1.Effects of Polyglycosides of Tripterygium Wilfordii Hook on Inflammation and Immune Suppression

Purpose To study the anti-inflammatory,anti-allergy and immunosuppressive effect of polyglycosides of tripterygium wilfordii Hook (TⅡ). Methods Animal model of acute and subacute inflammation,adjuvant arthritis,type Ⅳ and Ⅲ allergic reaction were used.Serum haemolysin level,lymphocyte proliferation and T-cell subsets in spleen were tested. Results TⅡ 60,120 mg/kg and pre(prednisone)10 mg/kg had marked inhibitory effect on carrageenine induced swelling,tampon induced granulation and on the prevention of adjuvant arthritis.The delayed type hypersensitivity reaction(type Ⅳ) and arthus reaction (type Ⅲ) was distinctly suppressed.It had a suppressive action on serum haemolysin and lymphocyte proliferation with and without concanavatin A which was dose dependent.In the T-cell subsets:TⅡ 60 mg/kg decreased TS and TⅡ 120 mg/kg inhibited both TS and TH,preacted mainly on TH and rendered the decreased ratio of TH/TS. Conclusions TⅡ has suppressive effect on inflammation,allergic reaction and immune function.
2.Role of Let-7 family in pathogenesis of Alzheimer disease

Alzheimer disease (AD) is a fatal neurodegenerative disorder,and the most common cause of dementia in the elderly.Accumulated evidences in AD research suggest that alterations in the microRNA (miRNA) network could contribute to risk for the disease.As the second discovered miRNA,Let-7 has an important effect on cell proliferation,differentiation,apoptosis,immune response,tumorigenesis,metastasis,and so on.We review in this article the role of Let-7 family in the pathogenesis of AD to provide a strong basis for future research aimed at understanding the potential contribution of miRNAs to AD pathophysiology.
3.Simultaneous Determination of the Contents of Geniposide and Liquiritin in Xinshenning Tablets by HPLC

OBJECTIVE:To establish a method for simultaneous determination of the contents of geniposide and liquiritin in Xinshenning tablets. METHODS:HPLC was performed on the column of Inertsil ODS-3 with mobile phase of acetonitrile-water (gradient elution) at flow rate of 1.0 ml/min,the column temperature was 25 ℃,detection wavelength was 238 nm and the vol-ume was 10 μl. RESULTS:The mass concentration range was 16.3-407.9 μg/ml for geniposide(r=0.999 9) and 7.4-185.9 μg/ml for liquiritin(r=0.999 9);RSDs of precision,stability and reproducibility tests were no more than 1.63%;average recoveries were 99.05%(RSD=1.28%,n=9)and 98.00%(RSD=1.84%,n=9),respectively. CONCLUSIONS:The established method is accu-rate,sensitive and simple,and can be used for the contents determination of geniposide and liquiritin in Xinshenning tablets.
4.Clinical observation of sevoflurane for anesthetic induction in elderly patients

Objective To observe the onset time,cardiovascular system reactions and side effects of anesthetic introduction with sevoflurane and to explore its reliability and safety in elderly patients. Methods Sixty American Society Anesthesiologists(ASA)Ⅰ~Ⅲ patients aged 65~78 years who were scheduled for selective operation were selected in the study.They were randomly divided into sevoflurane inhalation induction group(sevoflurane group)and propofol induction group (control group).In sevoflurane group,the concentration of inhaled sevoflurane was regulated according to downgrading method during the induction,and it was decreased by 1% every 30 seconds from the initial concentration of 7% to the maintained concentration of 4%.In control group,propofol was titrated by 1.5~2.5 mg/kg.When bispectral index(BIS)was≤60,sufentanil was administered intravenously and the patients were paralyzed with rocuronium and intubated.The extinction time of lash reflex and the achievement time of BIS≤60 in two groups were recorded.The mean arterial blood pressure(AMBP),heart rate(HR),oxygen saturation(Sp02)and BIS of the basic values before intubation,and 1 min,3 min,5 min after intubation were also recorded,respectively.The side effects during intubation such as laryngeal spasm,body movements,irritating cough,restless movement during intubation were noted.The consciousness during intubation and satisfaction with induction were followed in patients after operation. Results The extinction time of lash reflex and the achievement time of BIS≤60 were significantly longer in sevoflurane group than those in control group (both P<0.01).In sevoflurane group.there were no differences in MBP and HR between 1 min postintubation and pre-intubation.Whereas,in control group,AMBP and HR were lower before intubation than basic values,and were higher 1 rain post-intubation than pre-intubation(P<0.05).There were irritating cough cases both in sevoflurane group and in control group(5 vs.3),and there was no laryngeal spasm case.All patients were satisfied with the anesthesia and had no consciousness during intubation. Conclusions Sevoflurane is a safe and effective induction agent and has little side effects.It is an ideal choice for elderly patients.
5.Application of streamline liner of pharyngeal airway in airway management of general anesthesia

