1.Determination of Amygdalin in Semen Pruni by HPLC

Objective To establish a HPLC method for the determination of amygdalin in Semen Pruni. Methods The analytical column was Supelco ODS Cis column {4.6 mm ? 250 mm, 5 ?m), with methanol-water (20 : 80) as mobile phase. The flow rate was 1.0 mL/min, and the detection wavelength was 210 nm. Results The linear range for amygdalin was 0.06613~4.232 ?g (r=1.0000). The average recovery was 95.91% (RSD=2.29%). Conclusion The method is accurate and reliable, and can be used in the determination of amygdalin in Semen Pruni.
2.Effect of mild moxibustion on cancer-related fatigue, serum ghrelin and adiponectin in patients undergoing chemotherapy after colorectal cancer surgery

Objective: To observe the effect of mild moxibustion on cancer-related fatigue, serum ghrelin and adiponectin in patients undergoing chemotherapy after colorectal cancer surgery. Methods: Seventy patients were divided into a control group and an observation group according to the random number table method, with 35 cases in each group. Patients in both groups were treated with conventional FOLFOX4 chemotherapy regimen. Patients in the control group also received routine treatments such as symptomatic treatment of complications, health education, nutritional support, and exercise intervention. Patients in the observation group received mild moxibustion on the basis of interventions used in the control group. The Piper fatigue scale-revision (PFS-R), Karnofsky performance status (KPS) and spleen and kidney yang deficiency syndrome were scored, and serum ghrelin and adiponectin levels were measured before and after treatment. Results: After treatment, total score of PFS-R in the control group did not change significantly (P>0.05), while in the observation group it was significantly reduced (P<0.05) and lower than that in the control group (P<0.05); KPS scores in both groups were increased (both P<0.05), and it was significantly higher in the observation group than in the control group (P<0.05); total score of spleen and kidney yang deficiency syndrome in the control group did not change significantly (P>0.05), while it was significantly reduced in the observation group (P<0.05) and lower than that in the control group (P<0.05); serum ghrelin and adiponectin levels in the control group had no significant changes (both P>0.05), but the levels were significantly increased in the observation group (both P<0.05) and higher than those in the control group (both P<0.05). Conclusion: Based on the routine treatments, mild moxibustion can relieve fatigue, improve quality of life, and improve the symptoms of spleen and kidney yang deficiency in patients undergoing chemotherapy after colorectal cancer surgery, which may be related to the regulation of serum ghrelin and adiponectin levels.
3.Port catheter system placement in pulmonary artery via percutaneous subclavicle vein treatment for refractory multiple metastatic tumors in the two lungs:efficacy evaluation and technique discussion

Objective To evaluate the efficacy of port catheter system(PCS)placement in pulmonary artery via percutaneous subclavicle vein treatment for multiple metastatic tumor in the two lungs and discuss the PCS technique.Methods Fifteen multiple metastatic tumor patients(13 hepatocellular carcinomas,one mandible grand adenocarcinoma,one oral bottom squamous carcinoma)were carried out with pulmonary artery PCS placement by way of percutaneous subclavicle vein.FPA/FPM/GP chemotherapy scheme were introduced every 4~6 weeks.Results The success rate of PCS placement technique was 93.3%(14/15).One case failed.Percutaneous subclavicle veins were performed 14 cases in right side and 1 in left one.Following up 2~43 months,2~7 chemotherapy cycles(mean 5 cycles)were accomplished,and the clinical CR and PR were achieved in 1 and 3 cases respectively with clinical efficacy rate 28.6%(4/14).Major side reaction was late wound healing in 1 case.Conclusion PCS placement in pulmonary artery treatment for multiple metastatic tumor in the two lungs is effective,and mastering operation technique is the key for increasing operation suc- cess rate.
4.Meta analysis on nucleo(s)tide analogues sequential/sequential combined with pegylated interferon for the therapy of HBeAg-positive chronic hepatitis B

