1.Carbetocin combined with oxytocin for prevention of bleeding after artificial abortion

Objective To analyze the carbetocin combined with oxytocin for prevention of bleeding after artificial abortion .Methods 110 cases of pregnant women undergoing artificial abortion were randomly divided into observation group and control group.Observation group were treated with carbetocin combined with oxytocin treatment, the control group were treated by oxytocin treatment, the negative blood loss, menstrual recovery changes and curative effect in two groups of patients were compared.Results The total efficiency of the observation group was 96.36%, was significantly higher than the control group 78.18%, the difference was statistically significant (P<0.05), the 24h negative bleeding volume, vaginal bleeding time, menstruation time, first menstrual duration in the observation group were lower than those in control group, the difference was statistically significant (P<0.05).The vaginal bleeding time less than 7d in observation group of patients accounted for 30.91%, was significantly higher than the control group 18.18%; the time of vaginal bleeding >14d accounted for 3.64%, significantly lower than the control group 21.82%, the difference was statistically significant (P<0.05).The vaginal blood
2.Values of different scores for diagnosing short-term prognosis of HBV-related acute-on-chronic liver failure

Objective To investigate the diagnostic values of model of end-stage with incorporation of serum sodium (MELD-Na) score, chronic liver failure-sequential organ failure assessment (CLIF-SOFA) score and APASL-ACLF research consortium score (AARC-ACLF) for evaluation of prognosis of hepatitis B virus related acute-on-chronic liver failure (HBV-ACLF). Methods A total of 72 consecutive patients with HBV-ACLF were included in the study and divided into two groups (group A and group B) according to the prognosis in three-month. Group A were included 29 patients with stable disease or better after medical treatment at least for 3 months, and group B included 43 patients who were dead after treatment or received liver transplantation as failure of medical treatment. When the patients were diagnosed as ACLF or after admission, the data were collected. Results of the laboratory examination were collected when the international normalized ratio (INR) was minimum. Data of total bilirubin (TBIL), prothrombin time (PT), INR, serum creatinine (Cr), serum sodium (Na), albumin (ALB), MELD-Na, CLIF-SOFA and AARC-ACLF scores were calculated respectively. The comparative analysis was performed. Areas under the receiver operating characteristic curve (AUC-ROC) of MELD-Na and CLIF-SOFA scores were used to assess the short-term prognosis in patients with acute-on-chronic liver failure. Results The values of TBIL, INR, MELD-Na, AARC-ACLF and CLIF-SOFA were significantly higher in group B than those in group A (P<0.05). The serum level of Na was significantly lower in group B than that of group A (P<0.05). The area under curve (AUC) values generated by the ROC curves was higher for CLIF-SOFA score (AUC 0.887) than that of MELD-Na score (AUC 0.764) (Z=2.255, P<0.0167). The AUC values generated by the ROC curves showed no significant differences between CLIF-SOFA score and AARC-ACLF score (AUC 0.825) or MELD-Na score and AARC-ACLF score (Z=1.361, 1.127, P>0.0167). The cut-off scores of MELD-Na, CLIF-SOFA and AARC-ACLF were 23.84, 8.50 and 8.50 respectively. Conclusion MELD-Na, CLIF-SOFA and AARC-ACLF scores have appreciable values to evaluate the prognosis in patients with HBV-related ACLF. AARC-ACLF is better than that of MELD-Na and CLIF-SOFA in assessing prognosis of HBV-related ACLF.

Corneal endothelial cell(CEC)is the most critical part for the cornea, of which activity can influence the postoperative vision.It is very important for the clinical cornea preservation considering the function and its self-purification of donor cornea.There are a variety of classical methods, which can significantly prolong the saving time of donor cornea with its good quality of CEC.We reviewed the published papers about present preservation methods of cornea, which can give us many suggestions for the clinical cornea preservation.
4.Clinical study on 27 children with Epstein-Barr virus encephalitis

Objective To explore the clinical characteristics and diagnosis of Epstein-Barr(EB) virus encephalitis(EBE) in children. Methods The verification of EBE was based on detection of EBV DNA in cerebrospinal fluid (CSF) by fluorogenic quantitative PCR(FQ-PCR) and Nest-PCR. The clinical and CSF changes of 27 EBE cases and 26 controls were analyzed and compared. Results EB-DNA in CSF by FQ-PCR of 13 cases of EBE was (2.82?2.03)?10 3copies.There was no significant difference in clinical manifestations between EBE and HSE groups, except that WBC in CSF of EBE was lower than those of HSE.Conclusions EBE is not infrequent, about 10 % of encephalitis in children. EBE is always present independently, which is not a complication of infectious mononucleosis(IM).Detection of EBV-DNA in CSF is a sensitive and specific test for diagnosing of EBE. Early treatment may be beneficial to the prognosis of EBE.
5.Effect of Diabetic Status on the Preventive Efficiency of Methylcobal on Peripheral Neuropathy in Rats

