1.Effect of comprehensive medical goal appraisal on hand hygiene compli-ance rate of health care workers

Objective To evaluate the comprehensive medical goal appraisal system on hand hygiene compliance rate of health care workers(HCWs).Methods Comprehensive medical goal appraisal system was adopted to inter-vene hand hygiene compliance rate of HCWs in a comprehensive hospital ,hand hygiene compliance rates of HCWs and consumption of instant hand sanitizer per bed-day before (December 2012)and after intervention (January 2013-June 2014)were compared.Results Hand hygiene compliance rate after intervention was higher than before interven-tion (85.17% [18 208/21 379]vs 39.92%[853/2 137]),hand hygiene compliance rate enhanced by 113.35%(χ2 =2 590. 81,P <0.001).Hand hygiene compliance rates of HCWs of different departments,different occupations and different hand hygiene moments were all higher than before intervention (all P <0.001);after intervention ,hand hygiene compliance rate revealed a increased tendency,and has maintained high since October 2013 (>90%),consumption of instant hand sanitizer before and after intervention was 7.24 mL/bed-day(4 200 L/579 841 bed-day)and 10.54 mL/bed-day(9 323.5L/884 489 bed-day)respectively,the consumption after intervention increased by 45.58% compared with that before intervention. Conclusion Comprehensive medical goal appraisal can effectively enhance hand hygiene compliance rate ,and maintains at a high level;the measure can affect hand hygiene behavior of HCWs by hawthorne effect,and is an effective and long-term measure to improve hand hygiene compliance of HCWs.
2.Influence of astragalus injection on retina of simulated high altitude hypoxia rat

·AIM: To investigate the effects of astragalus injection on the metabolic and pathological changes of the retinal free radical metabolism and pathology in high-altitude hypoxic rats. ·METHODS: Sixty healthy SD rats without any eye disease were selected,and the random grouping method was divided into two groups: astragalus injection group (intervention group) and physiological saline group (control group),each group of 30 rats. In each group,6 were randomly selected for injection of astragalus injection (15mL/kg) and physiological saline (15mL/kg) in the pre-simulated chamber. The rats were sacrificed immediately and removed the eyeballs after the rats were taken out of the simulated module. We observed the changes of retinal morphology with the HE staining method, and determine the retinal SOD and MDA content with colorimetry. · RESULTS: HE staining showed no morphological changes in the two groups of retinas at 2h, and as the time of hypoxia was prolonged, the retinal edema gradually increased,but the intervention group was less edema than the control group. The activity of SOD in both groups decreased with the increase of anoxia time of high altitude, and the comparison of different time points in the group was statistically significant(P<0.05). The content of MDA in both groups increased gradually, and the comparison between different time points in the group was statistically significant (P<0.05). The SOD in the two groups of retinas was significantly different at same time point(P<0. 05), except for at 2h without statistical significance. The MDA showed the same situation as SOD. ·CONCLUSION: Astragalus injection can reduce the damage degree of retinopathy in rats under the high altitude hypoxia environment, that the mechanism may be related to free radicals, enhance the activity of SOD, reduce the MDA content of lipid peroxides,and enhance antioxidant capacity.

BACKGROUNDVaspin is a newly-identified adipocytokine related to obesity and insulin sensitivity. However, its pathophysiologic role in humans remains largely unknown. The aim of our study was to investigate the relationship between serum vaspin level and glucose metabolism or obesity in Chinese adults.
METHODSA total of 123 subjects, including 84 subjects with normal glucose tolerance (NGT) and 39 subjects with diabetes, were enrolled in this study. Anthropometric parameters, abdominal fat areas, plasma glucose concentration, serum insulin, lipids, and vaspin level were measured in each participant.
RESULTSSerum vaspin concentration was significantly higher in diabetic patients than that in NGT subjects (592 (438 - 695) pg/ml vs 380 (294 - 517) pg/ml, P = 0.020) in women. In all participants, age, fasting plasma glucose concentration (FPG), 2-hour post-load plasma glucose (PG2h), hemoglobin A1c (HbA1c) and high-density lipoprotein cholesterol (HDL-c) significantly increased from the lower tertile to the higher tertile of vaspin. Univariate linear regression analyses revealed that vaspin level was only positively correlated with age (beta = 0.340, P = 0.002) in NGT subjects. And vaspin was positively associated with FPG (beta = 0.365, P = 0.023), PG2h (beta = 0.526, P = 0.001), HbA1c (beta = 0.388, P = 0.016), and HDL-c (beta = 0.353, P = 0.027), while negatively with homeostasis model assessment of beta cell function (HOMA-beta) (beta = -0.361, P = 0.024) in diabetic patients. In stepwise multivariate regression analyses, age was independently associated with circulating vaspin in NGT subjects, whereas PG2h was an independent predictor of vaspin in diabetic patients. In addition, there was no significant difference of serum vaspin level between men and women. And no significant correlations between vaspin and body fat indexes were detected.
CONCLUSIONSSerum vaspin level is higher in diabetic patients than that in NGT subjects in women. Age predicts serum vaspin level in NGT subjects, while PG2h is independently associated with vaspin in diabetic patients.

