1.Role of hypoxia-inducible factor-1 alpha in retinoblastoma development

?Retinoblastoma is a common eye malignant tumor which has high degree of malignancy.The traditional treatment methods are destructive, and the prognosis is poor. Hypoxia-inducible factor-1αhighly express in retinoblastoma and regulate the development and progression of retinoblastoma ( by influencing the proliferation and differentiation of tumor cells and involving in the generation of vascular mimicry ) . This article reviewed the role of hypoxia inducible factor-1αin the development of retinoblastoma which will initiate a new avenue for the treatment of retinoblastoma.
2.Epidemic characteristics of falls and its related injury among elderly female in Changing district of Shanghai

Objective To study the epidemic characteristics of falls among older female adults.Methods Stratified random sampling was applied in this study.Totally 969 elderly female adults aged 60 years and over from communities of Changning district were recruited and self-designed questionnaire was used in survey.Results Among 969 interviewees,238 (24.6%) had an injury in the past years,and 4.7% had more than one fall.Among those who fell in the past year,84.5% had suffered from different kinds of fall-related injury with injury incidence of 20.7 %,and 32.4 % suffered from fracture with the fracture rate of 7.9%.There was a significant difference in fall incidence between different age groups,and the incidence increased with aging (x2 =30.26,P<0.01).After a fall,71.6% cases needed certain medical service,and 11.8% were hospitalized for treatment.Among those suffered from fall-related injury,the activities of 29.3% patients were decreased,and 3.0% lost the ability to live independently.Most falls (31.2%) occurred at home.Among those,61.8% of patients considered the reason of their falls was the interaction between intrinsic hazards and environmental hazards.Conclusions High incidence and severe consequence of fall appear among elderly female adults.More attention should be paid to this population.
3.Study on treatment of tibial avulsion fractures of posterior cruciateligament combined with anterior cruciate ligament

Objective To explore the clinical efficacy of one-stage cannulated screw fixation in treatment of tibial avulsion fracture at the insertion of posterior cruciate ligament(PCL) under posterior direction version and in treatment of tibial avulsion fracture at the anterior cruciate ligament (ACL)under arthroscopy.Methods From January 2010 to June 2015,25 patients with tibial avulsion fractures of the posterior cruciate ligament combined with the anterior cruciate ligament in our hospital were retrospectively analyzed.Firstly,arthroscopic exploration was conducted in all patients in their horizontal position,aiming to observe whether combined injures existed or not and to confirm fracture fragments and the degree of tibial avulsion fractures of the cruciate ligaments.All patients recieved arthroscopic fragment fixation by using cannulated screws,then,were treated with a minimally posteromedial incision in the prone position by cannulated screw fixation.All knee joints were protected by adjustable orthopedic brace for 4-6 weeks.The clinical knee joint function of patients were measured by using International Knee Documentation Committee (IKDC) scoring scale and Lysholm scoring scale.The X-ray or CT image were used to evaluate healing process of the fractures.Results All patients were followed up for 6-24 months,the median follow-up time was 15 months.All fractures healed in one stage,time of fracture healing ranged from 9 to 24 weeks,and the median time was 12 weeks.The average IKDC scores at the finial follow-up were as follows:claudication (4.88±0.29)points,support (4.65±0.36)points,pain (4.78±0.49)points,swelling (4.88±0.52)points,squat (4.85±0.35)points,all were higher than those before operation,there were statistically significant differences when compared with those assessed before operation (P<0.05).The average Lysholm score at the finial follow-up was (93.4±2.8)points,the rate of excellent and fine was 96.0%,no statistically significant difference was found compared with that at the healthy side (P>0.05).Conclusion The one-stage cannulated screw fixation in treatment of tibial avulsion fracture at the insertion of PCL under posterior direction version and in treatment of tibial avulsion fractures at the ACL under arthroscopy is easy and convenient to operate,which has the advantages of reliable fixation and satisfactory recent curative efficacy.
5.PER-1 Type Extended-spectrum ?-Lactamases Produced by Gram-negative Bacilli

