1.Chronic bladder entrapment and bowel herniation after traumatic symphysis pubis diastasis

Objective To explore the clinical features and operative treatment of chronic bladder entrapment and bowel herniation after traumatic symphysis pubis diastasis. Methods One patient of late pelvic posttraumatic mal-alignment as Tile C2 type, which was associated with serious symphysis separation and combination of chronic bladder entrapment and bowel herniation, was admitted to our hospital in August 2002. The X-ray, three-dimensional CT reconstruction, MRI, and the bladder cystography were performed respectively in order to confirm the conditions. With the usage of ilioinguinal approach, the symphysis pubis diastasis was exposed and restored firstly; then, the malunion site of the left iliac was corrected; finally, after the reduction and internal fixation of the pelvis, the bowel herniation was resolved and the inferior abdominal wall defect was repaired with artificial materials. During the operation, the abnormal conditions were ob-served as follow: 1) The distance of symphysis pubis separation was about 7.5 cm; 2) the full-thick of the ectus abdominis was torn longitudinally tear along the middle line, and the transversalis fascia was fibrosis and adhered to the peritoneum, which consisted of hernia capsule; 3) the small bowel had been entrapped over the bladder through the gap of rectus abdominis and the right Hesselbach triangle. Results The pa-tient was followed up of 6 months. The limb discrepancy had been corrected satisfactory with a sound frac-ture healing and a good gait recovering after operation. The urinary function has recovered with a normal ap-pearance of penis. The patient felt stronger then to relieve the bowels than he did preoperatively. The erected dysfunction did not improve postoperatively. Conclusion The combined osteotomy and rigid fixation through anterior and posterior pelvic ring in the same stage is an effective method to close the symphysis seperation, decrease the volume of pelvic cavity, construct the pelvic floor indirectly, repair the inferior ab-dominal wall defect, cure the bowel herniation, and improve the bladder entrapment.
2.Classification and Detection of ?-Lactamases Produced by Citrobacter freundii

OBJECTIVE To study the classification of the ?-lactamases produced by Citrobacter freundii.METHODS To detect the ?-lactamases produced by 33 strains of C.freundii with nitrocefin test,a multi-disk test(synergy test and antagonized test) for the inducible AmpC ?-lactamases(IABLs),AmpC ?-lactamases(ABLs) phenotype test,extended-spectrum ?-lactamases(ESBLs) screening test,and carbapenem-hydrolyzing ?-lactamases(CHBLs).RESULTS In the 33 strains,29 strains produced the ?-lactamases(87.9%);the nitrocefin tests of 27 from the 29 strains were positive(81.8%);two strains produced penicillinases,3 strains produced cephalosporinases,5 strains produced broad-spectrum ?-lactamases(BSBLs),11 strains produced ESBLs alone,and 6 strains produced ESBLs+ high-productive ABLs,the rate of ESBLs produced by the 33 strains of C.freundii was 51.5%,and 2 strains produced CHBLs(all produced ESBLs+ metallo-?-lactamase).CONCLUSIONS The rate of ?-lactamases,rate of ESBLs and co-produced rate is very high.There are six kinds of ?-lactamases produced by the 33 strains of C.freundii,mainly BSBLs and ESBLs.
3.Classification and Detection of ?-Lactamases Produced by Providencia spp

OBJECTIVE To study the classification the ?-lactamases produced by Providencia spp.METHODS To detect the ?-lactamases produced by 19 strains of Providencia spp with nitrocefin test,a multi-disk test(synergy test,antagonized test for the inducible AmpC ?-lactamases(IABLs),AmpC ?-lactamases(ABLs) phenotype test and extended-spectrum ?-lactamases(ESBLs) screening test,carbapenem-hydrolyzing ?-lactamases(CHBLs).RESULTS The nitrocefin tests of all of 19 strains were positive,producing the ?-lactamases(100.0%);eight strains produced broad-spectrum ?-lactamases(BSBLs,42.1%) and ESBLs(42.1%),respectively,one strain produced overexpression of AmpC,3 strains produced CHBLs.CONCLUSIONS The rate of ?-lactamases produced by the 19 strains of Providencia spp is very high,there are three kinds of ?-lactamases,mainly BSBLs and ESBLs.
4.Construction and expression in vitro of a mifepristone inducible recombinant adenovirus carrying enhanced green fluorecence protein