Objective To systematically evaluate the efficacy and safety of nucleo(s)tide analogues (NAs)sequen-tial/NAs sequential combined with pegylated interferon (Peg-IFN)for the treatment of HBeAg-positive chronic hepatitis B(CHB).Methods PubMed,Cochrane Library,Embase,and Chinese Medical databases (CNKI,Wan-fang and VIP)from database establishment to March 25,2017 were retrieved,randomized controlled trials of NAs sequential/sequential combined with Peg-IFN for the treatment of CHB after application of NAs to achieve virologic response were included in study,Meta analysis was performed by RevMan 5.3 software,HBeAg seroconversion rate and HBsAg negative conversion rate at the end of treatment were compared.Results Nine studies were eventu-ally included,4 were about NAs sequential Peg-IFN,5 about NAs sequential combined with Peg-IFN.At the end of treatment,compared with using NAs monotherapy for antiviral treatment,NAs sequential/sequential combined with Peg-IFN therapy can improve HBeAg seroconversion rate(31.2% vs 11.7%;OR,3.69 [95%CI ,2.43 -5.60];P < 0.01 )and HBsAg negative conversion rate(11.5% vs 0.5%;OR,9.31 [95%CI ,2.72 - 31.89];P <0.01).According to the results of subgroup analysis,HBeAg seroconversion rate in NAs sequential Peg-IFN therapy group was higher than control group (25.3% [42/166]vs 10.0% [17/170];OR,3.1 [95%CI ,1.66 -5.79];P <0.01);HBeAg seroconversion rate in NAs sequential combined with Peg-IFN therapy was higher than control group (36.8%[63/171]vs 13.5%[23/171];OR,4.24[95%CI ,2.41 -7.46];P <0.01).Sequential/se-quential combination therapy showed more adverse reaction,most of which can be tolerated or improved after symp-tomatic treatment.Conclusion For the treatment of HBeAg-positive CHB,after application of NAs to achieve viro-logic response,NAs sequential/sequential combined with Peg-IFN therapy for 48 weeks can significantly increase HBeAg seroconversion rate and HBsAg negative conversion rate.
5.Value of CD4 + and CD8 + T-lymphocyte counts for clinical diagnostic classification and prognosis of coronavirus disease 2019

Objective:To assess the value of CD4 + and CD8 + T-lymphocyte counts for the diagnostic classification and prognosis of coronavirus disease 2019 (COVID-19). Methods:A total of 95 COVID-19 adult patients admitted to Shanghai Public Health Clinical Center, Fudan University from January to March 2020 were recruited. The CD4 + and CD8 + T-lymphocyte counts among ordinary, severe and critical patients, as well among the cured, improved, unimproved and death patients were compared. The area under receiver operating characteristic curve (AUROC) was used to evaluate the value of CD4 + and CD8 + T-lymphocyte counts for the clinical diagnosis and prognosis of COVID-19. The comparison among groups was performed by Mann-Whitney U test. Results:A total of 95 COVID-19 cases including 68 common, 11 severe and 16 critical cases were enrolled. The counts of CD4 + and CD8 + T-lymphocyte of patients in common, severe and critical groups were 419 (309, 612), 267 (212, 540), 141 (77, 201)/μL, and 238 (153, 375), 128 (96, 172), 92 (51, 144)/μL, respectively, with significant differences ( Z=24.322 and 15.956, respectively, both P<0.01). The counts of CD4 + and CD8 + T-lymphocyte of the death, unimproved, improved, and cured patients were 149 (143, 349), 315 (116, 414), 344 (294, 426), 745 (611, 966)/μL, and 106 (43, 501), 176(67, 279), 194(188, 432), 429(276, 564)/μL, respectively, with significant differences ( Z=36.083 and 16.658, respectively, both P<0.01). The optimal cut-off point of CD4 + T-lymphocyte counts was 237/μL for critical COVID-19 with AUROC 0.911 (95% confidence interval ( CI) 0.833-0.989, P<0.01), with the sensitivity of 86.1% and specificity of 87.5%. For predicting severe and critical cases, the optimal cut-off point of CD4 + T-lymphocyte counts was 405/μL with AUROC 0.863 (95% CI 0.727-0.999, P=0.001), with the sensitivity of 78.6% and specificity of 74.6%. Conclusions:The conditions of patients with COVID-19 are aggravated with CD4 + and CD8 + T-lymphocyte counts decreasing. CD4 + T-lymphocyte counts may be an indicator for diagnostic classification of COVID-19 and prognostic indicator for severe and critical patients.

