1.Effect of acupuncturing Back-Shu points on gastrin, motilin of chronic atrophic gastritis model in rats

Objective To study the effect of gastrin, motilin of chronic atrophic gastritis model in rats by acupuncturing at back-shu points. Methods 60 SD rats were randomly divided into blank control group, model control group, omeprazole group, back-shu points group, with 15 rats in each group, and to establish the model of chronic atrophic gastritis by irregular diet that given rats odd and even days. The blank control group, model control group were fed with normal saline, 2ml/time, 1 time/day. The omeprazole group was given oral gavage(Omeprazole 0.44 mg/kg, Clarithromycin 6.92 mg/kg, Metronidazole 7.6 mg/kg), 1 time/day. Back-shu points group received acupuncture at “Geshu”“Ganshu”“Pishu”“Weishu”“Shenshu”, retaining 15 min, 1 time/day. After 30 days, the content of ELSIA was used to detect the serum gastrin, motilin. Results ①GAS of model control group(54.25±5.70)pg/ml was significantly decreased compared with blank control group(66.63± 5.99)pg/ml, the difference was statistically significant (P<0.05). Compared with the model control group, GAS content of omeprazole group(62.70±8.55)pg/ml and back-shu Points group(66.27±3.79)pg/ml were higher, showed significant differences(P<0.05). ②MOT of model control group(223.60±17.83)pg/ml was significantly increased compared with the blank control group(188.45 ± 8.90)pg/ml, the difference was statistically significant(P<0.05). MOT of omeprazole group(200.16 ± 4.38)pg/ml and back-shu Points group(190.11±10.04)pg/ml were lower than those in the model control group, there was statistically significant difference(P<0.05). Conclusion Acupuncturing at back-shu acupoints can increase the content of GAS, reduce content of MOT, regulate gastrointestinal hormone in rats of chronic atrophic gastritis model.
2. Extraction, isolation, and structure elucidation of two new triterpene glycosides from sea cucumber Holothuria nobilis

Objective: To study the constituents from the sea cucumber Holothuria nobilis. Methods: The compounds of the sea cucumber were separated by multi-chromatography and their structures were elucidated by physico-chemical and spectral analyses. Results: Two new triterpene glycosides were isolated and their structures have been deduced by extensive spectral analysis (IR, 2D-NMR, and ESI-MS) and chemical structures as 3-O-{3″″-O-methyl-β-D- glucopyranosyl-(1→3)-β-D-glucopyranosyl-(1→4)-β-D- quinovopyranosyl-(1→2)-4′-O-sulfate-β-D-xylopyranosyl} -holosta-22, 25-epoxy-9-ene-3β, 12α, 17α, 25β-tetraol- sodium salt (nobiliside I, 1) and 3-O-{β-D-quinovopyranosyl-(1→2) 4′-O-sulfate-β-D-xylopyranosyl}-holoshillaside 18 (16)-lactone-22, 25-epoxy-9-ene-3β, 12α, 17α-triol sodium (nobiliside II, 2). Conclusion: Two aglycons are both new compounds named nobiliside I and nobiliside II.
3. Two new triterpene glycosides from Stichopus variegatus

Objective: To study the constituents from the sea cucumber, Stichopus variegatus. Methods: The crude saponins of S. variegatus were desulfated with pyridine-dioxane (1:1), the desulfated products were separated by multi-chromatography to afford two compounds. Results: On the basis of chemical methods and spectral evidences, especially 2D-NMR and ESI-MS, these two compounds were identified as 3-0-[β-D-xylopyranosyl-(1→2)-4′-O- sulfate-β-D-xylopyranosyl]-holosta-9-ene-3β,12α,17α, 25β-tetraol sodium salt, named as variegatuside A (1) and 16β-acetoxy-3-O-β-D-xylopyranosyl-holosta-22,25-epoxy-9-ene-3β, 12α,17α-triol, named as variegatuside B (2). Conclusion: Compounds 1 and 2 are new compounds.
6.The analysis of 145 cases of improper avademic behavior in medical journal