ObjectiveTo investigate the effect of varied hyperglycemia and durations on preventing peripheral neuropathy (PN) with methylcobal in diabetic rats.Methods80 Sprague Dawley rats were selected and randomly chosen 16 as normal control (NC) group, others 64 animals as diabetic model induced by alloxan. The model rats were evenly divided into relatively good control (GC) group and relatively poor control (PC) group based on levels of hyperglycemia regulated with exogenous insulin. 16 rats of each GC/PC group were intramuscularly injected with methylcobal (500 μg/kg). The sensory nerve conduction velocity (SNCV), motor nerve conduction velocity (MNCV) and evoked potential amplitute (EPA) of sciatic nerve of rats were detected by evoked electromyogram respectively at 2nd, 8th and 12th week after onset of diabetes. Fructosamine in serum was also detected.ResultsThe rats injected with methylcobal had a significantly delayed reduction of SNCV, MNCV and EP in GC group (fructosamine,1.0 mmol/L) than that in PC group (fructosamine, 1.2 mmol/L) ( P<0.05~0.01). Up to 8th week after onset of diabetes, there was no obvious difference in physioelectroparameters between the GC group and NC group. At 12th week after onset of diabetes, physioelectroparameters in the PC group fell to the level of non-methylcobal injected group. No effect of methylcobal on blood glucose revealed in any groups ( P>0.05).ConclusionMethylcobal has a beneficial effect on delaying the onset of diabetic PN, and is of high efficacy with a good control of diabetic state, especially at early stage.
6.Characteristics of heroin spongiform leucoencephalopathy detected by hydrogen magnetic resonance spectroscopy

BACKGROUND:Point resolved spectroscopy (PRESS) or single-voxel spectroscopy (SVS) is always used in the previous researches of magnetic resonance spectroscopy (MRS) and its regions of interest are mainly located in focal zones which can be observed with magnetic resonance imaging (MRI); however, both of them cannot manifest the changes of focal marginal zone. Contrarily, hydrogen magnetic resonance spectroscopy (1H-MRS) can det ect the all regions of brain.OBJECTIVE: To observe the 1H-MRS manifestations of patients with heroin spongiform leucoencephalopathy (HSLE) so as to analyze metabolic regularities of N-acetyl aspartate (NAA), creatine (Cr) and bilineurine (Cho) in brain.DESIGN: Case-contrast observation.SETTING: Department of Neurology, Nanfang Hospital.PARTICIPANTS: Three HSLE patients including 2 males and 1 female who were diagnosed with clinical imaging were selected from the Department of Neurology, Nanfang Hospital from August 2005 to August 2006, and all of them were regarded as the case group. In addition, 10 healthy volunteers were regarded as the control group.METHODS: Siemens Megnetom Vision Plus 1.5T superconductive magnetic resonance (MR) system and standard head coil were used in this study, and then, all subjects were checked with 1H-MRS.MAIN OUTCOME MEASURES: Levels of NAA, Cr and Cho in white matter of frontal, parietal and occipital lobes, metabolic maps of them and ratios of NAA/Cr and Cho/Cr.RESULTS: All 13 subjects were involved in the final analysis. ① NAA level: The level of NAA in white matter of frontal,parietal and occipital lobes of case 1 was lower than that of the subjects in the control group (79.50±21.65, 96.75±16.14,77.05±22.47; 146.07±15.49, 117.77±14.56, 120.83±16.02; P < 0.05, 0.01); meanwhile, white matter of parietal lobes of case 2 and case 3 was also lower than that of subjects in the control group (87.50±7.89, 80.65±11.73, P < 0.01). ② Cr level: There were no significant differences of the Cr level of all subjects in both case group and control group (P> 0.05).③ Cho level: Except white matter of frontal lobes in case 1, the level of Cho was lower in the case group than that in the control group (P < 0.01). ④ Ratio of NAA/Cr was lower in the case group than that in the control group, and the radio of Cho/Cr was decreased remarkably. ⑤ Metabolic maps of NAA and Cr manifested a low signal in focal site. ⑥ Ratio of Cho/Cr was obviously reversed in focal marginal zone, but wave of lactic acid was not observed at the same time.CONCLUSION:The area with abnormal metabolites in HSLE patients showed by 1H-MRS is obviously larger than the visible lesion area showed by MRI.There are abnormal metabolites in the adjacent area of HSLE lesions.
7.Effects of Rehabilitation Surgery Assisted to FOLFOX Chemotherapy on Postoperative Recovery,Complica-tions and Quality of Life in Patients with Colorectal Cancer

