1.Comparison of MPCNL and ESWL for monotherapy of renal calculi in infants

Objective To compare the efficiency between mini-percutaneous nephrolithotomy (MPCNL) and extracorporeal shock wave lithotripsy (ESWL) for monotherapy of renal calculi in infants less than 3 years.Methods Forty-six infants were treated with either SWL (22 infants) or MPCNL monotherapy (24 infants).The mean age was (22.6 ± 8.7) months vs (23.5 ± 6.6) months and the stone size was (21.4 ± 3.5) mm vs (21.7 ± 1.7) mm,and there were no significant difference.Results For MPCNL,mean operating time was (76.2 ± 23.4) min and mean hospital stay was (14.13 ± 5.8) d.The stone-free rates were 84.0% (21/25) after first session and 96.0% (24/25) after second-look procedure.Postoperative fever happened in 4 (16.0%) cases.Hemoglobin drop was (8.5 ± 4.4) g/L and no one needed blood transfusion.For ESWL,the stone-free rate were 31.8% (7/22) after first session and 86.3% (19/22) after second session for 11 infants (50.0%).Mean hospital stay was (6.6 ± 2.3) d and 10 cases (45.5%)had complications.Hemoglobin drop was (10.6 ± 12.7) g/L.MPCNL was lower than ESWL in complications rate and re-treatment rate,and the stone-free rate was higher,but the hospital stay and operation time was longer (P < 0.05).GFR revealed improve or stable after operation in both groups.Conclusions For a higher success rate,lower complication rate and re-treatment rate,MPCNL was an effective option for the management of relatively larger stones in children (even in infants).
2.Determination of five kinds of trace ginkgolic acids in diterpene ginkgolides meglumine injection materials by LC-MS/MS.

To develop a LC-MS/MS method for the determination of five kinds of trace ginkgolic acids in diterpene ginkgolides meglumine injection materials, the column was Agilent ZORBAX Eclipse plus C18 (3.0 mm x 50 mm, 1.8 µm), and the mobile phase consisted of methanol-water (containing 0.2% formic acid) (95:5) at a flow rate of 0.5 mL · min(-1). The multiple reaction ion monitoring (MRM) with an ESI interface in the negative ion mode was selected. The results showed that the linear ranges of five kinds of ginkgolic acids were in the range of 0.2-36.0 µg · L(-1) (r ≥ 0.999 5). The lowest limit of quantification (LOQ) of ginkgo acid C13: 0, C15:1, C17:2, C15:0 and C17:1 were 0.18, 0.18, 0.21, 0.10 and 0.20 µg · L(-1), respectively. The average recovery was between 73.28% and 87.56%, and the average content of total ginkgolic acids in three batches of samples was in the range of 0.023-0.028 µg · g(-1), which was much lower than 2 µg · g(-1) prescribed in drug registration standards. This method is simple and rapid with high sensitivity, which can be used for the determination of five kinds of trace ginkgolic acids in diterpene ginkgolides meglumine injection materials.

3.Short-term follow up of bipolar arthroplasties for aged cases with intertrochanteric fractures

Objective To evaluate the short term effect of bipolar arthroplasties in aged cases with intertrochanteric fractures.Methods There were 23 males and 15 females(at age of 70-93 years, average 76 years)with unstable intertrochanteric fractures treated with the third generation cementing techniques and bipolar arthroplasties that were followed up for average 2.4 years.The clinical effect was evaluate with Harris scale,X-ray films and complications.Results Of all,34 cases(89.5%)could walk freely.The average Harris scale was 84.2 points.The average period until walk with full-weight load was 5.6 weeks.The greater trochanters was united in 35 cases(92%)during average 4.2 months.No peri-prothetic ossteolysis and loosening or subsidence occurred.Conclusions Cemented bipolar arthro- plasty has good advantages of reduced laying up period and few complications.The short-term outcome is satisfactory hut the long-term outcome needs deeper observation.
4.Effect of Shenqi Fufang(参芪复方)on Pathological Morphology and Serum Inflammatory Factors of Gastrocnemius in Type 2 Diabetic Rats

