Liver transplantation is one of the most effective treatment of hepatocellular carcinoma.And rapamycin is a new class of immunosuppressors already in use after liver transplantation.Recently.more and more studies find that rapamyein not only can exert immunosuppressive effects but also has antitumor properties.Therefore,the research about rapamycin in hepatocellular carcinoma has become a focus.Herein,we review the progress of rapamycin in the treatment of hepatocelhlar carcinoma together with current results and future developments.

AIM: To establish the determination of ginsenoside R e and R g1 in Compound Jiangtang Oral Liquid(Radix Ginseng, Fructus Schisandrae Chinensis, Radix Rehmannia, Radix Asparagi, etc.) by HPLC. METHODS: C 18 ODS was used as a stationary phase, acetonitrile-0.05% phosphoric acid (96∶400) as a mobile phase and detection wavelength at 203nm. RESULTS: The linear range of ginsenoside R e concentration was from 0.66 to 3.29?g?mL -1 and correlation coefficient was 0.9991, the linear range of ginsenoside R g1 concentration was from 1.05 to 5.27?g?mL -1 and correlation coefficient was 0.9998. The average recovery of sample was 98.2% and RSD 2.2% ( n =5), respectively. CONCLUSION: The method is simple and convenient, accurate with a good reproducibility and can be used for determination of ginsenoside R e and R g1 in Compound Jiangtang Oral Liquid.

Objective To evaluate the analytical variables of the reversed-phase high pressure liquid chromatography (HPLC) with 5-Fluorouracil(5-Fu) as an internal standard for determining creatinine in human serum and urine. The method was used as a candidate reference method for accuracy assessment of routine creatinine test kits.Methods We tested the accuracy, precision, linearity of the reversed-phase HPLC.We analyzed split samples of a panel of 85 patients’ serum and 94 patients’ urine and compared the results of the routine test kits and HPLC by means of bias plots (percentage differences of results) and standard linear regression.Results The linear range of HPLC method was up to 2 210 ?mol/L, the within-run CV(n=5) was below 2.5% and the between-day CV(n=10) was less than 4.5%. The analytical recovery rate was 96.3%～102.4%. All of the test kits correlated very well with HPLC.Conclusions We recommend the reversed-phase HPLC with 5-Fu as an internal standard as a candidate reference method for determining creatinine in human serum and urine.The enzymatic method with creatininase coupled sarcosine oxidase is suitable for routine work in clinical laboratories.

Objective:To investigate the effects of iron sucrose combined with recombinant human erythropoietin(EPO)on perioperative anemia and early functional recovery in elderly patients with intertrochanteric fractures.Methods:A retrospective comparative analysis was performed on 78 elderly patients who received PFNA treatment for intertrochanteric fractures in our hospital from July 2016 to December 2018.In the observation group, 40 cases were treated with iron sucrose combined with erythropoietin during the perioperative period.No perioperative intervention measures were taken for the control group.Hemoglobin(Hb)levels at 1, 3, 5, 7 days and before discharge and Harris scores of hip joints before surgery, 2 weeks and 3 months after surgery were compared between the two groups.Results:Hb levels were higher in the observation group than in the control group at 5 d[(98.8±8.6 g/L vs.92.2±8.5 g/L, t=3.389, P=0.001)]and 7 d[(105.4±8.9)g/L vs.(94.8±8.5)g/L, t=5.370, P=0.000]after surgery and before discharge[(115.4±8.6)g/L vs.(100.6±8.2)g/L, t=7.771, P=0.000]. Harris scores were higher in the observation group than in the control group 3 months after surgery(82.4±3.8 vs.75.8±2.5, t=8.933, P=0.000). Pearson' s correlation analysis showed that Hb levels at 5d and 7d after surgery were positively correlated with Harris scores at 3 months after surgery.Hb levels before discharge had an effect on Harris scores at 3 months after surgery. Conclusions:The combined administration of iron sucrose and EPO perioperatively can promote postoperative Hb recovery, reduce the incidence of anemia, accelerate early functional recovery and improve the quality of life in elderly patients with intertrochanteric fractures.

