1.Investigation of the risk factors associated with early mortality of the rapid two-stage arterial switch operation

Objective To investigate the risk factors associated with early mortality of the rapid two-stage arerial switch operation, which has a significantly higher overall mortality than that of ASO procedure for D-TGA with a intact ventricular septun. Methods The data we reviewed involving patients who underwent rapid two-stage switch operations from September,2002 to September, 2007 in our center, 13 patients were male and 8 were female, their age at operation ranged from 29 to 250 days afer birth, and the body weight was 3.5 to 7 kg. Chi-squared test and multivariant logistic regression methods were used for the analysis of demographic data, pre-diagnosis information, operation data, interval data combined with the operation time,left ventricle training condition, and the early post-operative outcomes. Results The operative mortality was high at initial stage, and then decreased gradually. The logistic multivariant regression analysis indicated that the mortality of left ventricular training operation was associated with the diameters of BT shunt ( P =0.003 ); the mortality of two-stage switch was associated with feminie ( P = 0.006 ) and pre-operative p. LV/RV ( P < 0.001 ). Conclusion Patients with transposition of the great arteries and intact ventricular septum who missed the optimal time for switch operation should receive rapid two-stage switch operations, which provide an opportunity for the correction of the deformity. The key factor associated with the success for operation was good in heart function after left ventricular training.
2.Study on HPLC fingerprint determination Huanglianjiedu Decoction

Objective:To establish a HPLC fingerprint determination which could separate the main principles of alkaloids, flavonoids and iridoids in Huanglianjiedu Decoction. Methods: The HPLC method was used and chromatography conditions were C 18V column, binary or ternary solvent-delivery gradient elution and diode-array detector. Results: A HPLC fingerprint determination method was established, which could separate main principles of the three effective groups of Huanglianjiedu Decoction. Conclusion: The ingredients of traditional Chinese medicine consist of several effective groups with different chemicals and physical properties, compared with the plant standard extracts, further studies are required for establishing HPLC fingerprint.
3.Comparison of HLA serologicul typing and molecular typing in acute leukemia between pre-and post complete remission

Objective To compare the results of HLA serological typing and gene typing in acuteleukemia before and after achieved complete remission.then make sure whether the HLA matching could be performed before remission of acute leukemia.Methods The peripheral blood samples of 20 patients with acute leukemia at onset and in complete remission were performed for the HLA serological typing and gene typing by complement-dependent microcytotoxicity(CDC)assay and PCR-sequence for specific primer. The class Ⅰ HLA antigens reaction was quoted as NIH score.and paired-sample t test Was performed.The class Ⅰand class Ⅱ HLA allele specificity of acute leukemia at onset Was compared with that in complete remission.Results No absence or change of the class Ⅰ and class Ⅱ HLA allele specificity in acute leukemia was observed before treatment and after complete remission.There were significant differences (P<0.05) for HLA-A. B and Bw6 reaction at onset from complete remission.The expression of HLA-A, B and Bw6 was significant down-regulated.This phenomenon did not occur on Bw4 (P>0.05).The relationship of down-regulation of class Ⅰ HLA antigens and the amount of blast in peripheral blood Was not observed.Conclusion HIA typing could be performed by molecular technique when patients with acute leukemia were diagnosed.It would shorten the period of HLA-matching,and increase the opportunity of finding an appropriate allergenic hematopoietic stem cell donor.

6.Factors affecting the postoperative function of malleolar fracture

Objective To discuss the factors which affect the postoperative functions of the ankle joint. Methods A retrospective study was done of 102 patients who had been diagnosed as malleolar fracture and operated on in our institute between January 2005 and January 2008. We recorded their age, gender, body mass index(BMI), fracture type (AO type), time from injury to operation and presence or ab-sence of cast immobilization. Their ankle functions were evaluated by X-ray and the Baird-Jackson evaluation system in regular follow-up. Relationship between the above-mentioned factors and the postoperative functions of the ankle joint was statistically analyzed, using univariate logistic regression and multiple stepwise logistic regression. Results A total of 102 patients were followed up for 24.7 (11 to 43) months. A negative correlation between the age, fracture type, reduction and postoperative function was found. The gender, body mass index (BMI), time from injury to operation and presence or absence of east immobilization, however, had no association with the postoperative function. The conservative treatment of the deltoid ligament injury complicated with the lateral malleolar fracture and/or improper treatment of the syndesmotic injury led to poor function. Conclusions The older a patient and the more serious a fracture, as well as the more unsat-isfactory the reduction, the poorer the postoperative ankle functions may be. To some extent, rational treat-ment of the deltoid ligament injury complicated with the lateral malleolar fracture and the syndesmotic injury may also determine the postoperative function of the ankle joint.
7.Fixation features of foveoschisis in pathological myopia