Objective To evaluate the efficacy of streamline liner of pharyngeal airway ( SLIPA) in airway management of general anesthesia. Methods Sixty ASA Ⅰ -Ⅱ patients undergoing elective breast surgery were randomized into 2 groups (30 cases each): SLIPA group (group S) and laryngeal mask airway (LMA) classic group (group L). Anesthesia was induced with propofol 2 mg/kg, sufentanil 0.15 μ g/kg and vecurium 0.08 mg/kg. SLIPA or LMA was inserted with digital manipulation. Anesthesia was maintained with 1% - 2% sevofluran and 50% N2O in 50% oxygen. The parameters of controlled ventilation were same in both groups. The attempt times, seal pressure and grade of fiberoptic (FOB ) laryngoscopy were measured. SpO2, PErCO2 and peak airway pressure were monitored every 15 minutes after successful laryngeal mask insertion. The complications such as regurgitation of gastric contents and sore throat were assessed by anesthesiologist after surgery. Surgery time, anesthesia time, extubation time and emergence time were recorded. Results Success rate of LMA insertion in both groups were 100%. The first attempt success rates were 90%( 27/30) in group S and 60%( 18/30) in group L (P< 0.05), the FOB grade 4 were 67%(20/30) in group S and 37% (ll/30)in group L(P< 0.05), the maximum seal pressures were (23.6 ± 4.7)cmH2O (1 cm H2O = 0.098 kPa) in group S and (18.8 ± 4.5) cm H2O in group L (P<0.05). SpO2, PErCO2 and peak airway pressure were within normal ranges. The occurrence of sore throat was similar in group S and group L (7 cases vs 6 cases). No signs of regurgitation were detected. Conclusion The SLIPA proved to be a simple, safe and effective airway instrument with little complications during the course of general anesthesia.
6.NMDA receptors in central medial thalamus participate in propofol-induced unconsciousness

Objective To investigate the role of NMDA receptors in central medial thalamus (CMT) in the unconscious?ness induced by general anesthesia. Methods A total of 60 rat models for microinfusion were assigned into 4 groups (n=15 for each group). After induction with propofol, 10 mmol/L (NMDA10 group), 20 mmol/L (NMDA 20 group) and 40 mmol/L (NMDA40 group) of NMDA and normal saline (group C) with equal volume were microinfused into CMT. The incidence of purposeful movement and recovery time of righting reflex were observed in each group respectively. Infusion sites were local?ized by histological method. Results When the microinfusion site localized within CMT, comparing with group C, the recov?ery time of righting reflex reduced notably in three NMDA groups (P<0.05). The recovery time was significantly shorter in NMDA20 group and NMDA40 group than that of NMDA10 group. The incidence of purposeful movement during propofol an?esthesia was higher in NMDA20 group and NMDA40 group than that of group C (P<0.05). When the microinfusion site lo?calized out of CMT, the recovery time of righting reflex was remarkably longer than that within CMT in three NMDA groups (P<0.05), and there was no significant difference in the incidence of purposeful movement and recovery time between four group (P>0.05). Conclusion Microinfusion of NMDA agonist into CMT reverses propofol anesthesia, indicating that NMDA receptor in CMT may contribute to the propofol-induced unconsciousness.
7.Characteristics analysis of epidemiological and antimicrobial resistance of 589 meliodosis bloodstream infection cases in Hainan