OBJECTIVE To investigate the prevalence of PER-1 type ESBLs-producing Enterobacteriaceae and their antimicrobial-resistance profile. METHODS Totally 167 strains of Escherichia coli, 154 strains of Klebsiella pneumoniae and 67 strains of Enterobacter cloacae were tested. All of them were isolated from all kinds of clinical specimens in four hospitals of Nanchang city from Aug 2003 to Jun 2004. Antimicrobial susceptibility test(AST) was determined by K-B disk diffusion test. double-disk test(CTX, CTX/CA and CAZ, CAZ/CA)and three- dimension extract test were used to determine ESBLs. PER-1 genes were amplified by PCR. The products of PCR were sequenced to identify its ?-lactamase encoding gene. RESULTS Sixteen isolates were PER-1 gene positive. The PER-1 gene positive rates of E. coli, K. pneumoniae and E. cloacae were 3.6%, 3.2% and 9.0% , respectively . The antimicrobial-resistant rates of isolates harboring blaPER-1 to cefotaxime, ceftaxidime, amikacin , cefoxitin , cefepime and imipenem were 100%,100%, 56%, 94%, 31% and 0%, respectively. CONCLUSIONS blaPER-1 is detected in E. coli, K. pneumoniae and E. cloacace, and has diffused in Enterobacteriaceae . Imipenem and amikacin may be used to treat the infection caused by bacteria harboring blaPER -1.

7.Clinical effects of internal fixation with Herbert screws for the treatment of Pipkin femoral head fractures.

OBJECTIVETo explore the therapeutic effects of internal fixation with Herbert screws for the treatment of Pipkin type I and type II femoral head fractures.
METHODSFrom January 2008 to December 2012, 23 patients with Pipkin type I and type II femoral head fractures were treated with open reduction and internal fixation by Herbert screws through Kocher-Langenbeck approach. Twenty-three patients (aged 18 to 52 years with a mean of 35.5 years, including 18 males and 5 females patients, 8 left hips and 15 right hips) with femoral head fractures and posterior hip dislocation. The fracture was classified according to Pipkin classification based on the radiographic findings, 5 patients had type I and 18 had type II fractures. The duration time from admission to the operation ranged from 6 to 72 h (averaged 32 h). The clinical and radiographic outcomes of the patients were measured using Thompson-Epstein scoring scale. The Harris hip score(HHS) was used to evaluate and compare hip functions at the latest follow-up between affected and healthy sides.
RESULTSAll the patients were followed up, and the duration ranged from 20 to 48 months (averaged 30 months). According to Thompson-Epstein system, 12 patients got an excellent result, 6 good, 4 fair and 1 poor. The average HHS at the finial follow-up was 87.80 ± 8.46 (ranged from 66 to 95), which is similar to that in the healthy side 90.10 ± 6.35 (ranged from 72 to 98) (t = 1.044, P = 0.302). The complications such as deep infection, and deep vein thrombosis were not found. At the 3rd year during follow-up,4 patients had avascular necrosis of femoral head. At the 1st year of follow-up, 1 patient had hip pain after walking,screws loosening and shift after trauma,and serious complications of traumatic arthritis. All the 5 patients were treated with total hip arthroplasties.
CONCLUSIONThe treatment of internal fixation with Herbert screws through the Kocher-Langenbeck approach is effective for Pipkin type I and type II femoral head fracture. The method is reliable and valuable for recommendation. However, such fracture may have avascular necrosis of femoral head and complication of traumatic arthritis, which should be observed carefully in clinic with preparation of the prevention and treatment measures.

8.Role of Rho-kinase in remote ischemicpostconditioning against myocardial ischemia/reperfusion injury

