1.Biological treatment for intervertebral disc degeneration.
China Journal of Orthopaedics and Traumatology 2016;29(6):576-580
Neck shoulder pain or lumbocrural pain caused by intervertebral disc degeneration (IDD) could seriously affect the qualities life of patients. Current treatments mainly focus on alleviating pain and the symptoms of nerve compression, which could not radically stop the process of intervertebral disc degeneration, but conversely lead to high recurrence rate. In recent years, scholars have turned to study the biological treatment for repair and rebuild the intervertebral disc by biological molecular therapy, gene therapy, cell therapy and tissue engineering to solve the problem of intervertebral disc degeneration, while most of the above methods are still in animal experiments or in vitro experiments and the clinical application is still a long way to go.

Objective To evaluate the engraftment state after allogeneic peripheral blood stem cell transplantation(Allo-PBSCT)in 2 exposed patients in Shandong “10?21” radiation accident. Methods The dynamic cytogenetic analysis was performed by chromosome aberration(CA)analysis,R-band karyotyping analysis and micronucleus(MN)assay of peripheral blood and/or bone marrow samples of 2 patients(A and B).Results The results showed that the transplantation was successful(complete chimerism),and the engraftment state was stable. No difference was observed between the results of cytogenetic analysis from peripheral blood samples and bone marrow samples. These results were in fairly good accordance with those results by short tandem repeats-polymerase chain reaction(STR-PCR)and clinical manifestations. Conclusions The results suggested that the dynamic observations of dicentric and ring aberrations and MN can be used as an indication to estimate the engraftment state after Allo-PBSCT,but only for the patient with acute radiation sickness. Moreover, the cytogenetic analysis performed on bone marrow samples is better than that on peripheral blood samples when the lymphocytes were reduced sharply in peripheral blood.

Medical Journal of Chinese People's Liberation Army 1981;0(04):-
Objective A biological dose estimation by cytokinesis-block micronucleus(CBMN)assay was performed on 2 patients exposed to accidental radiation.Method Micronucleus assay and dose estimation by CBMN in peripheral blood and bone marrow were performed in 2 patients(A and B)suffering from radiation sickness. Results No binucleated lymphocytes were observed in peripheral blood of 2 patients. The results of bone marrow examination showed that only one binucleated cell was discovered in patient A.The rough estimation of radiation dose of A was above 20Gy according to the degree of reduction of binucleated cell in our previous study. The micronucleus frequency was 2.42 per cell and the dose estimation was 8.7(8.0-9.4)Gy for patient B.These results were approximately in accordance with that of dose estimations by physical method, chromosome aberration analysis,ESR method, and clinical manifestations. Conclusion MN assay is simple, quick and accurate. It is a reliable biodosimetry in addition to chromosome aberration analysis.

HER2, a member of epidermal growth factor receptor family proteins, is overexpressed in about 30% of human breast cancer. Increased levels of HER2 are associated with poor patient prognosis and enhanced metastasis. RNA interference (RNAi) is developed recently as a new technique which can inhibit gene expression specifically in mammalian cells. On the basis of previous study,in which two target sequences with favorable RNAi effect on HER2 were identified, a series of dual promoter siRNA-expressing vectors containing two opposing U6 and H1 promoters were constructed. After transfection of HER2-overexpressing SKBR3 breast cancer cells with the siRNA-expressing vectors, downregulation of HER2 was identified quantitatively. Subsequently, the siRNA-expressing cassettes were subcloned into lentiviral vectors by LR recombination reaction and lentivirus was prepared successfully. The results from infection of SKBR3 cells with siRNA-expressing lentivirus demonstrated that lentiviral-mediated RNAi could downregulate HER2 expression efficiently through fluorescent quantitative PCR (FQ-PCR), western blot, and FACS analysis. Furthermore, cell growth was inhibited in cell proliferation assay after treatment with siRNA lentivirus.A new tool for clarifying the function of HER2 in cancer metastasis and developing the gene therapy drug was offered.

