the arteriovenous ifstula were surveyed by the kidney disease quality of life short form(KDQOL-SF1.3), demographic data questionnaire, visual analogue scale and pain self-effcacy questionnaire. Results: The median score of puncture-related pain was 5 and the score of pain self-efficacy was (31.42±14.59). The quality of life in the patients undergoing maintenance hemodialysis is poor. KDQOL-SF1.3 was (69.45±24.19), SF-36 was (49.82±19.17) and ESRD-targeted was (55.46±18.37). Multivariate analysis demonstrated that the quality of life was positively correlated with the patient gender (β=0.152,P<0.05, OR=1.638, 95% CI 1.241–1.954), working position (β=0.307,P<0.05, OR=2.069, 95% CI 1.206–-3.148), using time of arteriovenous ifstula (β=?0.815, P<0.05, OR=0.223, 95% CI 0.095–0.741), the score of pain (β=-0.017,P<0.05, OR=1.004, 95% CI 0.886–1.431) and pain self-effcacy (β=-0.409,P<0.05, OR=0.803, 95% CI 0.710–0.984). hTere existed negative correlation between the quality of life score and the puncture-related pain score in these patients (r=-0.472,-0.465,-0.381,P<0.01), positive correlation between the quality of life score and the score of pain self-efficacy (r=0.647, 0.203, 0.518,P<0.05), and negative correlation between the puncture-related pain score and the score of pain self-effcacy(r=-0.745,P<0.01). Conclusion: Puncture-related pain is a crucial inlfuential factor on the quality of life in the patients undergoing maintenance hemodialysis through internal arteriovenous ifstula.

Objective To observe the effect of inhaled glucocorticoids combined with theophylline in smoking patients with bronchial asthma.Methods Seventy-three patients with bronchial asthma were enrolled in this study and they were divided into observation group (34 cases,smoker) and control group(39 cases,no-smoker).They all accepted inhalation of budesonide 200 μ g/suck,bis in die,plus aminophylline tablet (0.1 g,ter in die,per os) for 3 months.After treatment for 3 months,the basal control rate was compared between two groups.The levels of asthma control test (ACT) score,peak expiratory flow (PEF),the first second forced expiratory volume accounted for the percentage of prediction (FEV1％pred),eosinophil leukocyte (EOS) count and immunoglobulin E(IgE) in two groups before and after treatment for 3 months were compared too.Results After treatment for 3 months,the basal control rate in observation group was 88.24％ (30/34),in control group was 92.31％(36/39),and there was no significant difference(P＞ 0.05).After treatment for 3 months,the levels of ACT score,PEF and FEV1％pred in two groups were increased and the levels of EOS count and IgE were decreased than those before treatment,and there were significant difference (P ＜0.05).After treatment for 3 months,the levels of ACT score,PEF,FEV1％ pred,EOS count and IgE in observation group were (22.43 ± 2.64) scores,(292.52 ± 98.64) L/min,(74.87 ± 4.83)％,(270 ± 180) × 106/L,(68.25 ± 13.89) U/L,in control group were(22.81 ± 2.27) scores,(300.34 ± 100.45) L/min,(75.26 ± 5.04)％,(210 ± 170) × 106/L,(65.47 ± 11.28) U/L,and there were no significant differences (P ＞ 0.05).Conclusions Low dose inhaled glucocorticoids combined with theophylline in treatment smoking and non-smoking asthmatics patients are equal.It can improve asthma symptoms and lung function,improve hormone sensitive and reduce airway inflammation.

Objective To explore the efficient way to purify and to enrich umbilical cord mesenchymal stem cells which have abilities to differentiate into chondrocytes. Methods Umbilical cord was acquired from new born infants and chopped into 1 mm2 cubics, then they were pretreated with colleganase and trypsin so as to get the mixed cell suspension. CD45(-),CD90(+) cells were acquired from the mixed cell suspension via FCM sorting and were cultured in the medium as umbilical cord mesenchymal stem PO cells. The PO cells were detached and passaged by multiplex collagenase NB4 and the positive rate of CD90 and CD45 of the P1-P3 cells were observeed via FCM ; the P3 cells were induced to differentiated into chondrocytes. Results FCM results showed that the positive rate of CD90 on P1, P2, F3 was (80.86 ± 7.85)%,(95.86± 3.28)%,(97.15± 1.43)%, while the positive rate of CD45 on P1, P2, P3 cells was (2.53 ±0.28)% and (0.97 ± 0.48)% and (0.05 ± 0.01)% respectively; P3 cells had differenciated into chondrocytes. Conclusion FCM presorting and Multiplex collagenase detachment procedures can swiftly acquire highly purified human umbilical cord mesenchymal stem cells which have the ability to differentiate into chondroeytes.

OBJECTIVE:To discuss the characteristics and regularities of adverse drug reactions(ADR)occurred in our hospital.METHODS:A total of 352 ADR cases submitted to State ADR Center by our hospital from 2006 to 2008 via network system were analyzed statistically in respect of patients'age and sex,the drugs involved,routes of administration,ADR-involved organs or systems,and turnover etc.RESULTS:Of the total 352 ADR cases,the majority(67.6%)were induced by antimicrobial drugs;44.9% manifested as lesions of skin and its appendants,and 43.2% were induced orally.CONCLUSION:It is urgent to intensify the publicizing of ADR-related knowledge so as to offer guidance for clinic rational,safe and effective drug use.

Multilevel models are applicable to both the quantitative data and categorical variables. We used the methods, including the multilevel models, analysis of covariance and CMH chi-square test, to analyse different types of data, to explore the application of multilevel models in the analysis of the multicenter clinical trial center effect. The results showed that the analysis of covariance is more sensitive to find the center effect for quantitative data, while multilevel models are more sensitive to categorical variables. It can be seen that results with different analytical methods for center effect are not the same, and the most appropriate method should be selected in accordance with the characteristics of data, the objective of research, and the applicable conditions of the various methods in practical use.

Objective To investigate the effect of hyperbaric oxygen therapy on T-lymphocyte subpopulations in rats with acute pancreatitis. Methods 56 rats were randomly divided into three groups the sham group ( n = 8 ),control group( n = 24) and hyperbaric oxygen therapy group( treatment group, n = 24), then the control group and treatment groups were divided into three subgroups of 8 rats each undergoing euthanasia on days 1,3,7 after the acute pancreatitis induction. The CD4+ ,CD8+ subpopulations of T-lymphocytes in peripheral blood were detected respectively at the fist day and each day of the euthanasia. The sham group was used to make sure that the model was successfully induced. After euthanasia the pancrea was examined using electron microscopy. Results In the control group, the CD4+ cells in AP rats was significantly decreased and the ratio of CD4+/CD8+ also decreased. After 7days of HBO therapy,compared with the control group, the CD4+ lymphocytes of peripheral blood in the treatment group markedly increased( P ＜0.01 ). The CD8+ lymphocytes also increased to a certain extent. And the CD4+/CD8+ ratio of peripheral blood was obviously increased(P ＜0. 01 ). Also more severe pathological changes appeared in the untreated group than in the treatment group. Conclusion Hyperbaric oxygen therapy could improve the oxygen supply in acute panereatitis, regulate T cell immune function.
