OBJECTIVETo explore curative effects of advanced elastic intramedullary nail technology in treating children with femoral subtrochanteric fracture.

METHODSForm March 2009 to December 2010, 16 children with femoral subtrochanteric fracture were treated with advanced elastic intramedullary nail technology. There were 12 males and 4 females ranging in age from 7 to 15 years old with a mean of 9.8 years old. According to fracture classification, 9 cases were stable fractures (traverse or short oblique type) and 7 cases were unstable fractures (comminuted or long oblique type). Radiographs and complications were retrospectively reviewed. Fracture healing were evaluated according to Flynn criteria.

RESULTSAll children were followed up from 11 to 32 months with an average of 21 months. No incision infecton,bone nonunion and breakage of screw occurred. The fracture healing time was from 8 to 16 weeks with an average of 10.1 weeks. According to Flynn criteria, 14 cases got excellent results, 1 moderate and 1 poor.

CONCLUSIONAdvanced elastic intramedullary nail technology for the treatment of children with femoral subtrochanteric fracture is a safe and effective fixation, which is good for early mobilization, shorter the duration of hospital stays and reduce complications.

To observe the serum samples and the anti-inflammatory effect of Tripterygium wilfordii in treating RA by using the pharmacokinetic-pharmacodynamic model, make a correlation analysis on concentration-time and effect-time curves, and explore RORγt, IL-17, STAT3, IL-6 mRNA transcriptional levels in rats by PCR. Methotrexate, tripterine and high-dose T. wilfordii could down-regulate RORγt, IL-17, STAT3, IL-6 mRNA transcriptional levels in AA rat lymph nodes. The study on PK-PD model showed correlations between inflammatory factors and blood concentration of T. wilfordii. T. wilfordii and its main active constituent tripterine could show the inflammatory effect and treat RA by inhibiting IL-17 cytokine.

Background/Aims: Non-alcoholic liver disease and alcoholic liver disease begin as simple steatosis that may progress to steatohepatitis and ensuing liver-related complications such as cirrhosis and hepatocellular carcinoma (HCC). We explored differences in characteristics between non-alcoholic steatohepatitis (NASH) and alcoholic steatohepatitisrelated (ASH) HCC. Methods: NASH and ASH patients were identified from our department’s prospective HCC database. A total of 54 and 45 patients met predefined inclusion and exclusion criteria for the NASH-HCC and ASH-HCC groups, respectively. Clinical, biochemical and tumor characteristics were studied. Results: NASH-HCC patients were older compared to ASH-HCC patients (72±9 vs. 66±9 years, P<0.001) and less male predominant (65% vs. 98%, P<0.001). Prevalence of diabetes mellitus (78% vs. 36%, P<0.001) and hypertension (80% vs. 58%, P<0.001) were significantly higher in the NASH-HCC group. Liver function tests and Child-Pugh scores were similar. There were no differences in alpha-fetoprotein level, lesions found at diagnosis (unifocal/multifocal) or prevalence of portal vein invasion. In both groups, almost half of the patients were in TNM stage 4 at the time of diagnosis and more than 50% of patients were not suitable for any therapy. Median survival in the NASH-HCC and ASH-HCC groups were 13 and 7 months respectively (P=0.113). Conclusions Despite significant differences in demography of the NASH-HCC and ASH-HCC groups, liver and tumor characteristics were comparable. Most patients were diagnosed late and were not amenable to curative or locoregional therapies. Better characterization of patients with NASH and ASH at risk of HCC is necessary to optimize screening, surveillance, and management strategies.

Systematical studies are lacking on the influencing factors and mechanisms of the heparin enhanced sperm capacitation, although many studies have shown that heparin enhanced sperm capacitation. The effect of heparin concentration and exposure time, incubation temperature and co-culture with oviductal epithelial cells or cumulus cells on goat sperm capacitation were investigated in this study. The motility, membrane and acrosome integrity and capacitated percentage of goat spermatozoa were assessed after different heparin treatments, and rates of fertilization and embryo cleavage were compared after in vitro insemination of oocytes with spermatozoa capacitated by different heparin treatments. The major results are summarized as follows: 1) When spermatozoa were capacitated with heparin at 5, 10, 25, 50 and 100 microg/mL for 45 min, 50 and 100 microg/mL heparin treatments produced the highest capacitated percentages of 55% and 56%, respectively, but the percentage of spermatozoa with intact acrosomes in the 100 microg/mL heparin treatment decreased significantly (P < 0.05) in comparison with that in the control group, indicating that the optimal heparin concentration for goat sperm capacitation would be 50 microg/mL. 2) Capacitated percentage of spermatozoa increased with extension of treatment time when goat sperm were treated with 50 microg/mL heparin for 0, 10, 20, 30, 45, 60 or 120 min. Although heparin treatments for 45 to 120 min did not differ significantly (P > 0.05) in capacitated sperm percentages, sperm motility and membrane integrity decreased significantly when treated with heparin for 120 min. This suggested that the optimal exposure time of heparin at 50 microg/mL for goat sperm capacitation would be 45 to 60 min. 3) Significantly higher capacitated percentages of spermatozoa were obtained when goat sperm were treated at 42 and 38.5 degrees C than at 15 and 37 degrees C, but sperm motility and acrosome integrity were significantly lower when spermatozoa were treated at 42 degrees C than they were treated at other temperatures. Temperature of 38.5 degrees C would, therefore, be the optimal temperature for goat sperm capacitation. 4) The capacitated percentage of spermatozoa was significantly higher when goat sperm were co-cultured with oviductal epithelial cells than when treated with heparin alone or co-cultured with cumulus cells, but sperm motility and membrane and acrosome integrity did not differ significantly among the three treatments. Rates of fertilization (91.3%) and cleavage (72.2%) were significantly higher in the oviductal epithelial cell co-culture group than those in the heparin alone group. This indicated that co-culture with oviductal epithelial cells significantly enhanced goat sperm capacitation by heparin treatment.

