1.Neutrophil gelatinase associated lipocalin and β2 microglobulin clinical value for the early prediction of diabetic nephropathy

Objective To explore the neutral grain cell gelatinase associated lipid lipocalin(NGAL ) and β2 microglobulin (β2‐MG) predictive value of diabetic nephropathy(DN) .Methods From January 2015 to April 2016 in the hospital for 78 cases of DN patients recorded as the observation group ,and then 50 cases of healthy volunteers were choosen as the control group at the same period .NGAL ,β2‐MG ,BUN ,SCr and GFR levels were compared in the two groups .And the positive rate and correlation were ana‐lyzed .Results The levels of NGAL and β2‐MG in the observation group were(473 .21 ± 127 .09)ng/mL ,(2 .76 ± 0 .45)mg/L , which were significantly higher than that in the control group(61 .36 ± 14 .230)ng/mL ,(0 .81 ± 0 .14)mg / ,respectively .The posi‐tive rate of NGAL and β2‐MG in the observation group were 96 .15% (75/78) ,91 .03% (71/78) ,which were significantly higher than that of the control group of 2% (1/50) ,6% (3/50) .While the BUN ,SCr in observation groups were significantly higher than that in the control group ,while the level of eGFR was significantly lower than that of the control group ,the differences were statis‐tically significant(both P< 0 .05) .NGAL ,β2‐MG and BUN ,SCr showed positive correlation(R = 0 .812 ,P= 0 .000 ;R = 0 .728 ,P=0 .001 ;R = 0 .748 ,P= 0 .001 ;R = 0 .685 ,P = 0 .021 ) ,and eGFR showed a negative correlation (R = - 0 .415 ,P = 0 .000 ;R=- 0 .371 ,P= 0 .000) .Conclusion The level of NGAL and β2‐MG in DN patients is significantly increased ,NGAL ,β2‐MG levels are significantly correlated with BUN .
2.The effect of ulinastatin on the improvement of the renal function and oxygen metabolism in patients with craniocerebral injury

Objective To investigate the effect of ulinastatin on renal function and oxygen metabolism in brain injury patients . Methods 50patientswithcraniocerebralinjuryinthehospitalwereenrolledinthestudy,andwererandomlydividedintocontrol group(received conventional craniocerebral injury treatment ,25 cases) and observation group(received conventional craniocerebral injury treatment and ulinastatin treatment ,25 cases) .Then the renal function and oxygen metabolism related indicators of the two groups before the treatment and at third ,seventh and fourteenth day after the treatment were compared .Results The renal function and oxygen metabolism related indicators of observation group at third ,seventh and fourteenth day after the treatment were all ob‐viously better than those of control group ,the differences of the two groups after the treatment were significant(P<0 .05) .Conclu‐sion The effect of ulinastatin on the renal function and oxygen metabolism of patients with craniocerebral injury are great ,and it has positive clinical significance for the improvement of patients′body function state .
3.Missed diagnosis in 1 case of esophageal foreign body of button battery in infant: a case report and review of the literature.

To be on the alert on infants with esophageal foreign body, and to pay more attention to the button battery esophageal foreign body, the clinical data of a 12-month-old infant with button battery esophageal foreign body, which was missed diagnosis for up to 4 months, is analyzed. And the related literature is reviewed. An esophagoscopy was carried out to remove the foreign body. A favorable outcome was achieved. When the infants have unexplained gastrointestinal symptoms, we should consider the possibility of an esophageal foreign body. We should pay attention to the button battery due to its highly corrosive to the esophagus. Timely diagnosis, reasonable operation are the keys to cure.

4.Review of juvenile myopia risk factors

Myopia is the most important cause of visual impairment in adolescents. However, its etiology is complex. In recent years, a large number of epidemiological studies have been done on risk factors of myopia. Most of these studies is cross- sectional study, not longitudinal cohort study. Overall, the incidence of myopia is the result of the interaction between genetic susceptibility and environmental exposure. This review is about the risk factors for myopia.
5.Observation on mind-refreshing and orifice-opening needling method plus swallowing disorder therapeutic apparatus for deglutition disorder of stroke patients in the convalescent stage

