1.MSCT characteristics of septic pulmonary embolism caused by intravenous drug abuse

Objective To analyze MSCT characteristics of septic pulmonary embolism (SPE) caused by intravenous drug and to improve diagnosis. Methods The MSCT findings of 16 patients of SPE induced by intravenous drug were analyzed retrospectively from March, 2001 to September, 2008. Results Peripheral or sub-pleural zones were commonly affected mainly within upper lung. Patchy, nodular and cavity shadows were detected respectively in 9 (56.25%), 12 (75.00%) and 8 patients (50.00%), while pulmonary cysts in 14 patients (87.50%). Six patients underwent CTPA, and pulmonary arteries filling defect was found in 2 patients. Pleural effusion and pneumothorax were also found in 10 and 3 patients, respectively, whereas miscellaneously shaped lesions were deteced in all 16 patients. Conclusion MSCT is an important method for diagnosing SPE caused by intravenous drug abuse. Pulmonary arteries filling defect is the direct sign and the cysts and nodular shadow with or without cavity in peripheral or sub-pleural pulmonary zones are characteristic findings.
2.In vitro Antiviral activity of a berberine derivant HB-13 against herpes simplex virus

Objective To evaluate the in vitro antiviral activity of HB-13,a compound derivant from berberine and its prodrug berberine,against herpes simplex virus type 1 (HSV-1) and type 2 (HSV-2). Methods Vero cells were cultured in vitro and infected with HSV.Then,various concentrations of HB-13, berberine,and aciclovir were used to treat these infected cells.The cytopathic effect was observed to deter- mine the antiviral effects and cytotoxicity of HB-13 and berberine.Results For HB-13,berberine and acy- clovir,the half toxicity concentration (TC_(50)) to Vero cells was 31.99,380 and more than 800?g/mL, respectively;the average half inhibitory concentration (IC_(50)) against HSV-1 was 1.328,more than 100,and 0.443?g/mL,respectively,the treatment index (TI) against HSV-1 was 24.09,less than 3.80,and more than 1805.87,respectively;the IC_(50) against HSV-2 was 1.344,more than 100,and 0.679?g/mL,respectively,the TI against HSV-2 was 23.80,less than 3.80 and more than 1178.20,respectively.Conclusion HB-13 possesses marked antiviral activity against HSV-1 and HSV-2 in vitro,while berberine does not.
3.Hybrid procedure for infants with ventricular septal defect and coarctation of aorta: a review of 20 cases

Objective To evaluate the early clinical efficaoy of hybrid procedure for infants less than six months old with ventricular septal defect and coarctation of aorta.Methods From January 2010 to July 2011,20 patients with ventricular septal defect and coarctation of aorta received hybrid procedure in our center.The body weight was (4.5 ± 1.6) kg ( ranged from 1.9 kg to 6.5 kg) and the age was ( 56 ± 45 ) days ( ranged from 18 days to 6 months).The pressure gradient of the coarctation of the aorta ranged from 30 mm Hg to 56 mm Hg,5 patients of them were diagnosed as hypoplasty of aortic arch.The size of the ventricular septal defect ranged from 8 mm to 16 mm.Results The mortality was zero in all the 20 cases during the surgery,and the mobidity was 20% (4/20).The complications were pneumonia in 2 cases,infective endocarditis in 1 case and pneumothorax in 1 case.The diameter of coarctation of the aorta ranged from 1.5 mm to 3.4 mm,and the size of the balloon ranged from 4 mm to 12 mm.The pressure gradient of the coarctation of the aorta decreased to 0 to 27 mm Hg.The bypass time ranged from 40 minutes to 87 minutes,and the crossclamp time of the aorta ranged from 20 minutes to 41minutes.The atrial septal defects were repaired and the patent ductuses were ligated during the surgery without leaving the sternum open.The total operation time was (4.0 ± 0.7 ) hours ( ranging from 3.0 hours to 5.2 hours).The mean ventilation time was (2.2 ± 1.4) days and mean ICU stay time was (5 ± 3 ) days.All the patients were followed up for ( 10.0 ± 3.6) months without aneurysm in arch and obstruction in airway.The residual obstructive pressurc gradicnt in the aortic arch ranged from 12 mm Hg to 35 mm Hg and 2 patients received reintervention.One patient received re-balloon dilation and the other received surgery.The cardiac function reached NYHA Ⅰ - Ⅱ in all eases.Conclusion The early outcome of the hybrid procedure (balloon dilation of the coartation of the aorta and surgical repair of ventricular septal defect) for infants with ventricular septal defect and coarctation of aorta was satisfying,which could avoid from circulatory arrest.It is a relatively safe procedure which could be the optional method for one-stage surgical repair.
5.Correlation analysis of P, IFN-γ level of artificial insemination during pregnancy and pregnancy outcome

Objective To investigate the correlation of P,IFN-γlevel of artificial insemination during pregnancy and pregnancy outcome.Methods 468 patients with infertility were done artificial insemination treatment cycles.The dydrogesterone were taken orally after surgery to luteal support.The blood P and IFN-γlevel of 54 patients with pregnant were observed.Results The correlation analysis of P,IFN-γ level of normal pregnancy group showed that the P level was negatively correlated with IFN-γlevel (r =-0.594,P < 0.05).In the biochemical pregnancy group,the P level was decreased from the ninth days begin,and compared with the normal pregnancy group,the difference was significant(t =12.319,P <0.05).In the abortion group,the P level was decreased from the sixteenth days begin,and compared with the normal pregnancy group,the difference was significant (t =7.663,19.315,all P <0.05).Compared with the normal pregnancy group,the IFN-γ level of the biochemical pregnancy group and abortion group was significantly increased,the difference was significant (P < 0.05).Conclusion The use of luteal support methods to raise the level of P in vivo has a positive effect on the prevention of biochemical pregnancy and spontaneous abortion,and monitoring the blood P and IFN-γlevel after artificial insemination can predict the prognosis of patients.
6.Measurement of mandibular incisive canal by cone beam CT

