Objective To determine the concentration difference in protoporphyrin in cancerous intestine and to discuss its application in laser-induced autofluorescence diagnosis for colorectal cancer at early stage.Methods We detected the concentration of protoporphyrin Ⅸ in 30 patients with colorectal cancer and 30 control patients, as well as that in 60 cases of intestinal tissues (30 control tissues and 30 cancer tissues).Results The concentration of blood protoporphyrin Ⅸ in patients with colorectal cancer was significantly higher than that in the controls (P<0.05). Protoporphyrin Ⅸ concentration in the cancer tissue was significantly higher than that in the control tissues (P<0.05).Conclusion That the concentration of protoporphyrin in cancerous intestine becomes abnormally high may be the material basis for spectrum intensity peak of (644.3±5.7)nm in laser-induced autofluorescence diagnosis for colorectal cancer at early stage.

Objective To summarize continuous veno- venous hemofiltration (CVVH) treatment of infants with complex congenital heart disease in children with acute renal failure after treatment. Methods A retrospective analysis was carried out in six cases of infants with congenital heart disease with application of CVVH. The children were given monitoring and anticoagulant treatment. The renal function, electrolytes and urine volume were compared before and after the treatment. Results The amount of urine increased, blood circulation and electrolytes remained stable in six patients 17.5-135.0 hours after CVVH. Conclusions The CVVH therapy can be applied to treat infants with congenital heart disease complicated with acute renal failure, but CVVH treatment should focus on anticoagulants and blood volume since complicated state of illness, low age, low body weight of children patients.

BACKGROUND:To explore the methods for secondary limb revascularization after endovascular repair is one of the most urgent research topics.
OBJECTIVE:To investigate the biological mechanism about the transplantation of autologous peripheral blood stem cel s to lead and perfect the effect of ischemic tissue for secondary revascularization.
METHODS:Forty-two patients with critical low limb ischemia were selected and treated with endovascular repair as first revascularization and transplantation of autologous peripheral blood stem cel s as secondary revascularization. The secondary revascularization was carried out at 3 months after the first blood flow remodeling. The patients receiving secondary revascularization were fol owed for 4 years.
RESULTS AND CONCLUSION:After transplantation of autologous peripheral blood stem cel s, the patients complained that the rest pain of the lower leg was relieved obviously and intermittent claudication distance was significantly lengthened. Limb salvage rate was 100%. Skin temperature index was 1.6±0.3, transcutaneous oxygen pressure was (5.00±1.26) kPa, ankle-brachium index was 0.9±0.2, photoplethysmograpy index was 0.8±0.1, saturation of blood oxygen was (79.4±20.4)%, and digital subtraction angiography score was (1.3±0.2)

Objective To observe the efficacy of arsenic trioxide(ATO) combined all trans retinoic acid (ATRA) versus cytara‐bine (Ara‐C) combined ATRA in the treatment of acute promyelocytic leukemia(APL) .Methods We enrolled 65 patients in our department during the period between January 2002 and August 2008 ,and they were randomly assigned to receive ATRA combined ATO (treatment group ,n= 27) or ATRA combined DA ,HA ,NA which were major of Ara‐C (control group ,n= 38) .Then observe the differences of between the two groups ,such as complete remission(CR) ,the time to complete remission ,overall survival(OS) ,e‐vent free survival(EFS) ,the 5 years disease free survival (DFS) and adverse reactions .Results The CR rate of treatment group (ATRA + ATO) and control group (chemotherapy + ATRA) was 81 .48% and 68 .42% ,respectively ,and the time to complete re‐mission was (28 .50 ± 3 .97)d and (30 .56 ± 2 .39)d ,respectively ,showed that there was no statistical difference between the two groups ( P > 0 .05 ) .The 5 years DFS of the CR patients in the two groups was 51 .9% (ATRA + ATO ) and 50 .0%(Chemotherapy + ATRA) ,respectively ,showed that there was no statistical difference between the two groups(P > 0 .05) .The 5 years EFS of the CR patients in the two groups was 48 .1% and 39 .5% ,respectively ,showed that there was no statistical difference between the two groups(P> 0 .05) .The 5 years DFS of the patients in the two groups was 55 .6% and 67 .6% ,respectively ,showed that there was no statistical difference between the two groups(P > 0 .05) .Bone marrow suppression in the treatment group was significantly lower than in the control group(P< 0 .05) .Conclusion ATRA + ATO can prolong the CR rate ,OS ,EFS and 5 years EFS of newly diagnosed APL patients .ATRA combined with chemotherapy has similar efficacy ,ATRA + ATO has lower bone marrow suppression than the ATRA combined with chemotherapy ,thus may reduce the risk of early death .

