1.Rheumatoid arthritis combined with pigmented villonodular synovitis: A case report and literature review.

We reported a case of rheumatoid arthritis (RA) combined with pigmented villonodular synovitis (PVNS) from Peking University People's Hospital. The clinical data were introduced and the related literature was reviewed. The clinical features, treatment and prognosis of the disease were summarized to improve clinicians' understanding of this rare disease and avoid misdiagnosis and delayed diagnosis. A 45-year-old female, with 15 years of RA history and unregular treatment, was admitted to the hospital with the complaint of aggravating pain and swelling in the right knee for 3 months. The puncture of the right knee was performed and there was a large amount of bloody synovial fluid that could not be explained by her RA history. Moreover, the magnetic resonance imaging (MRI) of the right knee revealed hemosiderin deposition with low-intensity signals on both T1-weighted and T2-weighted images which suggested PVNS to us. Then, the patient underwent knee arthroscopy and biopsy to assist in diagnosis. The arthroscopic appearance and pathology were consistent with PVNS and the hyperplastic synovium was removed during arthroscopy. After the operation, she did active functional exercises and took disease-modifying antirheumatic drugs to control RA. She recovered well and remained asymptomatic after half a year of follow-up. Also, there was no recurrence of the right knee. As we all know, RA is a systemic autoimmune disease characterized by chronic synovitis and joint damage. And PVNS is characterized by synovial proliferation and infiltrative process. Both of them are synovial involvement and the clinical manifestations are quite similar. PVNS has occasionally been reported in association with RA. So it is difficult to make a clear diagnosis of RA combined with PVNS. Literature was searched with RA+PVNS in the WanFang Medical Network Database and China National Knowledge Infrastructure and there were no related Chinese cases. Then we searched literature from PubMed with RA+PVNS. The cases were still rare and eventually 2 related articles were yielded including 2 similar patients. It is necessary to fully understand the disease development, complicated MRI appearance and various pathological morpho-logy. They can contribute to making a correct diagnosis which is effective to guide the proper treatment.

3.Empirical study of hepatocellular carcinoma's gene therapy using hTERTp-TK/GCV in HepG2 which was mediated by chitosan nanoparticles vehicles

Objective To compare the transfection efficiency of galactosylated chitosan nanoparticle vehicle with chitosan nanoparticles vehicle,and observe the therapeutic effect of pGL3-hTERTp-TK on HCC cell line HepG2. Methods Preparing the chitosan and galaetosylated chitosan. Constructing the pGL3-hTERTp-TK plasmid and the Ch/DNA and GC/DNA complexes.Transfecting the HepG2 and the normal hepatic cell L-02 with chitosan/DNA and galactosylated chitosan/DNA complexs.Detecting the fluorescence and the expression of luciferase gene using the fluorescent microscope and the scintillation counter.Detecting the cell growth and apoptosis through the Caspase-3 and the flow cytometry. Results Two clear straps appeared in the agarose gel.The locations were 300 bp and 1100 bp.The relative lueiferase activity of pGL3-hTERTp-Luc+mediated by galactosylated chitosan was powerful than which of pGL3-hTERTp-Luc+mediated by chitosan in HepG2 by the scintillation counter.However,the relative luciferase activity was very weak in L-02.The same results were observed by fluorescent microscope.When the concertration of the GCV was 10 μg/ml(t=51.40,P=0.000),the HepG2 cell inhibition which was transfected by GC-pGL3-hTERTp-TK was obviously different from the L-02 cell inhibition which was transfected by GC-pGL3-hTERTp-TK in the statistics.The significant apoptotic rate was 65.28%in HepG2 which was transfected with GC/DNA,whereas it was only 10.80% in L-02. The significant apoptotic rate in HepG2 which was transfected with GC/DNA was very higher than the other groups (LSD, P <0.05). The significant apoptotic rate (10.80%) in L-02 which was transfected with GC/DNA was very higher than the group which was Ch-pGL3-control (LSD, P =0.000). The average fluorescence intensity of the HepG2 which is transfected by the Ch-pGL3-hTERTp-TK was 168.02± 3.68. The average fluorescence intensity of the HepG2 which is transfected by the GC-pGL3-hTERTp-TK was 204.45 ±3.45. The two groups had a significant difference in the statistics (t=-12.504, P<0.05). Conclusion Galactosylated chitosan has higher transfection efficiency than chitosan. GC-pGL3-hTERTp-TK could specially attack HCC cell line and almost has no influence on normal hepatic cells.
4.Study on Determination Method for Bacterial Endotoxins in Nedaplatin for Injection