Objective To study the effects of high fat and calories diet on pharmacokinetics of melatonin prolonged -release tablet in healthy Chinese subjects.Methods Ten healthy subjects were randomly as-signed to receive single oral dose of melatonin prolonged-release tablet 2 mg after overnight fasting or breakfast by a randomized crossover design. Plasma concentrations of melatonin were measured by HPLC -MS/MS. The pharmacokinetic parameters were calculated, and the effect of the high fat and calories diet on the pharmacokinetics of melatonin was evalu-ated.Results Under fasted and fed conditions, the main pharmacoki-netic parameters of melatonin were as follows:Cmaxwere (1.40 ±0.68), (1.13 ±0.38)ng? mL-1, t1/2 were (2.53 ±1.10), (2.64 ±1.85)h, tmax were (1.28 ±0.85), (2.28 ±1.52)h, AUC0-10 h were (4.69 ±2.21), ( 4.09 ±1.58 ) ng? mL-1 ? h, AUC0-∞ were ( 4.98 ±2.45 ) , (4.32 ±1.64) ng? mL-1? h.Conclusion The absorption of melato-nin was reduced by food intake.
6.Serum vaspin level in relation to postprandial plasma glucose concentration in subjects with diabetes

7.Thyroid nodular diseases in the population indergoing medical examination and the analysis of its relative factors in Dalian City,Liaoning Province

Objective To investigate the characteristics about the prevalence of thyroid nodules detected on color Doppler uhrasonography(US) in people residing in Dalian City who undergo regular physical examinations, as well as its relative factors. Methods All thyroid sonographic and questionair procedures were performed in the 6020 people above 18-year-old living in the four districts of Dalian City for at least 5 years, who were examined at the department of health medical center of Dalian Municipal Central Hospital from May 2006 to March 2007. Urinary iodine concentration was measured in 2039 healthy adults selected by age layers in our study population. Moreover, urinary iodine concentrations were determined in 220 children aged 8-10 years old who were randomly chosen from four communities (55 children per elementary school from each community). The analysis of logistic regression was conducted for the risk factors linked to thyroid nodules. Results The prevalence of thyroid nodules in the 6020 adults was 38.5%(2319/6020), in which nodules sized between 0.3 and up to 0.5 cm were found in 17.1% (1030/6020), and those above 0.5 cm in 21.4% (1289/6020). Ultrsonography revealed solitary nodules in 17.2% (1036/6020), multinodular goiter in 21.3% (1283/6020). Fifty-four point nine percent (1272/2319) thyroid nodules showed solid internal echographic structures, 30.2%(701/2319) mixed and 14.9%(346/2319). The thyroid nodule detected rate in female individuals was 46.1% (1102/2393), among whom multinodular goiter [59.1% (651/1102)] was more than solitary nodules[40.9(451/1102)] in female; while only 33.6%(1217/3627) of male were detected to have thyroid nodule, there was a difference between the genders (χ2=95,079,P<0.01). The mediam urinary iodine concentration(MUI) was 184.32 μg/L in children and 216.75 μg/L in the health adults, moreover, it was 216.55 μg/L and 217.00 μg/L in the people with thyroid nodules and those without nodules without a significant difference (P>0.05). The rate of thyroid nodules gradually increased with age(χ2=344.998, P<0.01). The occurance of thyroid nodules was significant associated with gender and age(P<0.01). Conclusions The nutritional iodine intake in the four communities of Dalian City are adequate. The prevalence of thyroid incidentalomas is relatively high in this group of people receiving medical examination.
8.Improving rational use of antimicrobial agents by infection prevention and control