Objective To investigate the expression levels and the roles of IL-17 and IL-23 in children with Mycoplasma pneumoniae (MP) pneumonia. Methods One hundred and three children with pneumonia admitted to pediatric department from February to May in 2012 were divided into MP pneumonia group and non-MP pneumonia group according to the results of MP antibody tests. Meanwhile, 42 healthy children were chosen as normal controls. Serum levels of IL-17, IL-23 and MP antibodies were measured in all children. Immunoglobulin, C reactive protein, total white blood cell count and granulocyte count were detected in children with pneumonia. Results The serum levels of IL-17 and IL-23 were signiifcantly different among three groups (P<0.05). The children in MP pneumonia group had higher levels of IL-17 and IL-23 than those in non-MP pneumonia group (P<0.05). Compared with the control group, levels of IL-17 and IL-23 were higher in two pneumonia groups (P<0.05). There was no signiifcant difference in levels of IL-17 and IL-23 between pneumonia patients with normal and with abnormal levels of immunoglobulin (P>0.05), while IL-17 and IL-23 levels were both positively correlated with granulocyte count (P<0.05).Conclusion IL-17 and IL-23 may be involved in the immune response of MP pneumonia and may contribute to the clearance of pathogens.

Objective:To compare the effects of rotational night shifts on the micturition patterns of fe-male nurses.Methods:A total of 58 nurses without lower urinary tract symptoms were recruited,who worked in the Peking University People’s Hospital during January and June in 2014.The nurses aged 20 -43 years were divided into two groups,the night-shift group (n =28)and the non-shift group (n =30).The alcohol or coffee intaking were forbidden.In the night-shift group,nurses had worked on rota-tional shifts for at least 6 months.Their average age was (26.75 ±4.11)years.In the non-shift group, nurses took regular day-time work,whose average age was (27.80 ±5.60)years.A voiding diary was kept for 7 consecutive days at the end of 6 months,starting 2 days before their night duties until 4 days after completion of their night duties.For comparison,the non-shift group with regular shifts completed a 7-day voiding diary.In the 7-day recording voiding diary,the nurses were required to have the normal in-take of liquid about 1 500 -2 000 mL/d.The frequency volume charts of nocturia,the 8-hour interval urine production and frequency were compared between the two groups.Results:Nocturia frequency was increased in the night-shift group [0.5 (0 -2.4)]compared with the non-shift group [0 (0 -2),P =0.02].The volume of nocturia was increased in the night-shift group [125 mL (0 -660 mL)]compared with the non-shift group [0 mL (0 -340 mL),P <0.01].The 8-hour interval indices showed that urine production changed with shift (P <0.01).In the consecutive 7 days,the nocturnal volume of the night-shift group increased on the day after night shift.When the night-shift nurses returned to daytime duty, the volume of urine decreased but nocturnal urine production remained high,and the frequency of noctu-ria also increased significantly (P <0.05).Compared with the 8-hour interval indices,the night-shift group’s voiding volume [(542.35 ±204.66)mL]and voiding frequency (2.24 ±0.69)were more than those of the non-shift group at the afternoon time (from 2 pm to 10 pm).During the 8 h interval night time (from 10 pm to 6 am),the volume of nocturia in the night-shift group [(309.74 ±162.74) mL]was more than that in the non-shift group [(199.38 ±153.98)mL,P =0.01];the frequency of nocturia in the night-shift group (1.31 ±0.52)was increased than that in the non-shift group (0.82 ± 0.55,P <0.01).Conclusion:The rotational shifts affect the micturition patterns of nurses who go through the night shift work,which increases the volume and frequency of the nocturia.

Objective To determine the minimum effective local anesthetic dose (ED50 ) of spinal ropivacaine for arthroscopic meniscecto-my .Methods Twenty-five patients undergoing arthroscopic meniscectomy under combined spinal-epidural anesthesia received intrathecal ropivacaine .ED50 of ropivacaine was determined by the sequence method .The effective criteria were as follows :the level of sensory block reaching at least T12 within 20 min after injection of the local anesthetic ;the motion block reaching the Bromage Scale score ≥2 within 20 min after injection of the local anesthetic ;no supplemental epidural anesthetic at least 1 h after injection of the local anesthetic .The initial dose was 15mg and according to the effective or ineffective results in previous patient ,a dose of ropivacaine 1 mg was decreased or increased . Results ED50 of ropivacaine for the arthroscopic meniscectomy was 12 .24 mg(95% CI 12 .09-12 .39) .Conclusion The dose of ropiva-caine more than 12 .24 mg for arthroscopic meniscectomy is the best .But the anesthetic effect may be related with the specific gravity of the local anesthetic and the patients′factors(height ,weight ,age) .