Objective To provide data support for the prevention and control of melioidosis by analyzing epidemiological characteristics of melioidosis bloodstream infection and antibiotic resistance of its pathogen in Hainan Province from 2012 to 2021. Methods Data was collected from Hainan Antimicrobial Resistance Surveillance System, and WHONET 5.6 software was used to merge and analyze data. Results The case numbers of melioidosis bloodstream infection showed an increasing trend year by year from 2012 to 2021. The ratio of male to female patients was 4.6∶1, with 347 cases (58.91%) of patients aged 40-<60, the age group with the highest incidence. The number of cases from July to November were 424 cases (72.0%). Haikou and Sanya reported the largest case number, with 261 and 116 cases respectively. But there were no case reported from central area including Chengmai, Tunchang, Qiongzhong and Baoting. Burkholderia pseudomallei strains were isolated from both blood and other specimens in 105 of the 589 patients, with respiratory tract and wood specimens being the sites with the highest number of strains isolated other than blood; the resistance rates of Burkholderia pseudomallei to five antibiotics showed no obvious trend of change, with the lowest resistance rate to imipenem among the five drugs at 0.6%, followed by ceftazidime at 2.2%. Conclusion During the typhoon season, elderly, middle-aged male in coastal areas should pay attention to avoid or reduce their contact with mud and water. For patients with melioidosis bloodstream infection, imipenem or ceftazidime should be selected as early as possible for initial treatment.
8.Combination of Jiehe pellet and the standardized anti-tuberculosis therapeutic regimen for pulmonary tuberculosis complicating cervical lymph node tuberculosis in the aged

Objective To assess the efficacy of combination of Jiehe pellet and the standardized anti-tuberculosis therapeutic regimen for pulmonary tuberculosis complicating cervical lymph node tuberculosis in the aged. Methods A total of 103 aged patients with pulmonary tuberculosis complicating cervical lymph node tuberculosis were enrolled and randomly allocated to either a standardized anti-tuberculosis therapeutic regimen group (control group with 51 patients) or a standardized anti-tuberculosis therapeutic regimen plus Jiehe pellet group (treatment group with 52 patients). The patients in the control group and the treatment group received the treatment with 2HRZE/4HR and 2HRZE/4HR plus Jiehe pellet for 6 months, respectively. The abscessed lymph nodes were treated by either total excision or incision and drainage after 4 weeks of medicine treatment in both groups. Sputum smear was examined for acid-fast bacilli. The CD8 cells expressing natural killer T cells receptors NKG2A, NKG2D in peripheral blood were detected by flow cytometry. The treatment outcome was measured at the end of treatment. Results The rates of lesion resolution (78.85%vs. 58.82%;χ2=4.439, P<0.05) and cavity closure (62.86% vs. 35.48%;χ2=3.893, P<0.05) in the treatment group were significantly higher than those in the control group. In the end of 2, 4 and 6 months of treatment, cumulative rates of sputum conversion from positive to negative in the treatment group were significantly higher than those in the control group (χ2 were 5.343, 5.067 and 4.118,all P<0.05). The CD8 cells expressing NKG2A after treatment in the treatment group were significantly lower than those before treatment in the treatment group (t=9.510, P<0.01) and after treatment in the control group (t=9.832, P<0.01);the CD8 cells expressing NKG2D after treatment in the treatment group were significantly higher than those before treatment in the treatment group (t=10.622, P<0.01) and after treatment in the control group (t=10.433, P<0.01). The serum levels of IL-6 and TNF-αafter treatment were significantly lower than those before treatment in both groups (t were 17.344 and 21.142 in the treatment group, 10.984 and 12.203 in the control group;all P<0.01 );the serum levels of IL-6 and TNF-α after treatment in the treatment group were significantly lower than those after treatment in the control group (t were 7.832 and 5.478,all P<0.01). The serum IL-10 levels after treatment were significantly higher than those before treatment in both groups (t were 12.454 in the treatment group, 7.934 in the control group; all P<0.01 ); and the serum IL-10 level after treatment in the treatment group was significantly higher than that after treatment in the control group (t=4.720, P<0.01). The effective rate for cervical lymph node tuberculosis in the treatment was significantly higher than that in the control group (88.5%vs. 64.7%;χ2=6.855, P<0.01). Conclusion Combination of Jiehe pellet and the standardized anti-tuberculosis therapeutic regimen may improve immune function, increase the rate of sputum conversion from positive to negative, and facilitate lesion resolution in aged patients with pulmonary tuberculosis complicating cervical lymph node tuberculosis.
9.Measuring the volume of temporal lobe in healthy Chinese adults of the Hart nationality on the high-resolution MRI