Objective To explore the severe fever with thrombocytopenia syndrome bunyavirus (SFTSV) existence time in the body,and the correlation between viral load and the severity and prognosis of disease.Methods The clinical data of 125 SFTS patients from May 2015 to October 2016 in Weihai Central Hospital in Shandong province were analyzed retrospectively.Patients were divided into low viral load group and high viral load group according to the SFTSV RNA levels.Neurological symptoms,bleeding tendency,the incidence of myocardial damage and severe pneumonia,laboratory biochemical index and prognosis of two groups were compared.SFTSV RNA of 46 cases were detected dynamically.Data with homogeneity of variance were tested by t test,and data with heterogeneity of variance was tested by rank sum test.Results Among the 125 cases,64 were male and 61 were female.The mean age was (59.0±3.6) years old.One hundred and one cases were cured,and 24 died.SFTSV RNA loads in low viral load group(81 cases) were (3.08± 1.01) copies/mL,and those in high viral load group (44 cases) were (5.69 ± 0.99) copies/mL,with statically significant difference (t =11.78,P＜0.05).By the dynamic detection of SFTSV RNA load in 46 patients,viral loads in most patients were gradually declined after 1 week of onset,and cleared after 23 days.The incidence of neurological symptoms,bleeding tendency,severe myocardial damage and pneumonia of two groups showed significant difference (x2 =92.987,38.711,75.889 and 54.680,respectively,all P＜0.05).The viral loads of patients who died varied from 1.06× 104 copies/mL to 5.78 × 107 copies/mL.White blood counts of two groups showed no significant difference (t =0.181,P＞ 0.05).The platelet counts of two groups had significant difference (t =2.869,P＜0.05).AST and γ-GT of two groups also had significant difference (P＜0.01 and 0.05,respectively).creatine kinase,creatine kinase isoenzyme,lactic dehydrogenase and hydroxybutyrate dehydrogenase of two groups all had significant difference (P＜0.01 or 0.05).Serum sodium,blood calcium and glucose of the two groups had significant difference (P＜0.01 or 0.05).activated partial thromboplastin time of the two groups showed significant difference as well (t=5.623,P＜0.01).Conclusions After the onset of SFTSV infection,the virus existence in the body may less than 4 weeks.Viral loads are closely associated with disease severity and prognosis.The higher the viral loads are,the heavier organ dysfunction could be and the higher mortality is.

Objective To understand non-typhoid Salmonella in diarrhea patients from Guangdong province in order to timely discover the outbreaks caused by them as well as to grasp the serotypes, antibiotic resistance and pulse-field gel electrophoresis (PFGE) types of those strains isolated from this surveillance program. Methods Salmonella strains from diarrhea patients were detected and all the positive strains were tested by serum agglutination, antibiotic susceptibility and PFGE. Results 71 nontyphoid Salmonella strains were isolated from 1128 stoop samples, with a positive rate of 6.29 %. All the strains were divided into 29 serotypes, with Salmonella serotype enteritidis and typhimurium showing the most common serotypes. Most of the strains were sensitive to cephalosporins and quinolones. The antibiotic resistance rates of S. typhimurium were higher than S. enteritidis and S. stanley. Other than S. enteritidis, all the serotype strains did not have the same type of PFGE. 17 S. enteritidis strains digested by Xba Ⅰ were divided to 8 PFGE types while the PFGE 4 type appeared the most common one. 12 S. enteritidis strains were typed again by Sfi Ⅰ and Not Ⅰ , and there were still 3 groups of strains showing the same PFGE pattern. Conclusion Most of the infection caused by non-typhoidal Salmonella was sporadic in Guangdong province in 2007. Cephalosporins and quinolones seemed the best in curing the infection of non-typhoidal Salmonella at the clinics.

OBJECTIVETo explore a flow cytometry (FCM)-based method for discriminating aneugen- or clastogen-induced micronuclei.

METHODSCells were stained with anti-CD71-FITC and PI, and the PI fluorescent signal intensity of micronucleated reticulocyte (MN-RET) in the peripheral blood of NIH mouse treated with COL or CP was detected by flow cytometry.