Objective To investigate the safety and effectiveness of high intensity focused ultrasound (HIFU ) in the treat‐ment of uterine fibroid (<4 cm) .Methods Retrospectively studied 79 single fibroid (<4 cm) patients that treated with HIFU in our hospital from January 2011 to February 2013 .Evaluated the improvement of post‐treatment ,shrinkage of fibroid volume and HIFU‐related adverse reaction .Results All the 79 patients were successfully cured by HIFU treatment .The shrinkage rate of fi‐broid was 79 .0% (four point spacing 57 .9% ,92 .1% ) after 2‐year treatment ;Symptom severity score (SSS) and uterine fibroid symptom and quality of life score (UFS‐QOL) were significantly improved ;and there was no adverse reaction .Conclusion HIFU can be safely used in the treatment of small uterine fibroids (diameter<4 cm ) .

Objective: To investigate the pharmacokinetic and pharmacodynamic changes of Yangyin Tongnao Granules (YTG) in cerebral ischemia reperfusion rats after the compatibility of main effective parts (total alkaloids, total flavonoids, total saponins and total phenolic acids). Methods: By using the orthogonal design to research the main effective parts of YTG, nine different dosage ratios combinations were formed, which were used for oral administration in cerebral ischemia reperfusion rats. High performance liquid chromatography-diode array detector (HPLC-DAD) was used to determine the concentration of puerarin, ferulic acid, and ligustrazine in rat plasma at different time points. The non-compartmental model was used to fit the pharmacokinetic parameters by Drug and Statistics (DAS) 3.2.6 software. The total quantum statistical moment analysis method and comprehensive evaluation method were used to evaluate the total pharmacokinetic characteristics. Meanwhile, the content of superoxide dismutase (SOD) and catalase (CAT) were determined by enzyme-linked immunosorbent assay (ELISA) kits. Finally, the PK-PD model and the quantitative equations between drug concentration and efficacy were obtained. Results: The pharmacokinetic characteristics of puerarin, ferulic acid and ligustrazine in cerebral ischemia-reperfusion rats were different. Total statistical moment and comprehensive score study showed that different combinations had different effects on ACUT, mean retention time (MRT), and comprehensive evaluation. The effective parts inhibited the reduction of oxidation indexes such as SOD and CAT. Sigmoid-Emax models were adopted in all PK-PD models, and the fitting results had a good correlation with the measured data. The R values were more than 0.85. Conclusion: Compatibility of YTG activity parts had a certain effect on their pharmacokinetic behaviors and antioxidation in model rats. The total quantum statistical moment analysis and comprehensive evaluation method can be used to study the pharmacokinetics of multi-component traditional Chinese medicine prescriptions. PK-PD model could be used to predict and evaluate the correlation between pharmacokinetics and pharmacodynamics of traditional Chinese medicine prescriptions.

Objective To investigate the level and influencing factors of falls efficacy among older patients with type 2 diabetes (T2DM) .Methods A total of 218 older patients with T2DM were investigated by the modified falls efficacy scale(MFES) and Morse Fall Scale (MFS) ,questionnaire and observation were both used .Results The average score of falls efficacy was 8 .15 ± 2 .91 .The scores were lowest in the items of walking up and down stairs and going to bed and getting out of bed ,and highest were in the items of dressing and sitting down and standing up from a chair .The multivariate linear regression analysis showed that dura‐tion of diabetes ,diabetic complications and fall history were the main factors influencing their falls efficacy .Conclusion Falls in eld‐erly T2DM patients were in the medium level ,and it′s closely related with duration of diabetes ,its complications and fall history .

Objective To evaluate the value of acute kidney injury network (AKIN) criteria for predicting hospital mortality in post-cardiosurgery elderly patients. Methods From October 2006 to January 2007, the elderly patients who underwent coronary artery bypass grafting or valve replacement operation were enrolled in this study. The medical data included gender, age, operation type, perioperative hemodynamic parameters, urine output, biochemical parameters and outcome. Renal function was assessed according to the AKIN criteria, and severity of illness was determined after surgery by calculating Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ ) score. Results A total of 225 patients underwent cardiac surgery during this period, 169 patients were male (75. 1%), while 56 were female (24. 9%), mean age was (66. 7±5. 0) years old. The overall hospital mortality rate was 5.8% (13/225). According to AKIN criteria, there were 125 patients with acute kidney injury (55.6%), and the hospital mortality of stage 1, 2 and 3 patients were 2. l%(2/96), 9. 1% (1/11) and 50. 0% (9/18) respectively. A significant increase was observed in mortality based on AKIN criteria (P<0. 01). By applying the area under the receiver operating characteristic curve, the AKIN criteria had a good discriminative power. Conclusions Both the incidence and mortality rate of acute kidney injury in the post-cardiosurgery elderly patients are high, the AKIN criteria is a simple and valuable method with a good prognostic capability for evaluating acute kidney injury.