Aim To explore the role of Rho-kinase in remote ischemic postcondi-tioning and its possible mechanism.Methods Thirty male Sprague-Dawley rats were divided into five groups(n=6): sham group(Sham), ischemia/reperfusion group(I/R), remote ischemic postconditioning group(RIPostC), I/R with Rho-kinase inhibitor fasudil group(I/R+Fas) and RIPostC with Rho-kinase activator lysophosphatidic acid group(RIPostC+LPA).Throughout the whole process of experiment, mean arterial pressure(MAP), heart rate(HR) and Ⅱ lead electrocardiogram were continuously monitored.At the end of the reperfusion, plasma creatine kinase(CK) and lactate dehydrogenase(LDH) were measured.Myocardial histopathologic changes were observed by hematoxylin and eosin(HE) staining.Infarct size was measured using 2,3,5-triphenyltetrazolium chloride(TTC) staining.The expressions of phospho-myosin light chain(p-MLC) were detected with Western blot analysis.Results Compared with Sham group, the MAP and HR of other groups decreased, while the amplitude of ST segment increased.Compared with I/R group, MAP and HR increased, the amplitude of ST segment decreased, plasma CK and LDH activity decreased, myocardial pathological morphology and infarct size were improved significantly, infiltration of inflammatory cells was reduced, and the expression of p-MLC decreased in RIPostC and I/R+Fas group.Compared with RIPostC group, RIPostC+LPA group attenuated the effects of RIPostC, and the recovery of the above indicators were inhibited.Conclusion Rho-kinase signaling pathway might mediate remote ischemia postconditioning against myocardial ischemia/reperfusion injury.
9.Effect of zhimu and huangqi on cardiac hypertrophy and response to stimulation in mice.

OBJECTIVETo observe the effect of Zhimu and Huangqi on increasing heart rate and cardiac hypertrophy, and its bradycardia effect, to evaluate the exercise capacity and response to stimulation and to discuss the use of Zhimu in treating heart failure.
METHOD1. Rats were treated with isoproterenol 1.25 micrograms.kg-1 i.p., and then the increasing of heart rate and the bradycardia effect were measured after administrated with Zhimu or Huangqi for 3 days. 2. Mice cardiac hypertrophy model was established by s.c. ISO 1 mg.kg-1, 2 times per day for 14 days and heart-weight-index was measured after Zhimu and Huangqi was given orally alone or jointly for 14 days. 3. Abdominal aorta banding operation was done in mice and 3 weeks after operation, they were administrated for 2 weeks, and then run-time(exercise capacity), quiet heart rate, ISO response heart rate after i.p. isoproterenol and heart-weight-index were measured. 4. Cardiac hypertrophy model (inject ISO 14 days) mice were administrated for 12 days, and the mortality and dying time of mice in cold (-20 degrees C) and heat(45 degrees C) stimulative condition were observed.
RESULTZhimu could cut down the increasing of heart rate induced by ISO, decrease significantly heart-weight-index in cardiac hypertrophy mice, reduce the quiet heart rate and prolong the run time in abdominal banding model. Zhimu combined with Huangqi could also improve the ISO response in abdominal banding model mice, reduce the mortality and delay dying time of mice in stimulative condition.
CONCLUSIONZhimu combined with Huangqi can slow down heart rate, enhance the reserve force of the heart, and improve the response capacity of cardiac hypertrophy mice in stimulative condition.

10.Study on the effect of zhimu combined huangqi on improving renal hypertension rat's cardiac dysfunction.

OBJECTIVETo observe the effect of Zhimu and Huangqi used singly or combinatively on improving experimental cardiac dysfunction, and mainly to observe zhimu's effect on restraining sympathetic nerve and blocking beta-adrenergic-recepter and huangqi's effect of improving hemodynamics on heart failure.
METHODTwo-clib one kidney operation was done to make renal hypertension rat model, 8 weeks after operation, rats were divided into groups and medicated for 6 weeks, and then their heart rate and blood pressure were measured, left ventricle was cannulated to estimate heart function, and heart-weight-index and left-ventricle-weight-index were measured.
RESULTZhimu could slow rats heart rate obviously, prevent cardiac remodeling, but did not affect cardiac function remarkably; Huangqi could reduce blood pressure, heighten +dp/dtmax and -dp/dtmax remarkably; the combined use of the two drugs could decrease plasma catecholamine concentration, adjust myocardium cAMP content, and improve heart function obviously.
CONCLUSIONZhimu and Huangqi can protect experimental cardiac dysfunction, and the combined use is better than the single use, which shows it better to use the two drugs combinatively in treating heart dysfunction.
