1.Clinical effects of internal fixation with Herbert screws for the treatment of Pipkin femoral head fractures.

OBJECTIVETo explore the therapeutic effects of internal fixation with Herbert screws for the treatment of Pipkin type I and type II femoral head fractures.
METHODSFrom January 2008 to December 2012, 23 patients with Pipkin type I and type II femoral head fractures were treated with open reduction and internal fixation by Herbert screws through Kocher-Langenbeck approach. Twenty-three patients (aged 18 to 52 years with a mean of 35.5 years, including 18 males and 5 females patients, 8 left hips and 15 right hips) with femoral head fractures and posterior hip dislocation. The fracture was classified according to Pipkin classification based on the radiographic findings, 5 patients had type I and 18 had type II fractures. The duration time from admission to the operation ranged from 6 to 72 h (averaged 32 h). The clinical and radiographic outcomes of the patients were measured using Thompson-Epstein scoring scale. The Harris hip score(HHS) was used to evaluate and compare hip functions at the latest follow-up between affected and healthy sides.
RESULTSAll the patients were followed up, and the duration ranged from 20 to 48 months (averaged 30 months). According to Thompson-Epstein system, 12 patients got an excellent result, 6 good, 4 fair and 1 poor. The average HHS at the finial follow-up was 87.80 ± 8.46 (ranged from 66 to 95), which is similar to that in the healthy side 90.10 ± 6.35 (ranged from 72 to 98) (t = 1.044, P = 0.302). The complications such as deep infection, and deep vein thrombosis were not found. At the 3rd year during follow-up,4 patients had avascular necrosis of femoral head. At the 1st year of follow-up, 1 patient had hip pain after walking,screws loosening and shift after trauma,and serious complications of traumatic arthritis. All the 5 patients were treated with total hip arthroplasties.
CONCLUSIONThe treatment of internal fixation with Herbert screws through the Kocher-Langenbeck approach is effective for Pipkin type I and type II femoral head fracture. The method is reliable and valuable for recommendation. However, such fracture may have avascular necrosis of femoral head and complication of traumatic arthritis, which should be observed carefully in clinic with preparation of the prevention and treatment measures.

3.Study on the application of Knox method to temporal-spatial cluster for infectious diseases.

OBJECTIVEApplication of Knox method was discussed in exploring temporal-spatial cluster for infectious diseases.
METHODSThe theory of Knox method was introduced in detail. As an example, the temporal-spatial cluster of epidemic cerebrospinal meningitis cases from April to May in 2005 was explored in Beijing and the statistical process of Knox method was clearly demonstrated with related results well interpreted.
RESULTSOnly when time and distance threshold values were set as 13 days and 29.67 km, potential temporal-spatial cluster could be detected in epidemic cerebrospinal meningitis cases.
CONCLUSIONAccording to the characteristics of the epidemics, under Knox method, if appropriate threshold values of time and distance were chosen, information on temporal-spatial cluster of infectious diseases could be obtained. According to the records of different time and distance threshold values, the nature of an infectious disease might be explored. Thus Knox method could help us to strengthen the early warning system on infectious disease control strategy.

4.The expression of the estrogen receptor within the mammary gland of the breast hypertrophy and hypogenesis.

OBJECTIVETo investigate the expression of the estrogen receptors(ER) in breast hypertrophy and micromastia.
METHODSThe quantity of ER within breast tissue in 13 patients with mammary hypertrophy strictly elected was determined using DCC single dot test. The results were compared with that measured in 13 patients with micromastia.
RESULTSThe average of the quantity of the ER was respectively (5.33 +/- 5.70) fmol/mg and (1.17 +/- 0.86) fmol/mg in mammary hypertrophy ground and in the micromastia group. There was obvious significant between the two groups in a statistics basis (P < 0.05) .
CONCLUSIONSThe breast hypertrophy is related to the excessive expression of ER within breast tissue,while the micromastia and breast hypotrophy are related to the lower level or the expression inhibition of ER.

5.Changes of Na(+) channels in rat hippocampal CA1 neurons in early development after birth.

The purpose of this research is to investigate the critical period of voltage-gated Na(+) channel development in hippocampal CA1 neurons. Changes of Na(+) currents in acutely isolated hippocampal CA1 neurons of rats at different ages (0-4 weeks after birth) were recorded using the whole-cell patch-clamp technique. The results indicated that the maximum current density of Na(+) channels was increasing with age, and the amplitudes in 1, 2, 3 and 4 weeks respectively grew by (42.76 ± 4.91)%, (146.80 ± 7.63)%, (208.79 ± 5.28)% and (253.72 ± 5.74)% (n = 10, P < 0.05) compared with that in 0 week. The current density in CA1 neurons of 1-2 weeks after birth increased more significantly than those of other groups. The activation curve of Na(+) channel shifted to the left. The half-activation voltages (mV) in 0-2 weeks were -39.06 ± 0.65, -43.41 ± 0.52, -48.29 ± 0.45 (n = 10, P < 0.05), respectively, showing significant age-dependent decrease, and there were no significant changes in other groups. The slope factors of activation curve for each group did not change significantly. There were no regular changes in inactivation curve and no significant changes in half-inactivation voltage. The slope factors of inactivation curve in 1-2 weeks were: 5.77 ± 0.56, 4.42 ± 0.43 (n = 10, P < 0.05). The inactivation rate of the second week after birth was faster than that of the first week, and there were no significant changes during 0-1 week and 2-4 weeks. The recovery from inactivation curve of Na(+) channel shifted to the left. The recovery time declined in 1-3 weeks. Changes of action potential properties were consistent with Na(+) current. These results suggest that the period of 1-2 weeks after birth may be the critical development period of voltage-gated Na(+) channel in hippocampal CA1 neurons. During this time, the distribution of Na(+) channel increases significantly; the activation curve of Na(+) channel shifts to the left; inactivation rate increases as well as recovery time shortens.