Objective To explore the effect of Shenqi Fufang (参芪复方,SF) on skeletal muscle damage of type 2 diabetes mellitus (T2DM) and its possible mechanism.Methods Eighteen rats with spontaneous type 2 diabetes mellitus (GK) were randomized into the model group,SF group and sitagliptin group with six rats in each group,and six more Wistar rats were selected as the control group.Each group was given high fat diet for 8 weeks to build T2DM model except for the control group.At the same time,the control group and the model group were given normal saline 5 ml/(kg-d) by gastric perfusion,the SF group was given SF extract 1.44g/(kg · d) by gastric perfusion,and sitagliptin group was given sitagliptin phosphate suspension 16ml/(kg · d) by gastric perfusion.Eight weeks later,the levels of serum insulin-like growth factor 1 (IGF-1),tumor necrosis factor-αr (TNF-a) and interleukin-1β (IL-1β)were measured in each group,the gastrocnemius muscle was taken out to get the wet weight and observe the pathological changes,and the expression of p70s6k1 protein was detected.Results Compared with the control group,the wet weight of the gastrocnemius muscle,the serum IGF-1 level and p70s6k1 protein expression in the model group significantly decreased (P < 0.05),while the levels of serum TNF-α and IL-1β significantly increased (P < 0.05).Through pathological observation,there existed gastrocnemius muscle cells atrophy,a larger area of edema and interstitial lymphocyte infiltration.Compared with the model group,the wet weight of the gastrocnemius muscle,IGF-1 content and p70s6k protein expression in the sitagliptin group and the SF group significantly increased,the contents of TNF-α and IL-l β significantly decreased (P <0.05),and there were no obvious muscle cell atrophy and edema,and also little inflammatory cell infiltration.There was no significant difference between the sitagliptin group and SF group (P > 0.05).Conclusion SF could reduce the diabetic skeletal muscle injury;the potential mechanism seems to reduce the inflammatory factor content,improve IGF-1 resistance and maintain the skeletal muscle mass.
5.Zijin powder in inhibiting H22 mice solid and ascites liver cancer: experimental study.

OBJECTIVETo observe the rate of Zijin powder in inhibiting H22 mice solid and ascites liver cancer and the relation between quatity and effect.
METHODThe Kunming mics, transplanted by H22 liver cells, were divided into a model group, a cyclophosphamide group and three groups of Zijin powder in high dose, medium dose, and low dose. Then observation was made on the rate of Cancer.
RESULTThe inhibiting rates of Zijin powder of three groups (high dose group, medium dose group and low dose group) for H22 mice solid liver cancer were 30.8%, 38.31% and 48.59% respectivily. The inhibiting rates of three groups of Zijin powder (low dose group, medium dose and high dose group) for H22 mice ascites liver cancer were 6.77%, 15.59% and 14.90 % respectivily.
CONCLUSIONZijin powder has better effect on H22 mice solid liver cancer, and its effect is greatly increasing with the increased dosage.

6.Fingerprint analysis of Resina Draconis from different manufactuers by UPLC coupled with chemometrics.

This study is to establish an UPLC fingerprint of Resina Draconis from different manufacturers, which can provide a comprehensive evaluation for its quality control. The analysis was performed on a Phenomenex Kinetex 2.6 μ C18 100A column by agradientelution program with acetonitrile-water as mobile phase at a flow rate of 1.7 mL x min(-1). The column temperature was 40 degrees C and the detection wavelengthwas 280 nm. The fingerprints of 18 batches of Draconis Resina were further evaluated by chemometrics methods including similarity analysis (SA), hierarchical clustering analysis (HCA) and principal component analysis (PCA). As a result, there were 15 common peaks, 13 of which had been identified by LC-Q-TOF MS, and the similarity degrees of 15 batches of the samples was more than 0.9, and the samples were divided into 4 clusters by their quality difference. The method is reproducible, simple and reliablethat it can be used for quality control and evaluation of Resina Draconis from different manufacturers.