Objective:To establish a detection method for bacterial endotoxins in nedaplatin for injection. Methods:The experi-ment was carried out and the result was evaluated according to the appendix XIE of Chinese Pharmacopoeia 2010 Vol. II. Results:There was no interference from nedaplatin for injection to bacterial endotoxin in TAL reaction. Conclusion:The method is feasible to detect bacterial endotoxin in nedaplatin for injection, which can replace the rabbit pyrogen test.
5.Determination of Three Kinds of Phenylpropanoids in Stem Rind of Daphne giraldii by HPLC

Objective To establish a HPLC method for the determination of daphnetin, 7-OH coumarin and syringing in Zushima. Methods The separation was performed on YMC-Pack ODS (4.6 mm?250 mm, 5 ?m) column with a mobile phase consisting of methanol-0.5% acetic acid-water (19︰8︰1) as gradient elution at the flow rate of 1.0 mL/min. The dual detective wavelength was set at 327 nm and 266 nm. The column temperature was 30 ℃. Results Baseline separation was achieved for the analysis of the three index components. The average recoveries were 98.4%~100.2%. Conclusion This method is convenient, rapid and accurate. It can be used for quality control in production of Zushima.
6.Survey on Pervasive Developmental Disorder in 2-6 Year-old Children in Beijing

Objective: Exploring the epidemiological state of pervasive developmental disorder in 2-6 year-old children in Beijing. Methods: A total of 21866 children aged 2-6 year-old were recruited from permanent residents of Beijing by 2-phase cluster sampling.The Clancy Autism Behavior Scale was used for screening.The Childhood Autism Rating Scale and DSM-Ⅳ were used for diagnosis.Results: 16 children were diagnosed as pervasive developmental disorder (14 with autistic disorder, 1 with atypical autistic disorder, 1 with Rett syndrome).The false negative rate was 0.80 ‰.The prevalence of pervasive developmental disorder was 0.73 ‰, the adjusted prevalence was 1.53 ‰,the average annual detective rate was 0.11‰.The prevalence rate had no difference in distribution between urban-rural, among different age groups, sex and nations(?2=0.11~1.85,P=0.739-0.173).The low family income was related to high prevalence significantly ( tendency ?2=4.70,P=0.030 ).25% children with pervasive developmental disorder had not been identified by parents or had not been to clinics.80% children with pervasive developmental disorder had not get intervention.All children had demand to get intervention. Conclusions: The prevalence of pervasive developmental disorder in 2-6 year-old children in Beijing was not low.Low family income is the risk factor.The rate of intervention was low.The government should pay close attention to the children with pervasive developmental disorder and their needs.
7.Effect of traumatic occlusion on the expression of OPG and RANKL mRNA in the rat alveolar bone

Objective To investigate the expression change of receptor activator of NF ?B ligand (RANKL) and osteoprotegerin (OPG) mRNA in rat alveolar bone and the possible mechanism of alveolar bone resorption under traumatic occlusion. Methods Thirty SD rats weighed 270g?20g were used and divided into 6 groups, of which one group was as control. A square wire thickened about 1mm was bonded with Super bond composite resin on to the occlusal surface of left upper first molar of the rats in experiment groups. The animals were executed after 1,3,7,14,28 days. The left jaw were processed for the study of the expression of RANKL and OPG mRNA with RT PCR. Results Expression of RANKL and OPG mRNA in alveolar bone was significantly different between experiment and control groups. During occlusal trauma, expression of RANKL and OPG mRNA enhanced in 1 day group. The ratio of RANKL/OPG mRNA expression raised in 1、14 and 28 day groups. Conclusion The ratio of RANKL/OPG mRNA expression changed during occlusion trauma. RANKL and OPG might be involved in the restructure of periodontal tissue after traumatic occlusion
8.Analysis of Drug Use in 273 Sample Hospitals of Yangtze River Basin in 2014