Objective To construct a mifepristone inducible recombinant adenovirus carrying enhanced green fluorecence protein for future gene therapy in vivo.Methods Full length red fluorescent gene and mifepristone regulator cassette was subcloned into PDC313 shuttle plasmid.The positive clone was identified by sequencing.Recombinant adenovirus were produced after cotransfection of the shutter vector PDC-EGFP and adenovirus DNA helper plasmid pBHGloxE1,3Cre into 293 cells.The recombined adenovirus was purified by CsCl density gradient centrifugation and its titer was determined by end point dilution assay.Fluorescence microscopy and FCM were confirmed by the activation of this regulatable recombinant adenovirus vector in infected cells.Results The adenovirus vector containing enhanced green fluorecence protein gene was identify by PCR.The virus titer of Ad-RUEGFP was 6.6?1010pfu/ml.Without the absence of mifepristone,no significant enhanced green fluorecence protein activation was observed,whereas in the presence of mifepristone activation of the enhanced green fluorecence protein was positive correlation with the inducer under definite range.Conclusion A replication-defective recombinant adenovirus vector containing enhanced green fluorecence protein was successfully constructed,which can be inducible by mifepristone.
5.Analysis of GDNF mRNA gene expression on both severed sections of the sciatic nerve in rats

Objective To investigate the gene expression of GDNF mRNA on both ends of the severed sections of sciatic nerve in rats. Methods The specimen of sciatic nerve proximal and distal to the cut off site was taken respectively before and after sciatic nerve severance. The level of GDNF mRNA on both cutting sections was observed and compared by using of semi quantitative RT-PCR method with ? Actin as a inner consult before nad after sciatic nerve cut off. Results A minimum expression of GDNF mRNA was found on sciatic nerve before the operation, however, there was an increase of 20% at 24 hours in the distal section after sciatic nerve cut off, 60% at 7th day, 85% at 14th day and 90% at 28th day. While it decreased by 10%, 38%, 45% and 52% at 1st day, 7th day, 14th day and 28th day in the proximal section respectively. Conclusion The decreasing and increasing of GDNF mRNA level at the proximal and distal sciatic nerve section is the result of the disruption of “neuron axon target”axis after sciatic nerve cut off. This may provide a theory basis in treatment of SCI by use of the foreign GDNF.
6.Ultrasound diagnosis and differential diagnosis of renal arteriovenous fistula

Objective:To summarize the ultrasound features of renal arteriovenous fistula(RAVF)under various ultrasound imaging modes,so as to avoid missed diagnosis of RAVF on initial ultrasound examination.Methods:The clinical data of 6 patients with RAVF,including the ultrasound evidence,the timing of ultrasound diagnosis,the modes of ultrasound diagnosis, and the agreement between ultrasound diagnosis and selective renal arterial angiography,were retrospectively analyzed.Results: 2D ultrasound had 1 case of correct diagnosis,1 case of misdiagnosis and 4 cases of missed diagnosis.Color Doppler and spectral Doppler both had all the 6 cases correctly diagnosed.Ultrasound angiography in 3 cases demonstrated that the contrast agent reached the renal veins earlier than reached the renal parenchyma;large fistula lumen was associated with ischemia of downstream areas.3D ultrasound vividly reflected the structure of fistula lumen volume,and provided us with the profiles of blood signal in fistula at different planes and angles,improving our knowledge of blood flow on the fistula.Conclusion: Ultrasound is the first line screening method for RAVF.Color Doppler plays a decisive role in the diagnosis of RAVF and pulsed spectral Doppler plays a synergetic role and contributes to differential diagnosis.Contrast-enhanced ultrasound may help to discover the abnormality of venous circulation and the ischemic parenehyma area due to shunting.2D ultrasound has a poor diagnostic value and is liable to lead to misdiagnosis and missed diagnosis.
7.Determination of catechin in Songling-Xuemaikang capsules by HPLC

Objective To establish a HPLC method for determination of the content of catechin in Songling- Xuemaikang capsules. Methods The analysis was performed on Alltima C18 column (250 mm × 4.6 mm, 5 μm) with mobile phase consisting of methanol (A) -l% Acetic Acid (B) for gradient elution. The flow rate was 1 ml/min and the column temperature was 25 ℃. Results The results showed that catechin was well separated with the good linearity in 0.00756-0.05040 μg/μl. The average recovery of catechin was 100.147%. Conclusions The method could be applied to determine catechin in Songling-Xuemaikang capsules, which would make sure the quality control of capsules.