Objective To understand the status of the improper avademic behavior in medical journal,and to explore the prevention measures.Methods Total of 1 321 article in Zhongguo Yishi Jinxiu Zazhi were included in this study.Similarity detection system of Wanfang data and Google and baidu search engine were used to duplicate checking.The suspected existence of contributions for the improper avademic behavior,to check the whole paper of relevant original manuscript,and analyzed the characteristics of improper avademic behavior.Results One hundred and forty-five improper avademic behavior were found in 1 321 articles,accounted for 11.0%.The similarity of 58 paper were higer than 50.00%,accounted for 40.0%.Among them,the similarity of 10 papers were more than 90.00%,8 papers were 80.00%-89.99%,12 papers were 70.00%-79.99%,13 papers were 60.00%-69.99%,15 papers were 50.00%-59.99%,13 papers were 40.00%-49.99%,accounted for 9.0%,24 papers were 30.00%-39.99%,accounted for 16.6%,50 papers were 20.00%-29.99%,accounted for 34.5%.In 145 papers,copying accounted for 35.2% (51/145),data forge or tamper accounted for 28.3%(41/145),breach of medical ethics and academic moral accounted for 28.3% (41/145),fraud accounted for 6.9% (10/145),a draft cast more accounted for 1.4% (2/145).Original articles in the clinic were 92 papers,accounted for 63.4%;case reports were 28 papers,accounted for 19.3%;review were 25 papers,accounted for 17.2%.76 papers with a single author,accounted for 52.4%;38 papers with two authors,accounted for 26.2%,31 papers with more than three authors,accounted for 21.4%.Conclusions Contributions of suspected improper avademic behavior ratio is higher,and have certin characteristic.Medical journal editors should strengthen the responsibility consciousness,improve the ability to identify misconduct papers.
7.Inhibitory effect and its molecular mechanisms of MicroRNA-34a on human na-sopharyngeal carcinoma CNE-2 cell line subcutaneous xenograft tumor in nude mice

Objective:To investigate the inhibitory effect and its possible molecular mechanisms of MicroRNA-34a(miR-34a) on the human nasopharyngeal carcinoma CNE-2 cell line subcutaneous xenograft tumor in nude mice.Methods: The human nasopharyngeal carcinoma CNE-2 cell line was cultured in vitro.miR-34a and Scrambled miRNA recombinant plasmids were successfully established and stably transfected into CNE-2 cells.Fifteen six-week-old male nude mice were divided randomly into three groups:miR-34a group(5 mice) ,Scrambled miRNA group(5 mice) ,Blank control group(5 mice).Different CNE-2 cells were subcuta-neously injected on the back near right lower limb.Tumor volumes were examined every 7 days.Mice were executed on the 35 days,and the eventual average tumor volumes and weights were examined.Total RNA and protein were isolated from tumors,and the expression of miR-34a,CDK6,and Bcl-2 mRNA and protein were determined by qRT-PCR and western blot,respectively.Results: The relative expressions of miR-34a was significantly up-regulated in miR-34a transfected group compared to Scrambled miRNA transfected group (P<0.05).Eight subcutaneous xenograft tumor models were successfully established.The nude mice of miR-34a transfected group appeared a smaller tumor volume compared to the other two groups at the beginning of 21th day( P<0.05).The eventual average tumor volumes in miR-34a group,Scrambled miRNA group and blank control group were(351.37±98.19)mm3,(798.75±91.04)mm3 and
(849.62±101.32) mm3 ,respectively,and the eventual average tumor weights in miR-34a group,Scrambled miRNA group and blank control group were(0.81±0.13)g,(1.47±0.21)g and(1.58±0.37)g,respectively.Both the eventual average tumor volumes and weights in miR-34a group were lower compared to the other two groups(P<0.05).qRT-PCR results revealed that the expression of miR-34a in miR-34a transfected group was significantly higher than in the other two groups,while the mRNA and protein expression of CDK6 and Bcl-2 were lower than the other two groups ( P<0.05 ) .Conclusion: miR-34a may inhibit the growth of human nasopharyngeal carcinoma CNE-2 cell line subcutaneous xenograft tumor in nude mice by down-regulating CDK6 and Bcl-2.
8.Meta analysis of BISAP score system in predicting the severity of acute pancreatitis