RESULTSThe ratio of the median of the intensity of MN-RET fluorescent signals to that of nucleated cell was low in the cyclophosphamide treated mouse, while the median was high in the colchicine treated mouse.

CONCLUSIONThe flow cytometry-based micronucleus assay can be used to discriminate primarily smaller MN induced by the clastogen exposure from the larger MN induced by an aneugen.

objective To explore the difierence of arousal index between the children with obstructive sleep apnea-hypopnea syndrome(OSAHS)and children with primary snoring.Furthermore,to explore the correlation between the above mentioned arousals and the apnea-hypopnea index(AHI)and lowest oxygen saturation(LSaO2).Methods Between March 2007 and February 2008,102 children suspected of OSAHS underwent overnight polysomnogram monitoring in our medicine sleep center.OSAHS was diagnosed according to the general criterion[Draft of guidelines for the diagnosis and treatment of pediatric sleep apnea hypopnea syndrome(Urumqi)which was published in Chin J Otorhinolaryngol Head Neck Surg in February,2007]. One-hundred and two children were divided into two groups according to the guidelines.Sixty six children[56 boys,10 girls;aged 4-17 years,(7.01±2.24)years(x±s)]who were diagnosed as OSAHS were enrolled in study group and 36 children[29 boys,7 girls;aged 4-13 years,(7.44±2.68)years]who were diagnosed as primary snoring made up control group.The difference of spontaneous arousal index,total arousal index and arousal index related to respiratory stimuli and limb movement were compared between the two groups.Besides this,the correlation between spontaneous arousal index,total arousal index and arousal index related to respiratory stimuli and limb movement and AHI and LSaO2 were also analyzed.Furthermore,the study group were divided into three subgroups according to AHI (≤10 times/h,10 times/h＜AHI≤20 times/h,＞20 times/h).Spontaneous arousal index,total arousal index and arousal index related to respiratory stimuli and limb movement were compared among the three groups.Results The increased total aronsal index and arousal index related to respiratory stimuli and the decreased spontaneous arousal index of study group were significant as compared to those of control group (Mann-Whitney U,z value,-3.148,-3.866,-2.791;P value,0.002,0.000,0.005,respectively).The increased arousal index related to respiratory stimuli were significant as being compared among the three groups.Arousal index related to respiratory stimuli wag correlated with AHI(COefficient correlation:0.734.P=0.000).Other kinds of arousals and arousal index related to respiratory stimuli were not correlated with LSaO2(Spearman rank correlation analysis).When compared to control group,stage I increased and stage REM decreased and the difference was significant(z were-2.423,-3.519;P were 0.015,0.000).Conclusions The arousal index related to respiratory stimuli were increased and spontaneous arousal index were decreased in children with OSAHS.Arousal index related to respiratory stimuli is more suitable to show the degree of sleep fragment than other arousal index.

OBJECTIVE To explore the anti-tumor effect and mechanism of Xiaoai Jiedu Decoction on colon cancer.METH-ODS CT26 cells were injected subcutaneously into the right armpit of C57 mice to establish the tumor-bearing model.These mouse were sacrificed after 14 d administration.Mononuclear cells of the peripheral blood were isolated and the distribution of Treg cells were detected by flow cytometry.The expression of Foxp3 protein in tumor tissue was detected by immunohisto-chemistry method.RESULTS Xiaoai Jiedu Decoction significantly inhibited the proliferation of colon cancer in mice,decreased the distribution of CD4+Foxp3+Treg cells in peripheral blood and decreased the expression of Foxp3 protein in tumor tissues.CONCLUSION Xiaoai Jiedu Decoction can significantly inhibit the proliferation of colon cancer by decreasing the distribution of Treg cells in peripheral blood and the expression of Foxp3 in the tumor tissues,thus affecting the downstream signal pathway.

OBJECTIVE To study the anti-proliferative activity and mechanism of Xiao'ai Jiedu decoction against breast cancer cell lines MDA-MB-231.METHODS The cell morphology was observed by microscope.MTT method was used to in-vestigate the inhibitory rate of Xiao'ai Jiedu decoction against breast cancer cell lines MDA-MB-231.Flow cytometry analyzed the cell cycle distribution.Western blot detected the expression of Cyclin B1 and CDK1 and the expression of upstream signa-ling molecules P53,P21 in PI3K/AKT signaling pathway.RESULTS Compared with the control group,MDA-MB-231 cells of Xiao'ai Jiedu decoction group showed number-decreased,loosely arranged,morphology-smaller and rounder.Xiao'ai Jiedu decoction inhibited the proliferation of MDA-MB-231 cells(P<0.05,P<0.01).Xiao'ai Jiedu decoction down-regulated the ex-pression of Cyclin B1 and CDK1,inhibited the PI3K/AKT signaling pathway,and up-regulated the expression of P53 and P21 (P<0.05,P<0.01).CONCLUSION Xiao'ai Jiedu decoction can inhibit the proliferation of MDA-MB-231 breast cancer cells,perhaps related to inhibiting the PI3K/AKT signaling pathway.