6.Treatment of the complications of polyacrylamide hydrogel injection for augmentation mammaplasty.

OBJECTIVEThe purpose of this study was to search for a perfect treatment for the complications after polyacrylamide hydrogel injection for augmentation mammaplasty.
METHODS48 patients who accepted polyacrylamide hydrogel injection for augmentation mammaplasty were included in this study. Operations were performed for the complications of polyacrylamide hydrogel injection. The clinical data were analyzed and the experience in the treatment of the complications was summarized.
RESULTSAll patients were satisfied with the result of the operation. The B-ultrasonic examination showed that no visible polyacrylamide hydrogel remained in the cavity.
CONCLUSIONThe best therapy for the complications of polyacrylamide hydrogel injection was removing of the polyacrylamide hydrogel and the pathologic tissue and irrigation of the cavity.

7.Study on the application of Rogerson Spatial Pattern Surveillance Method in real-time surveillance for infectious diseases.

Application of Rogerson Spatial Pattern Surveillance Method was discussed in real-time surveillance and early monitoring for infectious diseases. Theory of Rogerson Spatial Pattern Surveillance Method was introduced in detail. Data from the program on 'Monitoring epidemic cerebrospinal meningitis with Rogerson Spatial Pattern Surveillance Method' by the Chinese Center for Disease Control and Prevention provided evidence as how to choose crucial parameters such as K, H, n0 and tau, how to simulate surveillance process and how to interpret results. Rogerson Spatial Pattern Surveillance Method seemed to be able to continuously monitor the changes of infectious diseases. New cases could be added to this model at any moment and this model could provide early warning information in time when the excess of cases occurred. Simultaneously it could provide the time and site of the extra cases. Rogerson Spatial Pattern Surveillance Method could implement dynamic and real-time surveillance for infectious diseases. Simultaneously it could avoid the problem of multiple tests that causing large type I error. It greatly helped in providing reliable information for controlling infectious diseases in early epidemic period. Hence,it had important value of application in early warning on infectious diseases.

8.Treatment of fracture of fifth metatarsal base and degree III lateral ligament injury of ankle with compressed cannulated screw and suture anchors.

OBJECTIVETo investigate the clinical outcomes of surgical treatment of fracture of the fifth metatarsal base combined with degree III lateral ligament injury of ankle.
METHODSFrom January 2008 to December 2011, 32 patients with fracture of the fifth metatarsal base combined with degree III lateral ligament injury of ankle were treated with surgery. Fractures were fixed with compressed canulated screw and ligaments were repaired with suture anchors. After operation, ankle joints were fixed in neutral position and slightly valgus position by plaster slab. Taking out stitch was performed at 2 weeks after operation and non-weight loading walking by double crutches support started; after the 6 weeks, remove the gypsum and part-weight loading walking by brace protection; at the 8 weeks after operation, completely weight loading walking was permitted. American Orthopaedic Foot and Ankle Society (AOFAS) was used to evaluate the clinical effect.
RESULTSThirty-two patients were followed up from 8 to 18 months with an average of 12 months. All fractures obtained healing with an average time of 12.5 weeks (ranged, 8 to 24 weeks). According to the standard of AOFAS, 18 cases got excellent results and 14 good.
CONCLUSIONThe method that fracture fixation with compressed canulated screw and ligament repair with suture anchors can obtain satisfactory effects in treating fracture of the fifth metatarsal base and degree III lateral ligament injury of ankle.

9.Correlation of polymorphism of the coding region of glutathione S-transferase M1 to susceptibility of nasopharyngeal carcinoma in south China population

Background and Objective:Glutathione S-transferase M1 (GSTM 1)deficiency may increase the risk of nasopharyngeal carcinoma (NPC).This study was to evaluate the correlation of the single nucleotide polymorphism (SNP) in the coding region of GSTMl gene to NPC susceptibility in southern China population.Methods:In total 239 NPC patients and 286 age-matched healthy controls were entered into the study.Among them.225 out of 239 NPC patients and 273 out of 286 controls were used for statisticaI analysis.SNP screening of all exons.relevant intron-exon boundaries.and the promoter region of GSTM 1,in total 4739bp,was performed by PCR direcl sequencing.The loci T1270533G and C1256088Cwere selected for the case-controI study using the tetra-Primer ARMS-PCR.as well as the sequencing method.Results:In totaf 29 SNPs of GSTM 1 were identified by sequencing.Missense mutation occurred in the polymorphic loci of T1270533G and C1256088C.However.no evident relationships between the variant of T1270533G and clinicaI phenotypes of NPC were obsewed in the NPC group and healthy control group(OR=0.1 70,95%CI=0.95-0.306for homozygote TT).The deletion frequency of C1256088C was 45%(45/100)for NPC patients and 42%(42/100)for controls.Conclusions:The polymorphism of T1270533G does not affect the detoxification function of GSTM1.The T1270533G JOCUS has no apparent association with genetic susceptibility to NPC in the southern China population.The IOSS rate of C1256088C is high in this study.