Objective:To understand the recognition status of cancer pain in basic medical staff from small towns to provide the basis for the improvement of cancer pain management in these areas. Methods:The medical staff of Hengjiang Town and subordinate villages was selected. The study area is situated in southwest China. Centralized questionnaires regarding cancer pain were collected and analyzed. A program and education of cancer pain were provided for these medical workers. Results: The medical staff from Hengjiang asserted that only 17%of cancer pain patients receive treatments. Approximately 70%of the medical staff did not consider the popularization and explanation of cancer pain treatment in their patients. Approximately 64%of the medical staff was not familiar with standardized cancer pain control, 87%did not believe that narcotics could suffice the need of patients, and 44%did not participate in the training for cancer pain control. Conclusion: The medical staff in Hengjiang possesses less knowledge on the importance of cancer pain. Hence, further training is necessary. The specific management of cancer pain as a part of community chronic diseases is mandatory.

Journal of Modern Laboratory Medicine 2016;31(3):12-15
Objective To investigate the relationship between manganese superoxide dismutase (Mn-SOD)9 Ala/Val genetic polymorphisms and the levels of blood lipid and homocysteine (HCY).Methods The genotypes of Mn-SOD 9 Ala/Val ge-netic polymorphisms were identified by sequencing method, the serum activities of T-SOD and Mn-SOD were detected by colorimetric method, the serum level of HCY was detected by enzymatic method, and the serum levels of cholesterol (TC), triglyceride (TG),high density lipoprotein cholesterol (HDL-C)and low density lipoprotein cholesterol (LDL-C)were de-tected by end-point method in 137 patients with coronary heart disease (CHD)and 85 controls. Results Compared with the control group, the VV genotype and V allele of Mn-SOD 9 Ala/Val genetic polymorphisms in the CHD group were higher, while the serum activities of T-SOD and Mn-SOD in the CHD group was significantly lower. The serum activities of T-SOD and Mn-SOD of the Mn-SOD 9 VV genotype was significantly lower than the Mn-SOD 9 AA genotype.Compared with the Mn-SOD 9 AA genotype, the serum levels of TC,TG,LDL-C and HCY of the Mn-SOD 9 VV genotype were significantly higher, while the serum level of HDL-C was significantly lower. The serum activity of Mn-SOD was negativelycorrelated with the serum levels of TC,TG,LDL-C and HCY and positively correlated with the serum level of HDL-C.Conclusion The antioxidative ability in patients with CHD was decreased.Mn-SOD 9 Ala/Val genetic polymorphisms led to lipid metab-olism disorders by affecting the Mn-SOD activity, promoting the development of CHD.HCY resulted in increased oxidative substances by self-oxidation and inhibition of the Mn-SOD activity, increasing the risk of CHD.

OBJECTIVE:To systematically review the efficacy of agomelatine in the treatment of depression in acute phase, and provide evidence-based reference for the clinical treatment. METHODS:Retrieved from Cochrane Library,Medline,EMBase, CJFD,VIP,CBM,Wanfang Database,WHO Clinical Trials Registry Platform and American Clinical Trials Registry Platform, the randomized controlled trials (RCT) about agomelatine versus placebo in the depression patients in acute phase. After quality evaluation and data extraction,Meta-analysis was conducted by using Rev Man 5.2 statistics software. RESULTS:A total of 7 RCT were included, involving 2 378 patients. Results of Meta-analysis showed the effective rate in agomelatine group was obviously bet-ter than placebo group [RR=1.43 ,95%CI(1.29 ,1.59),P<0.001] ,remission rate was obviously better than placebo group [RR=1.27 ,95%CI(1.03 ,1.57),P=0.02] ,and the endpoint score of depression scales was obviously lower than placebo group [MD=-2.92,95%CI(-3.65,-2.20),P<0.001],there was statistically significance. CONCLUSIONS:Agomelatine is effective in the treatment of depression patients in acute phase. However, due to the limit of methodological quality and sample size,it remains to be further verified with more rigorously designed and long-term follow-up of large-scale RCT.
