Objective To determine the mechanism of nitric oxide synthase(NOS) and prostacyclin(PGI2) acting on splanchnic hyperdynamic circulation of portal hypertention(PHT). Methods Ninety-six Sprague-Dawley rats were divided into three groups, namely, intrahepatic portal hypertension(IHPH, n=31), prehepatic portal hypertension(PHPH, n=33) and sham-operated controls(SO, n=32). Animals of each group were received indomethacin(INDO) either on a short term or long term with saline as control. Portal venous pressure, together with the concentration of nitric oxide (NO) and PGI2 in serum was measured. The constitutive nitric oxide synthase(cNOS)and inducible nitric oxide synthase(iNOS)activity in the abdominal aorta and small intestine of these rats were detrmined by spectrophotometry method. RT-PCR was performed to measure the levels of iNOS and cNOS mRNA in the arteries and guts mentioned above. Results Although INDO decreased the concentration of PGI2 in serum, the long-term INDO-treated group restored splanchnic hyperdynamic circulation in both IHPH and PHPH rats, concomitant with enhanced expression of iNOS and concentration of NO(P0.05). Conclusion Overproduction of NO inducing hemodynamic abnormalities of PHT is synthesized principally by increase of iNOS. There may be a possible interaction between PGI2 and NO in hyperhemodynamics of PHT, while PGI2 may not be a mediator in the formation and development of hyperdynamic circulatory state.

Objective To determine the contents of organic acids in different processed products and different parts of Schisandra Chinensis Fruits by HPLC;To discuss the influence of different processing methods on contents of organic acids.Methods Ecosil C18-AQ Column (250 mm×4.6 mm, 5μm) was used with mobile phase of methanol-0.5% acetic acid (15∶85), at a flow rate of 0.8 mL/min, and the detection wavelength was 260 nm for protocatechuic acid;mobile phase of acetonitrile∶15 mmol/L potassium dihydrogen phosphate buffered saline solution=3∶97 (phosphoric acid adjusting pH=3), at a flow rate of 1.0 mL/min, and the detection wavelength was 210 nm for citric acid.Results The contents of protocatechuic acid and citric acid in Schisandra Chinensis Fruits, wine steaming and vinegar steaming Schisandra Chinensis Fruits were 0.0098%, 0.0124%, 0.0121% and 14.8293%, 14.1694%, 14.3650%, respectively. The contents of protocatechuic acid and citric acid in pulp, seeds were 0.0123%, 0.0090%, and 22.8810%, 3.8990%, respectively.Conclusion The protocatechuic acid was increased significantly after being processed, but citric acid showed a small change and had a slight downward trend. The two organic acids in pulp were higher than the two in seeds.

Objective To analyze the MRI features of fibrocystic breast disease (FBD). Methods 39 patients with pathologically proven FBD were retrospectively reviewed. The imaging protocol consisted of pro-contrast imagings and bilateral breast volume im-aging for breast assessme NT(VIBRANT). MRI features were interpreted based on the morphologic and enhancement kinetic de-scriptors defined on ACR DIRADS-MRI lexicon. Results There were 3 types on ACR BI-RADS-MRI lexicon in FBD,including 13 focal mass-type showing malignant enhancement kinetic pattern mostly;15 cyst-type with non-enhancement or mild enhance-ment,of them,1 case with periductal mastiffs showing strong enhancement and rapid up-slope and wash-out mimicking a breast cancer. 6 non-mass type usually showing benign enhancement kinetic pattern. In 5 patients,MRI did not show definite abnormal evi-dence. Conclusion MRI is of significant value in diagnosing FBD.

