1.Factors of prognosis in cervical spondylotic myelopathy: a review.

Cervical spondylotic myelopathy (CSM) is a common cause of spinal cord dysfunction clinical disease. Surgery is the main therapeutic tool for CSM. However, there are obvious differences in clinical functional recovery after operation. For the past few years, the influence factors of prognosis in cervical spondylosis myelopathic has been widely concerned. Age, nerve function, course of desease, imaging findings,surgical method and related factors became the investigative point for prognosis of cervical spondylotic myelopathy. Present viewpoint showed that the older patient, preoperative worse nerve function, longer the course of disease would result in worse outcomes. Imaging examination maybe can indicate the prognosis, but the correlation is unclear. Selection of surgical method and approach should be based on the principles of sufficient decompression, stabilize the alignment of the cervical spine, keeping backward extension of cervical spine, maintain effective decompression, preventing complications. Therefore, the treatment of cervical spondylotic myelopathy should be on the basis of pathogenic condition and imaging examination at early stage and a suitable usrgical procedure should be performed to obtain a better prognosis.

2.A clinical analysis of percutaneous coronary intervention in 65 patients with coronary chronic total occlusions

Objective: To analyze influencing factors of percutaneous coronary intervention (PCI) on therapeutic effect in patients with coronary chronic total occlusions (CTO). Methods: Clinical data, lesion features and PCI therapeutic results of 65 patients with 72 CTO lesions, who received PCI in our hospital from Jan 2010 to Dec 2012, were retrospectively analyzed. Results: PCI success rate of CTO lesion was 91.67% (66/72); compared with patients with CTO occlusion 3~12 months, there was significant decrease in PCI success rate (97.78% vs. 81.48%) in those with CTO occlusion >12 months; compared with patients with occlusion length ≤15mm, there was significant decrease in PCI success rate (97.96% vs. 78.26%) in those with occlusion length >15mm; compared with patients with mouse tail-like broken ends, there was significant decrease in PCI success rate (96.55% vs. 71.43%) in those with knife cut-like broken ends, P<0.05 all; PCI failed in six lesions, in which four because guidewire failed to pass through lesions and two because balloon failed to pass through lesions; incidence rate of complications was 7.69% during PCI, there were no major adverse cardiovascular events during admission in all patients; symptoms relieving rate of angina pectoris was 90.16% after PCI. Conclusion: Success rate of percutaneous coronary intervention is related to lesion features, CTO occlusion duration etc.
3.Resolution of alpha-cyclohexyl mandelic acid enantiomers by enantioselective extraction in separation system containing tartaric esters and beta-cyclodextrin.

AIMTo establish a resolution method for alpha-cyclohexyl mandelic acid enantiomers by enantioselective extraction and to observe the effects of all kinds of tartaric esters, pH, the concentration of D-tartaric esters and beta-cyclodextrin on the enantioselectivity.
METHODSResolution of alpha-cyclohexyl mandelic acid enantiomers by enantioselective extraction with tartaric esters and beta-cyclodextrin has been studied.
RESULTSThe distribution behavior of alpha-cyclohexyl mandelic acid enantiomers in the separation system was studied. The effects of all kinds of tartaric esters, pH, the concentration of D-tartaric esters and beta-cyclodextrin on the enantioselectivity has been examined in the two-phase.
CONCLUSIONResults showed that the complex formed by D-iso-butyl-D-tartaric ester with R enantiomer is stabler than that with S enantiomer. With the increase of pH, the partition coefficient and separation factor decreased. The concentration of beta-cyclodextrin and D-tartaric ester had visible effect on the enantioselectivity.

4.Protective effect of total flavonoids of epimedium on the kidney in experimental diabetic rats.

