Chinese Journal of Ocular Fundus Diseases 2016;32(5):514-517
Objective To observe the efficacy of pars plana vitrectomy (PPV) combined with endoscopic cyclophotocoagulation (ECP) for refractory glaucoma. Methods Twenty-eight patients (30 eyes) diagnosed with refractory glaucoma were enrolled in this study. The intraocular pressure was ranged from 28 to 55 mmHg (1 mmHg=0.133 kPa),with the mean of (46.3±10.3) mmHg.There were 24 eyes with initial visual acuity <0.05,1 eye with 0.05-0.08 initial visual acuity and 5 eyes with ≥0.1 initial visual acuity. The mean number of intraocular pressure-decreased drugs was 4.All the eyes underwent PPV combined with ECP.The cyclophotocoagulation ranged from 90°-360°,with the mean of (232.5 ±77.6)°.The cyclophotocoagulation was <180° in 4 eyes,180°-270° in 6 eyes,>270° in 20 eyes. The follow-up ranged from 12 to 20 months, with the mean of 15.2 months. The intraocular pressure, vision and the mean number of intraocular pressure decreased drugs before and after surgery were comparatively observed. The relationship between cyclophotocoagulation area and postoperative intraocular pressure was analyzed. Results At the final follow up,the intraocular pressure ranged from 12 to 36 mmHg,with the mean of (18.5± 4.4) mmHg,which decreased 49.4% compared with preoperative intraocular pressure (t=15.537,P< 0.01).Among 30 eyes, the intraocular pressure was ≤ 21 mmHg without any intraocular pressuredecreased drugs in 24 eyes (80.0%),1 eye (3.3%) with ≤21 mmHg who treatment by local intraocular pressure decreased drugs, lose control of intraocular pressure in 5 eyes (16.7%).The vision was <0.05 in 15 eyes,0.05-0.08 in 3 eyes and ≥0.1 in 12 eyes. The difference of vision distribution before and after surgery was not significant (x2=6.197,P>0.05).Compared with preoperative vision, the postoperative vision was improved in 8 eyes (26.7%),stabled in 15 eyes (50.0%),decreased in 7 eyes (23.3%).The mean number of intraocular pressure-decreased drugs was reduced from 4 to 1 (t=8.402,P<0.01).The cyclophotocoagulation area was positive related to postoperative intraocular pressure (r=4.160,P<0.05).There were no ocular and systemic complications during and after surgery except slight inflammation. Conclusion PPV combined with ECP for refractory glaucoma can reduce intraocular pressure, stabilize or improve the visual acuity.
2.Effect of Recombination Human Erythropoietin on the Apoptosis of Glutamate-induced Retinal Cells
Acta Medicinae Universitatis Scientiae et Technologiae Huazhong 2014;(2):227-230
Objective To study the protective effect of recombination human erythropoietin (rhEPO) on the apoptosis of retinal neurons induced by glutamate. Methods The primary retinal neurons of postnatal SD rats were cultured in vitro for 7 days and divided into 3 groups :control group ,glutamate group and rhEPO pretreatment group. The neurons in the rhEPO pre-treatment group were afterwards allocated to three subgroups in terms of different rhEPO treatments (0.15 ,0.30 or 0.50 U/mL rhEPO for 12 h).Those in glutamate group and rhEPO pretreatment group were treated with glutamate at the concentration of 20μmol/L for 30 min for establishment of the apoptosis model.Twenty-four h later ,the apoptosis index (AI) was assayed by TUNEL and the expressions of BCL-xL mRNA and protein was detected by RT-PCR and immunocytochemistry respective-ly.Results The AI was significantly higher in the glutamate group than in the control group (P<0.01).The AI was signifi-cantly reduced ,and the expression level of BCL-xL mRNA and protein was markedly dose-dependently increased in the rhEPO pretreatment groups compared with the glutamate group (P<0.01).Conclusion The rhEPO pretreatment can inhibit the glu-tamate-induced apoptosis of retinal neurons by up-regulating the expression of BCL-xL .

OBJECTIVE:
To explore and summarize the operation method and operation stage for cochlear implant with chronic suppurative otitis media, to provide the clinical reference for cochlear implant with chronic suppurative otitis media.
METHOD:
The clinical data of 6 cases of cochlear implant with chronic suppurative otitis media from Jun 2006 to Mar 2009 in our hospital was analyzed retrospectively. The operation stage. surgical skill, possible risk and prognosis was analyzed and summarized.
RESULT:
3 of 6 cases received single stage subtotal petrosectomy and cochlear implant. 3 of 6 cases received subtotal petrosectomy, they received staged cochlear implant 1 to 6 months later. No complications occurred, all of the cochlear implanted had good open set speech perception.
CONCLUSION
Staged operation was the first choice for cochlear implant with chronic suppurative otitis media. Single stage operation took potential risks, it should be done cautiously. The key points for the operation was the clearance of the pathological tissue totally, this required good surgical skill and operation instrument.

6.Research progress in inhibition of corneal neovascularization by anti-VEGF therapy
Journal of Shanghai Jiaotong University(Medical Science) 2009;29(7):886-889
Corneal neovascularization is a significant and sight-threatening complication of many ocular surface disorders, and may cause corneal sear and rejection reaction after corneal grafting. Recent studies have revealed that vascular endothelial growth factor (VEGF) plays an important role in corneal neovascularization, and inhibition of VEGF has become a main strategy for treatment of corneal neovascularization. This article reviews the research progress in inhibition of corneal neovascularization by anti-VEGF therapy.
7.Whole-genome expression analysis of synovial fibroblasts in rheumatoid arthritis
Chinese Journal of Orthopaedics 2017;37(11):691-697
Objective To identify the crucial gene implicated in rheumatoid arthritis (RA) pathogenesis by comparing microarray-based gene expression profiles of synovial fibroblast in arthritis patients and that in control. Methods The public datasets were obtained from NCBI GEO and EBI ArrayExpress. The qualified microarray-based gene expression profiles were integrated and normalized using the method implemented in GeneSpring software.Furthermore, the differentially expressed genes (DEGs) were identified using significance analysis of microarrays (SAM) method. The online tool DAVID and STRING were applied to conduct the enrichment analysis and gene product interaction analysis respectively. Results There were two datasets that were qualified and analyzed in the present study.A total of 336 significant DEGs were identified by comparing the whole-genome gene expression profiles from synovial fibroblast of RA patients and control group.Among these DEGs,261 were significantly downregulated and 75 upregulated.About 13.6% of the downregulated genes were associated with extracellular matrix degradation. The COL9A3 and COL4A5,indispensable component of hyaline cartilage and basement membrane respectively, were significantly downregulated,as well as genes in WNT family, including WNT2,WNT11,and WNT16.In contrast, matrix metalloproteinase 13 (MMP13) was found to be significantly upregulated in RA patients.MMP13 is a matrix metallopeptidase that degrade extracellular matrix and hyaline cartilage, and it could possibly interact with other proteins to regulate morphogenesis. Conclusion Molecular mechanisms underlying RA pathogenesis were investigated by analyzing the public datasets.A few genes that associated with extracellular matrix degradation, construction and regulation, including MMP13,WNT2,WNT11,WNT16,COL9A3 and COL4A5,could be regarded as therapeutic targets in RA treatment.