Objective:To observe the clinical effect of mind-refreshing and orifice-opening needling method plus swallowing disorder therapeutic apparatus for deglutition disorder of stroke patients in convalescence stage.Methods:A total of 136 patients conforming to the inclusion criteria were randomized into three groups by the random number table,including group A of 46 cases,group B of 44 cases and group C of 46 cases.Patients in group A received swallowing disorder therapeutic apparatus treatment,patients in group B received mind-refreshing and orifice-opening needling method treatment,and patients in group C received mind-refreshing and orifice-opening needling method plus swallowing disorder therapeutic apparatus treatment.The treatment was given once a day for 10 d as a course,the whole treatment lasted for 4 courses.Therapeutic evaluation items including water-swallowing test (WST),standardized swallowing assessment (SSA) and modified Barthel index (MBI) were measured before treatment,after treatment and at follow-up visit (2 months after treatment).Results:After treatment,scores of WST and MBI in all three groups increased significantly (all P<0.05),while the SSA score dropped significantly (all P<0.05).After treatment and during follow-up visit period,score of WST in group C was significantly higher than that in group A and group B (both P<0.05),while the difference between group A and group B showed no statistical significance (P>0.05);the SSA score in group C was substantially lower than that in group A and group B (all P<0.05),the difference between group A and group B showed no statistical significance (P>0.05);the MBI scores in group B and group C were substantially higher than that in group A (all P<0.05),the difference between group B and group C showed no statistical significance (P>0.05).After treatment and during follow-up visit period,the differences in overall therapeutic effect between group A and group B showed no statistical significance (P>0.05),while the overall therapeutic effect in group C was substantially better than that in group A and group B (all P<0.05).Conclusion:Both mind-refreshing and orifice-opening needling method and swallowing disorder therapeutic apparatus can alleviate symptoms of deglutition disorder of stroke patients respectively,and the combination of two methods can improve the therapeutic effect.
6.The classification and prognostic implication of islet β cell function before and in remission period in newly diagnosed diabetic patients with ketosis

ObjectiveTo observe β cell function in newly diagnosed diabetic patients with ketosis before and in remission period and evaluate its classification and predictive value.MethodsA total of 206 patients newly diagnosed as diabetic ketosis who had been treated with intensive insulin therapy in our hospital and entered in the honeymoon after the withdraw of insulin therapy were followed for 36 months from onset of diabetes.They were divided into two groups of type 1 and type2 diabetes ( group A and B),according to the dependence or independence on insulin treatment. The β cell function of the two groups before and in remission period was compared by oral glucose tolerance test (OGTF).β cell function was measured with the AUC of insulin and C-peptide and homeostatic model assessment β-cell function (HOMA-β),while homeostatic model assessment insulin resistant (HOMA-IR) for insulin resistant.The duration of the honeymoon and the change of insulin and C-peptide curve before and in honeymoon were also observed.ResultsThe AUC of insulin and C-peptide,the HOMA-β and the HOMA-IR before and after the intensive insulin treatment were lower in group A than that in group B [ before the insulin treatment:(10.18 ±2.36)mIU · h · L-1 vs (20.28 ±6.89)mIU · h · L-1,(1.56 ±0.53) μg · h · L-1 vs (3.75 ±0.67) μg · h · L-1,3.68 ± 1.08 vs 18.20 ±6.59,1.22 ±0.49 vs 3.06 ± 1.54,respectively;after the insulin treatment:(29.86 ± 8.65 ) mIU · h · L-1 vs (93.35 ± 19.42 ) mIU · h · L-1,( 3.99 ± 0.79 )μg · h · L-1 vs ( 12.54 ±3.83) μg · h · L-1,8.50 ±2.46 vs 56.17 ± 19.42,0.63 ±0.56 vs 1.42 ±0.78,respectively ].The duration of the honeymoon in group A was significantly shorter than in group B [ (7.9 ±5.2) months vs (20.9 ± 9.9 ) months ].In oral glucose insulin and C-peptide release test,the peak of insulin and C-peptide releasing curve in group A was brought forward by a half to 1 hour after intensive treatment while delayed in group B by 1 or 2 hours.The releasing peak of insulin and C-peptide in group A was less than two folds of the basic value,while four to ten fold of the basic value in group B.The positive ratio of glutamic acid decarboxylase antibody,insulin autoantibody and insular cellular antibody in group A and group B were 21.2% vs 4.8%,18.1% vs 3.3%,9.2% vs 10.6%,respectively.ConclusionsOf all the patients newly diagnosed as diabetes ketosis who had entered into the honeymoon after intensive insulin therapy,91% were type 2 diabetes.Inferior β cell function before insulin therapy,weaker remission after insulin therapy and shorter duration of remission period suggest the classification of type 1 diabetes.
7.Preclinical study of bladder cancer diagnosis using tumor homing peptide fluorescent molecular probe

Objective To evaluate the features of optical molecular imaging of bladder tumor cells labeled by tumor homing peptide fluorescent molecular probe, and to explore the theoretical foundation of optical molecular imaging for bladder cancer diagnosis.Methods After prepared the FITC-CSNRDARRC fluorescent molecular probe, laser scanning confocal microscope, immuno fluorescence and multispectral fluorescence in vivo optical molecular imaging system have been used to evaluate the binding sites, the affecting factors of binding rates, the specificity and the targets.BIU-87 bladder tumor cell line, BIU-87 bladder tumor cell line, 68 cases of paraffin bladder tumor tissue samples, 16 cases of paraffin glandular bladder inflammatory samples, 43 cases of paraffin renal clear cell carcinoma samples, 68 cases of paraffin gastric adenocarcinoma samples, 29 cases of urine exfoliated cells suspected bladder cancer and BIU-87 bladder cancer nude xenograft have been used in this study.Results The binding site of FITC-CSNRDARRC fluorescent molecular probe were at the nucleus of labeled bladder tumor cells.The binding rates were correlated linearly with the dose of probe and the grade of pathology.The in vitro and in vivo studies demonstrate that the probe has a binding specificity with bladder tumor. When the FITC-CSNRDARRC fluorescent molecular probe labeled tumor cells, bright green spots were observed under laser scanning confocal microscope.The bright green spots were more apparent after stained by DAPI again.The tissue samples and tumor cells in the urine can be successful labeled and identified by fluorescence microscope.Optical molecular imaging of in vivo xenograft tumor tissues showed fluorescent spots under EMCCD.Conclusions The labeled loci of single cell by FITC-CSNRDARRC probe have been identified. The spatial resolution of optical molecular image is related to sensitivity of CCD, and the optical molecular imaging cannot be imaged by the conventional endoscope camera.
8.Establishment of a Minipig Model of Ischemic Heart Failure with Acute Myocardial Infarction by Coronary Balloon Occlusion and Injection of Intermixture of Microthrombi and Plastic Microspheres