7.Quality analysis of Guizhi Fuling capsule before and after application of in-process quality control in pharmaceutical production.

The effects of application of in-process quality control in Guizhi Fuling capsule production were evaluated by 192 batches data analysis. Using a statistical analysis method, each batch of data were to be counted to research for the difference between 96 samples adopting the technologies of in-process control or not. According to quality standards of Guizhi Fuling capsule, all measurements of the 192 batches of the drugs before and after the application of process control technology were analyzed, and they were within the rules. There was a significant difference between adopting the technologies of process control or not. Application of in-process control technology can improve the uniformity of lot-to-lot for Guizhi Fuling capsule.

8.Transmission models of tuberculosis in heterogeneous population.

OBJECTIVETo review the transmission models of tuberculosis in heterogeneous population.
DATA SOURCESThe data used in this review were adopted mainly from the studies of models of tuberculosis reported from 1995 to 2006.
STUDY SELECTIONRelevant literature on transmission models of tuberculosis in heterogeneous populations are referenced.
RESULTSCasual/random factors and genetic factors are the main reasons for epidemics of tuberculosis in recent years. Mass public transport is playing the primary role in casually close contact which can facilitate the transmission of tuberculosis. Genetic susceptibility not only varies endemic prevalence levels, but also drastically alters the effects of treatment for tuberculosis patients. Detailed studies further exhibit that casual contact and genetic factor are responsible for over 30% - 40% of the total new cases in recent years. The prevalence of tuberculosis could double (from 33% to 60%) if a genetically susceptible phenotype is present in only 30% of the population. And some challenges have emerged along with these exciting results.
CONCLUSIONSCasual/random contact, public transport and genetic susceptibility are responsible for most new tuberculosis cases and a wide variation in endemic tuberculosis levels between regions. Hence, the transmission model of tuberculosis in a heterogeneous population can provide more clues to underlying mechanism of tuberculosis transmission than in a homogeneous population. However, many challenges remain for us in understanding transmission of disease.

9.Analysis methodology of multi element in four herbs of guizhi fuling capsules.

This paper was focused on establishing a ICP-MS method with microwave digestion for simultaneous determination of lead, copper, arsenic, cadmium, mercury, magnesium, manganese, nickel, thallium in cassia tuckahoe capsule and its five raw herbal materials. Internal standard method was adopted to reduce matrix effect and other interference effects. The method established was shown to be simple with high sensitivity, strong specificity and good reproducibility. Linear relationship is good as R2 ≥ 0.999 3 while the average recovery was among 75.84% - 118.9%. The detection limit was 0.016 - 4.593 μg x L(-1). Data in this paper provided the basis for control of deleterious element in Guizhi Fuling capsules, and further more it was with referencing values for control of deleterious element in other crude drug.

10.Efficacy and safety of vardenafil in the treatment of erectile dysfunction: a report of 700 cases.

OBJECTIVETo evaluate the efficacy and safety of long-term on-demand use of vardenafil in the treatment of erectile dysfunction (ED).
METHODSWe conducted a questionnaire investigation among 891 ED patients treated by on-demand use of oral vardenafil at 20 mg every 3 days from March 2007 to January 2010, covering the general information of the patients, their need for and attitudes towards the treatment, clinical efficacy and adverse events of the drug, and satisfaction of the patients and their partners after 12 weeks of treatment.
RESULTSTreatment and follow-up were completed in 700 patients, of whom 504 (72%) achieved sufficient hardness and duration of penile erection and overall sexual satisfaction, 84 (12%) admitted to improvement of erectile hardness and duration but not adequate satisfaction, and the other 112 (16%) experienced no significant changes. Significant differences were found in IIEF-5 scores, Rigiscan test results and partners TSS scores before and after the treatment (P < 0.05). Most frequent adverse events included flushing (15%), dizziness and headache (10%), dyspepsia (3%), and nasal congestion (1%).
CONCLUSIONLong-term on-demand use of oral vardenafil, in addition to its safety and good tolerance, can effectively improve the erectile function of ED patients, their success rate of sexual intercourse, and overall quality of sexual life.
