1.Treatment effect of enteral nutriention rich in diet fiber in diarrhea due to bacteria disorder after severe brain injuries

Objectives: To investigate the treatment effect of enteral nutrition rich in diet fiber in diarrhea because of bacteria disorder after severe brain injuries.Methods:20 cases with severe brain injuries were supported with enteral nutrition rich in diet fiber after the occurrance of diarrhea because of bacteria disorder as clinical treatment group.20 cases with the same conditions were treated with traditional methods as control.Results: Diarrhea of all patients in clinical treatment group had been controlled during 7-10 days,and the feeding quantities reached total calories.The nutrition state was improved,and the body internalenvironment was stable during the 20 days. Conclusion: Enteral nutrition rich in fiber can treat diarrhea caused by bacteria disorder after severe brain injuries.

3.Assessment of sedative effects by intravenous injection of fentanyl on neonates with mechanical ventilation

Objective We aimed to assess the sedative effects by intravenous injection of fentanyl on neonates with mechanical ventilation. Methods We divided 142 neonates with mechanical ventilation into the observation group (92 cases) and the control group (50 cases). The observation group received intravenous injection of fentanyl, while the control group was given no sedative medication. The pain and sedation was assessed before ventilation, 1 and 2 hours after ventilation and later every 4 hours by adoption of NPASS evaluation inventory. Results The scores of pain at different time points 2 hours after medication in the observation group were lower than those in the control group, especially 24 hours after medication. The mean airway pressure and oxygen concentration required by the observation group was lower than those of the control group. But no difference was seen in blood oxygen saturation between the two groups. Conclusions Intravenous injection of fentanyl was proper in the application of sedative treatment for neonates with mechanical ventilation. N-PASS inventory could effectively assess the pain and sedation condition and ensure the safe medication.

5.Neural Mechanisms of Forward Conditional Reasoning:A Functional MRI Study

Purpose Two kinds of forward conditional reasoning including modus ponens (MP) and denial of the antecedent (DA) are the most common deductive reasoning in daily life. The present study aimed to explore the difference between the two neural machanisms. Materials and Methods Three kinds of tasks including MP, DA, and baseline were administrated to 14 undergraduates [7 males and 7 females, mean age (23.4±1.3) years] by using functional magnetic resonance imaging (fMRI) technique. fMRI data was analyzed. Results The reaction times were significantly slower for MP and DA than that of BS (P<0.001), while the effect was not significant for accuracy. In contrast to baseline, both MP and DA tasks activated the left inferior frontal cortex (BA 9), inferior parietal lobule (BA 40), and postcentral gyrus. Bilateral caudate significant activation was found in MP task when compared with DA, whereas the frontal, parietal, occipital lobe and anterior cingulate cortex were acivated more in the DA than in MP task. Conclusion Both forward conditional reasoning of MP and DA commonly activate the left frontoparietal cortex, while significant dissociations can be seen in the regions of caudate and bilateral frontal, parietal and occipital. These results firstly demonstrates the different neural mechanism underlying the two forward conditional reasoning, which might help to further uncover the brain mechanism of conditional reasoning.
6.Treatment effect of rational enteral nutrition in stress ulcer bleeding after severe brain injuries

Objective: To investigate the treatment effect of rational enteral nutrition in stress ulcer bleeding after severe brain injuries.Methods: 30 cases with severe brain injuries accompanied with stress ulcer bleeding were supported by enteral nutrition as clinical treatment group.50 cases with the same conditions were managed with traditional treatment methods as control.Results: All patients in clinical treatment group had no stress ulcer bleeding any more. The nutritional state was improved. On the other hand,the stress ulcer bleeding occurred again and again and the nutritional state was poor.Conclusion: Rational enteral nutrition can treat upper gastrointestinal stress ulcer bleeding in severe brain injuries effectively.
8.Blood-saving effect of tranexamic acid in off-pump coronary artery bypass

Objective To investigate the blood-saving effect of tranexamic acid in off-pump coronary artery bypass (OPCAB). Methods Two hundred and sixty ASA Ⅰ- Ⅲ and NYHA Ⅰ- Ⅲ patients of both sexes,aged 18-64 yr, with body mass index 16-22 kg/m2 , undergoing OPCAB, were randomly divided into 2 groups (n = 130 each): control group (group C) and tranexamic acid group (group T) . Anesthesia was induced with iv injection of midazolam 0.1 mg/kg, fentanyl 5-10μg/kg and pipecuronium 0.1 mg/kg. The patients were tracheal intubated and mechanically ventilated. PEr CO2 was maintained at 35-45 mm Hg. A bolus of tranexamic acid 1 g was infused intravenously within 30 min after indution followed by continuous infusion at 400 mg/h until the end of operation in group T. While equal volume of normal saline was given in control group. Anesthesia was maintained with inhalation of isoflurane and intermittent iv injection of fentanyl and pipecuronium. Venous blood samples were taken before induction, at the end of operation and at 24 h after operation for determination of Hb, platelet count (P1t), prothrombin time (PT) and international normalized ratio (INR). The volume of chest tube drainage was collected and recorded at 6 and 24 h after operation. The requirement for transfusion of allogeneic red blood cells and fresh frozen plasma was also recorded. Results There was no significant difference in Hb, Plt, PTand INR at each time point between the two groups ( P > 0.05). The requirement for transfusion of allogeneic red blood cells and fresh frozen plasma was significantly reduced in group T as compared with group C ( P < 0.05 or 0.01 ). No deaths and complications occurred during hospital stay in the two groups. Conclusion Tranexamic acid exerts the blood-saving effect in OPCAB.
9.Identification of antler powder components based on DNA barcoding technology.

In order to authenticate the components of antler powder in the market, DNA barcoding technology coupled with cloning method were used. Cytochrome c oxidase subunit I (COI) sequences were obtained according to the DNA barcoding standard operation procedure (SOP). For antler powder with possible mixed components, the cloning method was used to get each COI sequence. 65 COI sequences were successfully obtained from commercial antler powders via sequencing PCR products. The results indicates that only 38% of these samples were derived from Cervus nippon Temminck or Cervus elaphus Linnaeus which is recorded in the 2010 edition of "Chinese Pharmacopoeia", while 62% of them were derived from other species. Rangifer tarandus Linnaeus was the most frequent species among the adulterants. Further analysis showed that some samples collected from different regions, companies and prices, contained adulterants. Analysis of 36 COI sequences obtained by the cloning method showed that C. elaphus and C. nippon were main components. In addition, some samples were marked clearly as antler powder on the label, however, C. elaphus or R. tarandus were their main components. In summary, DNA barcoding can accurately and efficiently distinguish the exact content in the commercial antler powder, which provides a new technique to ensure clinical safety and improve quality control of Chinese traditional medicine