Objective To explore the morphological features of temporal lobe of healthy Chinese Han adults on the high-resolution MRI and provide morphological data of temporal lobe for the construction of database for Chinese Standard Brain.Methods This is a clinical multi-center study.Three hundred healthy Chinese volunteers (male 150, and female 150) recruited from 15 hospitals were divided equally into five groups according to their age, i.e., 18-30 ( Group A ), 31-40 ( Group B ), 41-50 ( Group C ), 51-60(Group D), 61-70( Group E).All subjects were scanned using T1WI 3D MPRAGE sequence and volumes of standardized temporal lobe were collected.The bilateral volumes of standardized temporal lobe were compared by variance analysis between male and female subjects and among five age groups.Results The mean volumes of left and right temporal lobe were (97 126±15 703) mm3 and (97 015±15 545) mm3 respectively for men, and (95 123±14 564) mm3 and (96 423±13 407) mm3 for women.The difference temporal lobe volume between male and female wasn't significant on the same side (F = 1.336, 0.127 ,P =0.249, 0.722).The left temporal lobe volumes of Group A-E were (93 873±13 351 ), (95 566±11 964), (10 1890±14 511 ), (93 972±14 050) and (95 636±19 864)mm3 respectively, and those on the right side were (93 409±10 984), (98 158±16 392), (102 079±15 112), (95 448±11 123 ) and (94 658±16 928) mm3.There were significant differences among 5 groups between left and right temporal lobe volume( F = 2.940, 3.514, P = 0.021, 0.008 ).Further pairwise comparison revealed that left and right temporal lobe volume in Group C is higher than those of Group A and D(P<0.05), while there were no significant differences between the other groups ( P > 0.05 ).Conclusion High-resolution MRI could offer detailed images and precise morphological data of temporal lobe, which provides morphological data of temporal lobe for the construction of database for Chinese Standard Brain.
10.Risk factors for periventricular-intraventricular hemorrhage in premature infants treated with mechanical ventilation.

OBJECTIVETo study risk factors for periventricular-intraventricular hemorrhage (PVH-IVH) in premature infants treated with mechanical ventilation.
METHODSA total of 205 premature infants who were admitted to the neonatal intensive care unit (NICU) and treated with mechanical ventilation between January 2009 and December 2011 were enrolled. They were classified into PVH-IVH and non-PVH-IVH groups according to the results of head ultrasonography performed at 3 to 7 days after birth. Single factor and multivariate logistic regression analysis were used to identify risk factors for PVH-IVH.
RESULTSSingle factor analysis indicated 9 factors associated with the development of PVH-IVH, including a gestational age of <32 weeks, a birth weight of <1500 g, intrauterine distress, severe asphyxia, vaginal delivery, maternal perinatal infection, premature rupture of membranes (PROM) at ≥8 hours, mechanical ventilation duration of ≥7 days and ventilator-associated pneumonia (VAP) (P<0.05). Multivariate logistic regression analysis showed that a birth weight of <1500 g (OR=2.665), intrauterine distress (OR=2.177), severe asphyxia (OR=5.653), maternal perinatal infection (OR=4.365) and VAP (OR=2.299) were independent risk factors for the development of PVH-IVH (P<0.05).
CONCLUSIONSVery low birth weight, intrauterine distress, severe asphyxia, maternal perinatal infection and VAP are closely associated with an increased risk of PVH-IVH in premature infants treated with mechanical ventilation. These clinical risk factors should be given more attention in the prevention of PVH-IVH.
