3.Exploring the effects of Chinese medicine in improving uterine endometrial blood flow for increasing the successful rate of in vitro fertilization and embryo transfer.
Journal of Integrative Medicine 2011;9(12):1301-6
Endometrial blood flow is directly related to endometrial receptivity thereby affecting in vitro fertilization and embryo transfer (IVF-ET) outcomes. In recent years a growing number of studies have shown that traditional Chinese medicine (TCM) can play a role to improve endometrial blood flow and embryo transfer. Studies have confirmed that formulas based on reinforcing kidney and activating blood can promote the formation of uterine endometrial blood vessels by adjusting expressions of a variety of vessel growth factors, and regulating nitric oxide level for inhibition of vascular smooth muscle contraction of the uterus. Treatments based on differentiation of syndromes are key to the theory of TCM. Differentiation of syndromes should be combined with biomedical disease diagnosis. It is also necessary to further clarify other endometrial blood flow disorders using TCM diagnostic methods. In these cases, drugs for reinforcing kidney and activating blood are relevant, but other medicines for smoothing liver qi and nourishing blood, as well as tonifying the spleen and generating blood may also be effective. Future clinical studies should focus on the observation of different types of TCM syndromes and the research on compatibility ratio and dose-dependent relationship of Chinese medicines. Acupuncture has been used during IVF-ET for more than 10 years. Electro-acupuncture can control the expansion of the uterine arteries by inhibiting the sympathetic nerves. However, acupuncture has not been proved to regulate endometrial arteries. To sum up, TCM can be applied to improve endometrial blood flow so as to increase birth rates in IVF-ET. The combination of Chinese and Western medicines and acupuncture application will increase their combined effect, thereby obtaining greater clinical benefits.

Recently the combination of acupuncture with assisted reproductive technology (ART) to increase the outcomes of ART is being widely studied. In this article, the literatures concerning random controlled clinical trials since 2002 are reviewed and the designs of the trials, especially the timing of acupuncture, are evaluated. Over the past 5 years, the related clinical trials have primarily showed that acupuncture done immediately before and after embryo transfer might increase the assisted reproduction rates, but still requiring further high quality trials with large samples; in addition, different stimulation modes could produce different result, and so far there has not been a consensus as to the optimal time-point for the acupuncture intervention during the in vitro fertilization-embryo transfer (IVF-ET) course. Since the effects of acupuncture change with women's endocrine cycles, it is important and possible to make a breakthrough in ART outcomes if acupuncture is performed at a suitable time point during the cycle of IVF/ET combined with ART.

BACKGROUND: The copper-superoxide dismutase (Cu-SOD) can not play an effective role in free radical-scavenging in the process of hypoxia or reaeration because of blood-brain barrier, whether the treating effect can be enhanced by chitosan nanoparticles is uncertain. OBJECTIVE: To observe the ability of chitosan nanoparticles loaded the Cu-SOD plasmid on free radical-scavenging in ischemia-reperfusion brain tissue. DESIGN: An observation experiment. MATERIALS: The Cu-SOD plasmid was provided by the Center Laboratory of Tangdu Hospital, Forth Military Medical University of Chinese PLA. A total of 36 rats were prepared ischemia-reperfusion models, and randomly divided into the control, physiological saline (PS) plus Cu-SOD plasmid and chitosan nanoparticles loaded the Cu-SOD plasmid groups. METHODS: The chitosan nanoparticles were prepared by cross-linking method. The Cu-SOD plasmid was absorbed to chitosan nanoparticles by electrostaficforces, mixed the 50.0 ?L nanoparticles with 10.0, 25.0, 50.0, 75.0, 100.0 ?L SOD plasmid respectively. The electrophoretic analysis was used to examine the combination condition of the chitosan nanoparticles with superoxide dismutase plasmid, and the embedding rate was calculated with ultraviolet spectrophotometer. Chitosan nanoparticles, SOD and Cu-SOD plasmid were infused into the rat after brain ischemia-reperfusion. Further more, to detect the content of malondialdehyde and superoxide dismutase in brain tissues. MAIN OUTCOME MEASURES: The combination condition of the chitosan nanoparticles with superoxide dismutase plasmid, and the activity of malondialdehyde (MDA) and SOD were measured. RESULTS: Chitosan nanoparticles and superoxide dismutase in volume ratio of 50: 75 could harvest the largest embedding rate with 100%, which had good protective effect on DNA. The contents of MDA in brain tissues of the Chitosan nanoparticles loaded the Cu-SOD plasmid group were obvious lower than that of the control (P