Objective To investigate the value of BISAP scoring system in predicting the severity of acute pancreatitis.Methods Medline,EMBASE,Science Direct,Springer link,CBM,Cnki,Wan fang and VIP database were retrieved by computer between January 2000 and March 2013,and articles of BISAP score system in predicting the acute pancreatitis were collected according to strict inclusion criteria.Quality assessment was made by QUADAS scale.Meta-Disc1.4 software was used to analyze the heterogeneity of included articles and perform quantitative synthesis,as well as calculate the pooled sensitivity and specificity,positive likelihood ratio,negative likelihood ratio,and draw the ROC curve,and the results were presented with 95% CI.Results A total of 11 articles were included,and there were 7 Chinese articles and 4 English articles.According to QUADAS scale,there were 4 articles of A grade,5 articles of B grade,2 articles of C grade.There were 6 studies using 2 as BISAP cut-off value,and 9 articles using 3 as cut-off value,while 4 studies using 2 cut-off values.Pooled analysis showed diagnostic odds ratio of 8.03 (95 % CI 5.66-11.38) when cut-off value was 2,and diagnostic odds ratio of 7.49 (95% CI 5.35-10.49) when cut-off value was 3.There were moderate heterogeneity in both groups (I2 =63.3 %,P =0.018; I2 =56.1%,P =0.019).When BISAP cut-off value was 2,the pooled analysis of sensitivity,specificity,positive likelihood ratio,negative likelihood ratio and area under curve (AUC) were 59% (95% CI56%-63%),82% (95% CI 80%-83%),3.50 (95% CI2.96-4.14),0.45 (95% CI0.36-0.56) and0.82; and when BISAP cut-off value was 3,the corresponding values were 44% (95% CI41%-47%),90% (95% CI 89%-91%),4.59 (95% CI 3.31-6.37),0.64 (95% CI 0.61-0.68) and 0.64.The former had a high sensitivity,low specificity and large AUC,while the latter had a low sensitivity,high specificity and small AUC.Conclusions The best cut-off value of BISAP in predicting SAP is 2.When cut-off value is 2,the misdiagnosis rate is low,and the false positive rate is in the acceptable range,which is suitable for clinical application.
9.The application of simulated patient role play in the inquiry and the medical record writing teaching

Objective To explore the application and effectiveness of simulated patient role play in the inquiry and the medical record writing teaching.Methods Carrying out the teaching model of simulated patient role play in the Second Clinical Medical College of Southern Medical University to the undergraduate students of grade 2009 snd 2010 in diagnose inquiry and medical record writing training class.A self-designed student questionnaire mainly about atmosphere of class,study enthusiasm,Self-evaluation of learning efficiency,the effect of the medical record writing in-class training and so on was used to do the survey.On the other hand,questionnaire for teachers included students' performance,students' writing ability,etc.100 students and 50 teachers were investigated with the survey.The percentage of the options of each question on the questionnaire was calculated in order to analyze the new teaching mode.Results The random questionnaire survey showed that 85% (85/100) of the students believe that the new teaching mode can activate classroom atmosphere and improve learning motivation.About 62%(62/100) of the students said medical record writing errors can be obtained and corrected timely in-class training.Most students in grade 2009 have better capacity in medical record writing than those students who did not contact with new teaching mode.Conclusion The teaching model of simulated patient role play in the analogue inquiry and medical record writing is efficient and feasible in medical education,which can improve students' practical ability and is expected to be applied in undergraduate medical education.
10.Change of heart rate turbulence in coronary artery disease patients and its predictive value

Objective: To explore change of heart rate turbulence (HRT) in patients with coronary artery disease (CAD) and its predictive value. Methods: A total of 58 CAD patients were regard as CAD group. The 50 normal persons with negative CAG were regard as normal control group.
All subjects received 24h dynamic ECG, CAG and echocardiography examination. Left ventricular ejection fraction (LVEF) and HRT indexes were compared between two groups. Results: Compared with normal control group, there was significant rise in turbulence onset [TO, (-1.34±1.09)% vs. (2.32±0.60)%] and significant reduction in turbulence slope [TS, (11.14±2.27) ms/RR vs. (3.44±0.60)ms/RR] in CAD group, P<0.01 both; in CAD group, along with coronary artery lesion aggravated (single-, double-, multi-vessel coronary disease), there was significant rise in TO [(1.35±0.52)% vs. (2.56±0.83)% vs. (3.01±1.62)%] and significant reduction in TS [(6.49±1.79) ms/RR vs. (3.33±1.02) ms/RR vs. (1.66±0.30) ms/RR], the difference was significant in two-two comparison among all groups, P<0.05 or <0.01; compared with CAD without cardiac event group, there was significant rise in TO [(1.68±0.61)% vs. (2.24±0.24)%], and significant reduction in TS [(5.87±1.22) ms/RR vs. (1.65±0.23) ms/RR] and LVEF[(66.18±2.64)% vs. (56.36±3.34)%] in CAD with cardiac event group, P<0.01 all. Conclusion: Weakened HRT phenomenon exists in CAD patients. HRT can be treated as an index evaluating severity of CAD patients’ condition and an effective predicting index for recurrent cardiac events in CAD patients.
