Objective To evaluate the clinical value of three-dimensional reconstruction and virtual hepatectomy for laparoscopic precise liver resection of right liver tumor.Methods 31 cases of right liver tumor from June 2012 to October 2015 at Department of Hepatobiliary Surgery in Affiliated Hospital of Nantong University,were analyzed retrospectively.Patients were divided into three dimensional reconstruction group (n =15) and control group (n =16),Operation time,bleeding volume during operation,postoperative complications and days of hospitalization after operation in two groups were observed.Results All operation was successfully performed with pure laparoscopic right liver tumor resection in three dimensional reconstruction group.In control group,1 case was completed by hand-assisted laparoscopic liver resection,1 case was converted to open operation,the remaining 14 cases underwent total laparoscopic right liver tumor resection.Operation time in 3-dimensional reconstruction group was (141 ± 36)min vs.(207 ± 66)min in control group,bleeding volume was (274 ±88)ml vs.(418 ± 189)ml,hospitalization after operation was (9 ± 3)days vs.(13 ±6)days (all P ＜0.05).Conclusions Preoperative three-dimensional reconstruction and virtual hepatectomy can ensure the localization of parenchyma tumor during the laparoscopic precise liver resection,clarifing the relationship between tumor operation section and peritumoral vascular or biliary ducts,fulfilling the idea of precise liver resection.

AlM: To explore the different ages of congenital nasolacrimal duct obstruction in infants, take different treatment methods at different times.
METHODS:The 87 cases of 102 children were divided into three different age groups: the first group of 25d-3mo of age 21 cases 26 eyes; The second group >3mo-7mo 31 cases 36 eyes;The third group >7-24mo of age 35 cases 40 eyes. For the first group of infants, the implementation of the lacrimal sac nasolacrimal duct massage + eye drops; for the second group of infants, carry lacrimal pressure washing treatment; for the third group of infants, the implementation of the nasolacrimal duct probing treatment.
RESULTS: The first group of children through the nasolacrimal duct sac massage + drops tobramycin eye drops treatment unobstructed 12, the cure rate was 46. 2%;The second group of children through pressurized irrigation treatment lacrimal patency by 33, the cure rate was 91. 7%; The third group of children through the nasolacrimal duct probing unobstructed 36 treatment, the cure rate was 90. 0%. The second and third group were better than the first group (χ2=15. 71, P<0. 01;χ2=15. 27, P<0. 01);the treatment effect of the second and third groups was no significant difference (χ2=0. 02, P>0. 05).
CONCLUSlON:lnfants with congenital nasolacrimal duct obstruction should distinguish between ages, taking different treatments, in order to obtain a better therapeutic effect, and lacrimal pressure washing is the preferred way of treating infants with congenital nasolacrimal duct obstruction.

OBJECTIVETo introduce the framework of evidence-based practice with a case of castration-resistant prostate cancer (CRPC) as an example.

METHODSA clinical question was formulated according the clinical scenario. A systematic search was conducted for the published literature in the databases of PubMed, EMBASE, Cochrane Library, Clinical Trial Registries, and Web of Knowledge up to Dec 2014. The identified literature was reviewed for quality appraisal before the evidence was applied to clinical practice.

RESULTSThe treatment was effective and the patient achieved disease remission.

CONCLUSIONEvidence-based practice should be integrated with clinical scenario, current evidence, and patients' willingness, and follow a systematic framework.

The expressions of Ki67 and FHIT were detected by immunohistochemical staining in 15 cases of adrenocortical carcinoma, 42 cases with adrenocortical adenoma,6 cases of adrenocortical hyperplasia, and 10 cases of normal adrenocortical tissue. The results showed that the highest expression of Ki67 and the lowest expression of FHIT were found in adrenocortical carcinoma. There were significant differences in the Ki67 and FHIT between adrenocortical adenoma and adrenocortical carcinoma ( both P < 0. 05 ). There existed negative correlation between the expressions of Ki67 and FHIT( r=-0. 712, P<0.05 ). Ki67 over-expression and loss of FHIT expression may be involved in the occurrence and development of adrenocortical carcinoma. It is suggested that combined detections of Ki67 and FHIT may have reference significance in the differentiation of adrenocortical adenoma from adrenocortical carcinoma.

In this study, OVA-induced asthma mice was taken as the model, and orally administered with different concentration of ethanol extracts of crude and processed Stemona tuberosa, in order to determine the cytokine level released from Th1 and Th2 in splenocytes. RT-PCR was carried out to determine the genetic expression of T-bet/GATA-3 in lung, and compare the differentiation between ethanol extracts of crude and processed S. tuberosa in therapeutic effect on asthma in mice. According to the results, compared with the crude samples, processed samples significantly increased the levels of inflammatory factor INF-gamma (P < 0.05) and decreased IL-5 (P < 0.05) in splenocytes. According to the RT-PCR results, the administration of processed samples could increase the ratio of T-bet/GATA-3 (P < 0.05). The experiment showed that ethanol extracts of both crude and processed S. tuberosa could treat asthma by regulating Th1/Th2 ratio, but processed samples showed more notable effect. This indicated that crude and processed S. tuberosa had significant pharmacological difference. Therefore, it was more rational to apply processed S. tuberosa in clinical treatment of asthma and chronic cough, which layed a foundation for further revealing the processing mechanism of S. tuberosa.