ObjectiveTo explore the influence of gender,age and types of peer acceptance on the ability of theory of mind among rural parent-absent children.Methods 312 rural parent-absent children participated in the study (including 162 boys,150 girls; 102 members of 8 year-old,119 members of 9 year-old,and 91 members of 10 year-old).Their ability of theory of mind and types of peer acceptance were examined by the 2nd-order False-beliefs Tasks and Peer nomination.The date were analyzed by Multivariate analysis of variance (ANOVA),LSD checkout and Simple effect analysis.ResultsThere were statistically significant effects on ages in theory of mind,types of peer acceptance ( F=4.57,P=0.011 ; F =3.82,P =0.005 ) and the interaction among gender,ages and types of peer acceptance(F=3.20,P =0.042; F =2.50,P =0.043 ).The least significant different(LSD) assay of the ability of theory of mind age and types of peer acceptance shows:there were statistically significant lies in the divergence of age 8 and age 9 group,and also in age 9 and age 10 group,(I-J =-0.51,P =0.016;I-J =- 0.90,P =0.000 ).In the same way,there were also statistically significant lies in the divergence of the ability of theory of mind performance-popular and rejected ( I-J =1.05,P =0.001 ) ; popular and neglected ( I-J =0.84,P =0.003 ) ; popular and controversial( I-J =1.03,P =0.002) ; rejected and average ( I-J =- 0.66,P =0.02 ) ; controversial and average ( I-J =- 0.64,P=0.034).Results of simple effect shows:there were statistically significant lies in the gender divergence of the 9 age group (F =4.98,P =0.026),the age divergence of girls (F =7.50,P =0.001 ),the gender divergence of controversial groups (F=5.76,P =0.017 ),the peer acceptance divergence of boys(F=4.97,P =0.001),and also the peer acceptance divergence of girls(F =2.54,P =0.04).Conclusion There is substantial connection between the ability of theory of mind of rural parent-absent children and ages and types of peer acceptance.

OBJECTIVE:To study the effects of indometacin on apootosis and proliferation of cervical cancer Hela cell. METH-ODS:Hela cell was cultured in vitro as study object,and cultured with 0(blank control),200,400,600,800 and 1 000 μmol/L indometacin for 24,48 and 72 h. The inhibitory rate of indometacin to the proliferation of Hela cells was detected by MTT assay. After treated with 0(blank control),400,600 and 800 μmol/L indometacin for 24 h,the change of cellular morphology was ob-served by invert microscope;cell cycle phase and apoptosis were analyzed by flow cytometry. RESUITS:Indometacin of 600, 800,1 000μmol/L could inhibit the proliferation of Hela cell,which was positively correlated to drug concentration and time. Com-pared with blank control,indometacin could induce that Hela cell transformed from polygonous to round in appearance,and result-ed in cell apoptosis and necrosis;the proportion of cells at G0/G1 phase increased,while the proportion of cells at S phase reduced;the apoptotic rate of cells raised. CONCLUSIONS:Indometacin could inhibit the proliferation of Hela cell,block cell cycle at G0/G1 phase and induce apoptosis.

Cartilage antuumor component (CATC) was isolated from a 1 mol/L guanidine hydrochloride extract of bovine cartilage by acetone fractioned precipitation and superfiltration. Using human skin fibroblasts as a normal control, it was demonstrated that CATC inhibited the DNA synthesis of Hela, QGY7703 tumor cell lines and bovine artery endothelial cells, but accelerated the normal cells, when the concentration was below 1250 ?g/ml. At the concentration of 5 000 ?g/ml, CATC inhibited the two cell lines. With human tumor stem cell assay, CATC inhibited the stem cell growth of Hela and QGY7703 cell lines. These suggest that CATC has the effects of inhibiting angiogenesis and tumor cells.

Objective To explore the values of manual anastomosis with two operations out of anus in laparoscopic anal sphincter preserving resection of ultra low rectal cancer. Methods Radical excision of ultra low rectal cancer was performed with ultrasonic scalpel in 12 patients based on the concept of total mesorectal excision (TME) and ultra low coloreclal/anal anastomosis was performed applying manual anastomosis with two operations out of anus. Results All the operations were finished successfully, without conversions to open for surgery. One case had anastomotic leakage, and there were no bleeding and infection of abdominal cavity, anastomotic stenosis and other complications. The operating time was 185-310 (218 ±10) min, the blood loss was 160-450 (232 ±8) ml,the length of hospital stay was 9-14 (11 ±3) d. All patients were followed up 6-36(18 ± 2) months, local recurrence was not found but 1 case had liver metastasis.Conclusions The manual anastomosis with two operations out of anus in laparoscopic anal sphincter preserving resection of ultra low rectal cancer is safe,economical,effective,minimally invasive, and has the benefits of less bleeding during the operation and shorter hospital stay. It should be widely used.