Objective This study was conducted to measure the mandibular incisive canal (MIC) by cone beam computed tomography(CBCT)and to assess 3?dimensional structure, course and adjacent structure of the MIC , in order to ensure safe region for surgery of the mandibular interforaminal area. Methods A total of the CBCT images from 83 patients were studied and measured. The detection rates of MIC were calculated The diameter of the MIC and the distances from MIC to the labial and lingual cortices and the inferior border of the mandible and tooth tips were measured at canines and lateral incisors. Results MIC was found in 67.8%of the patients. In the position of canine, the diameter of MIC, the mean distance of the MIC to the tips of the teeth, to the buccal border of the mandible , to the lingual border of the mandible and the inferior border of the mandible were (1.3±0.4), (8.9±2.9), (4.3±0.9), (5.2±1.1) and (8.6± 1.5) mm, respectively. In the position of the incisor, the distance of MIC, the mean distance of the MIC to the tips of the teeth, to the buccal border of the mandible , to the lingual border of the mandible and the inferior border of the mandible were (1.0 ± 0.3), (13.3 ± 4.0), (4.2 ± 0.9), (5.9 ± 0.9) and (8.9 ± 1.8) mm, respectively. Conclusion CBCT clearly show three dimensional structure, course and adjacent structure of the MIC. Preoperative CBCT can provide vital information for surgery of the mandibular interforaminal area.
7.Effects of High Intensity Interval Training on Type 2 Diabetes Mellitus:A Meta-analysis

Objective To evaluate the effect of high intensity interval training(HIIT)on type 2 diabetes mellitus(T2DM). Methods Published articles from the earliest date available to September the first,2016 were recalled from PubMed,Em-base,Cochrane Library,EBSCO,Web of Science,CNKI and CBM Database.Two reviewers selected indepen-dently the randomized controlled trials(RCTs)about HIIT for patients with T2DM,and extracted data indepen-dently and analyzed with RevMan 5.3. Results Eight trials that represented 275 participants were evaluated.HIIT could significantly improve glycated hemo-globin(SMD=-0.72,95%CI-1.39~-0.04,P=0.04)and body mass index(MD=-0.60,95%CI-1.03~-0.18,P=0.005)more than the control group,but no difference was found in the levels of fasting blood-glucose,fasting in-sulin,insulin resistance,and indexes of blood fat,body mass,body fat mass and waist circumference(P>0.05). Conclusion There is no evidence showed that HIIT is more effective on blood glucose,blood fat and body composition in the patients with T2DM,which needs further research.
8.How to cover the defect of great toe after the wrap-around flap transfer

Objective To explore the methods for coverage of the defect of great toe after the wrap-a- round flap transferand decrease the morbidity of donor site in great toes.Methods Twenty-five patients received three kinds of procedure for immediate resurfacing of donor defect of the great toes during wrap-around flap transferAmong them9 cases received the free flaps for coverage of defect in donor great toes12 cases was repaired by local pedieled dorsal or plamarpedis flapsand the other cases were treated by the nail-flap of second toe.Results All the flaps were survivalTwo patients received the flap thinning procedure in 6 months laterall patients were satisfied with cosmetic and functional outcomeThe appearance and sensory function of donor toe repaired by second toe nail-flap was best among three methods.Conclusion Accord- ing the detect situation of great toesthree kinds of flap were selected for immediate coverage of donor site, which can decrease the complication of donor great toe at the most.
9.Detection of FLT3 gene and FLT3/ITD mutation by polymerase chain reaction-single-strand conformation polymorphismin in patients with acute lymphoblastic leukemia

Objective To analyze Fms-like tyrosine kinase 3 (FLT3) gene and FLT3 internal tandem duplication (ITD)mutation in acute lymphoblastic leukemia (ALL) patients of different immunological subtypes. Methods Polymerase chain reaction-single-strand conformation polymorphism (PCR-SSCP) was used to detect FLT3 gene and FLT3/ITD mutation in 63 ALL cases. Results Among the 63 ALL cases, FLT3 gene was detected in 41 (61.5%) cases. The positivity rate of FLT3 gene in pre-pre B-lineage ALL, pre-B-ALL, B-lineage ALL and T-lineage ALL cases were 93.3% (14/15), 77.8% (14/18), 41.7% (5/12) and 28.6% (4/14), respectively. The positivity rate of FLT3 gene was significantly higher in pre-pre B-ALL/pre B-ALL subtypes (84.8%) than in B-ALL subtypes (41.7%, P<0.005), and the rate was significantly higher in B- ALL subtypes (73.3%)than in T-ALL subtypes (28.6%, P<0.001). Two cases (3.2%) were found to have FLT3/ITD mutation, which were also positive for myeloid antigen expression and diagnosed as acute mixed-lineage leukemia, showing leukocytosis and high percentage of bone marrow blast cells with poor prognosis. Conclusions FLT3 gene can be detected in both B- and T-lineage ALL patients, but more frequently in the former. In B-lineage ALL patients, FLT3 gene is more frequent in cases with undifferentiated than those with differentiated blast cells. FLT3/ITD is rarely detected in ALL patients and FLT3/ITD mutation detection might be helpful to identify the genotypes and evaluate the prognosis of acute leukemia.