Objective To evaluate the efficacy of infection prevention and control measures on the management of rational use of antimicrobial agents.Methods Patients who were admitted in a hospital from 2011 to 2015 were as the research object,a series of infection prevention and control intervention measure were taken,efficacy of intervention measures were evaluated.Results After the implementation of comprehensive intervention measures,compliance rate of hand hygiene increased year by year,from 38.17 % in 2011 to 87.16 % in 2015,difference was statistically significant (x2 =48.50,P<0.05).Incidence of healthcare-associated infection dropped from 1.45% to 1.06%,difference was statistically significant (x2 =42.50,P<0.05);antimicrobial use density in 2011-2015 were 63.1,44.4,40.0,40.8,and 40.5 respectively,which showed a decreasing tendency.Conclusion Effective infection prevention and control measures have obvious effect on promoting management of rational use of antimicrobial agents,it is helpful for reducing the clinical use density of antimicrobial agents.
9.Effects of long-term intervention of moxa smoke on T lymphocyte subsets and CD4+ CD25+ Treg in peripheral blood of Wistar rats.

OBJECTIVETo investigate the cellular immune regulation of the long-term intervention of moxa smoke.
METHODSThirty-two Wistar rats were randomly divided into a blank group, a low concentration group, a medium concentration group and a high concentration group, 8 cases in each group. In addition to the blank group, rats in the other groups were exposed to the corresponding concentration moxa smoke for 20 min every day, the T lymphocyte subsets and proportion of the CD4+ CD25+ Treg in CD4+ T cells in peripheral blood were tested by flow cytometry after 6 months.
RESULTSCompared with the blank group, the proportions of CD3+ CD4+, CD3+ CD8+ T cells and CD3+ CD4/CD3+ CD8+ in the other 3 moxa smoke groups were not significantly different (P > 0.05), while the proportions of the CD4+ CD25+ Treg in CD4+ T cells were significantly lower (P < 0.05), but no statistically significant differences among those 3 moxa smoke intervention groups (P > 0.05).
CONCLUSIONLong-term moxa smoke intervention has no significant effect on the proportions of CD3+ CD4+, CD3+ CD8+ T cells and CD3+ CD4+/CD3+ CD8+, but it can decrease the proportions of the CD4+ CD25+ Treg in CD4+ T cells in peripheral blood of rats. The way produced by pretreatment with moxa smoke may play immunomodulatory effect.

10.A single institution, retrospective study of treatment experience in primary mediastinal germ cell tumors: elucidating the significance of systemic chemotherapy.

BACKGROUNDPrimary malignant germ cell tumors (GCTs) of mediastinum are rare neoplasms. We introduce our institutional experience in managing patients with primary malignant GCTs of the mediastinum, focusing on the analysis of therapeutic modalities.
METHODSA retrospective review was done in 39 consecutive patients with mediastinal malignant GCTs treated in our institution between 1991 and 2007.
RESULTSA total of 39 patients were enrolled in this study with a median age of 27 years. The 5-year overall survival (OS) and progression-free survival (PFS) rates of the whole population were 60.2% and 57.7%, respectively. Stratified by the histology, 18 patients (46.2%) had seminoma and 21 patients (53.8%) had nonseminomatous germ cell tumors (NSGCTs). The 5-year OS rate of patients with seminoma was 87.4% as compared with 36.7% in patients with NSGCTs (P = 0.0004). The 5-year PFS rate was also significantly higher in seminoma patients (87.4% vs. 31.6%, P = 0.003). For 19 patients with NSGCTs managed with multi-modality treatment, chemotherapy exposure appeared to impact the prognosis. The 5-year OS rate was 44.9% in patients with chemotherapy exposure as compared with 20.0% in patients without it (P = 0.43).
CONCLUSIONOur study confirmed the significance of systemic chemotherapy in the treatment of primary mediastinal GCTs.