OBJECTIVE:To investigate the effect of rapid rehabilitation surgery assisted to fluorouracil,oxaliplatin,leucovo-rin(FOLFOX)on postoperative recovery,complications and quality of life in patients with colorectal cancer. METHODS:60 pa-tients with colorectal cancer were randomly divided into control group(30 cases)and observation group(30 cases). After resection of colorectal cancer,control group received 85 mg/m2 Oxaliplatin for injection,2 h intravenous infusion,d1+200 mg/m2 Calcium le-vofolinate for injection,intravenous infusion,once a day,d1-2+400 mg/m2 Fluorouracil injection,intravenous infusion,once a day, d1-5 then 600 mg/m2 for 22 h intravenous infusion,d1-2. Observation was additionally given rapid rehabilitation surgery. 2-week was regarded as 1 course,it lasted 12 courses. Urinary retention time,anal exhaust time,first defecation time,postoperative hospital-ization time,scores of life quality (overall health,emotional function,mental health,physiological functions,physical function,so-matic pain,vitality,social function)in 2 groups were observed,and the incidence of postoperative complications was recorded. RE-SULTS:The urinary retention time,anal exhaust time,first defecation time,postoperative hospitalization time in observation group were significantly shorter than control group,the incidence of postoperative complications in observation group was signifi-cantly lower than control group,overall health,emotional function and mental health score were significantly higher than control group,the differences were statistically significant(P<0.05),but there were no significant differences in physiological functions, physical function,somatic pain,vitality,social function score in 2 groups (P>0.05). CONCLUSIONS:Rapid rehabilitation sur-gery assisted to FOLFOX chemotherapy can reduce the incidence of postoperative complications in patients with colorectal cancer, improve quality of life,shorten hospitalization time and promote postoperative recovery.
8.Spontaneous ovulation in in vitro fertilization-embryo transfer cycles using gonadotropin-releasing hormone antagonist:a large-sample retrospective study

Objective To investigate the premature spontaneous ovulation rates in in vitro fertilization-embryo transfer (IVF-ET) cycles using gonadotropin-releasing hormone antagonist (GnRH-ant) and gonadotropin-releasing hormone agonist (GnRH-a), as well as the risk factors for premature spontaneous ovulation. Methods The rates of premature spontaneous ovulation in a total of 10 612 cycles using GnRH-ant or GnRH-a were compared. Matched case-controlled study and binary logistic regression model were conducted to analyze the risk factors for premature spontaneous ovulation. Results The spontaneous ovulation rate in the whole for GnRH-a cycles was 0.15%(13/8 514), compared with a 1.62%(34/2 098) in GnRH-ant cycles (P<0.01). Further matched controlled study and regression analyze found out that higher basal FSH level was a predominant risk and prediction factor for spontaneous ovulation (OR=1.20, P=0.009). Conclusions In GnRH-ant cycles, spontaneous ovulation rate is about 10 times than which in GnRH-a cycles. Diminished ovarian function is a predominate risk factor for premature spontaneous ovulation.
9.Chemical constituents in roots of Angelica dahurica var. formosana

Objective:To investigate the chemical constituents in the roots of Angelica dahurica var.formosana.Methods: The dried rhizomes of Angelica dahurica var.formosana were chopped and extracted with 80% EtOH for 3 times under reflux.The EtOH extract was isolated by chromatography on silica gel using a gradient solvent system(PE:EtOAc & CHCl_3MeOH) and Sephadex LH-20.The structures of the contents were elucidated by nuclear magnetic resonance and MS spectral analysis.Results: Seventeen compounds were identified,namely,isoimperatorin(1),imperatorin(2),bergapten(3),alloimperatorin(4),xanthotoxol(5),cnidilin(6),byakangelicin(7),bergaptol(8),byakangelicol(9),pabulenol(10),oxypeucedanin hydrate(11),desmodimine(12),palmitic acid(13),stiamasterol(14),?-sitosterol(15),?-daucosterin(16),and sucrose(17).Conclusion: Compounds 8 and 12 have been isolated from Angelica dahurica var.formosana for the first time.Compound 12 has been firstly isolated from Umbelifera plants.All the courmarin compounds belong to linear 6,7-furanocourmarins.
10.Applied anatomy of stellate ganglion block

Objective To study the anatomical characteristics of the stellate ganglion (SG) for providing morphological data for SG block. Methods The shape, size, position, and relation of 30 cases of SG in 15 adult cadavers were observed and measured. Results Most of the SG was stellate-shaped. The average volume of the stellate ganglion was (0.86?0.07)cm3. The position of the SG could be localized according to the adjacent bone or muscle marks, and the shortest distance was between the SG and the transverse process of the 7th cervical vertebrae. There were many important structures around the SG, in which the vertebral artery was closely related to the SG. Conclusion During performance of the SG block, the surface projection, puncture point, and medication dosage could be judged on the basis of these quantitative anatomic data.