Objective To observe the fixation features of foveoschisis in pathological myopia patients.Methods 36 patients(42 eyes)with>-6.00D myopic degree and foveoschisis who diagnosed by optical coherence tomography were included.The patients were divided into foveoschisis with retinal detachment group(11 eyes),foveoschisis with macular hole group(12 eyes)and only foveoschisis group(19 eyes).Micro-perimeter MP-1 was used to identify the fixation location and fixation stability.Results The eccentric fixation were formed above the foyea in the foveoschisis with retinal detachment and foveoschisis with macular hole group,the fixation location was formed on fovea in the only foveoschisis group.The fixation stability of the foveoschisis with retinal detachment,foveoschisis with macular hole and only foveoschisis group were(23±4)0A,(59±6)% ,(91±11)% respectively,the difference was statistically significant(F=243.47,P<0.01).Conclusions The fixation location is formed on foyea in the patients with foveoschisis in pathological myopia.The eccentric fixation is formed above the fovea in the patients who has foveoschisis with retinal detachment and macular hole in pathological myopia.
8.Detection of phenotype and analysis of genotype in vancomycin-resistant Enterococci

Objective To research the resistant characteristics, genotype and prevalence of vancomycin-resistant Enterococci( VRE) . Methods K-B disc diffusion method was used to determine the susceptibility, minimum inhibi-tory concentration ( MIC) for vancomycin of VRE was detected by E-test method;VRE was then subjected to PCR for resistance related genes;6 strains sequencing results of PCR product were contrastively analyzed the amino acid sequence by BLAST. Results 6 VREFm strains were found from 193 strains enterococci;6 VREFm strains were completely resistant to high unit gentamicin, ampicillin, ciprofloxacin, teicoplanin,but were sensitive to linezolid and macrodantin,MIC for vancomycin was more than 256 mg/L;Genotypes were vanA;Amino acids in vanA gene have changed with Asn83→Asp( AAC→GAC) . Conclusion VRE in our hospital is mostly multi-drug resistant , which bringS difficulty in clinical treatment for its infection. So the hospital should strengthen the prevention and monitoring, to prevent the spread of VRE strains in the hospital and popular.
9.Experience of Transabdominal Gastroesophageal Stapling in Management of Variceal Bleeding in Portal Hypertension(Report of 48 cases)

Objective To investigate the effect of transabdominal gastroesophageal stapling in management of variceal bleeding in portal hypertension with Hassabi operation and sum up our clinical experience.Methods Retrospective analysis of 48 cases with portal hypertension operated by transabdominal gastroesophageal stapling between May,1998 and Oct.2005.The elimination of oesophagus venous、the rebleeding rate、the operation mortality、the stricture and the fistula rate and the index that influences the liver function were maily observed.Results No case suffered rebleeding and hepatal encephalopathy during the hospitaligation of the patients.Esophageal varices disappeared or reduced in all cases.One case resulted from stricture and one case died of multiple organ failure.Conclusions Transabdominal extensive oesophagogastric devascularization combined with gastrooesophageal stapling is an effective and safe procedure for control of acute variceal haemorrhage with satisfactory short-term control,but the long-term effect needs to be long follow-up for the patients.

Nonalcoholic fatty liver disease (NAFLD)is prevalent in patients with characteristics of metabolic syndrome.The main clinico-pathological manifestations of NAFLD are fatty degeneration of liver parenchyma cells and fat storage,which can progress to liver cirrhosis and eventually lead to liver failure.Toll -like receptor 4 (TLR4)signaling pathway is involved in the development and progression of NAFLD.At present,the role of TLR4 in the onset of NAFLD is highly valued by clinical doctors.This paper reviews the structure of TLR4, the composition of TLR4 signaling pathway,the regulation of TLR4 expression,the function of TLR4,and the relationship between TLR4 and NAFLD,suggesting that TLR4 signaling pathway is going to be an important research field in the treatment of NAFLD in the future.