OBJECTIVETo investigate the influence of total flavonoids of epimedium (TFE) on the streptozocin (STZ)-induced kidney injury in diabetic rats and discuss the possible mechanism.
METHODSDiabetes was produced by a single injection of streptozocin (40 mg/kg, iv) in male SD rats. The rats were randomly divided into three groups (n = 10): control group, model group and TFE group (100 mg/kg, ig). Animals were sacrificed 12 weeks later. The level of blood glucose, blood urea nitrogen (BUN) and creatinine (Cr) as well as the renal index were determined. Detect the specific biochemical of renal tissue: superoxide dismutase (SOD), malondialdehyde (MDA). Use masson staining to observe the morphology of the renal tissue. Immunohistochemistry was employed to determine the protein levels of transforming growth factor-beta1 (TGF-beta1).
RESULTSCompared to control group, the enhancement of blood glucose, renal index, BUN and Cr was found in model group, which was significantly attenuated by treatment with TFE. Meanwhile, elevated MDA level in renal tissue as well as decreased SOD activities in renal tissue were significantly remitted by TFE. Furthermore, TFE decreased the expression of TGF-beta1.
CONCLUSIONTFE can evidently relieve renal damage in rats with diabetic nephropathy induced by STZ, which might be related to antioxidation and modulating the expression of TGF-beta1 protein.

5.Experimental study on inhibition of skin graft contraction with papaverine cream.

OBJECTIVETo evaluate the effect of exteral use of papaverine cream on enhancement of skin graft.
METHODSFive mini-pigs were used for the study. 20 full-thickness wounds were created on the back of each animal. Along the midline of the abdomen, a 40 cm x 2 cm full-thickness skin graft was harvested and the wound sutured.The full-thickness graft was prepared and trimmed to 2 cm x 2 cm of 0.6 mm thickness split-skin grafts, which were transplanted to each wound with tie-over bolsters. The sutures were removed 2 weeks after the operation. Twelve pairs of 100%-survived skin grafts were selected and divided into group A (the left side) and group B (the right side), with each pair locating on the same and opposite position. From the day of suture removal, 2% papaverine cream was used to group A while plain cream was used to group B. The grafts were measured and observed for healing condition at the time of suture removal and the first, second, third, fourth, fifth, and sixth month. The surface area of the graft was measured and the shrinking ratio was calculated. After the animals were killed at the sixth month, samples of the skin grafts were taken and processed with 10% formalin, routine paraffin wax and Hematoxylin-eosin staining. The tissue structure was observed and the results were analyzed statistically.
RESULTSThe grafts in two groups did not have significant differences at the time of suture removal. Observations from the first to the sixth month showed that the two groups had significant differences in skin graft contracture and histological changes. HE stains demonstrated that the skin grafts in group A had less fibroblasts, more microvessels and orderly-arranged collagenous fibers.
CONCLUSIONSExternal use of papaverine cream can inhabit the contraction of skin grafts. It is a safe, effective, simple, and reliable method,which has the advantages of easy delivery,cheapness, less injury and infection,and no side effects.

6.Effects of transcription factor GATA-2 on transcriptive regulation of iASPP gene.

iASPP can prompt the cell proliferation and inhibit the apoptosis of many cells. There are putative binding sites of transcription factor GATA-2 upstream of iASPP transcription start site. GATA-2 plays an important role in the proliferation and differentiation of hematopoietic stem cells (HSC) and progenitors. This study was aimed to explore the role of GATA-2 protein in iASPP gene transcription. Firstly, the expression of iASPP and GATA-2 protein in some leukemia cell lines was detected by Western blot. Second, The expressive vector of pCMV5-GATA2 and the luciferase reporter vectors containing possible binding sites of GATA-2 were constructed and co-transfected into HEK293 and CV-1 cells. Then the luciferase activity was assayed by luminometer. Also, ChIP assays were performed to further confirm the specific binding of GATA-2 to iASPP promoter. The results showed that GATA-2 was overexpressed in most cell lines with high level of iASPP. GATA-2 exhibited a significant effect on luciferase activity of reporter gene iASPP and in a dose-dependant manner. The relative luciferase activity was up-regulated to about two-fold of the empty vector control when the transfection dose of pCMV5-GATA2 plasmid was increased to 100 ng. While the effect was more significant in CV-1 cells and showed a 6.7-fold increase. The ChIP assay demonstrated the in vivo specific binding of GATA-2 to iASPP. The binding sites of GATA2 were located between nt -361 ∼ -334 in upstream of iASPP gene transcription start site. It is concluded that transcription factor GATA-2 can bind with the cis-regulatory region of the iASPP promoter and up-regulate iASPP expression.

7.Preliminary application of the fusion cage of biomimetic n-HA/PA66 composites in anterior cervical intervertebral fusion.