OBJECTIVE:To provide reference for clinical rational drug use. METHODS:The drug sales amount,DDDs, DDC and major pharmaceutical enterprises in 273 sample hospitals of Yangtze River basin in 2014 were statistically analyzed. RE-SULTS:The growth of consumption sum slowed down gradually. The anti-infective drugs,cardiovascular drugs and nervous sys-tem drugs occupied the medical mainstream market. Top 3 drugs in the list of consumption sum in consecutive 4 quarters were monosialotetrahexosylganglioside,thymic peptide and lansoprazole. Nearly half of top 15 drugs were auxiliary drugs;Chinese pat-ent medicines grew increasingly and appeared in the list. The sales enterprises were mainly domestic enterprises,which concentrat-ed in Sichuan,Chongqing,Jiangsu province,and other places. CONCLUSIONS:The structure of drug use in sample hospitals is basically consistent with the spectrum of disease in China. However,there is deviation between auxiliary drug use and international guide,and the rationality of clinical drug use needs to be improved.
9.Association between C1GALT1,DEFA polymorphisms and its susceptibility to IgA nephropathy in Chinese Han and Hui population in Gansu Province

Objective:To investigate the association between the C1GALT1 rs1008898,the DEFA rs2738081 polymorphisms and susceptibility to IgA nephropathy in Chinese Han and Hui population in area of Gansu Province and explore molecular markers to predict IgA nephropathy Methods: In this study,there were 146 patients with IgAN and 180 normal controls in Han people and 83 patients with IgAN and 100 normal controls in Hui people.Two SNPs as rs1008898 and rs2738081 were analyzed with polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and polymerase chain reaction-gene sequencing technology.The genotype and allele frequency of rs1008898 and rs2738081 were compared between patients with IgAN and normal controls.Results: Rs1008898 GG genotypes and G allele were over represented in IgAN patients compared with controls.Distribution of rs1008898 polymorphism in patients with IgAN and normal controls showed no difference in Hui people.Neither Han nor Hui population,rs2738081 polymorphism had difference between IgAN patients and normal controls.Conclusion: The G allele of rs1008898 probably has correlation with the genetic susceptibility of IgAN in Gansu Han people.
10.Clinical Observation of Eprosartan in the Treatment of Hypertensive with Coronary Heart Disease

OBJECTIVE:To observe the efficacy and safety of eprosartan in the treatment of hypertensive patients with coro-nary heart disease. METHODS:160 hypertensive patients with coronary heart disease randomly divided into observation group and control group. All patients were given aspirin,nitroglycerin,low molecular weight heparin,statins and other conventional treat-ment;control group was additioanlly given 50 mg Losartan potassium tablet,orally,once a day. Observation group was additional-ly given 600 mg Eprosartan tablet,orally,once a day. The treatment course for both groups was 6 months. Clinical efficacy,sit-ting systolic blood pressure and diastolic blood pressure,alanine aminotransferase(ALT),aspartate aminotransferase(AST),urea (UREA),creatinine(Cr),uric acid(UA),total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C), low-density lipoprotein cholesterol (LDL-C),the Mini-Mental status (MMSE) scale and activities of daily living (ADL) scale scores before and after treatment and incidence of adverse reactions in 2 groups were observed. RESULTS:There was no signifi-cant difference in the total effective rate between 2 groups(P>0.05). After treatment,the sitting systolic blood pressure and diastol-ic blood pressure,MMSE and ADL scale scores in 2 groups were significantly lower than before,and sitting systolic blood pres-sure in observation group was lower than control group,the differences were statistically significant(P<0.05),and there were no significant differences in sitting diastolic blood pressure,MMSE and ADL scale scores between 2 groups(P>0.05),and no signifi-cant differences in ALT,AST,UREA,Cr,UA,TC,TG,HDL-C and LDL-C between 2 groups before and after treatment(P>0.05). There were no obvious adverse reactions during treatment. CONCLUSIONS:Eprosartan can effectively reduce sitting systol-ic blood pressure in hypertensive patients with coronary heart disease,and improve cognitive function,with good safety.