OBJECTIVETo observe the application effect of minimally invasive decompression, bone graft implantation and metal trabecular bone reconstruction system for early stage osteonecrosis of femoral head and discuss the treatment of hip-salvage operation in early stage osteonecrosis of femoral head;

METHODSFrom January 2010 to June 2011, 50 patients (62 hips) Which were osteonecrosis of femoral head of early stake,were treated with minimally invasive decompression, bone graft implantation and metal trabecular bone reconstruction system, including 31 males (40 hips), 19 females (22 hip) with an average age of 36.2 years old ranging from 22 to 54 years old. The course of disease was from 6 to 15 months (averaged 10.5 months). Among them, 19 cases (23 hips) were steroid-induced, 25 cases (33 hips) were alcohol-induced, 6 cases (6 hips) were idiopathic; According to ARCO stage, 28 hips were at stage I, 34 hips were at stage II. All of them were diagnosed as femoral head necrosis by imaging examination before operation. Then each patient was followed to assess by Harris hip score, curative effect, and conduct the femoral head survival analysis during the postoperation.

RESULTSAll patients had finished operation, the operation time was between 30 and 85 min, intraoperative blood loss was 50 to 220 ml, and 47 cases (58 hips) were follow-up from 24 to 46 months with an average of 34.05 months. As compared with preoperative, the Harris hip score at the last follow-up was improved, the difference was statistically significant (P<0.01). The Harris hip score, curative effect and survival time of femoral head in ARCO stage I was superior to these in ARCO Stage II, the difference was statistically significant (P< 0.05).

CONCLUSIONEffect of minimally invasive decompression,bone graft implantation combine with the metal trabecular bone reconstruction system for early stage osteonecrosis of femoral head was good,it could significantly improve the Harris hip score, increase the femoral head survival time, delay the hip replacement, and performance better in ARCO stage I.

To simplify the macrocyclic fragment and to modify the zinc binding group of the natural product apicidin, two series of S-hexyl (heptyl) ethanethioate derivatives were designed and synthesized. Twenty-six compounds were synthesized and confirmed with 1H NMR, IR, MS and HR-MS spectrum, which were not reported. Take vorinostat as control, their antiporliferative activities against cancer cell lines, MCF-7 and HL-60, were tested with MTT assay or trypan blue staining method. Generally in both series it was found that, the chiral carbon atom at 7 position is not necessary, compounds II-1, II-3, II-6 and II-13 showed good activity on HL-60 cells in vitro, with the IC50 values less than 10 micromol x L(-1). II-7 and II-8 showed stronger activity against MCF-7 than Vorinostat, with the IC50 of 3.19 and 6.29 micromol x L(-1), respectively.

OBJECTIVETo explore the influence of NH2-terminal sequence Ac-EEED peptide of alpha-smooth muscle actin (alpha-SMA) which is the specific antibody of alpha-SMA on wound contraction.

METHODS(1) Full skin loss wounds were created on the backs of Wistar rats. The wound edge was fixed by a hard plastic frame. The wounds in experimental group (EG) were applied topically with alpha-SMA fusion peptide containing Ac-DEDE at N-terminal (alpha-SMA -FP, 1 mg/ml) during 8 to 10 days after the injury, while gel only (0.5 mg/ml) and alpha-SMA -FP (1 mg/ml) were topically applied to the wounds in control group 1 and 2, respectively. The wound areas were determined at 1, 6 and 24 hours after the removal at the fixing frame at 10 days after injury. The wound contraction rates were determined by comparing the wound area after and before the frame removal. (2) The fibroblasts in the granulation tissue were isolated 9 days after injury and were cultured in deformable silicone substrate dish. The changes in cell contraction were observed before and after the fibroblasts were treated with alpha-SMA -FP (1 mg/ml) and after alpha-SMA -FP was washed away.

RESULTS(1) The wound contraction rates exhibited no evident difference at 1, 6 and 24 hours after the removal of fixing frame in control group 1 and 2 (P < 0.05). (2) There exhibited numerous wrinkles within the fibroblasts under the microscope before alpha-SMA -FP processing. But the wrinkles decreased and became shallow remarkably at 5 mins after alpha-SMA -FP processing and disappeared completely 30 mins later. The wrinkles recovered gradually after alpha-SMA -FP was removed. But the cells treated by gel and alpha-SKA -FP exhibited no such phenomenon.

CONCLUSIONalpha-SMA-AcEEED might specifically inhibit the contraction of granulation tissue and inhibit the contraction of fibroblasts, which was reversible.