This study was aimed to evaluate the preliminary efficacy and the safety of the fusion cage made of biomimetic nano-hydroxyapatite and polyamide 66 (n-HA/PA66) composites for the structural reconstruction and the restoration of height of vertebral body in the case of cervical spondylosis by anterior surgical procedures. 52 patients with cervical spondylosis, received the therapy by discectomy with or without vertebrae resection and decompression, and the fusion cage of n-HA/PA66 vertebra implant with bone chip, and titanium plate system was fixed. All cases were followed up for 6 to 25 months. All the patients' preoperative symptoms subsided without any serious complication, and no patient complained of lasting soreness. No effusion or flare was found, and no recurrence happened in the follow-up. The preoperative JOA score was 10.4, and post-operative JOA score 15.7. The X-ray films of all cases demonstrated successful fusion with good curvature and height, and there was no sinking or collapse. The stability was satisfactory; the reconstructive height of vertebra was maintained. No complications such as infection and screw broken came into being. The fusion cage of the biomimetic n-HA/PA66 composites can effectively restore the height and structure of vertebra. It may have the potential for use as a satisfactory prosthestic vertebral body replacement.

8.Knowledge levels of chronic obstructive pulmonary disease among physicians in primary care and related factors: a survey in Zhumadian City of Henan Province

Objective:To survey the knowledge levels of chronic obstructive pulmonary disease(COPD) among physicians in primary care and related factors.Methods:A questionnaire survey was conducted during December 2020 to December 2021 among 1 232 primary care doctors in Zhumadian City of Henan Province. The questionnaire included basic information, knowledge of COPD and its management, and the training received. The COPD-related knowledge levels and the influencing factors were analyzed using logistic regression model.Results:A total of 1 232 questionnaires were collected, and 1 221(99.1%) were valid, which were included in the analysis. The survey showed that the COPD-related knowledge level was significantly associated with the educational level of primary care physicians(χ2=9.91, P<0.05); and the Pearson correlation analysis showed that the knowledge level was positively correlated with educational level( r=0.09, P<0.05). In terms of risk factors, causes of acute exacerbations, and treatment and management of COPD, there was a linear relationship between the education level and knowledge level(χ2=13.86, P<0.001;χ2=20.93, P<0.001); the Pearson correlation analysis showed that the knowledge level was correlated with higher educational level( r=0.11, P<0.001; r=0.13, P<0.001). In terms of symptoms and diagnosis, there was no correlation between educational level and awareness level(χ2=1.90, P=0.168). Logistic regression model showed that compared to doctors aged 20-30, those aged 31-40 had a higher COPD-related knowledge level( OR=2.24, 95% CI: 1.16-4.30, P=0.016); compare to the doctors with associate degree, doctors with bachelor degree or above had a higher COPD-related knowledge( OR=1.71, 95% CI:1.24-2.37, P=0.001); compared to doctors without professional title, those with primary or middle-rank professional title had a higher knowledge level( OR=1.65, 95% CI: 1.28-2.13, P<0.001; OR=2.15, 95% CI: 1.46-3.16, P<0.001); compared to doctors with training time<3, those with training time ≥3 had a higher knowledge level( OR=1.15, 95% CI: 1.21-2.61, P=0.021). Conclusion:The COPD-related knowledge levels(especially in knowledge of risk factors and causes of acute exacerbations, and treatment and management of COPD) among doctors in primary care are affected by age, educational background, professional title, and the number of training sessions.
9.Experimental study on two-way application of drugs with neutral property for promoting blood circulation and removing blood stasis on cold and heat blood stasis syndromes II.

