2.Intelligent optimization of the structure of the large section highway tunnel based on improved immune genetic algorithm

As in the building of deep buried long tunnels, there are complicated conditions such as great deformation, high stress, multi-variables, high non-linearity and so on, the algorithm for structure optimization and its application in tunnel engineering are still in the starting stage. Along with the rapid development of highways across the country, It has become a very urgent task to be tackled to carry out the optimization design of the structure of the section of the tunnel to lessen excavation workload and to reinforce the support. Artificial intelligence demonstrates an extremely strong capability of identifying, expressing and disposing such kind of multiple variables and complicated non- linear relations. In this paper, a comprehensive consideration of the strategy of the selection and updating of the concentration and adaptability of the immune algorithm is made to replace the selection mode in the original genetic algorithm which depends simply on the adaptability value. Such an algorithm has the advantages of both the immune algorithm and the genetic algorithm, thus serving the purpose of not only enhancing the individual adaptability but maintaining the individual diversity as well. By use of the identifying function of the antigen memory, the global search capability of the immune genetic algorithm is raised, thereby avoiding the occurrence of the premature phenomenon. By optimizing the structure of the section of the Huayuan tunnel, the current excavation area and support design are adjusted. A conclusion with applicable value is arrived at. At a higher computational speed and a higher efficiency, the current method is verified to have advantages in the optimization computation of the tunnel project. This also suggests that the application of the immune genetic algorithm has a practical significance to the stability assessment and informationlzation design of the wall rock of the tunnel.

4.Role of microRNA-1 on fibrosis induced by high glucose cultured cardiac fibroblasts

Objective To investigate the effect of microRNA-1 (miR-1) on the fibrosis of cardiac fibroblasts induced by high glucose. Methods The primary cultured fibroblasts from 1-3 days old Sprague-Dawley (SD) rats, were randomly divided into 4 groups (n = 3): normal glucose + lentivector-vehicle (CON+Lv-Vehicle group), normal glucose + lentivector-miR-1 (CON+Lv-miR1 group), high glucose + lentivector-vehicle (HG+Lv-Vehicle group), high glucose + lentivector-miR-1 (HG+Lv-miR1 group). Fibroblasts were cultured in glucose concentration 5.5 mmol/L and 25 mmol/L DMEM culture, and were injected lentiviral vector carrying miR-1 silencer sequence or the same volume of lentiviral vector. After 12 hours, the medium was replaced with fresh complete medium. After 3 days when transfection efficiency was up to 90%, the cellular miR-1 content was detected by real-time reverse transcription-polymerase chain reaction (qRT-PCR). The secretion of collagen Ⅰ and Ⅲ were measured by enzyme linked immunosorbent assay (ELISA). Expression of collagen Ⅰ and Ⅲ, matrix metalloproteinase 2 and 9 (MMP-2, MMP-9), autophagy related protein LC3B-Ⅱ, P62/SQSTM1 and Cathepsin D were assessed by Western Blot. Results Compared with the CON+Lv-Vehicle group, the content of miRNA in the CON+Lv-miR1 group had no statistical significance. Compared with the CON+Lv-Vehicle group, high glucose increased the amount of miR-1 (2-ΔΔCt: 1.82±0.17 vs. 1.00±0.04), collagen Ⅰ and Ⅲ secretion (mg/L: 14.55±0.33 vs. 7.28±0.22, 157.50±13.22 vs. 61.25±8.54) and expression (gray value: 432.35±56.00 vs. 100.00±15.00, 320.35±47.00 vs. 100.00±15.00), the level of MMP-2, MMP-9 and the expression of autophagy related protein LC3B-Ⅱ and P62/SQSTM1 (gray value: 249.0±21.0 vs. 100.0±15.0, 142.3±20.0 vs. 100.0±16.0, 178±19 vs. 100±14, 378.3±20.0 vs. 100.0±15.0), decreased the expression of lysosomal associated protein Cathepsin D (gray value: 60±14 vs. 100±10), and the differences were statistically significant (all P < 0.01). Compared with the HG+Lv-Vehicle group, the amount of miR-1 in the HG+Lv-miR1 group was significantly decreased (2-ΔΔCt: 1.21±0.10 vs. 1.82±0.17), collagen Ⅰ and Ⅲ secretion (mg/L: 10.68±0.54 vs. 14.55±0.33, 87.25±13.55 vs. 157.50±13.22) and expression (gray value: 179.41±45.00 vs. 432.35±56.00, 173.41±50.00 vs. 320.35±47.00), the level of MMP-2, MMP-9 and the expression of autophagy related protein LC3B-Ⅱ and P62/SQSTM1 (gray value: 172.0±23.0 vs. 249.0±21.0, 90.0±17.0 vs. 142.3±20.0, 138±15 vs. 178±19, 265.0±17.0 vs. 378.3±20.0) in the HG+Lv-miR1 group were decreased and the expression of lysosomal associated protein Cathepsin D was higher (gray value: 110±17 vs. 60±14), and the differences were statistically significant (all P < 0.05). Conclusions The expression of miRNA-1 was up-regulated in cardiac fibroblasts cultured in high glucose, and miRNA-1 silencing inhibited cardiac fibroblast induced fibrosis in high glucose. The mechanism may be related to the recovery of autophagy flux, up-regulation of Cathepsin D expression and inhibition of collagen production.
5.Mason surgical treatment of recurrent rectovaginal fistula in 13 cases