Objective To evaluate the influence of self-etch adhesive,total-etch adhesive and glass ionomer cement on the marginal microleakage of class II restorations. Methods Thirty human premolars were randomly divided into 3 groups(n=10),and cuboid class II cavities(4.0 mm?3.5 mm?2.5 mm) were prepared.Restoration was performed using self-etch adhesive+nano-resin(self-etch group),total-etch adhesive + nano-resin(total-etch group) or glass ionomer cement(glass ionomer group).Half of each group underwent 200 thermocyclings and the other half underwent 500 thermocyclings.The specimens were immersed in 0.5% basic fuchsin for staining.Each tooth was then evaluated the microleakage at the axial wall and the gingival wall section by section under a stereomicroscope.The data were statistically analyzed. Objective To investigate the effects of total flavonoids in Gingko Biloba on glucose and lipid metabolism and liver function in rats with insulin resistance. Methods Forty SD rats were randomly divided into normal control group,model group,total flavonoids in Gingko Biloba group and rosiglitazone group(positive drug control group)(n=10).Models of insulin resistance were established by high glucose and high fat diet in model group,total flavonoids in Gingko Biloba group and rosiglitazone group.After treatment for 12 weeks,serum glucose,serum lipids,and parameters of insulin resistance,liver function and anti-oxidation capability were detected in each group,and histologic observations of liver tissues were conducted with adipose staining. Objective To assess the curative effect of percutem transilluminated with negative pressured on the potaried technique on the treatment of venous ulcer in lower extremity.Methods The clinical date of 300 cases involving 300 legs with venous ulcer in lower extremity,who underwent the percutum transilluminated negative pressured potaried technique using TRIVEXTM Ⅱ potaried system or the percutum transfixion surgical treatment from October 2005 to June 2009,were analyzed.Three hundred cases were randomly divided into potaried group and transfixion group.In potaried group,there were 190 cases involving 190 legs treated with TRIVEXTM Ⅱ potaried system.In transfixion group,110 cases involving 110 legs treated with percutum transfixion.The clinical indexes of skin infection rate and skin necrosis rate,shrinkage rate of wound area and skin depigmentation rate,ulcer healing rate and ulcer recurrence rate were calculated to assess the clinical curative effect on day 5,day 20,day 120 and day 360 after operation respectively.Results The rates of skin infection and skin necrosis were significantly decreased in potaried group compared with transfixion group on day 5 after operation (P0.05).Ulcer recurrence rate was remarkably lower in potaried group than that in transfixion group on day 360 (P
10.Correlation of adipose content and distribution of thigh with insulin resistance in subjects with normal glucose tolerance

Objective To establish a method of measuring adipose content and fat distribution of the thigh in normal glucose tolerance (NGT) subjects,and to investigate its relation to insulin resistance.Methods Insulin sensitivity was evaluated by hyperinsulinemic and euglycemic clamp technique,and femoral adipose content and fat distribution were determined by MRI in 30 individuals with NGT including 15 with normal weight and 15 overweighted or obese subjects.Results Compared to normal weight group,the subscutaneous adipose tissue of thigh (SCAT) [(176.7?21.6) cm~2 vs (115.0?12.8 ) cm~2,P<0.05],adipose tissue of thigh beneath the fascia (SFAT) [(75.4?4.4 ) cm~2 vs (57.5?4.7 ) cm~2,P<0.01] and intermuscular adipose tissue (IMAT) [(28.3?3.2) cm~2 vs (14.5?1.1 ) cm~2,P<0.01] were greater in overweight/obesity group.Overweight/ obesity group had lower insulin sensitivity( glucose disposal rate under steady state of hyperinsulinemic euglycemic clamp:4.54?0.43 vs 7.88?0.75,P<0.01).SFAT and IMAT were significantly correlated with insulin sensitivity.SFAT showed the most marked correlation with insulin sensitivity.Multiple stepwise regression analysis showed that the increased SFAT played a pivotal role in insulin resistance.Conclusion The adipose content and fat distribution are highly correlated with insulin sensitivity and the adipose tissue of thigh beneath fascia may play the most significant role in insulin sensitivity.