Chinese Journal of Rehabilitation Theory and Practice 2007;13(8):759-761
Objective To observe the respiratory depression of sufentanil with target-controlled infusion under propofol sedation or not.Methods40 patients, ASAⅠ or Ⅱ, scheduled for selective neurosurgery were randomly divided into the sufentanil group (group S) and the combined group (group SP) with 20 cases in each group. In the group SP, sedation was maintained with target-controlled infusion of propofol by plasmas concentration of 2 μg/ml. Target-controlled infusion of sufentanil was performed at the initial plasma concentration (Cp) of 0.2 ng/ml, and increased in a increment of 0.2 ng/ml at 3 minutes interval; while in the group S, only the infusion of sufentanil was performed. The respiratory rate (RR), minute ventilation (MV), partial pressure of end-tidal carbondioxide (PETCO2), pulse oxygen saturation (SpO2), mean arterial pressure (MAP), heart rate (HR), Observer's Assessment of Alertness/Sedation (OAA/S) were measured and respiratory depression was defined as one of the following end points were achieved: muscle rigidity, RR<6 bpm, MV<3 L/min, PETCO2>55 mmHg, SpO2<90% or apnea>15 s.ResultsThe RR, MV of patients in two groups decreased and PETCO2 increased significantly as increase of Ce ( P<0.05). The calculated effect site concentration (Ce) of sufentanil for respiratory depression was (0.46±0.14)ng/ml in the group S and (0.23±0.06)ng/ml in the group SP having a significant difference between two groups ( P<0.01).ConclusionSufentanil can dose-dependently suppress spontaneous respiration.

OBJECTIVETo observe the effect of Yijing Recipe (YR) on the apoptosis of testis spermatogenic cells and the protein expression of Bcl-2/Bax in rats with adenine induced infertility.
METHODSTotally 75 Wistar rats were randomly divided into 5 groups, i.e., the blank control group, the model group, the high dose YR group, the middle dose YR group, and the low dose YR group, 15 in each group. Except those in the blank control group, rats in the rest groups were intragastrically administered with adenine for 10 successive days. From the 11th day, rats in the blank control group and the model group were fed with equal volume of normal saline. Rats in the YR groups were intragastrically administered with YR at different doses (3.38 g/100 g; 1.69 g/100 g; 0.85 g/100 g), once daily for 20 consecutive days. All rats were killed by the end of the experiment and their testes extracted. The apoptosis of spermatogenic cells and the expression of Bcl-2/Bax proteins were detected by terminal deoxynucleotidyl transferase-mediated biotinylated UTP nick end labeling (TUNEL) and SABC method.
RESULTSCompared with the blank control group, the Bcl-2 protein expression decreased, the Bax protein expression increased, and the apoptosis index increased in the model group, showing statistical difference (P <0.01). Compared with the model group, the Bcl-2 protein expression increased in the three YR treated groups (P <0.01, P <0.05). The Bax protein expression level decreased in the high and middle dose YR groups (P <0. 01, P <0. 05). The apoptosis index decreased in the middle dose YR group (P <0.01).
CONCLUSIONYR could inhibit the apoptosis of spermatogenic cells through regulating the expression of Bcl-2 and Bax protein in the testis.