Objective To investigate the surgical strategy of recurrent rectuvaginal fistula. Methods Retrospectivly analyzed the clinical data of 13 patients with recurrent rectovaginal fistula from December 2001 to December 2008. The etiopathogenisis and the ways of treatment were analyzed. Results All the 13 patients with a transverse colostomy surgical repaired with Mason operation in the secondary intention,and recepted closure of colostomy in the third intention for treatment, all patients were cured. No recurrent fistula was identified with postoperative follow-up 4 - 84 months. Conclusions After recurrent rectovaginal fistula with multiple surgical treatment,the blood supply of local tissue is poor,the scar is serious. Selecting the appropriate timing of operation, adequate preoperative preparation, the application of transverse colostomy and Mason operation in different period could significantly enhance the successful operation rate.
6.The endoscopic features of gastric low-grade intraepithelial neoplasia with carcinomatous transformation

Objective This study aimed to evaluate the potential predictive factors for carcinomatous transformation of gastric low-grade intraepithelial neoplasia (LGIEN) on the basis of endoscopic features.Methods The study involved 312 gastric LGIENs that were histologically confirmed by endoscopic forceps biopsy (EFB) between May 2004 and September 2010,and then removed by endoscopic mucosal resection or endoscopic submucosal dissection were enrolled in this study.According to EMR/ESD postoperative pathological findings,they were divided into LGIEN group and the HGIEN group,and compared their endoscopic characteristics.Results There were not significant different between the two group,as the mean age、gender the diameter of the lesions、lesion sites and surface nodularity.The diameter of the lesions was 14.6 ± 8.2 mm in the LGIEN group and 22.0 ± 0.55 mm in the HGIEN group (P < 0.05).42 of 69 gastric adenomas (60.9%)larger than 20 mm in diameter showed HGIEN (P < 0.05).Hyperemia and mucosal ulceration were significant differences in the two groups.Conclusion The LGIEN lesions with these endoscopic characteristics,such as size (> 2 cm),surface hyperemia,ulceration should be considered for EMR/ESD.
7.Effect of early-stage esophageal squamous cell carcinoma treated with external beam radiotherapy and 252Cf neutron brachytherapy

Objective To observe and analyze the long-term curative effects and complications for early stage thoracic esophageal cancer patients treated with external beam radiotherapy (EBRT) and 252Cf neutron brachytherapy (NBT).Methods From May 2002 to May 2012,26 patients with early stage squamous cell carcinoma who underwent EBRT and 252Cf NBT were respectively analyzed.Patients were treated 5 days per week at 2 Gy/day for a total dose of 50 Gy with EBRT.The total radiation dose to the reference point was 12-16 Gy-eq in 3-4 fractions with 4 Gy-eq/fraction with 252Cf NBT.The 1-,3-and 5-year follow-up rates were all 100 %.Results The 1-,3-and 5-year survival rates were 95.5 %,95.5 % and 83.5 %,respectively.The early complication rates for grades 1 and 2 radiation esophagitis were 76.9 % (20/26) and 23.1% (6/26),respectively.The late complication rates for grades 0 and 1 (according to the RTOG/EORTC standard) were 84.6 % (22/26) and 15.4 % (4/26),respectively.Barium esophagography after treatments confirmed that the complete response rate was 100 %.Twenty-two patients were confirmed by endoscopy to have either normal mucosa or inflammation change.Conclusion EBRT combined with 252Cf NBT is an effective and safe treatment for early stage esophageal squamous cell carcinoma.
9.Correlation between 18F-FDG PET/CT SUV and immunohistochemical expression of tumor associated molecular markers in non-small-cell lung cancer

Objective To analyze the correlation between 18F-FDG SUV and immunohistochemical index including GLUT1, Ki-67, MVD, survivin and cyclinA in non-snall-cell lung cancer. Methods Thirty-three patients with NSCLC underwent preoperative PET/CT examination and surgical operation. All patients were divided into two groups according to the size of tumor (cutoff=3 cm), metastasis of mediastinal or hilar lymph nodes or not, and histological types of the cancer, respectively. The expression of GLUT1, Ki-67, MVD, survivin and cyclinA were estimated with SP immunohistochemical technique, and were analyzed statistically to reveal the correlation to FDG SUV. Results The rate of positive expression of GLUT1, Ki-67 and CD34 were 66.67%, 72.73% and 100%, respectively. The mean value of CD34 in all 33 patients was 12.6±2.9 (12-56). The rate of positive expression of survivin was 84.85%, and the corresponding data of cyclinA was 27.27%. Conclusion There is linear correlation between FDG PET SUV and GLUT1, but not between FDG PET SUV and Ki-67, MVD, survivin and cyclinA. The expressions of GLUT1, Ki-67, MVD, survivin and cyclinA are not related with the size of tumor, nor metastasis of lymph nodes. The expression of GLUT1 and Ki-67 is related with histological types of the cancer, but not with MVD, survivin and cyclinA.