OBJECTIVE: To evaluate the effect of hypoxia improvement in Hep-2 cell on cisplatin-induced apoptosis. METHOD: Human laryngeal squamous cell carcinoma Hep-2 cells and HIF-1α-RNAi-Hep-2 cells were cultured in normoxic, hypoxic and reoxygenation condition. The inhibition of cisplatin on cell proliferation was evaluated by MTT assay. The influence of cisplatin on cell cycle and apoptosis were detected by flow cytometry. RESULT: The inhibition of cisplatin on cell proliferation was reduced by hypoxia. After HIF-1α gene was silenced, the inhibition of cisplatin on Hep-2 cell proliferation was increased apparently, but was still interfered partly by hypoxia. Hypoxia could induce cell apoptosis decreased and enhance chemotherapeutic resistance. After reoxygenation, cell apoptosis induced by cisplatin was increased significantly (P < 0.05). HIF-1α-RNAi-Hep-2 cells under hypoxia also showed certain resistance to apoptosis but the sensitivity to cisplatin was higher than that of Hep-2 cells. When cells were returned from hypoxic condition to normoxic condition for some time, the apoptosis induced by cisplatin was increased significantly (P < 0.05). CONCLUSION The improvement of hypoxic microenvironment with HIF-1α gene knockout could increase the sensitivity of Hep-2 cells to chemotherapy.

Objective To investigate whether the effect of transient high glucose on inflammatory factors expression could be continuous in rat glomerular mesangial cell,and its relation with histone methylation modification.Methods Rat glomerular mesangial cells (HBZY-l) were divided into three groups:the high glucose group (25.0 mmol/L glucose),the hypertonic group (MA,5.5 mmol/L glucose+ 19.5 mmol/L mannitol) and the normal-glucose control group (5.5 mmol/L glucose),which were cultured for 24 h respectively.All 3 groups were then changed with normal-glucose medium to culture for 24 h,48 h and 72 h.Their protein,mRNA and supernatant were harvested.The protein expressions of mono-methylation of H3 lysine 4 (H3K4mel) was measured by Western blotting,and the mRNA expressions of NF-κB subunit p65 and set7/9 were determined by real timequantitative PCR.The expression of monocyte chemoattractant protein 1 (MCP-1) and vascular cell adhesion molecule 1 (VCAM-1) were detected by enzyme-linked immunosorbent assay.Results (1)Compared with those in normal control group,the expressions of H3K4mel protein and set7/9 mRNA were first up-regulated in high glucose group,then gradually down-regulated in the following 48 h normal-glucose medium (as compared with those at 0 h,all P ＜ 0.05).At 72 h there was no statistic difference between high glucose group and normal control group (all P ＞ 0.05).(2) Compared with those in normal control group,the up-regulated p65 mRNA,VCAM-1 and MCP-1 sustained at least for 72 h in high glucose group.Conclusions Transient high glucose can induce persistent inflammatory factors expression in rat glomerular mesangial cells,which may via histone modification.

Objective To investigate the expression vibration of microRNA-503(miR-503) and its effect on target gene Bcl-2,caspase enzyme activity and apoptosis of human renal tubular epithelial cells (HK-2) induced by high glucose,and to clarify the pathogenesis of renal tubular injury induced by high glucose.Methods HK-2 cells were cultured in normal glucose group (NG),mannitol hypertonic control group (MA),and high glucose group (HG).The morphology of apoptotic cells was observed using inverted microscope.The expression of miR-503 was determined using realtime quantitative PCR.The apoptosis rate of HK-2 cells was detected by Annexin V-FITC double dye using flow cytometry instrument.The expression of Bcl-2 and cleaved caspase-9 were detected by Western blotting.Results In the high glucose and mannitol groups HK-2 cell,an obviously increased apoptotic rate was observed under inverted microscope compared with normal glucose group (P ＜ 0.05).MA and HG up-regulated miR-503 expression (P ＜ 0.01),down-regulated anti-apoptotic protein Bcl-2 expression (P ＜ 0.05) and up-regulated cleaved caspase-9 (P ＜ 0.05).Conclusions The expression of miR-503 increases in HK-2 cells cultured by high glucose and mannitol.MiR-503 promotes apoptosis of HK-2 cells via activating mitochondrial apoptotic pathways and enhancing cleaved caspase-9 for Bcl-2 insufficiency.The tubular toxicity of high glucose is partly due to osmotic pressure.The miR-503 may be involved in diabetic tubular injury and may be a new therapeutic target of DN.