ObjectiveTo observe β cell function in newly diagnosed diabetic patients with ketosis before and in remission period and evaluate its classification and predictive value.MethodsA total of 206 patients newly diagnosed as diabetic ketosis who had been treated with intensive insulin therapy in our hospital and entered in the honeymoon after the withdraw of insulin therapy were followed for 36 months from onset of diabetes.They were divided into two groups of type 1 and type2 diabetes ( group A and B),according to the dependence or independence on insulin treatment. The β cell function of the two groups before and in remission period was compared by oral glucose tolerance test (OGTF).β cell function was measured with the AUC of insulin and C-peptide and homeostatic model assessment β-cell function (HOMA-β),while homeostatic model assessment insulin resistant (HOMA-IR) for insulin resistant.The duration of the honeymoon and the change of insulin and C-peptide curve before and in honeymoon were also observed.ResultsThe AUC of insulin and C-peptide,the HOMA-β and the HOMA-IR before and after the intensive insulin treatment were lower in group A than that in group B [ before the insulin treatment:(10.18 ±2.36)mIU · h · L-1 vs (20.28 ±6.89)mIU · h · L-1,(1.56 ±0.53) μg · h · L-1 vs (3.75 ±0.67) μg · h · L-1,3.68 ± 1.08 vs 18.20 ±6.59,1.22 ±0.49 vs 3.06 ± 1.54,respectively;after the insulin treatment:(29.86 ± 8.65 ) mIU · h · L-1 vs (93.35 ± 19.42 ) mIU · h · L-1,( 3.99 ± 0.79 )μg · h · L-1 vs ( 12.54 ±3.83) μg · h · L-1,8.50 ±2.46 vs 56.17 ± 19.42,0.63 ±0.56 vs 1.42 ±0.78,respectively ].The duration of the honeymoon in group A was significantly shorter than in group B [ (7.9 ±5.2) months vs (20.9 ± 9.9 ) months ].In oral glucose insulin and C-peptide release test,the peak of insulin and C-peptide releasing curve in group A was brought forward by a half to 1 hour after intensive treatment while delayed in group B by 1 or 2 hours.The releasing peak of insulin and C-peptide in group A was less than two folds of the basic value,while four to ten fold of the basic value in group B.The positive ratio of glutamic acid decarboxylase antibody,insulin autoantibody and insular cellular antibody in group A and group B were 21.2％ vs 4.8％,18.1％ vs 3.3％,9.2％ vs 10.6％,respectively.ConclusionsOf all the patients newly diagnosed as diabetes ketosis who had entered into the honeymoon after intensive insulin therapy,91％ were type 2 diabetes.Inferior β cell function before insulin therapy,weaker remission after insulin therapy and shorter duration of remission period suggest the classification of type 1 diabetes.

ObjectiveTo investigate rational surgical approaches for Siewert type Ⅰ adenocarcinoma of the esophagogastric junction (AEG),and analyze the prognostic factors.MethodsThe clinical data of 103 patients with Siewert type Ⅰ AEG who were admitted to the Renji Hospital from January 2005 to December 2009 were retrospectively analyzed.All patients were divided into transthoracic approach group (61 patients) and thoracoabdominal approach group (42 patients).The incidences of numbers of lymph node dissected and postoperative complications of the 2 groups were compared using the chi-square test,Fisher exact probability or the t test.The survival curve was drawn by the Kaplan-Meier method and the survival was analyzed using the Log-rank test.Prognostic factors were analyzed using the one-way analysis of variance and Cox regression model.ResultsNo perioperative death was observed in the 2 groups.There were significant differences in the number of lymph node dissected and number of metastatic lymph node between the 2 groups (t =2.18,2.29,P ＜ 0.05 ).There was no significant difference in splenic injury between the 2 groups (P ＞ 0.05 ).There were no significant differences in postoperative bleeding,anastomotic fistula and stricture,esophagogastric reflux,pulmonary infection and esteomyelitis between the 2 groups (x2 =0.07,0.94,0.22,1.41,0.17,P＞0.05).Of the 103 patients,97(94.2％) were followed up.The mean postoperative survival time was 26 months.The median survival time was 26 months,and the 3-yearsurvival rate was 35.9％.The 3-year survival rates of transthoracic approach group and thoracoabdominal approach group were 32.8％ and 40.2％,with no significant difference between the 2 groups ( x2 =0.37,P ＞ 0.05).The results of univariate analysis showed that radical or palliative resection,TNM stage,lymph node metastasis stage,tumor diameter and metastasis rate,degree of radical resection were independent factors influencing the prognosis of patients with Siewert type Ⅰ AEG (x2 =21.07,26.04,22.42,6.26,32.20,20.80,P＜0.05).The results of multivariate analysis showed that degree of TNM stage,lymph node metastasis rate and radical resection were independent factors influencing the prognosis of patients ( Wald =12.01,8.75,10.03,P ＜ 0.05 ).Conclusions Thoracoabdominal approach is a reasonable selection for patients with Siewert type I AEG.Degree of TNM stage,lymph node metastasis rate and radical resection were independent risk factors influencing the prognosis of patients.