Objective To evaluate the method of establishment of a minipig model of ischemic heart failure(HF) with acute myocardial infarction(AMI) by coronary balloon occlusion and coadministration of injecting of microthrombi and plastic microspheres.Methods A total of eighteen minipigs were selected.After coronary angiography,angioplasty balloons were placed in the mid-distal of left anterior descending(LAD).The balloon was inflated intermittently to occlude the LAD 3 times and then to occlude it continuously for 120 minutes.After the balloon was taken out,4F Judkins-type angiogrphic catheter was superelectively engaged in LAD and 3 mL intermixture of mierothrombi and plastic microspheres were injected at 10 minites interval until TIMI myocardial perfusion was grade<2 and left ventfieular end-diastolic pressure was maintained from 15 to 18 mmHg.Electrocardiogram(ECG),hemodynamic perameters,ultrasonic cardiogram,cTnI and CK-MB were measured.Myocardial infarction area was evaluated by histopathology.Results Fourteen days later,fifteen minipigs survived and fourteen satisfied the criteria(pulmonary capillary wedge pressure.PCWP>18 mmHg and eardio output (CO) decreased beyond 30% ). The changes of ECG, hemodynamic perameters, CKMB, cTnI and cardiac pathologic examination were in accordance with AMI. Conclusion A stable experimental method of establishment of minipig model of ischemic heart failure (HF) with acute myocardial infarction (AMI) by coronary balloon occlusion and coadministration of injecting of microthrombi and plastic mierospheres is succeded. This method has advantages such as closed chest, higher success rate and stability compared with the drug induced, taehycardia-pacing induced, coronary artery ligation induced or microsphere injection alone methods.
9.Study of correlation between serum levels of soluble intercellular adhesion molecule-1 and 99Tcm-MDP scintigraphy in patients with rheumatoid arthritis

Objective To study serum levels of soluble intercellular adhesion molecule-1 (sICAM-1) and its correlation with accumulation degree of 99Tcm-MDP on bone and arthritis and to study the role of sICAM-1 in the diagnosis of patients with early RA. Methods Serum levels of sICAM-1 were measured by immunoenzymetic assay (ELISA). 99mTc-MDP scintigraphy was carried out for the same patients. The significant test between serum levels of sICAM-1 of three groups was performed with analysis of variance; Correlations between serum levels of sICAM-1 and index of semiquatitive 99Tcm-MDP were obtained using sperman's ranks correlation method. Results Compared with control group,serum levels of sICAM-1 were significantly higher in standard group (group A) (P<0.01) and non-standard group (group B) (P<0.05). No significant difference was found between group A and B(P>0.05). Serum levels of sICAM-1 were positively correlated with 99Tcm-MDP scintigraphy. Most patients of non-standard group and standard group showed visible medium-high uptake of 99Tcm-MDP. While control group had not obviously uptake of 99Tcm-MDP. Conclusion Serum levels of sICAM-1, semiquatitive 99Tcm-MDP scintigraphy, accumulation degree of radiopharmuceuutial on bone and arthritis are helpful for diagnosis of early rheumatoid arthritis and judge-ment of the clinical status and improvement of the prognosis.
10.Assessment of DNA damage in radiation workers by using single cell gel electrophoresis

objective To assess the DNA damage of radiation workers in different grade hospitals,and to explore the correlation between the types of work or work time and the levels of DNA damage.Methods DNA single strand break were detected by using alkaline single cell gel electrophoresis(SCGE),and the comet was analyzed with CASP(Comet Assay Software Project).TDNA,TL,TM and OTM were calculated.Results The parameters of SCGE in the radiation greup were higher than those of control group(F=3.93,P<0.01).The significant difference was found not only among the different types of work or difierent work time,but also among the different grade hospitals(F=1.83,1.9 1,P<0.05).Conclusions Various levels of DNA damage could be detected in the radiation workers of the two hospitals.DNA damage of radiation workers is less serious in the higher-grade hospital than the lower grade one.Different types of work or work time might affect the DNA damage level.