OBJECTIVETo further study the characteristics of drugs with neutral property in two-way application and conditioned dominance by observing the action characteristic of 10 traditional Chinese medicines with neutral property in hemorheological indicators of heat stagnation and blood stasis syndrome and cold stagnation and blood stasis syndrome rats.
METHODThe model of heat stagnation and blood stasis syndrome rats was established by injecting carrageenan and dry yeast, while the model of cold stagnation and blood stasis syndrome rats was established by body freezing. Subsequently, 10 traditional Chinese medicines with neutral property, 5 traditional Chinese medicines with heat property and 5 traditional Chinese medicines with cold property were selected for intervention to observe the changes in such indicators as whole blood viscosity, plasma viscosity and hematocrit and analyze the action characteristics of drugs with neutral property.
RESULTANOVA showed that among six of the 10 traditional Chinese medicines with neutral property, including Typhae Pollen, Sarcandrae Herba and Sappan lignum, could obviously increase the hemorheological indicators of both heat stagnation and blood stasis syndrome and cold stagnation and blood stasis syndrome rats; five traditional Chinese medicines with cold property, such as Salviae Miltiorrhizae Radix et Rhizoma, Leonuri Herba, Rhei Radix et Rhizoma, could significantly ameliorate the hemorheological indicators of heat stagnation and blood stasis syndrome rats (P < 0.01 or P < 0.05), and Salvia Miltiorrhiza Radix et Rhizoma alone could ameliorate the hemorheological indicators of cold stagnation and blood stasis syndrome rats (P < 0.05); all of the five traditional Chinese medicines with heat property could significantly ameliorate the hemorheological indicators of cold stagnation and blood stasis syndrome rats (P < 0.01), among which Carthami Flos and Notoginseng Radix et Rhizoma could significantly ameliorate the hemorheological indicators of cold stagnation and blood stasis syndrome rats. According to the average high-shear blood viscosity analysis, drugs with neutral property showed similar action characteristics to those with cold property in ameliorating hemorheology indicators of heat stagnation and blood stasis syndrome rat and better effect than those with heat property in reducing whole blood viscosity; and traditional Chinese medicines with neutral property have the similar action characteristics to those with heat property in improving the hemorheology indicators of cold stagnation and blood stasis syndrome rat and better effect than those with heat property in reducing whole blood viscosity.
CONCLUSIONUnder the condition of heat stagnation and blood stasis syndrome, traditional Chinese medicines with neutral property show the similar action characteristics to those with cold property; but under the condition of cold stagnation and blood stasis syndrome, traditional Chinese medicines with neutral property show the similar action characteristics to those with heat property. This indicates that traditional Chinese medicines with neutral property show both heat and cold properties under he conditions of heat stagnation and blood stasis syndrome and cold stagnation and blood stasis syndrome.

10.Study on expression of PTEN gene and its pseudogene PTENP1 in acute leukemia and correlation between them.

OBJECTIVETo investigate the expression of PTEN (phosphatase and tension homology deletion on chromosome 10, PTEN) and its pseudogene PTENP1 in acute leukemia (AL) and correlation between them, and to explore the role of PTENP1 on the PTEN expression in AL cells.
METHODSPTEN and PTENP1 mRNA expression were evaluated in bone marrow (BM) samples from 138 newly diagnosed AL patients and 15 healthy controls by quantitative real-time RT-PCR (qRT-PCR). pCDH1-PTENP1 3'UTR-GFP lentivirus vectors were constructed. 293T cells were transfected by calcium phosphate precipitation to produce retrovirus. HL-60 cell line was infected with the retroviral vectors expressing pCDH1-GFP and pCDH1-PTENP1 3'UTR-GFP respectively. The flow cell sorter was used to sort the HL-60 with GFP positively expressed. The mRNA expression of PTEN and PTENP1 was detected by qRT-PCR, the expression of PTEN protein by western blot, and the impact of PTENP13'UTR on the proliferation of HL-60 cells by MTT assay.
RESULTSAML patients showed significantly lower PTEN and PTENP1 mRNA expression in BM compared to healthy controls. Correlation analysis showed that the expression of PTEN and PTENP1 mRNA were positively correlated (P < 0.05). The 108 cases of PTENP1(+) AML were classified according to the prognostic classification of 2011 NCCN Clinical Practice Guidelines in AML, there was no difference among different subgroups. HL-60 cell line was infected with the retroviral vectors expressing pCDH1-GFP (control group) and pCDH1-PTENP1 3'UTR-GFP respectively. Compared with the control group, PTENP1 mRNA level of HL-60 infected with the retroviral vectors expressing pCDH1-PTENP1 3'UTR-GFP increased significantly, and PTEN mRNA level also increased. While the PTEN protein level and the cell growth rate of the PTENP1 3'UTR group didn't change significantly.
CONCLUSIONPTEN and PTENP1 mRNA expression level of BM cells from AL patients is significantly lower. There is a positive correlation between expression of PTEN and PTENP1 mRNA. PTENP1 may regulate the expression of PTEN in mRNA level.
